A recent publication describes a sophisticated “liquid biopsy” procedure that analyses both tumoral DNA and proteins present in peripheral blood to provide sensitive detection of different types of cancer. These results represent a significant advance, but they still fall short of the sensitivity and specificity required, and it seems unlikely that they could progress to a level at which screening of the (asymptomatic) general population would have a really favourable benefit to risk ratio.

The general population increasingly uses digital services, meaning services which are delivered over the internet or an electronic network, and events such as pandemics have accelerated the need of using new digital services. Governments have also increased their number of digital services, however, these digital services still lack of sufficient information security, particularly integrity. Blockchain uses cryptographic techniques that allow decentralization and increase the integrity of the information it handles, but it still has disadvantages in terms of efficiency, making it incapable of implementing some digital services where a high rate of transactions are required. In order to increase its efficient, a multi-layer proposal based on blockchain is presented. It has four layers, where each layer specializes in a different type of information and uses properties of public blockchain and private blockchain. An statistical analysis is performed and the proposal is modeled showing that it maintains and even increases the integrity of the information while preserving the efficiency of transactions. Besides, the proposal can be flexible and adapt to different types of digital services. It also considers that voluntary nodes participate in the decentralization of information making it more secure, verifiable, transparent and reliable.

Background Tumor immunology research has led to the identification of a number of tumor-associated self antigens, suggesting that most tumors trigger an immunogenic response, as is the case in osteosarcoma, where the detection of natural serum IgM antibodies might achieve the diagnosis of osteosarcoma. Natural IgM antibodies to tumor-associated proteins may expand the number of available tumor biomarkers for osteosarcoma and may be used together in a serum profile to enhance test sensitivity and specificity. Natural IgM antibodies can be consistently detected in the peripheral blood sera months to years before the tumor is diagnosed clinically. The study of the level of a potential biomarker many months (or years) prior to diagnosis is fundamentally important. Integrated circulating and imaging markers in clinical practice treating osteosarcoma have potential applications for controlling tumor angiogenesis. Objectives To study the expression of natural IgM antibodies to the tumor antigens of angiogenesis in the peripheral blood sera of osteosarcoma patients and healthy individuals, and to develop serum-based predictive biomarkers. Methods Peripheral venous blood samples were collected from 117 osteosarcoma patients and 117 patients with other tumors. All diagnosis was histologically confirmed. Staging of patients was performed according to the Enneking Surgical Staging System. The control group consisted of 117 age- and sex- matched healthy individuals. In this study, novel immunoconjugates were designed, synthesized and then used to develop a rapid, specific and sensitive enzyme-linked immunosorbent assay (ELISA) method to detect angiogenin (ANG)–IgM directly in the peripheral blood sera of humans. Results Serum ANG–IgM levels are significantly higher in osteosarcoma patients than in healthy individuals ( P < 0.005). Serum ANG–IgM levels varied widely, but were highly dependent on the concentration of IgM (r = 0.85; P < 0.0005). We found ANG–IgM in the sera of 85% of newly diagnosed osteosarcoma patients and ANG–IgM levels were significantly higher in osteosarcoma patients compared to any other tumors ( P < 0.001). Conclusions These results demonstrated that the combined biomarker ANG–IgM has greater sensitivity and specificity in early diagnosis of osteosarcoma patients than the traditional biomarkers (ANG and vascular endothelial growth factor). Circulating ANG–IgM immune complexes can potentially serve as a biomarker for increased risk of osteosarcoma, because relatively high serum levels were also detected in otherwise healthy individuals with a first degree family history of osteosarcoma and in patients with a diagnosis of benign conditions. Immunological aspects of angiogenesis for managing osteosarcoma will have a practical value in early diagnosis, prognosis and monitoring response to antiangiogenic therapy.

The progressive aging of the world’s population makes a higher prevalence of neurodegenerative diseases inevitable. The necessity for an accurate, but at the same time, inexpensive and minimally invasive, diagnostic test is urgently required, not only to confirm the presence of the disease but also to discriminate between different types of dementia to provide the appropriate management and treatment. In this study, attenuated total reflection FTIR (ATR-FTIR) spectroscopy combined with chemometric techniques were used to analyze blood plasma samples from our cohort. Blood samples are easily collected by conventional venepuncture, permitting repeated measurements from the same individuals to monitor their progression throughout the years or evaluate any tested drugs. We included 549 individuals: 347 with various neurodegenerative diseases and 202 age-matched healthy individuals. Alzheimer’s disease (AD;n= 164) was identified with 70% sensitivity and specificity, which after the incorporation of apolipoprotein ε4 genotype (APOEε4) information, increased to 86% when individuals carried one or two alleles of ε4, and to 72% sensitivity and 77% specificity when individuals did not carry ε4 alleles. Early AD cases (n= 14) were identified with 80% sensitivity and 74% specificity. Segregation of AD from dementia with Lewy bodies (DLB;n= 34) was achieved with 90% sensitivity and specificity. Other neurodegenerative diseases, such as frontotemporal dementia (FTD;n= 30), Parkinson’s disease (PD;n= 32), and progressive supranuclear palsy (PSP;n= 31), were included in our cohort for diagnostic purposes. Our method allows for both rapid and robust diagnosis of neurodegeneration and segregation between different dementias.

Objectives: This study examined the physical and mental health of orchestra musicians of different types of orchestras compared to a reference sample of the general population and of two other professions. Methods: Professional musicians (n = 429) from nine opera and/or concert orchestras were surveyed with the Short Form-12 general health questionnaire (SF-12). Data were compared with a reference sample (n = 2805) with a sample of physicians (n = 549) and aircraft manufacturers (n = 822). Results: Compared to the reference sample and the two other professional groups, the musicians had a higher physical health score: 53.07 (SD 5.89) vs 49.03 (SD 9.35) reference, 51.26 (SD 7.53) physicians, and 49.31 (SD 7.99) aircraft manufacturers. The musicians’ mental health score was lower compared to the reference sample but did not differ from the other professional groups: 48.33 (SD 9.52) for musicians vs 52.24 (SD 8.10) reference, 48.26 (SD 10.06) physicians, and 48.54 (SD 9.59) aircraft manufacturers. Physical health but not mental health decreased with age in all groups. In physical and mental health, women scored lower than men. There was no significant difference in physical and mental health scores between musicians of concert and opera orchestras. Age and gender accounted for 3.6% of the variance of the physical health score, but none of the demographic characteristics or orchestral roles and functions was predictive for mental health scores. Conclusions: Musicians report better physical but poorer mental health than the general population, but they did not differ in mental health scores from physicians or aircraft manufacturers.

This chapter focuses on electoral ergonomics, defined as the interface between every aspect of electoral organization and the psychology of the voters. It argues that every small detail in the organization of the vote (electoral ergonomics) matters not only mechanically, but also because of the way it may trigger different psychological mechanisms and emotional reactions, and that as a result, the ergonomic interface will have different effects on different types of voter, such as ‘referees’ or ‘supporters’. Indeed, electoral ergonomics affects the way citizens experience the vote, their attitudes, their likeliness to vote in elections, and their actual electoral choice. The chapter then unpacks the theoretical and empirical logic behind the influence of electoral ergonomics, both in general and through specific case studies. These case studies include the impact of the use of remote voting on electoral experience in the general population, its influence on the electoral choice of young voters, and the effect of ballot-paper design (including paper vs electronic ballots) on the electoral experience.
