Evaluation of a Field-Ready Neurofunctional Assessment Tool for Use in a Military Environment

ABSTRACT Introduction The Office of Naval Research sponsored the Blast Load Assessment Sense and Test (BLAST) program to develop a rapid, in-field solution that could be used by team leaders, commanders, and medical personnel to provide a standardized approach to operationally relevant monitoring and analysis of service members exposed to single or repeated low-level blast. A critical piece of the BLAST team’s solution was the development of the Brain Gauge technology which includes a cognitive assessment device that measures neurofunctional changes by testing sensory perceptions and a suite of mathematical algorithms that analyze the results of the test. The most recent versions of the technology are easily portable; the device is in the size and shape of a computer mouse. Tests can be administered in a matter of minutes and do not require oversight by a clinician, making Brain Gauge an excellent choice for field use. This paper describes the theoretical underpinnings and performance of a fieldable Brain Gauge technology for use with military populations. Materials and Methods The methods used by the Brain Gauge have been documented in over 80 peer-reviewed publications. These papers are reviewed, and the utility of the Brain Gauge is described in terms of those publications. Results The Brain Gauge has been demonstrated to be an effective tool for assessing blast-induced neurotrauma and tracking its recovery. Additionally, the method parallels neurophysiological findings of animal models which provide insight into the sensitivity of specific metrics to mechanisms of information processing. Conclusions The overall objective of the work was to provide an efficient tool, or tools, that can be effectively used for (1) determining stand-down criteria when critical levels of blast exposure have been reached and (2) tracking the brain health history until return-to-duty status is achieved. Neurofunctional outcome measures will provide the scientific link between blast sensors and the impact of blast on biological health. This calibration process is strengthened with outcome measures that have a biological basis that are paralleled in animal models. The integrative approach that utilizes the Brain Gauge technology will provide a significant advance for assessing the impact of blast exposure and support rapid, science-based decision-making that will ensure mission success and promote the protection of brain health in service members.

This is a brief review of current evidence for the relationships between physical activity and exercise and the brain and cognition throughout the life span in non-pathological populations. We focus on the effects of both aerobic and resistance training and provide a brief overview of potential neurobiological mechanisms derived from non-human animal models. Whereas research has focused primarily on the benefits of aerobic exercise in youth and young adult populations, there is growing evidence that both aerobic and resistance training are important for maintaining cognitive and brain health in old age. Finally, in these contexts, we point out gaps in the literature and future directions that will help advance the field of exercise neuroscience, including more studies that explicitly examine the effect of exercise type and intensity on cognition, the brain, and clinically significant outcomes. There is also a need for human neuroimaging studies to adopt a more unified multi-modal framework and for greater interaction between human and animal models of exercise effects on brain and cognition across the life span.

Stress is one of the main risk factors that can induce humans to develop disorders such as depression, anxiety, or drug use. One of the main sources of stress is social interaction, which can lead to situations such as bullying at school or at work. In this article we will review the close relationship between exposure to stressful situations and increased cocaine or alcohol use. We will present the main results obtained with animal models, which allow us to study the brain mechanisms involved in the impact of stress on drug use. To conclude, we will detail the main mechanisms that explain the powerful effect of stress on drug use.

While people with multiple sclerosis (PwMS) historically were advised to avoid physical activity to reduce symptoms such as fatigue, they are now encouraged to remain active and to enlist in programs of exercise. However, despite an extensive current literature that exercise not only increases physical well-being but also their cognition and mental health, many PwMS are not meeting recommended levels of exercise. Here, we emphasize the impact and mechanisms of exercise on functional and structural changes to the brain, including improved connectome, neuroprotection, neurogenesis, oligodendrogenesis, and remyelination. We review evidence from animal models of multiple sclerosis (MS) that exercise protects and repairs the brain, and provide supportive data from clinical studies of PwMS. We introduce the concept of MedXercise, where exercise provides a brain milieu particularly conducive for a brain regenerative medication to act upon. The emphasis on exercise improving brain functions and repair should incentivize PwMS to remain physically active.

It has been proven that physical exercise improves cognitive function and memory, has an analgesic and antidepressant effect, and delays the aging of the brain and the development of diseases, including neurodegenerative disorders. There are even attempts to use physical activity in the treatment of mental diseases. The course of most diseases is strictly associated with oxidative stress, which can be prevented or alleviated with regular exercise. It has been proven that physical exercise helps to maintain the oxidant–antioxidant balance. In this review, we present the current knowledge on redox balance in the organism and the consequences of its disruption, while focusing mainly on the brain. Furthermore, we discuss the impact of physical activity on aging and brain diseases, and present current recommendations and directions for further research in this area.

Abstract With growing evidence that the coronavirus directly harms the brain and indirectly harms mental well-being due to social isolation and new, increased stressors, the GCBH recognized the urgent need to inform adults age 50+ about ways to their protect brain health as the pandemic continues. In our latest report, the GCBH describes the known neurological symptoms occurring in the short and long term for adults, providing 10 recommendations to protect brain health and urging research in 11 different areas. Calling for an all-of-society approach to protect the brain health of everyone, the GCBH described the negative effects of COVID-19 on people living with Alzheimer’s disease and other dementias and to the impact of health care inequalities. For example, people with dementia were twice as likely to catch the virus as those without dementia; African Americans with dementia had nearly three times the risk of COVID-19 as Caucasians with dementia. The GCBH also points out that caregivers for those living with dementia have experienced particular stress and provided resources and guidance. The Council spotlights the disproportionate toll of COVID-19 on the vulnerable, including racial and ethnic minorities and those living in low- to middle-income countries. After attending this session, participants will be able to identify the neurological impacts of COVID-19, understand the various ways to mitigate risks to brain health, and learn which areas of research will be critical in the future. These recommendations were developed and put forth by the Global Council on Brain Health Governance Committee and Issue Experts.

Sleep plays a crucial role in maintaining brain health. Insufficient sleep leads to an enhanced permeability of the blood–brain barrier and the development of diseases of small cerebral vessels. In this study, we discuss the possibility of detecting changes in the electrical activity of the brain associated with sleep deficit, using an extended detrended fluctuation analysis (EDFA). We apply this approach to electroencephalograms (EEG) in mice to identify signs of changes that can be caused by short-term sleep deprivation (SD). Although the SD effect is usually subject-dependent, analysis of a group of animals shows the appearance of a pronounced decrease in EDFA scaling exponents, describing power-law correlations and the impact of nonstationarity as a fairly typical response. Using EDFA, we revealed an SD effect in 9 out of 10 mice (Mann–Whitney test, p<0.05) that outperforms the DFA results (7 out of 10 mice). This tool may be a promising method for quantifying SD-induced pathological changes in the brain.

The Tonsillectomy in children or adults is an intervention commonly encountered in the ENT (Ear Nose and Throat) and Head and Neck surgeon practice. The current tendency is to perform this type of surgery in major ambulatory surgery centers. Two objectives are thus pursued: first of all, the increase of the patient quality of life through the reintegration into the family as quickly as possible and secondly, the expenses associated with continuous hospitalization are reduced. Any tertiary (multidisciplinary) sleep center must ensure the complete diagnosis and treatment (including surgery) of sleep respiratory disorders. Under these conditions the selection of patients and especially the implementation of the specific protocols in order to control the postoperative complications it becomes essential. The present paper describes our experience of tonsillectomy as treatment for selected patients with chronic rhonchopathy (snoring) and mild to moderate obstructive sleep apnoea. It was presented the impact of antibiotics protocols in reducing the main morbid outcomes following tonsillectomy, in our day surgery center. The obtained results can also be a prerequisite for the integrative approach of the patients with sleep apnoea who were recommended surgical treatment. Considering the wide range of therapeutic modalities used in sleep apnoea, each with its specific advantages and disadvantages, more extensive and multicenter studies are needed. Climate change is an important driver of biodiversity loss and extinction of endemic montane species. In China, three endemic Juniperus spp. (Juniperuspingii var. pingii, J.tibetica, and J.komarovii) are threatened and subjected to the risk of extinction. This study aimed to predict the potential distribution of these three Juniperus species under climate change and dispersal scenarios, to identify critical drivers explaining their potential distributions, to assess the extinction risk by estimating the loss percentage in their area of occupancy (AOO), and to identify priority areas for their conservation in China. We used ensemble modeling to evaluate the impact of climate change and project AOO. Our results revealed that the projected AOOs followed a similar trend in the three Juniperus species, which predicted an entire loss of their suitable habitats under both climate and dispersal scenarios. Temperature annual range and isothermality were the most critical key variables explaining the potential distribution of these three Juniperus species; they contribute by 16–56.1% and 20.4–38.3%, respectively. Accounting for the use of different thresholds provides a balanced approach for species distribution models’ applications in conservation assessment when the goal is to assess potential climatic suitability in new geographical areas. Therefore, south Sichuan and north Yunnan could be considered important priority conservation areas for in situ conservation and search for unknown populations of these three Juniperus species.
