Abstract Background and Aims Renal transplant and associated immunosuppression can influence bone volume. The aim of this study was to analyze the relations between bone biopsy data and levels of bone-related molecules [phosphorus (Pi), Calcium (Ca), Magnesium (Mg), parathyroid hormone (PTH), bone alkaline phosphatase (bAP), calcitonin, vitamin D (vitD), alpha-klotho, fibroblast grow factor (FGF) 23, sclerostin], obtained 1-year after transplantation with bone densitometry findings in the same time point in renal transplanted patients. Method We performed a prospective cohort study of a consecutive sample of de novo single renal transplanted patients in our unit. At inclusion, demographic, clinical and transplant-related data were collected, X-ray of the pelvis and hands (Adragão score) and echocardiographic findings were recorded. All patients were submitted to a laboratorial evaluation and a bone biopsy at baseline. Patients were followed for 12 months, after which performed laboratorial evaluation, 2nd bone biopsy, echocardiogram, X-ray of pelvis and hands, bone densitometry (DXA) and non-contrast cardiac CT. For this report we use the information of the 2nd analysis: laboratorial information, bone histology information, as well the densitometry evaluation. Continuous variables were presented as medians and categorical variables as frequencies. Associations between variables were performed using Wilcoxon rank-sum test, Fisher exact test, Kruskal Wallis rank test or Spearman correlation test. Multivariate analysis was performed using linear regression models. STATA software was used and p < 0.05 was considered statistically significant. Results We recruited 84 patients and, at the end of 12 months, we performed a 2nd evaluation in 69 patients. Median age 53 years, 48 men, 53 caucasian (78.8%), median BMI 24.6, median dialysis vintage 55 months. Patients had a median cumulative steroid dose of 5692.5 mg. Analyzing bone biopsies, we found that 28 patients had adynamic bone disease; 6 had hyperparathyroid bone disease; 2 had osteomalacia and 3 other abnormal mineralization; 8 patients presented only with osteoporosis. There was no significant difference between bone volume / total volume pre transplant (18%) and 1 year after transplantation (19%). Using DXA technique, 14 patients were classified has having osteoporosis, and all those had low volume at the bone biopsy. Nevertheless, in 4 patients low bone turnover was also present. The positive predictive value dropped from 100% to 57%, if we add the other abnormalities of bone, in addiction to the volume. DXA exam wasn’t a good tool to detect a normal bone volume, as the negative predicted value dropped from 78% (normal volume, irrespective of turnover and mineralization) to 37% (normal bone biopsy). Nevertheless, overall bone volume assessed by a bone biopsy correlated well with densitometry findings. Conclusion DXA exam isn’t a good tool to identify the bone quality. Nevertheless, once osteoporosis is detected the probability of the patient having low bone volume is high, but we still need a bone biopsy in order to exclude mineralization or turnover deviations.

Abstract Background and Aims Vascular calcification (VC) is associated to morbidity and mortality in chronic kidney patients. However, it remains to clarify the relationship between VC and renal osteodystrophy (ROD) evaluated by bone biopsy in peritoneal dialysis (PD) patients. Method In order to characterize the relationship between VC and ROD in prevalent PD population, we performed tetracycline-labelled bone biopsies with histomorphometric analysis according KDIGO guidelines. Hands and pelvis x-ray were performed to evaluate VC and to calculate Adragão Score. Exclusion criteria: previous kidney transplant, hemodialysis, treatment with agents interfering in bone metabolism (for example bisphosphonates). All patients were treated with biocompatible PD solutions, with calcium concentration of 1.25 mmol/L. Results Thirty-one patients participated in the study. Mean age was 52.32±11.09 years, 16 male, 6 with diabetes mellitus (DM), 22 on manual PD, median time on PD was 12 (3-61) months. Mean calcium, phosphate and PTH were 9.2±0.5 mg/dL, 4.9±1.0 mg/dL and 486.0±230.5 pg/mL, respectively. Most frequent diagnosed ROD pattern was adynamic bone disease (44.4%) followed by hiperparathyroid bone disease (33.3%) and normal bone (14.8%). VC was detected in 29% of patients and mean Adragão score was 1.13. However, 100% of diabetics had VC on x-ray compared to 12% of non-diabetics. Clinical, analytical and histomorphometric essential data is represented in table 1: Also no significant differences were observed between the patients with or without VC in ROD patterns on bone biopsy, glucose load in PD solutions, prescribed phosphate binders namely calcium-based binders, active vitamin D or calcimimetics. In diabetic patients, bone volume tended to be lower (17.26% vs 22.54%; p=0.072) comparing to non-diabetics. Bone formation rate was similar in diabetics and non-diabetics (25.39 vs 27.71µm3/µm2/y) as mineralization lag time (18.7 vs 20.1 days). Conclusion ROD evaluated by bone biopsy and the proportion of PD patients with VC on x-ray were similar to previous reports. However, in the present study a striking difference in VC was detected in diabetics – all diabetic patients had VC demonstrated radiographically. This finding was not explained by differences in calcium, phosphate, PTH levels or other studied factors. So DM could be a strong risk factor to VC in this population. Also, patients with VC were older, with higher sedimentation rate and lower total KT/V. It is possible that diabetic patients represent a different subgroup, particularly prone to lower bone volume and specially VC. They might benefit from higher dialysis dose and aggressive approaches to prevent or retard VC. This remains to be proved in properly designed prospective studies. In conclusion, age, sedimentation rate, total KT/V and presence of diabetes were associated to VC in prevalent PD patients. Diabetics may be a subgroup of PD patients with very high risk of VC.

Abstract Background Preterm infants may be more vulnerable to fractures due to various factors, including metabolic bone disease, but an increased risk of fractures up to the age of 2 is unproven. Objective To compare fracture patterns in premature and full-term children in the first 3 years of life. Materials and methods A retrospective study was conducted. We excluded any child who returned with the same injury, with known metabolic bone disease, with any disease or condition known to reduce bone density, who received any medication known to affect Vitamin D metabolism within 3 months of enrollment or who had fractures post-surgery/resuscitation. Variables such as the number of fractures sustained each year, age of presentation to the Emergency Department and mechanism of injury were compared between the preterm and term groups using statistical analysis (χ2 and Fisher exact test for categorical variables and Student’s t-test for continuous variables). Simple linear regression was performed on the total number of fractures sustained by age 3. Results Forty-four children with fractures were included. Of these, none were born extremely preterm, 24 (55%) were preterm, and 20 (45%) were born at term. Mean gestational ages of the preterm and term groups were 32 weeks 3 days and 39 weeks 6 days, respectively. There were no extremely low birth weight or very low birth weight children. There was no significant difference in the number of fractures sustained yearly, the age of presentation to the Emergency Department or the site of fracture between preterm and term groups. Linear regression showed that the total number of fractures sustained by age 3 years was unrelated to prematurity status, gender or birth weight category. Conclusion No significant difference in fracture number or pattern was identified.

Abstract Background and Aims Chronic kidney disease-mineral and bone disorder (CKD-MBD) is frequent in end-stage renal disease ESRD (ESRD) patients, increasing morbid-mortality. The aim of this study was to analyse the associations between the new CKD-MBD players [alpha-klotho, fibroblast grow factor (FGF) 23, sclerostin] and the usual markers of the disease [parathyroid hormone (PTH), bone alkaline phosphatase (bAP), vitamin D (vitD), phosphorus (Pi), Calcium (Ca) and Magnesium (Mg)], as well echocardiographic findings [left ventricular mass index (LVMI) measured by Devereux formula, valvular calcifications], vascular calcifications and patients outcomes. Method We performed a prospective cohort study of a sample of ESRD patients listed for renal transplant. All patients were submitted to renal transplant and were followed for 12 months. Patient and graft survival were recorded. At inclusion, demographic and clinical data were collected, laboratorial evaluation, bone biopsy and X-ray of the pelvis and hands (Adragão score) were performed. Continuous variables are presented as medians and categorical variables as frequencies. Associations between variables were performed using Wilcoxon rank sum test and Spearman correlation test. STATA software was used and p < 0.05 was considered statistically significant. Results We included 85 patients. Mean age 50.1±12.7 years, 59 men (69.4%), 66 caucasian (77.6%). The median LVMI was 108.5 (92 – 129) g/m2, with 32 patients presenting LVH and 19 valvular calcifications. Median Adragão score was 1 (0 – 2). At the end of 12 months, 4 patients died and 5 had graft failure (non-primary function). Alpha-klotho correlated with bAP (p=0.0006) and marginally with PTH and absence of valvular calcifications (p=0.05). FGF23 correlated with Pi (p<0.001), Ca (p=0.004), PTH (p=0.003), Mg (p=0.002), and inversely with bAP (p=0.003). There was a marginal association with Adragão score (p=0.06). We didn’t found correlations between FGF23 and alpha-klotho or dialysis vintage or echocardiographic characteristics. Sclerostin correlated negatively with bAP (p=0.007) and PTH (p=0.04). The 3rd sclerostin tertile was associated with high scores of vascular calcifications (p=0.02). Lower levels of sclerostin were associated with patient survival at the end of 12 months (p=0.02). Conclusion From these 3 new bone-derived hormones, sclerostin, a bone formation inhibitor, seems to act as a risk factor for vascular calcifications and worse outcomes.

Excavations in the 1990s at the medieval Chapter House of Worcester Cathedral, UK, revealed medieval human skeletal remains, some of which exhibited a distinctive purple coloration. The nature of the colored bone was investigated using solvents for stain extraction, scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDX), X-ray diffraction (XRD), X-ray fluorescence (XRF), plane polarized (PPL) and cross-polarized (XPL) light microscopy, and auto fluorescence (AF) microscopy. Normal bone from the cemetery was used as a control. The color does not arise from a stain soluble in normal organic solvents. EDX and XRD analysis showed no significant difference between purple and normal bone. XRF analysis shows the presence of trace levels of iron, manganese, zinc, and copper in the affected material. This exhibited a pink color in acid phase and a blue color in alkaline phase. These two states were reversible. The alkaline phase gradually changed irreversibly to yellow over time. These data suggest that the coloration is consistent with the presence of high levels of purple acid phosphatase (PAP) enzyme. The presence of trace amounts of iron, manganese, zinc, and copper suggests a plant or fungal origin for the putative PAP, possibly a member of the Aspergillus ficuum species.

Abstract Background As opposed to Staphylococcus. aureus bacteremia, there are no guidelines to recommend repeating blood cultures in Gram-negative bacilli bacteremia (GNB). Several studies have questioned the utility of follow-up blood cultures (FUBCs) in GNB, but the impact of this practice on clinical outcomes is not fully understood. Our aim was to study the practice of obtaining FUBCs in GNB at our institution and to assess it’s impact on clinical outcomes. Methods We conducted a retrospective, single-center study of adult patients, ≥ 18 years of age admitted with GNB between January 2017 and December 2018. We aimed to compare clinical outcomes in those with and without FUBCs. Data collected included demographics, comorbidities, presumed source of bacteremia and need for intensive care unit (ICU) admission. Presence of fever, hypotension /shock and white blood cell (WBC) count on the day of FUBC was recorded. The primary objective was to compare 30-day mortality between the two groups. Secondary objectives were to compare differences in 30-day readmission rate, hospital length of stay (LOS) and duration of antibiotic treatment. Mean and standard deviation were used for continuous variables, frequency and proportion were used for categorical variables. P-value < 0.05 was defined as statistically significant. Results 482 patients were included, and of these, 321 (67%) had FUBCs. 96% of FUBCs were negative and 2.8% had persistent bacteremia. There was no significant difference in 30-day mortality between those with and without FUBCs (2.9% and 2.7% respectively), or in 30-day readmission rate (21.4% and 23.4% respectively). In patients with FUBCs compared to those without FUBCs, hospital LOS was longer (7 days vs 5 days, P < 0.001), and mean duration of antibiotic treatment was longer (14 days vs 11 days, P < 0.001). A higher number of patients with FUBCs needed ICU care compared to those without FUBCs (41.4% and 25.5% respectively, P < 0.001) Microbiology of index blood culture in those with and without FUBCs Outcomes in those with and without FUBCs FUBCs characteristics Conclusion Obtaining FUBCs in GNB had no impact on 30-day mortality or 30-day readmission rate. It was associated with longer LOS and antibiotic duration. Our findings suggest that FUBCs in GNB are low yield and may not be recommended in all patients. Background:Dual energy X-ray absoprtiometry is the reference method to mesure bone mineral density (1). Loss of bone mineral density is significant if it exceeds the least significant change. The threshold value used in general population is 0,03 g/cm2 (2). Patients with obesity are known for having a higher bone mineral density due to metabolism and physiopathology characteristics (3,4).Objectives:The aim of our study was to determine the least significant change in bone densitometry in patients with obesity.Methods:We conducted an interventionnal study in 120 patients with obesity who performed a bone densitometry. We measured twice the bone mineral density at the lumbar spine, the femoral neck and the total hip in the same time (5,6). We determined the least significant change in bone densitometry from each pair of measurements, using the Bland and Altman method. We also determined the least significant change in bone densitometry according to each stage of obesity.Results:The least significant change in bone densitometry in patients with obesity is 0,046g/cm2 at the lumbar spine, 0.069 g/cm2 at the femoral neck and 0.06 g/cm2 at the total hip.Conclusion:The least significant change in bone densitometry in patients with obesity is higher than in general population. The crystal structure of pimecrolimus Form B has been solved and refined using synchrotron X-ray powder diffraction data and optimized using density functional techniques. Pimecrolimus crystallizes in the space group P21 (#4) with a = 15.28864(7), b = 13.31111(4), c = 10.95529(5) Å, β = 96.1542(3)°, V = 2216.649(9) Å3, and Z = 2. Although there are an intramolecular six-ring hydrogen bond and some larger chain and ring patterns, the crystal structure is dominated by van der Waals interactions. There is a significant difference between the conformation of the Rietveld-refined and the DFT-optimized structures in one portion of the macrocyclic ring. Although weak, intermolecular interactions are apparently important in determining the solid-state conformation. The powder pattern is included in the Powder Diffraction File™ (PDF®) as entry 00-066-1619. This study provides the atomic coordinates to be added to the PDF entry.

Background:Biological and targeted synthetic disease modifying anti-rheumatic agents (bDMARDs) increase the risk of serious infections (SIs), however there is limited ‘real-world’ evidence comparing the relative risk of SI for individual bDMARDs. (1,2)Objectives:This study examines the rates of SIs in a non-select Australian Northern Queensland (NQ) cohort of patients with various rheumatic diseases receiving treatment with a bDMARD, to define predisposing factors and directly compare the bDMARDs.Methods:A retrospective review was performed for all patients who received a bDMARD through the Townsville Hospital Rheumatology Department over the 5-year period between June 2013 and May 2018. Episodes of a SI were defined as infection requiring admission or use of intravenous antibiotics. For each bDMARD the rate of SI per 100 patient years (PYs) was calculated and patient demographics and comorbidities were analysed. Between group differences were assessed using independent samples t-tests or ANOVA. Where assumptions were violated, Mann-Whitney U tests or Kruskal-Wallis tests were used. For categorical variables, chi-square tests were used, except when assumptions were violated when Fisher’s Exact tests were used.Results:296 patients received bDMARDs with an overall SI rate of 11.7/100PYs. There was no significant difference in presence of SI by disease type with 24% of patients with rheumatoid arthritis versus 19% with psoriatic arthritis, 14% with ankylosing spondylitis and 29% with “other” (X2=3.11; df=3; p=0.37). Respiratory tract infections were the most common infection (46%) followed by skin and soft tissue infections (23%). The highest incidence rate of SI occurred with rituximab (29.72 SI/100PYs) followed by certolizumab (22.50 SI/100PYs) and tocilizumab (15.00 SI/100PYs). Duration of time on a bDMARD, disease duration and use of methotrexate or leflunomide were not shown to significantly increase the risk of SI for the entire cohort. The characteristics which were shown to significantly increase SI rates were; prednisone use, increasing age, chronic pulmonary comorbidity and specifically in those with rheumatoid arthritis male gender and total duration of bDMARD use.Conclusion:In this real-world NQ cohort of patients treated with a bDMARD for a rheumatic disease, we have identified a number of factors potentially contributing to the risk of the development of SIs. This study provides valuable data on SI rates in an Australian ‘real-world’ cohort that may assist clinicians’ choice of bDMARD in patients with a high baseline risk of infection and highlights the importance of minimising prednisone use in patients on bDMARDs.References:[1]Ramiro S, Sepriano A, Chatzidionysiou K, et al. This study was undertaken to investigate the integration of titanium micro-implants installed in conjunction with previously dentin-grafted areas and to study the morphological appearance, mineral content, and healing pattern of xenogenic EDTA-conditioned dentin blocks and granules grafted to cavities in the tibial bone of rabbits. Demineralized and non-demineralized dentin blocks and granules from human premolars were implanted into cavities prepared on the lateral aspects of the tibias of rabbits. After a healing period of six months, micro-implants were installed at each surgical site. Histological examinations were carried out after 24 weeks. Characterization of the EDTA-conditioned dentin blocks was performed by means of light microscopy, dental X-rays, scanning electron microscopy, and energy dispersive X-ray analysis (EDX). No implants were found to be integrated in direct contact with the dentin particles or blocks. On the EDTA-conditioned dentin surface, the organic marker elements C and N dominated, as revealed by EDX. The hydroxyapatite constituents Ca and P were almost absent on the dentin surface. No statistically significant difference was observed between the EDTA-conditioned and non-demineralized dentin, as revealed by BIC and BA. The bone-inductive capacity of the dentin material seemed limited, although demineralization by means of EDTA indicated higher BIC and BA values in conjunction with the installed implants in the area. A 12 h EDTA treatment did not fully decalcify the grafts, as revealed by X-ray analysis.
