Assessing the Integration of Health Center and Community Emergency Preparedness and Response Planning

ABSTRACTBackground: To assess the state of health center integration into community preparedness, we undertook a national study of linkages between health centers and the emergency preparedness and response planning initiatives in their communities. The key objectives of this project were to gain a better understanding of existing linkages in a nationally representative sample of health centers, and identify health center demographic and experience factors that were associated with strong linkages.Methods: The objectives of the study were to gain a baseline understanding of existing health center linkages to community emergency preparedness and response systems and to identify factors that were associated with strong linkages. A 60-item questionnaire was mailed to the population of health centers supported by the Health Resources and Services Administration’s Bureau of Primary Health Care in February 2005. Results were aggregated and a chi square analysis identified factors associated with stronger linkages.Results: Overall performance on study-defined indicators of strong linkages was low: 34% had completed a hazard vulnerability analysis in collaboration with the community emergency management agency, 30% had their role documented in the community plan, and 24% participated in community-wide exercises. Stronger linkages were associated with experience responding to a disaster and a perception of high risk for experiencing a disaster.Conclusions: The potential for health centers to participate in an integrated response is not fully realized, and their absence from community-based planning leaves an already vulnerable population at greater risk. Community planners should be encouraged to include health centers in planning and response and centers should receive more targeted resources for community integration. ABSTRAK   Pneumonia masih merupakan masalah kesehatan yang penting karena menyebabkan kematian bayi dan balita yang cukup tinggi yaitu kira-kira satu dari empat kematian yang terjadi. Tujuan penelitian ini adalah diketahuinya hubungan antara umur balita dan pengetahuan ibu dengan kejadian pneumonia pada balita di Puskesmas Cambai tahun 2016. Penelitian ini menggunakan metode Survey Analitik dengan pendekatan Cross Sectional. Populasi pada penelitian ini adalah semua ibu yang membawa atau memeriksakan balitanya yang umur < 5 tahun di Puskesmas Cambai tahun 2016, pada saat penelitian. Pengambilan sampel tersebut secara Non Random dengan teknik Accidental Sampling. Analisa data dilakukan secara univariat dan bivariat dengan uji statistik Chi-Square dengan tingkat kemaknaan α = 0,05. Hasil penelitian menunjukkan ada hubungan antara umur balita dan pengetahuan ibu dengan kejadian pneumonia pada balita di Puskesmas Cambai tahun 2016. Dari hasil penelitian ini, peneliti berharap petugas pelayanan kesehatan dapat meningkatkan pelayanan kesehatan bayi dan balita di Puskesmas Cambai , serta lebih sering melaksanakan penyuluhan tentang pneumonia dan penyuluhan mengenai makanan 4 sehat 5 sempurna, agar terciptanya balita sehat.     ABSTRACT Pneumonia is an important health problem because it causes the death of infants and toddlers are quite high, roughly one in four deaths. The purpose of this study is known huubungan between toddler age and maternal knowledge with pneumonia incidence in infants in health centers Cambai 2016. This study used a survey method with the Analytical cross-sectional approach. The population in this study were all mothers who carry their babies or check the age < 5 years at Health Center Cambai in 2016, at the time of the study. The sampling is non- random with accidental sampling technique. Data analysis was done using univariate and bivariate statistical test Chi - Square with significance level α = 0.05. The results of this study showed association between maternal age and knowledge toddlers with pneumonia incidence in infants in Health Center Cambai in 2016. From these results, researchers expect health care workers to improve health services in health centers babies and toddlers Cambai, and more frequently to conduct information about pneumonia and counseling about 4 healthy 5 perfect food , in order to create a healthy toddler.

Abstract Evidence-based decision-making is central to public health. Implementing evidence-informed actions is most challenging during a public health emergency as in an epidemic, when time is limited, scientific uncertainties and political pressures tend to be high, and irrefutable evidence may be lacking. The process of including evidence in public health decision-making and for evidence-informed policy, in preparation, and during public health emergencies, is not systematic and is complicated by many barriers as the absences of shared tools and approaches for evidence-based preparedness and response planning. Many of today's public health crises are also cross-border, and countries need to collaborate in a systematic and standardized way in order to enhance interoperability and to implement coordinated evidence-based response plans. To strengthen the impact of scientific evidence on decision-making for public health emergency preparedness and response, it is necessary to better define mechanisms through which interdisciplinary evidence feeds into decision-making processes during public health emergencies and the context in which these mechanisms operate. As a multidisciplinary, standardized and evidence-based decision-making tool, Health Technology Assessment (HTA) represents and approach that can inform public health emergency preparedness and response planning processes; it can also provide meaningful insights on existing preparedness structures, working as bridge between scientists and decision-makers, easing knowledge transition and translation to ensure that evidence is effectively integrated into decision-making contexts. HTA can address the link between scientific evidence and decision-making in public health emergencies, and overcome the key challenges faced by public health experts when advising decision makers, including strengthening and accelerating knowledge transfer through rapid HTA, improving networking between actors and disciplines. It may allow a 360° perspective, providing a comprehensive view to decision-making in preparation and during public health emergencies. The objective of the workshop is to explore and present how HTA can be used as a shared and systematic evidence-based tool for Public Health Emergency Preparedness and Response, in order to enable stakeholders and decision makers taking actions based on the best available evidence through a process which is systematic and transparent. Key messages There are many barriers and no shared mechanisms to bring evidence in decision-making during public health emergencies. HTA can represent the tool to bring evidence-informed actions in public health emergency preparedness and response.

To better understand existing capacity and help organizations plan for the strategic and expanded uses of data, a project was initiated that deployed contemporary, Hadoop-based, analytic technology into several multi-site community health centers (CHCs) and a primary care association (PCA). An initial data quality exercise was carried out after deployment, in which a number of analytic queries were executed using both the existing electronic health record (EHR) applications and in parallel, the analytic stack. Each organization carried out the EHR analysis using the definitions typically applied for routine reporting. The analysis using the analytic stack was carried out using those common definitions established for the Uniform Data System (UDS) by the Health Resources and Service Administration.  In addition, interviews with health center leadership and staff were completed to understand the context for the findings.The analysis uncovered many challenges and inconsistencies with respect to the definition of core terms (patient, encounter, etc.), data formatting, and missing, incorrect and unavailable data. At a population level, apparent underreporting of a number of diagnoses, specifically obesity and heart disease, was also evident in the results of the data quality exercise, for both the EHR-derived and stack analytic results.Data awareness, that is, an appreciation of the importance of data integrity, data hygiene and the potential uses of data, needs to be prioritized and developed by health centers and other healthcare organizations if analytics are to be used in an effective manner to support strategic objectives. While this analysis was conducted exclusively with community health center organizations, its conclusions and recommendations may be more broadly applicable.

During pregnancy, pregnant women would usually have complaints. Moderated exercise such as pregnancy exercise would make your feet and heels more comfortable. Percentage of antenatal care in health centers of West Navan especially for K1 and K4 already met the target of 95% . The purpose of this studied is analysis of factors associated with the implementation of the health center pregnancy exercise in Navan western region in 2013. This researched used analytic studied with cross -sectional correlation. The population in this study were all pregnant women in the region checkups Navan West Health Center in 2012 as many as 871 pregnant women then took sample of 59 samples using accidental sampling methode. Analyzed used univariate and bivariate analysis with the chi square test. These results it could be concluded that most of the samples have a relatively good knowledge (67.8 %), good attitude (69.5 %) , not working (55.9 %) , support pregnant women during pregnancy (67.8 %) and doing pregnancy exercise (71.2 %). Thechi-square statistical test found no relationship between knowledge , attitude and family support with the implementation of the pregnancy exercise with each value p = 0.005 , p = 0.001 and p = 0.005 then found that there was no relationship between job execution pregnancy exercise with p = 0.362.

Hypertension is a non-communicable disease (PTM) which is a very serious health problem both in the world and in Indonesia. One sign of hypertension is an increase in blood pressure. Hypertension is called the silent killer, because people with hypertension often show no symptoms. The research objective was to determine the relationship between diet and obesity with the incidence of hypertension at the Rawasari Public Health Center in Jambi City in 2019.This study used a cross sectional design, which was carried out in August 2019, 64 hypertension respondents at the Rawasari Health Center were taken using purposive sampling technique. The research variables were diet and obesity  which were analyzed using the chi-square test.This study showed that most respondents had hypertension (92.2%). most of the diet frequently (51.6%). and  nutritional status (50%). Chi-square analysis showed  that there was a relationship between diet and hypertension (p= 0.016) and chi-square analysis showed that there was no relationship between obesity  and hypertension (p= 0.0162). There is no significant relationship between diet and hypertension, and there is no significant relationship between nutritional status and hypertension

The hospital staff need to be competent to utilize the disaster plan to cope up with an emergency situation. Therefore, the present study has aimed to assess the knowledge of hospital staff of Dhahran Al Janoub General Hospital regarding the disaster management during Hazm Storm Support 1436/2015 in Saudi Arabia. The study has employed quantitative research design to assess the disaster management of hospital staff by recruiting 84 individuals (physicians, nurses, technicians, officers, and housekeepers) from Dhahran Al Janoub General Hospital. A questionnaire was given to respondents to gather information about disaster management. The obtained data was analysed using SPSS through chi-square analysis. The study results clearly depicted that the hospital staff with fewer years of experience had lesser knowledge about the disaster assessment as compared to the experienced employees. There was no statistically significant relationship identified between different job categories in the hospital and the level of knowledge about presence or absence of the emergency response plan. However, there was a statistically significant association found between different job professions and level of awareness regarding presence or absence of hospital command centres. The study concluded that the knowledge of emergency preparedness among the hospital staff was moderate and the hospital staff should participate and seek opportunities to prepare assessment for disaster management.

The purpose of the study was to find out the correlation between the parents role and women's marital age in the Religious Affairs Office of Kasihan district. The study used a cross sectional approach and the type of descriptive correlational study. The subjects of the study were 48 brides who were going to get married at the Religious Affairs Office of Kasihan district which were retrieved using quota sampling method. The study used bivariate chi square analysis. The results of the study revealed that most respondents got married at mature ages namely 21 - 34 years old. There is a correlation between parents role and women’s marital age in the Religious Affairs Office of Kasihan district with a significance value of 0,000 (p <0,05) and the value of the correlation coefficient was 0,558. The Religious Affairs Office of Kasihan district should continue to cooperate with Kasihan Health Center to develop the implementation of the bridal class program and reproductive health education for prospective brides.

Electronic medical records (EMRs) are at the forefront of the national healthcare agenda and this paper examines EMR implementation and usage based on data from the 2009 Commonwealth Fund National Survey of Federally Qualified Health Centers (FQHC). Chi-square analysis was used to examine differences in EMR implementation and usage. Logistic regression analysis was used to understand the adjusted associations between EMR implementation and usage. A significant finding of this study was that simple EMRs were implemented in more than half of FQHCs in the Northeast, Southern, and Western regions of the United States and EMRs in more than half of the FQHCs in the Southern and Western regions are not even utilized. These findings indicate simple EMR usage and full EMR implementation need improvement to meet the requirements of the American Recovery and Reinvestment Act by 2014, or face reduction in Medicare and Medicaid reimbursements.
