Introduction: Piezosurgery is an emerging boom in the field of maxillofacial surgery for precise, safe and effective osteotomies sparing the adjacent vital structures compared to conventional surgery. Corpus: It works on the principle of piezoelectric effect in which crystals in the piezoelectric substances get deformed on the application of an electric field. Various studies gave the evidence of improved wound healing and bone formation compared to conventional approaches. The soft tissue sparing capability with improved patient comfort and decreased blood loss gave the utmost importance for this surgical technique in the present as well as future world of surgery. Conclusion: Piezosurgery has emerging as a promising surgical modality with a wide range of clinical applications throughout the whole field of surgery.

Abstract Background Post-bariatric patients present a surgical challenge within abdominoplasty because of residual obesity and major comorbidities. In this study, we analyzed complications following abdominoplasty in post-bariatric patients and evaluated potential risk factors associated with these complications. Objectives The authors sought to determine the complications and risk factors following abdominoplasty in post-bariatric patients. Methods A retrospective study of patients who underwent abdominoplasty was performed from January 2009 to December 2018 at our institution. Variables analyzed were sex, age, body mass index (BMI), smoking, surgical technique, operative time, resection weight, drain output, and complications. Results A total of 406 patients were included in this study (320 female and 86 male) with a mean age of 44.4 years and a BMI of 30.6 kg/m2. Abdominoplasty techniques consisted of traditional (64.3%), fleur-de-lis technique (27.3%), and panniculectomy without umbilical displacement (8.4%). Overall complications recorded were 41.9%, the majority of these being wound-healing problems (32%). Minor and major complications were found in 29.1% and 12.8% of patients, respectively. A BMI value of ≥30 kg/m2 was associated with an increased risk for wound-healing problems (P = 0.001). The frequency of total complications was significantly related to age (P = 0.007), BMI (P = 0.004), and resection weight (P = 0.001). Abdominoplasty technique tended to influence total complications. Conclusions This study demonstrates in a fairly large sample of post-bariatric patients (n = 406) that abdominoplasty alone can be performed safely, with an acceptable complication rate. Age, BMI, and resection weight are shown to be significant risk factors for total complications. The role of surgical technique needs to be evaluated further. Level of Evidence: 4

Abstract Objectives Air-polishing has been used in the treatment of periodontitis and gingivitis for years. The introduction of low-abrasive powders has enabled the use of air-polishing devices for subgingival therapy. Within the last decade, a wide range of different low-abrasive powders for subgingival use has been established. In this study, the effects of a glycine powder and a trehalose powder on human gingival fibroblasts (HGF) were investigated. Methods HGF were derived from three systemically and periodontally healthy donors. After 24 h and 48 h of incubation time, mRNA levels, and after 48 h, protein levels of TNFα, IL-8, CCL2, and VEGF were determined. In addition, NF-κB p65 nuclear translocation and in vitro wound healing were assessed. Statistical analysis was performed by ANOVA and post hoc Dunnett’s and Tukey’s tests (p < 0.05). Results Glycine powder significantly increased the expression of proinflammatory genes and showed exploitation of the NF-κB pathway, albeit trehalose powder hardly interfered with cell function and did not trigger the NF-κB pathway. In contrast to trehalose, glycine showed a significant inhibitory effect on the in vitro wound healing rate. Conclusion Subgingivally applicable powders for air-polishing devices can regulate cell viability and proliferation as well as cytokine expression. Our in vitro study suggests that the above powders may influence HGF via direct cell effects. Trehalose appears to be relatively inert compared to glycine powder.

Radiofrequency ablation may be described as a thermal strategy to destroy tissue by increasing its temperature and causing irreversible cellular injury. Radiofrequency ablation is a relatively new modality which has found use in a wide range of medical applications and gained acceptance. RF ablation has been used to destroy tumors in the liver, prostate, breasts, lungs, kidneys, bones, and eyes. One of the early clinical applications was its use in treating supraventricular arrhythmias by selectively destroying cardiac tissue. Radiofrequency ablation has become established as the primary modality of transcatheter therapy for the treatment of symptomatic arrhythmias. Radiofrequency catheter ablation of cardiac arrhythmias was investigated using a finite-element based solution of the bioheat transfer equation. Spatial and temporal temperature profiles in the cardiac tissue were visualized.

There is growing interest in expanding the clinical applications for HBO2(hyperbaric oxygen therapy) into new medical and surgical fields. The pathophysiology of response towards wounds, infection, trauma, or surgery involves various chemical mediators that include cytokines, prostaglandins (PGs), and nitric oxide (NO). The beneficial role played by HBO2in wound healing, carbon monoxide poisoning, decompression sickness, and other indications is well documented. However, the exact mechanism of action is still poorly understood. This review addresses the effects of HBO2on PGs, NO, and cytokines involved in wound pathophysiology and inflammation in particular. The results of this review indicate that HBO2has important effects on the biology of cytokines and other mediators of inflammation. HBO2causes cytokine down-regulation and growth factor up-regulation. HBO2transiently suppresses stimulus-induced proinflammatory cytokine production and affects the liberation of TNFα (tumor necrosis factor alpha) and endothelins. VEGF (vascular endothelial growth factor) levels are significantly increased with HBO2, whereas the value of PGE2 and COX-2 mRNA are markedly reduced. The effect of HBO2on NO production is not well established and more studies are required. In conclusion, cytokines, PGs, and NO may play a major role in the mechanism of action of HBO2 and further research could pave the way for new clinical applications for HBO2to be established. It could be proposed that chronic wounds persist due to an uncontrolled pathological inflammatory response in the wound bed and that HBO2enhances wound healing by damping pathological inflammation (anti-inflammatory effects); this hypothetical proposal remains to be substantiated with experimental results.

Objective: Algae is the undisputed treasures of the sea and are a valuable raw material, providing unlimited opportunities for new drug discoveries. Marine algal products are in demand in the international market in the form of standardized algal extracts or semi-finished products. Methods: Aqueous and methanolic extracts of Oedogonium globosum and Oedogonium intermedium species were obtained maceration and hot percolation. The active principles from O. intermedium were isolated, purified by column chromatography, and characterized by spectral studies IR, λmax, 1HNMR and MS. The extracts of Oedogonium species were screened for their anti-microbial effects, acute dermal irritation and wound-healing activity studies. Results: Comparing to Oedogonium intermedium (45 %, 90 %, 87 %), very low extractive yields were obtained for Oedogonium globosum (10.80 %, 37 %, 28 %). At phytochemical screening, Terpenoids, Flavanoids and, Glycans were found to be present in a significant amount and upon their isolation, it was found that a collection of fractions from cold extract with Rf value in the range 0.32-0.34 as Glycans and those from the hot extract with 0.40-0.72 as Flavanoids and those from methanolic extracts with 0.23, 0.44 and 0.71 as for Terpenoids. Anti-bacterial study revealed out the fact of Oedogonium species could give higher inhibition to gram-positive than for gram-negative bacteria at (10 μg/10μl/disc) concentration. No symptoms of systemic toxicity and mortality were observed. Silver sulfadiazine, more potent in wound closure, the effect of methanolic extracts of O. intermedium (87 %) was almost at par to the standard (95 %) in action and significantly greater than O. globosum (72 %, P<0.05). Conclusion: Admittedly, Oedogonium type algal species can be known as medicinal algae with a plethora of a wide range of pharmacological activities. Thus, this research work may be considered further for extensive innovative discoveries of new lead molecules and any other pharmacological activities, in the future.

Abstract Ballota nigra, also known as black horehound is a common medical herb used in folk medicine around the world. First reported mentions of its medical properties and use goes as far as the 13th century. The use of black horehound depends on regions and countries. It is used mostly to treat e.g. mild sleep disorders, nervousness, upset stomach, wound healing. It can be used as an anti-inflammatory, antibacterial, antiprotozoal, antifungal drug. Moreover, it has been reported as a potential cancer drug. This extensive usage is particularly interesting for us. The aim of this review is to present available data on B. nigra pharmacological effects and known traditional uses gathered from a wide range of scientific articles published in 1997–2020.

Torch ginger has a wide range of good antimicrobial, antioxidant, anticancer, larvicidal and repellent activities. Active compounds in Torch ginger that affect pharmacological activities are phenols, polyphenols, flavonoids, and terpenoids. Based on these ingredients Torch ginger can be used to heal wounds. This study aims to observe the effect of Torch ginger Umbut extract on wound healing in mice. This study hopes to add information about the effect of Torch ginger umbut extract on the healing of cuts in mice so that it can be another alternative for wound healing and can be a reference for other researchers. This study used a completely randomized design with 5 treatments and 3 replications. Tests carried out on adult male mice. The results showed that the optimal wound healing in P2 is treatment with 10% Torch ginger umbut extract which requires a range of wound healing 7-8 days. Based on these results, Torch ginger umbut extract can heal wounds.

Background Recently, radial artery access has gained popularity for interventional neurovascular procedures due to patient comfort and fewer complications. However, there are instances where the radial artery approach is not feasible. In such cases, trans-ulnar artery access (TUA) can offer an alternate route. There is limited data regarding neuro-interventional procedures performed via this approach. This study aims to evaluate the feasibility and safety of trans-ulnar approach for a wide range of interventional neurovascular procedures. Materials and methods The data for all patients who underwent ulnar artery access for diagnostic or interventional neuroradiology procedures was retrospectively collected between September 2020 and March 2021. Patient demographics, procedural details, procedure success, and complications were recorded. Results During the study period, 23 patients underwent 24 trans-ulnar approach procedures. The mean age of patients was 50.1 ± 14.2 years. Fourteen diagnostic cerebral angiograms and ten interventional procedures were performed. All procedures were successfully completed via trans-ulnar approach without a switch to alternate access. No major access site complication was observed. Conclusion Ulnar artery access is a safe and feasible option for neurovascular procedures. It can be effectively utilized for diagnostic cerebral angiography and a wide range of interventional procedures.

Different biomaterials in the form of ceramics, metal alloys, composites, glasses, polymers, etc. have gained wide-range acceptance in the realm of medical sciences. Bioimplants from such biomaterials have been constructed and used widely for different clinical applications. With the continual progress, biomaterials that may be resorbed inside the body have been developed. These have done away with the major challenge of removal of an implant after it has served its intended function. Important factors are taken into consideration in design and development of implants from such biomaterials are mechanical properties, degradation rate, surface modification, rate of corrosion, biocompatibility, and non-toxicity. Given the importance of such materials in clinical applications, the chapter presents an overview of the bioresorable composites and their implants. The related properties and the functions served have been outlined briefly. Further, the challenges associated and the remedies to overcome them have also been delineated.
