Firstly, do no harm. This is held as the first law of clinical practice when considering any intervention to improve the health of our patients. This may at times be a difficult proposition, especially when the approach to treating a condition is fraught with risks and can carry the danger of adverse and unwanted side effects. Prescribing therapeutics is the time perhaps when this maxim should be most at the forefront of a clinician’s mind, as therapeutic interven­tions may not cause any immediately discernible danger or harm in the same way as operative interventions. It is therefore important for the prescriber to understand the relevant pharmacodynamics (the effects of the agent on the body) and pharmacokinetics (the effects of the body on the agent). To add a further layer of complexity, the reader should understand that pharmacological sciences are possibly the fastest evolving part of medicine. It would be a fair bet to say that, within the course of the reader’s undergraduate education, entire new classes of therapeutics will have emerged and established perceptions of other agents would have significantly changed. Practically speaking, this does not mean that it is necessary to mem­orize the nuances of all therapeutic agents (although you should have a good grasp of those you prescribe regularly); rather it is more important that a clinician understands how to recognize potential dangers and then be resourceful enough to mitigate against them, given the best know­ledge available at the time. Access to an up- to-date formulary and the will to use it are the surest way to navigate any prescribing pitfalls. ‘We don’t rise to the occasion, we fall to the most basic level of our training’. Thankfully, medical emergencies occur infrequently in the general practice setting. It is the rarity of such events that often leads to anxieties when dealing with them. This reaction is amplified by the caregiver’s nat­ural instinct to do something immediately, but often not knowing exactly what to do because the diagnosis is not immediately clear. The ABCDE approach, as advocated by the Resuscitation Council, is a safe and methodical way to approach any emergency. ABCDE is not only a hierarchy of importance for systems critical to life, but it also acts as an aide- memoire to undertake examinations and interventions when necessary. Most importantly, it buys time whilst the diagnosis is found or declares itself, without adversely affecting the outcome by inaction. Background: Biocompatible polymers are gaining great interest in the field of biomedical applications. The term biocompatibility refers to the suitability of a polymer to body and body fluids exposure. Biocompatible polymers are both synthetic (man-made) and natural and aid in the close vicinity of a living system or work in intimacy with living cells. These are used to gauge, treat, boost, or substitute any tissue, organ or function of the body. A biocompatible polymer improves body functions without altering its normal functioning and triggering allergies or other side effects. It encompasses advances in tissue culture, tissue scaffolds, implantation, artificial grafts, wound fabrication, controlled drug delivery, bone filler material, etc. Objectives: This review provides an insight into the remarkable contribution made by some well-known biopolymers such as polylactic-co-glycolic acid, poly(ε-caprolactone) (PCL), polyLactic Acid, poly(3- hydroxybutyrate-co-3-hydroxyvalerate) (PHBV), Chitosan and Cellulose in the therapeutic measure for many biomedical applications. Methods: : Various techniques and methods have made biopolymers more significant in the biomedical fields such as augmentation (replaced petroleum based polymers), film processing, injection modeling, blow molding techniques, controlled / implantable drug delivery devices, biological grafting, nano technology, tissue engineering etc. Results: The fore mentioned techniques and other advanced techniques have resulted in improved biocompatibility, nontoxicity, renewability, mild processing conditions, health condition, reduced immunological reactions and minimized side effects that would occur if synthetic polymers are used in a host cell. Conclusion: Biopolymers have brought effective and attainable targets in pharmaceutics and therapeutics. There are huge numbers of biopolymers reported in the literature that has been used effectively and extensively.

Background: Fever, is known as pyrexia, may occur due to infection, inflammation, or any tissue damage and disease states. Normally, the infected or damaged tissue initiates the enhanced formation of pro-inflammatory mediators like cytokines which further increases the synthesis of prostaglandin E2 (PgE2) near the hypothalamic area and thereby trigger the hypothalamus to elevate the body temperature. Objective: Antipyretics are the agents which reduce the elevated body temperature. The most commonly used antipyretic agent, paracetamol, may be fatal due to its side effects. Methods: In this review paper, Chemical Abstracts, Google Scholar, PubMed, and Science Direct were the sources for the published article to collect information regarding antipyretic activity. Results: This review compiles the antipyretic plants that may be useful to treat fever due to various diseases. Conclusion: These medicinal plants could be good alternatives for traditional allopathic antipyretics.

Panchakarma therapy is one of the vital branch of Ayurveda, which deals mainly with purification of the provoked Doshas from the body. Basti Chikitsa is regarded as the prime treatment modality among the Panchakarma. It is having not only curative action but also preventive and promotive actions. It is considered as best treatment for Vata Dosha. Yapana Basti is a subtype of Asthapana Basti, which is having the property to support life and promote longevity and widely used in various disorders. Rajayapana Basti is superior amongst all the Yapana Bastis described by Acharya Charaka as it is the king of Yapana. This Rasayana Yapana Basti performs dual function of both Anuvasana and Niruha; hence this is Srotoshodhaka and Brimhana at the sametime. That’s why there is no need to administer separate Anuvasana while giving Yapana Basti. There is an urgent need of standardizing the classical Panchakarma procedures in consideration of the need of today. The dosage schedule, exact procedures, medicaments, effects, and side effects are to be standardized so that uniform procedure of practice should be followed all over nation. Standardization is the need of hour for physicians, to prevent Atiyoga (over activity), Ayoga (less or no activity) and to get adequate effects in a systematic and sophisticated manner within desired time period.

This chapter explores Schopenhauer’s concept of force, which lies at the root of his philosophy. It is force in nature and thus in natural science that is inexplicable and grabs Schopenhauer’s attention. To answer the question of what this inexplicable term is at the root of all causation, Schopenhauer looks to the will within us. Through will, he maintains that we gain immediate insight into forces in nature and hence into the thing in itself at the core of everything and all things. Will is thus Schopenhauer’s attempt to answer the question of the essence of appearance. Yet will, as it turns out, cannot be known immediately as it is subject to time, and the acts of will, which we experience within us, do not correlate immediately with the actions of the body (as Schopenhauer had originally postulated). Hence, the acts of will do not lead to an explanation of force, which is at the root of causation in nature. Schopenhauer sets out to explain what is at the root of all appearances, derived from the question of an original cause, or as Schopenhauer states “the cause of causation,” but cannot determine this essence other than by stating that it is will; a will, however, that cannot be immediately known.

ABSTRACT In March 2020, concurrently with the outbreak of COVID-19 in Iran, the rate of alcohol poisoning was unexpectedly increased in the country. This study has attempted to make an overall description and analysis of this phenomenon by collecting credible data from the field, news, and reports published by the emergency centers and the Iranian Ministry of Health. The investigations showed that in May 20, 2020, more than 6150 people have been affected by methanol poisoning from whom 804 deaths have been reported. A major cause of the increased rate of alcohol poisoning in this period was actually the illusion that alcohol could eliminate the Coronaviruses having entered the body. It is of utmost importance that all mass media try to dismiss the cultural, religious, and political considerations and prepare convincing programs to openly discuss the side-effects of forged alcohol consumption with the public, especially with the youth. It must be clearly specified that “consuming alcohol cannot help prevent COVID-19.”

Introduction: Little is known about prehospital availability and use of medications to treat patients from hazardous materials (hazmat) medical emergencies. The aim of this study was to identify the availability and frequency of use of medications for patients in hazmat incidents by paramedics with advanced training to care for these patients.Methods: A prospectively validated survey was distributed to United States paramedics with advanced training in the medical management of patients from hazmat incidents who successfully completed a 16-hour Advanced Hazmat Life Support (AHLS) Provider Course from 1999 to 2017. The survey questioned hazmat medication availability, storage, and frequency of use. Hazmat medications were considered to have been used if administered anytime within the past 5 years. For analyses, medications were grouped into those with hazmat indications only and those with multiple indications.Results: The survey email was opened by 911 course participants and 784 of these completed the survey (86.1 percent). Of these 784 respondents, 279 (35.6 percent) reported carrying dedicated hazmat medication kits, ie, tox-boxes, and 505 (64.4 percent) did not carry tox-boxes. For those medications specifically for hazmat use, hydroxocobalamin was most commonly available, either within or not within a dedicated tox-box. Of the 784 respondents, 313 (39.9 percent) reported carrying hydroxocobalamin and 69 (8.8 percent) reported administering it within the past 5 years. For medications with multiple indications, availability and use varied: for example, of the 784 respondents, albuterol was available to 699 (89.2 percent) and used by 572 (73.0 percent), while calcium gluconate was available to 247 (31.5 percent) and used by 80 (10.2 percent) within the last 5 years.Conclusion: Paramedics with advanced training in the medical management of patients in hazmat incidents reported limited availability and use of medications to treat patients in hazmat incidents.

The article presents data from the Russian and foreign literature of the properties of melatonin, its role in the vital activity of the body and the regulation of circadian rhythms. The features of using the drug in desynchronosis, the optimal dosage and time of administration, as well as the possibility of its use in combination with non-pharmacological treatment methods are discussed. These methods include dosed light exposure and exercise. Additionally, new studies are considered, indicating the beneficial effect of melatonin and its agonists on the immune, cardiovascular, endocrine and nervous systems. Based on the collected data, it is asserted that melatonin can significantly accelerate the process of adaptation to a new time zone without the development of side effects. Therefore, this drug is the most effective and safe treatment for desynchronosis. It is also important to note that the study of the multifaceted effects of melatonin on body functions opens up great prospects in the treatment of many diseases.

Clinical holistic medicine (CHM) is short-term psychodynamic psychotherapy (STPP) complemented with bodywork and philosophical exercises, to be more efficient in treating patients with severe mental and physical illness. STPP has already been found superior to psychiatric treatment as usual (TAU) and thus able to compete with psychiatric standard treatment as the treatment of choice for all non-organic mental illnesses; we have found the addition of bodywork and philosophy of life to STPP to accelerate the process of existential healing and recovery (salutogenesis). In this paper we compare the side effects, suicidal risk, problems from implanted memory and implanted philosophy of CHM with psychopharmacological treatment. Method: Qualitative and quantitative comparative review. Results: In all aspects of risks, harmfulness, and side effects, we have been considering, CHM was superior to the standard psychiatric treatment. The old principle of “first do no harm“ is well respected by CHM, but not always by standard psychiatry. CHM seems to be able to heal the patient, while psychopharmacological drugs can turn the patient into a chronic, mentally ill patient for life. Based on the available data CHM seems another alternative to patients with mental illness. There seem to be no documentation at all for CHM being dangerous, harmful, having side effects of putting patients at risk for suicide. As CHM uses spontaneous regression there is no danger for the patient developing psychosis as, according to some experts, has been seen with earlier intensive psychodynamic methods. CHM is an efficient, safe and affordable cure for a broad range of mental illnesses.

This discussion of a review paper on endodontic materials focuses on the need for standardization of this group of materials, including clinical and biological investigations. It was concluded that the search for endodontic materials that are both adhesive to dentin and insoluble must continue. When such materials become available, the defense mechanism of the body would take care of the healing per se. This approach as well as techniques and materials which induce dentin bridge formation in the apical area were considered feasible for improving endodontic treatment. The need to report side-effects of endodontic materials was stressed, and it was suggested that such reports should be mandatory for clinicians and for manufacturers of endodontic materials.
