Abstract Background: To investigate the current status and problems of sepsis guidelines in clinical application among doctors in tertiary hospitals in China and to provide evidence-based case for the formulation, implementation, and training on sepsis diagnosis and treatment.Methods: Physicians from tertiary hospitals, mainly in emergency and critical care departments were enrolled into the survey. Participants completed a questionnaire on sepsis score, diagnostic indicators, fluid resuscitation, antibiotics choice, access to knowledge and application training, and the application of sepsis guidelines in clinical diagnosis and treatment. Results: Of 661 questionnaires that were distributed, 625 were returned. A total of 432 (69.12%) respondents had junior and intermediate professional titles and 193 (30.88%) had senior titles. Among them, 291 (46.56%) were self-taught, 148 (23.68%) were attending lectures, 153 (24.48%) were assigned ward rounds by senior doctors, 9 (30.88%) were taught in other ways, and 24 (3.84%) had no training. To learn about sepsis guidelines, 36.64% of respondents preferred attending lectures, 34.72% chose to study by themselves, and 27.04% preferred listening to senior doctors. Regarding the best way to learn about sepsis guidelines, 42.08% preferred to read by themselves, 41.76% preferred attending lectures, and 14.72% preferred listening to senior doctors. For formulation and implementation of sepsis diagnosis and treatment strategy, >90% of the respondents thought that they needed to rapidly give patients large volumes of fluid replacement but were not satisfied, while 89.6% doctors agreed that rehydration should follow the principle of “use with vasoactive drugs, not too much”. Of the respondents, 61.92% preferred empirical drugs in the use of sepsis antibiotics, 34.56% preferred high-level antibiotics with a wide coverage strategy, 2.4% would choose following instructions by senior doctors, and 0.16% did not know how to choose. Over 70% of the doctors surveyed thought that self-study guide, attending lectures, and senior doctors’ ward rounds were the most important factors affecting drug use, while 47.68% said that the information found on the internet was an important way of guiding drug use.Conclusion: Although the prevalence of sepsis guidelines is high among doctors in tertiary Chinese hospitals, homogeneity of clinical application is poor and greatly affects diagnosis and treatment. Thus, standardized training for sepsis diagnosis and treatment is needed.

UNSTRUCTURED Background: Since December 2019, Coronavirus Disease 2019 (COVID-19) emerged in Wuhan city and rapidly spread throughout China. Facing this kind of public health emergency, an efficient, fast and group communication method is needed. Method: As a director of the department Pulmonary and Critical Care Medicine in a tertiary hospitals, which is the only designated one for diagnosis and treatment of COVID-19 in a medium-sized city, I analyzed and summarized the “group function” of WeChat (Weixin, micro-message) App in working about COVID-19. Results: By February 16, 2020, we have completed 1,526 citywide consultations and treatment of 322 inpatients, including 97 patients diagnosed with COVID-19, with the help of 12 WeChat groups by handy. The advantages of WeChat group are as follows: 1. Work efficiency can be improved greatly, saving labor costs. 2. Accurate and intuitive information can be gotten fast and timely, avoiding close contacting with COVID-19 patients. 3. Data and message in WeChat groups can be saved, arranged and reviewed at any time. Conclusions: The “group function” in WeChat App plays a greater role in the public health emergent work about management, diagnosis and treatment of COVID-19.

Abstract OBJECTIVE: To assess the current status of disease-related knowledge and to analyze the relationship among the general condition, illness perception, and psychological status of patients with COVID-19. METHODS: A hospital-based cross-sectional study was conducted on 118 patients using convenience sampling. The general questionnaire, disease-related knowledge questionnaire of COVID-19, Revised Illness Perception Questionnaire (IPQ-R), and Profile of Mood States (POMS) were used to measure the current status of participants. RESULTS: The overall average score of the disease-related knowledge of patients with COVID-19 was (79.19 ± 14.25), the self-care situation was positively correlated with knowledge of prevention and control (r=0.265, P=0.004) and total score of disease-related knowledge (r= 0.206, P= 0.025); the degree of anxiety was negatively correlated with the knowledge of diagnosis and treatment (r= -0.182, P= 0.049). The score of disease-related knowledge was negatively correlated with negative cognition (volatility, consequences, emotional statements) and negative emotions (tension, fatigue, depression) (P<0.05); positively correlated with positive cognition (disease coherence) and positive emotion (self-esteem) (P<0.05). CONCLUSION It was recommended that we should pay more attention to the elderly and low-income groups, and increase the knowledge about diagnosis and treatment of COVID-19 and self-care in the future health education for patients.

The prognosis of patients with solid tumours has remarkably improved with the development of molecular-targeted drugs and immune checkpoint inhibitors. However, the improvements in the prognosis of pancreatic cancer and biliary tract cancer is delayed compared to other carcinomas, and the 5-year survival rates of distal-stage disease are approximately 10 and 20%, respectively. However, a comprehensive analysis of tumour cells using The Cancer Genome Atlas (TCGA) project has led to the identification of various driver mutations. Evidently, few mutations exist across organs, and basket trials targeting driver mutations regardless of the primary organ are being actively conducted. Such basket trials not only focus on the gate keeper-type oncogene mutations, such as HER2 and BRAF, but also focus on the caretaker-type tumour suppressor genes, such as BRCA1/2, mismatch repair-related genes, which cause hereditary cancer syndrome. As oncogene panel testing is a vital approach in routine practice, clinicians should devise a strategy for improved understanding of the cancer genome. Here, the gene mutation profiles of pancreatic cancer and biliary tract cancer have been outlined and the current status of tumour-agnostic therapy in these cancers has been reported.

AbstractExosomes are single-membrane, secreted organelles with a diameter of 30–200 nm, containing diverse bioactive constituents, including DNAs, RNAs, proteins, and lipids, with prominent molecular heterogeneity. Extensive studies indicate that exosomal RNAs (e.g., microRNAs, long non-coding RNAs, and circular RNAs) can interact with many types of cancers, associated with several hallmark features like tumor growth, metastasis, and resistance to therapy. Pancreatic cancer (PaCa) is among the most lethal cancers worldwide, emerging as the seventh foremost cause of cancer-related death in both sexes. Hence, revealing the specific pathogenesis and improving the clinical diagnosis and treatment process are urgently required. As the study of exosomes has become an active area of research, the functional connections between exosomes and PaCa have been deeply investigated. Among these, exosomal RNAs seem to play a significant role in the development, diagnosis, and treatment of PaCa. Exosomal RNAs delivery ultimately modulates the various features of PaCa, and many scholars have interpreted how exosomal RNAs contribute to the proliferation, angiogenesis, migration, invasion, metastasis, immune escape, and drug resistance in PaCa. Besides, recent studies emphasize that exosomal RNAs may serve as diagnostic and prognostic biomarkers or therapeutic targets for PaCa. In this review, we will introduce these recent insights focusing on the discoveries of the relationship between exosomal RNAs and PaCa, and the potentially diagnostic and therapeutic applications of exosomes in PaCa.

Pancreatic ductal adenocarcinoma (PDAC) exhibits the poorest prognosis of all solid tumors, with a 5-year survival of less than 10%. To improve the prognosis, it is necessary to advance in the development of tools that help us in the early diagnosis, treatment selection, disease monitoring, evaluation of the response and prognosis. Liquid biopsy (LB), in its different modalities, represents a particularly interesting tool for these purposes, since it is a minimally invasive and risk-free procedure that can detect both the presence of genetic material from the tumor and circulating tumor cells (CTCs) in the blood and therefore distantly reflect the global status of the disease. In this work we review the current status of the main LB modalities (ctDNA, exosomes, CTCs and cfRNAs) for detecting and monitoring PDAC.
