Migrant doctors’ narratives about patients

Abstract Narratives of personal and vicarious experience are part and parcel of being a doctor, as doctors routinely (re)interpret and (re)tell patients’ narratives when reflecting on clinical cases. Taking an interest in migrant doctors’ self-initiated narratives about patients in doctor-researcher interviews about cultural transitions, this study examines over thirty hours of audio-recordings of forty semi-structured interviews conducted as part of a collaborative project in Chile and Hong Kong. The study explores how migrant doctors construct their professional ‘self’ through narratives about patients, and how these narratives help migrant doctors legitimise their arguments and professional stance in criticizing cultural and societal attitudes towards health and illness, and the professional practices of local doctors. Finally, the paper reflects on the ways in which migrant doctors’ identity positionings provide space for the creation of a “symbolic territory” in which the practices of migrant doctors co-exist within the boundaries of the practices of local doctors in the host culture.

China resumed its sovereignty over Hong Kong in 1997. Since then drastic changes in this former British colony have occurred. One of these changes is a shift in language policy, from bilingualism (Cantonese and English) to trilingualism (Cantonese, English and Putonghua). The present study is aimed at investigating tertiary students’ use of Cantonese, English and Putonghua on a daily basis, analysing the roles and functions of each language and discussing how these may impact on language policy and language education.Research instruments included 52 students’ language diaries and written analyses, 51 hours of audio-recordings of verbal exchanges, and focus group semi-structured interviews. Results show that the students’ speech repertoire mainly consists of two languages: Cantonese and English and their various mixes. Cantonese is used to ensure understanding, consolidate solidarity and maintain social cohesion. The English-Cantonese mix has become a more powerful identity marker for educated people in Hong Kong than pure Cantonese. English and its supplement with Cantonese are often used in the domain of education. The majority of students seldom use Putonghua in everyday life, but there is a strong instrumental motivation to learn it. Measures are suggested to facilitate a more successful move from bilingualism to trilingualism.

This article reports a case study of an English as a Second Language (ESL) teacher in Hong Kong who conducted lessons via Zoom during the COVID-19 pandemic. The study focused on the factors influencing her technology integration in synchronous online teaching mode. Using data from classroom recordings, stimulated-recall and semi-structured interviews, this study uncovered how Zoom functioned as a substitute for face-to-face lessons. The findings revealed that although there were fewer interactions between the teacher and her students, teaching in synchronous online mode provided the teacher with opportunities to utilize certain online features to augment methods of checking student understanding. The study identified the teacher’s pedagogical beliefs, the context and professional development as factors that influenced the level of technology integration in her Zoom classes. The study concludes that embracing process-oriented pedagogies may be necessary for a higher level of technology integration among ESL teachers who have adequate professional development opportunities and school support.

In recent years, more and more young people from Hong Kong, Macao and Taiwan have come to universities in Chinese Mainland for higher education. However, due to the differences in political, economic and cultural environment between Hong Kong, Macao and Taiwan regions and Chinese Mainland, these young people have encountered psychological problems such as examination anxiety, interpersonal loneliness and lack of sense of meaning in life due to the related learning and life difficulties in Chinese mainland universities, which affect their growth and success. Many of the students originally thought that smoking was used in a more secluded environment to relieve stress, psychological counseling and energy recovery, but in fact smoking brings more harm. In the creation of a smoke-free campus, a variety of measures are adopted to give full play to the positive emotions of students to promote their mental health, which will help to achieve the creation of a smoke-free campus and share a healthy life on the campus. Based on the survey of a total of 658 undergraduates from Hong Kong, Macao and Taiwan in H University on their positive affect and mental health, the following conclusions are drawn through data analysis: undergraduates from Hong Kong, Macao and Taiwan in Chinese Mainland have positive affect at the upper-middle level, negative affect and overall affect at the lower-middle level; some undergraduates from Hong Kong, Macao and Taiwan in Chinese mainland universities have middle-level mental health status and significant differences in grade and family relationship satisfaction; family relationship satisfaction has an important impact on students' emotional and mental health; the negative emotional performance and mental health of senior students, science students and students with low family relationship satisfaction need more attention from educators. Correlation analysis shows that positive affect is positively correlated with health concerns, energy status, satisfaction and interest in life, mood status, control of emotions and behaviors, relaxation and tension, and total score of mental health (P < 0.01). Regression analysis shows that positive affect has significant positive predictive effects on six factors in mental health, such as "health concerns", "satisfaction and interest in life", "energy", "mental state", "control of emotions and behaviors", "relaxation and tension" and mental health. The enlightenment of this study lies in the suggestions that actions should be taken from the two levels of school education and home-school combination to strengthen the cultivation of positive affect of Hong Kong, Macao and Taiwan undergraduates in Chinese mainland universities, so as to improve their mental health level.

Purpose This paper aims to discover why such a public partnership project had been successful with a non-profit third-party alliance such as a smart city consortium (SCC) promoting smart city development. Design/methodology/approach This descriptive case study is primarily based on analysing data collected from various texts, public statements, media interviews and three semi-structured interviews with key members involved in the Covid-19 dashboard project. Findings The data and analysis reviews that both interpersonal and interorganisational trust, dedication and proactiveness of the leaders at SCC were major contributing factors to why SCC was able to partner with the Hong Kong Government in the Covid-19 dashboard in the first place and that the success was also a direct outcome of effective mass collaborative knowledge management activities. Research limitations/implications The research in leadership attributes and activities in the non-profit alliance has been few and this collaborative partnership between the alliance and the government is an example of the importance of further research in smart city leadership. Practical implications In deploying projects for mass collaboration and knowledge sharing in smart city development (which is multi-disciplinary in nature). there are still many new and evolving organisational practices and leadership matters that many business leaders and city managers can learn from. Social implications Smart city development projects involve the notion of sharing data in an open environment enabled by software and mediating tools. Successful projects such as this Hong Kong Covid-19 dashboard which serves a diverse audience can further promote the importance of an open data policy regime for the benefit of the public. Originality/value This case study covers a highly original and unique case study with the leaders at the SCC and representatives from the Hong Kong Government.

Abstract This paper investigates the Lebanese conversational style in relation to Lebanese cultural values. The study adopts a discourse analysis approach based on interactional sociolinguistic methodology for the analysis of audio-recordings and semi-structured interviews involving Lebanese nationals (multi-active culture) and members of linear-active cultures, in addition to participant observation. Four distinctive linguistic features characterizing the Lebanese conversational style are identified: topic (focus on personal topics and abrupt topic shift), pacing (overlap and fast pace), expressive phonology and intonation, and formulaic language. The findings of this study reveal that the Lebanese have a high-involvement conversational style as a result of their cultural values which reflect those of high-context, multi-active and collectivist cultures. Furthermore, a connection is made between cultural and communicative differences which can account for instances of stereotyping and misunderstandings between members of the two cultural groups.

PurposeThe capacity of landfills will reach saturation in the 2020s. There are more than 50,000 buildings in Hong Kong over 30 years old and which may require extensive refurbishment under the Mandatory Building Scheme. Additionally, most new owners/tenants tend to renovate their premises before moving in. Hence, there is an urgent need in Hong Kong, to explore strategies and measures to enable the development of effective refurbishment and renovation (R&R) waste management for such projects. The objectives of this paper are to investigate the process of R&R for identifying the perceived barriers and thereby the strategies for minimisation and management of R&R waste in Hong Kong.Design/methodology/approachDesktop study, semi-structured interviews, site observations and document reviews were used as the data collection methods to achieve the objectives of this research. Considering the nature and characteristics of the industry structure, the snowball sampling process was deployed for data collection. Thematic analysis and content analysis were used for data analysis. Waste minimisation and management strategies for R&R projects were then discussed and developed by the research team and a focus group meeting was held to validate the research findings. Six strategies were then proposed to the government.FindingsR&R projects contribute 10–20% of the construction and demolition waste. The barriers to recycling of R&R waste can be grouped into six major categories: (1) lack of sorting and storage spaces, (2) high cost, (3) insufficient government supporting policy, (4) complicated recycling processes, (5) immature recycling market and (6) insufficient public education. Also, six strategies are proposed in this study, which include (1) pre-refurbishment audit, (2) development of recycling market, (3) sea reclamation, (4) incineration, (5) government support and (6) education and research.Originality/valueThe strategies and measures proposed in this research could most adequately serve as reference for the government officials, building professionals and academic researchers. Such knowledge would make possible the development of effective strategies and measures for minimising and managing R&R waste.

This paper investigates web browser extensions as an under-researched media object for their capacity for activism. “Activist extensions” disrupt a webpage’s intended use and redirect users’ attention to social issues by modifying textual, visual, or auditory elements of the web user interface. The relevance of the study stems from the ubiquity of the web browser as a communication tool and the potential of browser extensions to counter its power in shaping how web content is delivered to users. Based on the notions of transduction and affordance, the critical vocabulary of the Situationist International, and the conceptualization of platform governance through the provision of infrastructural services, this paper asks: Through what mechanism do activist extensions redirect users’ attention to social issues? What are the potential implications for users? And, how can browser platforms affect the creation and distribution of activist extensions? The study adopts a mixed-methods approach that includes discursive interface analysis of the extensions’ modification of the browser interface, critical discourse analysis of user comments on these extensions, and semi-structured interviews with extension developers. Major findings of the study include: 1) the redirection of users’ attention from the webpage to social issues is achieved through the mechanism of $2 , 2) activist extensions function as $2 that provides users with a coping mechanism against certain online rhetoric, and 3) the creation and distribution of activist extensions are conditioned by an $2 imposed by the browser platform on extension developers.

Purpose: There is a growing desire for health professions educators to engage learners in more meaningful instruction. Many have tapped Wikipedia to offer an applied approach to engage learners, particularly as it relates to evidence-based medicine (EBM). However, little is known about the benefits and challenges of using Wikipedia as a pedagogical tool from the collective experience of educators who have sought to improve their instructional practice with it. This study aims to uncover and synthesize the perspectives of health professions education (HPE) instructors who have incorporated Wikipedia in their HPE courses. Methods: Applying a constructivist approach, the authors conducted semi-structured interviews with 17 participating HPE instructors who had substantively integrated Wikipedia into their curriculum. Participants were interviewed about their experiences of integrating Wikipedia editing into their courses. Thematic analysis was conducted on resulting transcripts. Results: Authors observed two broad themes among participants' expressed benefits of teaching with Wikipedia: 1) provides a meaningful instructional alternative that also benefits society and develops learners' information literacy and EBM skills, and 2) supports learners' careers and professional identity formation. Identified challenges included: 1) high effort and time, 2) issues with sourcing references, and 3) challenging interactions with skeptics, editors, and students. Discussion: Findings build on known benefits, such as providing a real-world collaborative project that contextualizes students' learning experiences. They also echo known challenges, such as the resource-intensive nature of teaching with Wikipedia. At the same time, findings extend the current literature by revealing a potential opportunity to approach crowd-sourced information tools, like Wikipedia, as a vehicle to engage and enculturate HPE students within a situated learning context. These findings present implications for HPE programs considering implementing Wikipedia and faculty development needed to help instructors harness crowd-sourced information tools' pedagogical opportunities as well as anticipate their challenges.

Physicians have been subject to increasing external control to improve their medical practice, and scholars have theorized extensively about their opposition to such control. However, little empirical attention has been paid to the views and reasoning that lie behind this opposition. An in-depth understanding is necessary for enhancing the effectiveness and efficiency of external controls, and continuous deep sedation until death (CDS) is an interesting case in this regard. This study aims to explore how physicians frame control measures for CDS. We conducted 47 semi-structured interviews with Belgian physicians in 2019. A qualitative framing analysis was performed to analyze their views and reasoning. This study reveals that physicians approach CDS practice and control measures with different emphases. Controlling by mechanisms of professional self-regulation and state governance are put forward as appropriate means to improve CDS practice. Policymakers should take into consideration physicians’ frames to develop sound control measures.
