Abstract. We investigated education and occupational influences as markers of cognitive reserve in relation to cognitive performance and decline on multiple fluid and crystallized abilities in preclinical dementia. From the total sample of 702 participants stemming from the OCTO-Twin Study (Sweden), aged 80+ at baseline in 1992–1993, only those who developed dementia during the study period (N = 127) were included in these analyses. Random effects models were used to examine the level of performance at the time of dementia diagnosis and the rates of decline prior to diagnosis. The results demonstrated that both fluid and crystallized abilities decline in preclinical stages, and that education and occupational class have independent moderating roles on the cognitive performance at the time of diagnosis, but not on the rates of decline.

The elderly population is growing worldwide, and therefore cognitive decline and dementia is a major problem for healthcare system. However, many elders do not develop dementia or significant cognitive impairment even though present brain lesions, such as cortical atrophy and/or lesions, leading to the concept of Cognitive Reserve (CR). The main objective of this review is to establish the recent findings of CR in elderly cognition and explore some of the cognitive markers related to CR. In order to accomplish that we carried out a search for papers published either in English or Portuguese language in the last 5 years in the Medline database using as keywords cognitive reserve, elderly and aging/ageing. We filtered 14 studies that specifically approached the neuropsychological aspects (e.g, memory, attention, orientation, executive function) and reviewed them in detail. Based on these papers regarding old-aged individuals, education appears to have several implications on CR by strengthening cognitive abilities, however does not appear to impact on cognitive decline. Besides, we realized that cognitive performance is one of the form to measure CR, even though the methods cannot be standardized, which may be the cause of some varied conclusions. Regarding CR, education was the most prevalent measure, and CR seems to have a beneficial effect on executive function and episodic memory and it seems to act by both neural reserve and neural compensation. Print exposure appears as a potential variable positively related to cognitive performance and CR.

Objective:We sought to determine whether MRI-based Cerebral Small Vessel Disease (CSVD) burden assessment, in addition to clinical and CT data, improved prediction of cognitive impairment after spontaneous Intracerebral Hemorrhage (ICH).Methods:We analyzed data from ICH survivors enrolled in a single-center prospective study. We employed three validated CSVD burden scores: global, cerebral amyloid angiopathy (CAA)-specific, hypertensive arteriopathy (HTNA)-specific. We quantified cognitive performance by administering the modified Telephone Interview for Cognitive Status (TICS-m) test. We utilized linear mixed models to model cognitive decline rates, and survival models for new-onset dementia. We calculated CSVD scores’ cut-offs to maximize predictive performance for dementia diagnosis.Results:We enrolled 612 ICH survivors, and followed them for a median of 46.3 months (Inter-Quartile Range: 35.5-58.7). A total of 214/612 (35%) participants developed dementia. Higher global CSVD scores at baseline were associated with faster cognitive decline (Coeff -0.25, Standard Error [SE] 0.02) and dementia risk (Sub-Hazard Ratio 1.35, 95% CI 1.10-1.65). The global score outperformed the CAA and HTNA scores in predicting post-ICH dementia (all p<0.05). Compared to a model including readily available clinical and CT data, inclusion of the global CSVD score resulted in improved prediction of post-ICH dementia (Area Under the Curve [AUC] 0.89, SE 0.02 vs. AUC 0.81, SE 0.03, p = 0.008 for comparison). Global CSVD scores ≥ 2 had highest sensitivity (83%) and specificity (91%) for dementia diagnosis.Conclusions:A validated MRI-based CSVD score is associated with cognitive performance after ICH, and improved diagnostic accuracy for predicting new onset of dementia.

Objective: To examine the association between physical activity and cognitive performance in a longitudinal study. Methods: We analyzed data from 17.333 noninstitutionalized persons aged 50 years or older in 11 European countries who participated in Wave 1 (2004/2005) and Wave 2 (2006/2007) of the Survey of Heath, Ageing, and Retirement in Europe (SHARE). Physical activity at baseline was measured as self-reported frequency of sports or activities requiring vigorous activity, and frequency of activities demanding a moderate level of activity. Cognitive function was measured at baseline and after a mean of 2.5 years of follow-up by delayed word recall and verbal fluency tests. The effects of physical activity at baseline on cognitive performance at follow-up were assessed in hierarchical multilevel random effects models adjusted for sociodemographic variables (age, education), somatic comorbidities, functional impairment (basic and instrumental activities of daily living, maximum grip strength), depressive symptoms, and body mass index. Results: After adjusting for potential confounders, we found physical inactivity (neither moderate nor vigorous) to be associated with a higher rate of cognitive decline over a mean follow-up of 2.5 years (β = –1.79 (SE = 0.17) for verbal fluency; β = –0.35 (SE = 0.04) for delayed word recall). Further analyses showed that vigorous activities more than once a week were especially related to change in cognition over time. Conclusion: Engagement in moderate and vigorous physical activities protects against cognitive decline in older age. Participation in physical activities may be of particular importance when other risk factors for cognitive decline are present.

Abstract Background: Elders with subjective cognitive decline (SCD) in the absence of objective cognitive impairment are at increased risk of developing mild cognitive impairment (MCI) or Alzheimer’s disease (AD). Previous study has reported that higher education was associated with better performance in cognitive performance in elderly people with SCD. However, it is not clear whether there is a differential effect of education on cognition between healthy controls (HC), individuals with SCD and patients with cognitive impairment (CI).Methods: We performed a cross-sectional study that included 186 HC, 279 SCD subjects and 79 patients with CI from the Sino Longitudinal Study on Cognitive Decline (SILCODE) project. A series of neuropsychological tests of memory, executive, language, and general cognitive function were used to assess the subjects cognitive performance. We performed multiple linear regression models to examine the effect of education on neuropsychological test scores in the total sample and then described differences in the effect according to three different groups. In addition, we presented the effect of education on total test score expressed as a global composite z-scores in the total sample and three different groups, respectively. Furthermore, we examined whether apolipoprotein E ε4 allele (APOE ε4) status would regulate the effect of education on the global composite score among three different groups.Results: Education has a positive effect on cognition in the total sample. Stratification for different groups showed that the positive effect of education on cognition is found in HC and SCD group, while education has a negative effect on cognition in the CI group. Furthermore, we found the APOE ε4 allele does not modify the positive effect of education on cognition in the HC group, but the APOE ε4 allele weaken that beneficial effect in the SCD group and maintain the negative effect in the CI group.Conclusions: High education may delay the progression from SCD to cognitive impairment and yield differential effects on cognition across the spectrum of AD. Furthermore, that differential effects are subjected to modulation by the APOE ε4 status.

Recent advances in latent space and related random effects models hold much promise for representing network data. The inherent dependency between ties in a network makes modeling data of this type difficult. In this article we consider a recently developed latent space model that is particularly appropriate for the visualization of networks. We suggest a new estimator of the latent positions and perform two network analyses, comparing four alternative estimators. We demonstrate a method of checking the validity of the positional estimates. These estimators are implemented via a package in the freeware statistical language R. The package allows researchers to efficiently fit the latent space model to data and to visualize the results.

Abstract Objective The prevalence and prognostic implications of pre-existing dyslipidaemia in patients infected by the SARS-CoV-2 remain unclear. To perform a systematic review and meta-analysis of prevalence and mortality risk in COVID-19 patients with pre-existing dyslipidaemia. Methods Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed in abstracting data and assessing validity. We searched MEDLINE and Scopus to locate all the articles published up to January 31, 2021, reporting data on dyslipidaemia among COVID-19 survivors and non-survivors. The pooled prevalence of dyslipidaemia was calculated using a random effects model and presenting the related 95% confidence interval (CI), while the mortality risk was estimated using the Mantel-Haenszel random effects models with odds ratio (OR) and related 95% CI. Statistical heterogeneity was measured using the Higgins I2 statistic. Results Eighteen studies, enrolling 74.132 COVID-19 patients [mean age 70.6 years], met the inclusion criteria and were included in the final analysis. The pooled prevalence of dyslipidaemia was 17.5% of cases (95% CI: 12.3-24.3%, p &lt; 0.0001), with high heterogeneity (I2=98.7%). Pre-existing dyslipidaemia was significantly associated with higher risk of short-term death (OR: 1.69, 95% CI: 1.19-2.41, p = 0.003), with high heterogeneity (I2=88.7%). Due to publication bias, according to the Trim-and-Fill method, the corrected random-effect ORs resulted 1.61, 95% CI 1.13-2.28, p &lt; 0.0001 (one studies trimmed). Conclusions Dyslipidaemia represents a major comorbidity in about 18% of COVID-19 patients but it is associated with a 60% increase of short-term mortality risk.

Abstract Dementia is arguably the most devastating condition of older adulthood with treatment options still elusive. Alzheimer’s is the most prevalent form of dementia where cognitive deficits relate strongly to underlying brain pathology. However, there exist cases in which cognitive performance does not match the corresponding level of neuropathology. Attempts to explain this phenomenon often include the concept of cognitive reserve (CR), whereby greater CR (e.g., more education or higher occupational position) presumably results in less impairment relative to the extent of pathology early in disease progression but also greater impairment once cognitive symptoms manifest. We examined the influence of CR proxy variables (education and occupation) on the relationship between hippocampal volume and cognitive performance on tests of executive control and memory using data from the Czech Brain Aging Study (CBAS). Participants were cognitively normal/with subjective cognitive decline but without actual impairment (CN; n=115; M(age)=66.43; M(education)=15.90; 37 men) or had amnestic mild cognitive impairment (aMCI; n=165; M(age)=71.37; M(education)=14.92; 85 men). We found that hippocampal volume was significantly related to executive control (b=-.0001, p=.03) and memory (b=.0002, p&lt;.001) for participants with aMCI, but only memory (b=.0002, p=.03) for CN participants. Occupational position moderated the association between memory and hippocampal volume in aMCI, with the result approaching significance (p=.07), whereby a greater link between memory problems and hippocampal atrophy was present in those previously in high occupational positions. No other moderations for occupational position or education emerged (ps&gt;.25). We found evidence for the concept of CR using occupational position as proxy.

AbstractObjectivesThe results of existing studies on bisphenol A (BPA) and puberty timing did not reach a consensus. Thereby we performed this meta-analytic study to explore the association between BPA exposure in urine and puberty timing.MethodsMeta-analysis of the pooled odds ratios (OR), prevalence ratios (PR) or hazards ratios (HR) with 95% confidence intervals (CI) were calculated and estimated using fixed-effects or random-effects models based on between-study heterogeneity.ResultsA total of 10 studies involving 5621 subjects were finally included. The meta-analysis showed that BPA exposure was weakly associated with thelarche (PR: 0.96, 95% CI: 0.93–0.99), while no association was found between BPA exposure and menarche (HR: 0.99, 95% CI: 0.89–1.12; OR: 1.02, 95% CI: 0.73–1.43), and pubarche (OR: 1.00, 95% CI: 0.79–1.26; PR: 1.00, 95% CI: 0.95–1.05).ConclusionsThere was no strong correlation between BPA exposure and puberty timing. Further studies with large sample sizes are needed to verify the relationship between BPA and puberty timing.

IntroductionProstate cancer is the most prevalent oncological disease among men in industrialised countries. Despite the high survival rates, treatments are often associated with adverse effects, including metabolic and cardiovascular complications, sexual dysfunction and, to a lesser extent, cognitive decline. This study was primarily designed to evaluate the trajectories of cognitive performance in patients with prostate cancer, and to quantify the impact of the disease and its treatments on the occurrence of cognitive decline.MethodsParticipants will be recruited from two main hospitals providing care to approximately half of the patients with prostate cancer in Northern Portugal (Portuguese Institute of Oncology of Porto and São João Hospital Centre), and will comprise a cohort of recently diagnosed patients with prostate cancer proposed for different treatment plans, including: (1) radical prostatectomy; (2) brachytherapy and/or radiotherapy; (3) radiotherapy in combination with androgen deprivation therapy and (4) androgen deprivation therapy (with or without chemotherapy). Recruitment began in February 2018 and is expected to continue until the first semester of 2021. Follow-up evaluations will be conducted at 1, 3, 5, 7 and 10 years. Sociodemographic, behavioural and clinical characteristics, anxiety and depression, health literacy, health status, quality of life, and sleep quality will be assessed. Blood pressure and anthropometrics will be measured, and a fasting blood sample will be collected. Participants’ cognitive performance will be evaluated before treatments and throughout follow-up (Montreal Cognitive Assessment and Cube Test as well as Brain on Track for remote monitoring). All participants suspected of cognitive impairment will undergo neuropsychological tests and clinical observation by a neurologist.Ethics and disseminationThe study was approved by the Ethics Committee of the hospitals involved. All participants will provide written informed consent, and study procedures will be developed to ensure data protection and confidentiality. Results will be disseminated through publication in peer-reviewed journals and presentation in scientific meetings.
