Vigorous physical activity and cognitive trajectory later in life: prospective association and interaction by apolipoprotein E e4 in the Nurses’ Health Study

Abstract Background The apolipoprotein E (APOE) e4 allele is a well-established genetic risk factor of brain ageing. Vigorous physical activity may be particularly important in APOE-e4 carriers, but data have been inconsistent, likely due to differences in the timing of the physical activity assessment, definition of cognitive decline and/or sample size. Methods We prospectively evaluated the association between vigorous physical activity and cognition assessed at least 9 years later, according to APOE-e4 carrier status. Biennially from 1986, Nurses’ Health Study participants reported their leisure-time physical activities. Starting in 1995-2001 and through 2008, participants (aged 70+ years) underwent up to four repeated cognitive telephone assessments (6 tasks averaged together using z-scores). Results Among 7,252 women, latent process mixed models identified three major patterns of cognitive change over 6 years: high-stable, medium-stable, and decline. Taking the high-stable cognitive trajectory as the outcome reference in multinomial logistic regressions, highest tertile of vigorous physical activity (≥5.9 metabolic-equivalent[MET]-hours/week) compared to lowest tertile (≤0.9 MET-hours/week) was significantly associated with subsequent lower likelihood of the medium-stable trajectory in the global score (OR[95%CI]=0.72[0.63,0.82]), verbal memory (OR[95%CI]=0.78[0.68-0.89]) and telephone interview of cognitive status score (OR[95%CI]=0.81[0.70-0.94]). Vigorous physical activity was also associated with lower likelihood of decline in category fluency (OR[95%CI]=0.72[0.56,0.92]). We observed some evidence (p-interaction=0.06 for the global score) that the association was stronger among APOE-e4 carriers than non-carriers (OR[95%CI]=0.60[0.39,0.92] versus 0.82[0.59,1.16]). Conclusion Midlife vigorous physical activity was associated with better cognitive trajectories in women in their seventies, with suggestions of stronger associations among APOE-e4 carriers.

Introduction: Skeletal muscle is the largest organ in the human body and vital to maintaining metabolic homeostasis. Increased skeletal muscle fat infiltration (i.e. myosteatosis) is now recognized as a major risk factor for cardio-metabolic diseases, independent of general obesity. Modifications in lifestyle, such as sleep, to reduce myosteatosis would be of great public health importance. However, studies of this relationship often use subjective data and are lacking in minority populations. The aim of this study was to examine the relation between objectively measured sleep duration and myosteatosis at the calf among African Caribbeans. Methods: Data were collected on men (n=393) and women (n=438) from the Tobago Health Study. Sleep duration and physical activity was collected using a SenseWear Pro Armband (BodyMedia, Inc.). Participants were instructed to wear the armband at all times, except in water, for 4-7 days. Measures of muscle density, intermuscular adipose tissue (IMAT), and area were obtained by peripheral QCT scans of the calf (Stratec XCT-2000). Model covariates included age, sex, BMI, diabetes, alcohol intake, smoking, and moderate to vigorous physical activity (MVPA). Linear regression was used to assess the relationship of sleep duration on skeletal muscle. Results: Mean sleep duration was 5.5 hours/day (Min 2.2, Max 11.6). Overall, participants were aged 58.7 years, had a BMI of 30.2 kg/m 2 , spent an average of 42 min/day in MVPA, and 18% were diabetic. In fully adjusted models, longer sleep duration was associated with smaller muscle area, but greater muscle density and less IMAT (all P<0.03). There was no interaction of sleep and sex on muscle density, area and IMAT (p-value=0.5184; 0.2730; 0.0954). Conclusions: In African Caribbean men and women, longer sleep duration was associated with less myosteatosis, as well as, less muscle area at the calf. Further research is warranted to understand this relationship longitudinally in order to determine how it may inform lifestyle guidelines in the Caribbean.

Lifestyle factors may individually protect against the development of mild cognitive impairment. We investigate the relationships between both self-reported physical activity and measured physical function with cognition in a population of elderly adults, more than half of whom follow vegetarian dietary patterns. Otherwise healthy adults (n = 127, mean age 74.9 ± 7.9 years, 61.3% current vegetarians) were assessed using a comprehensive neuropsychological battery. A principal components analysis derived processing speed, executive function, and memory/language factors. Participants reported current levels of vigorous physical activity on questionnaires, and physical function and mobility were measured with the Physical Performance Test (PPT) and Timed Up and Go (TUG) Test. Generalized linear models estimated β coefficients for cross-sectional associations between cognitive factors and indicators of physical abilities and self-reported physical activity. Better physical function indicated by PPT was associated with higher scores on the processing speed factor (β = 0.21 SDs for each 4.4-point increase in PPT score; p = 0.02). Faster TUG times were also associated with higher processing speed factor scores (β = 0.21 SDs increase for each 2.8 second less TUG time; p = 0.02). Self-reported levels of vigorous physical activity were not associated with any area of cognitive function; the association between PPT, TUG and processing speed was independent of physical activity. Associations between PPT and TUG and processing speed were stronger among participants who followed vegetarian dietary patterns. Better physical function may have an effect on cognition in a context of healthy lifestyles.

Abstract Context Time spent in moderate-to-vigorous physical activity (MVPA), but not in sedentary behavior (SB), is related to cardiometabolic risk among non-Hispanic white youth. Objective Examine associations of SB and MVPA with cardiometabolic risk factors among Hispanic/Latino youth. Design Cross-sectional analysis. Setting Four US communities. Participants Hispanic/Latino youth (N = 1,426) ages 8 to 16 years. Measurements Associations of MVPA and SB, measured using 7-day accelerometer data (independent variables), with markers of glucose and lipid metabolism, inflammation, and endothelial function (dependent variables), were assessed in multivariable linear regression models while adjusting for sociodemographic characteristics and accelerometer wear time. Additional models controlled for obesity measures. Results SB comprised a mean (SD) of 75% (13%) of accelerometer wear time; mean (SD) time of MVPA was 35 min/d (22 min/d). Deleterious levels of high-density lipoprotein–cholesterol (HDL-C), triglycerides, insulin resistance, C-reactive protein, and plasminogen activator inhibitor-1 were associated with lower levels of MVPA and higher levels of SB (all P &lt; 0.05). Associations of MVPA with log-transformed triglyceride concentrations (β per 15-min/d increment, −0.039; SE, 0.018; P = 0.037) and SB with HDL-C (β per 30-min/d increment, −0.63; SE, 0.26; P = 0.018), but not those with other markers, remained significant after adjusting for MVPA or SB and further adjustment for body mass index and waist circumference. Higher SB tertiles were associated with lower soluble receptor for advanced glycation end products in fully adjusted models (P for trend = 0.037). Conclusions Physiological precursors of diabetes and cardiovascular disease were associated with MVPA and SB among US Hispanic/Latino youth, a group that bears a disproportionate burden of metabolic disorders.

Maintenance of recommended physical activity levels is associated with favorable cardiovascular outcomes. Consequently, the American Heart Association (AHA) included physical activity in the new Ideal Cardiovascular Health concept. Although physical activity is known to decline with age, the age-specific probability of transitioning between AHA defined levels (ideal, intermediate, poor) is unclear. This knowledge could inform interventions seeking to increase or maintain physical activity levels across the life-course. We estimated age-specific prevalence of ideal (≥150 min/wk moderate or ≥75 min/wk vigorous or equivalent combination), intermediate (1-149 min/wk moderate or 1-74 min/wk vigorous or equivalent combination) and poor (0 min of moderate and vigorous) physical activity among adult (16+) European Americans (EA) and African Americans (AA) from the 2007-2010 NHANES (n=13,418) and Hispanic/Latino participants (H/L) from the 2008-2011 HCHS/SOL (n=14,291). Prevalence estimation incorporated data from identical NHANES and HCHS/SOL self-report questionnaires. We then used the prevalence estimates to calculate age, race, and sex-specific probabilities of transitioning between levels using Markov-type state-transition models. These models are designed to estimate transition probabilities from cross-sectional data assuming time-constant transitions, and have been updated to accommodate complex sampling procedures. The estimated 5-year probability of maintaining ideal physical activity levels remained high through the fourth decade of life, after which race and sex differences were observed. For example, the estimated 5-year probabilities of transitioning from ideal to intermediate physical activity levels were similar at age 40 for EA and AA men (1.2% [95%CI: 0.3, 2.3] vs. 2.2% [95%CI: 1.0, 3.6]), but by age 65 diverged to 5.3% (95%CI: 3.9, 6.9) vs. 11.6% (95%CI: 8.5, 15.2), respectively. During middle age, the estimated probability of transitioning from intermediate to poor physical activity levels was accelerated, although variation by race was noted. For example, the estimated probability of transitioning from intermediate to poor physical activity levels was <25% for EA, AA and H/L men through age 40, but by age 65 this increased to 32.6% (95%CI: 22.0, 44.1) for EA men and 41.1% (95%CI: 26.1, 58.0) for AA men while remaining low (11.1% (95%CI: 0.3, 24.3) for H/L men. Across all race-sex groups, individuals with poor physical activity levels had a 0% estimated probability of transitioning to ideal or intermediate physical activity levels, regardless of age. These results indicate need for interventions aimed at maintaining ideal physical activity levels beginning in young adulthood, particularly in AAs who transition from ideal levels at earlier ages than other race/ethnic groups. Innovative efforts may be needed for those with poor physical activity.

Introduction: Evidence regarding the associations between accelerometer-measured moderate-vigorous physical activity (MVPA) and cardiovascular health (CVH) indicators among Hispanic/Latino adults are unavailable. Methods: Examined cross-sectional data from 12,008 Hispanic/Latino adults aged 18–74 years participating in the Hispanic Community Health Study/Study of Latinos. Accelerometer-measured MVPA was assessed categorically and dichotomously per 2008 PA guidelines. Adverse and ideal CVH indicators were determined by standard cut-points for blood glucose, total cholesterol, blood pressure, body mass index (BMI), and smoking. A composite of low CV risk, defined as achieving all ideal CVH indicators, was included. Adjusted Poisson regression models and complex survey design methods were used for all analyses. Results: Compared to high MVPA, lower MVPA categories were associated with higher prevalence of all adverse CVH indicators, except hypertension, and with lower prevalence of low CV risk and ideal blood glucose, blood pressure, and BMI. Similarly, non-adherence to PA guidelines was associated with a higher prevalence of diabetes (16%), hypercholesterolemia (9%), obesity (28%), and smoking (9%); and lower prevalence of low CV risk (24%), ideal blood glucose (6%), ideal blood pressure (6%), and ideal BMI (22%). Conclusion: Overall, high accelerometer-measured MVPA and meeting PA guidelines were associated with favorable CVH in Hispanic/Latino adults.

Background:To examine the association between accelerometer-assessed physical activity and visual acuity among a nationally representative sample of adults with evidence of diabetes.Methods:Six hundred seventy adult participants with diabetes (age 20 to 85) from the 2003–2006 NHANES cycles constituted the analyzed sample. Participants wore an accelerometer for 7 days to quantify time spent in sedentary behavior, light-intensity physical activity and moderate-to-vigorous physical activity. Visual acuity was objectively assessed for each eye.Results:In multivariable models, every 1-hour increment in daily sedentary behavior was associated with 23% greater likelihood (OR = 1.23; 95% CI: 1.01–1.52) of having uncorrected refractive error as opposed to normal sight. Performing more than 5 minutes of daily moderate-to-vigorous physical activity was associated with a 82% lower likelihood of having vision impairment as opposed to normal sight (OR = 0.18; 95% CI: 0.06–0.50) while every 1-hour increment in daily light-intensity physical activity was, after adjustments, independently associated with a 38% lower likelihood of vision impairment (OR = 0.62; 95% CI: 0.42–0.92).Conclusion:People with diabetes spending more time in sedentary behavior and less time performing light or moderate-to-vigorous physical activity are more likely to have poorer vision.

Introduction: Sedentary behavior is associated with cardiometabolic risk independent of physical activity, but the underlying mechanisms are not fully understood. Hypothesis: Sedentary behavior may influence serum metabolite profiles which are associated with cardiometabolic risk factors. Methods: The analyses included 2711 participants (43% men) of the HCHS/SOL aged 18 to 74 years, free of CVD and other major chronic diseases. Serum metabolomics was performed using an untargeted approach. Physical activity and sedentary behavior were measured by accelerometer (Actical) for a 7-day period. Survey linear regressions were used to examine associations between sedentary behavior and metabolites. Metabolite scores were calculated by summing up z-scores of significant metabolites within the same sub-pathway. Results: Of 624 known metabolites, 95 were significantly associated with sedentary behavior ( P <0.05 after FDR correction), adjusting for socioeconomic and lifestyle factors including moderate to vigorous physical activity ( Figure 1A ). The majority of significant metabolites were clustered into 7 sub-pathways. Three sub-pathway scores (branched-chain amino acids [BCAA], acylcarnitine and medium-chain fatty acid metabolism) were positively associated with sedentary behavior, while the other 4 sub-pathway scores (acylcholine, plasmalogen, nicotinate, and arginine) were inversely associated with sedentary behavior. Among these scores, the BCAA score showed unfavorable correlations with all cardiometabolic traits including obesity measures, glycemic traits, blood lipids and blood pressure. The acylcholine score showed favorable correlations with obesity measures and glycemic traits, though positive correlations with blood lipids were observed ( Figure 1B) . Conclusions: Our study identified multiple serum metabolites associated with sedentary behavior independent of physical activity, suggesting potential mechanistic links between sedentary behavior and cardiometabolic diseases.
