The relevance of the emergence of innovative technologies, more sophisticated diagnostics methods, opportunities for intensive therapy, antibacterial prophylaxis, surgical methods of treatment involving minimally invasive surgery, does not solve the problem of high overall mortality in severe acute pancreatitis at this time. This indicator for the last ten years remains at a high level (10-30%), and in case of inflammatory forms of severe pancreatitis, reaches 85%. Traditional laboratory criteria for the diagnosis of pancreatic necrosis may involve leukocytosis with the destruction of the leukocyte formula and appearance of myelocytes and metamylocytes in it; improvement of leukocyte index of intoxication; improvement of the nuclear index of the destruction; blood glucose growth; growth of blood urea; growth of neutrophil-lymphocytic coefficient; change in endogenous creatinine clearance; improvement of blood transaminases. The indicator enzymes have the greatest value for the diagnosis of gastrointestinal pancreatitis in the diagnostic plan. The determining of α-amylase activity is a sensitive and valuable criterion for diagnostics of gastrointestinal pancreatitis and control over the effectiveness of treatment. Also, talking about laboratory diagnostics of acute pancreatitis it is worth noting about acute-phase proteins. The increase in the level of acute-phase proteins in inflammation is a compensatory reaction associated with the ability to inhibit cells released during destruction and cause secondary tissue damage by proteolytic enzymes, as well as to suppress autoimmune aggression. Due to this, the inflammatory process in the body is limited and autoaggression is restrained. The purpose of the study was to study the proteins of the acute phase in the blood of patients with acute pancreatitis of varying severity. Materials and methods. The study is based on the results of examination of 88 patients with acute pancreatitis of varying severity who were treated at the V.T. Zaytsev Institute of the General and Urgent Surgery of Academy of Medical Sciences of Ukraine, Kharkiv, as well as 45 donors (control group). Results and discussion. The determination of the activity of serum enzymes in the dynamics of acute destructive pancreatitis was carried out. The parameters were verified before the operation and after the operative treatment in dynamics, as well as before and after the second operation in the dynamics. Markers of tissue destruction can simultaneously serve as indicators of the severity of the inflammatory process in the pancreas and determine the quality of the performed surgical intervention. The diagnostic value of ferritin and lactofferin as markers of an acute destructive process was proved on the basis of a significant increase in their level in blood serum and exudates of patients. It has been shown that ferroproteins are reliable markers of the inflammatory process, among others, including classical acute phase proteins. At the same time, their identification is distinguished by its explicitness in the diagnosis of not only latent destructive states. Conclusion. The detected changes in the concentration of clinical and laboratory parameters of alpha-2-macroglobulin, lactoferrin, C-reactive protein, ferritin in the blood serum correlate with the degree of the inflammatory process and purulent-destructive changes in the pancreas

Acute pancreatitis (AP) is an acute inflammation of the pancreas, usually occurs suddenly with a variety of clinical symptoms, complications of multiple organ failure and high mortality rates. Objectives: To determine the value of combination of HAP score and BISAP score in predicting the severity of acute pancreatitis of the Atlanta 2012 Classification. Patients and Methods: 75 patients of acute pancreatitis hospitalized at Hue Central Hospital between March 2017 and July 2018; HAP and BISHAP score is calculated within the first 24 hours. The severity of AP was classified by the revised Atlanta criteria 2012. Results: When combining the HAP and BISAP scores in predicting the severity of acute pancreatitis, the area under the ROC curve was 0,923 with sensitivity value was 66.7%, specificity value was 97.1%; positive predictive value was 66.7%, negative predictive value was 97.1%. Conclusion: The combination of HAP and BISAP scores increased the sensitivity, predictive value, and prognostic value in predicting the severity of acute pancreatitis of the revised Atlanta 2012 classification in compare to each single scores. Abstract Background Analyses of blood biomarkers involved in the host response to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral infection can reveal distinct biological pathways and inform development and testing of therapeutics for COVID-19. Our objective was to evaluate host endothelial, epithelial and inflammatory biomarkers in COVID-19. Methods We prospectively enrolled 171 ICU patients, including 78 (46%) patients positive and 93 (54%) negative for SARS-CoV-2 infection from April to September, 2020. We compared 22 plasma biomarkers in blood collected within 24 h and 3 days after ICU admission. Results In critically ill COVID-19 and non-COVID-19 patients, the most common ICU admission diagnoses were respiratory failure or pneumonia, followed by sepsis and other diagnoses. Similar proportions of patients in both groups received invasive mechanical ventilation at the time of study enrollment. COVID-19 and non-COVID-19 patients had similar rates of acute respiratory distress syndrome, severe acute kidney injury, and in-hospital mortality. While concentrations of interleukin 6 and 8 were not different between groups, markers of epithelial cell injury (soluble receptor for advanced glycation end products, sRAGE) and acute phase proteins (serum amyloid A, SAA) were significantly higher in COVID-19 compared to non-COVID-19, adjusting for demographics and APACHE III scores. In contrast, angiopoietin 2:1 (Ang-2:1 ratio) and soluble tumor necrosis factor receptor 1 (sTNFR-1), markers of endothelial dysfunction and inflammation, were significantly lower in COVID-19 (p < 0.002). Ang-2:1 ratio and SAA were associated with mortality only in non-COVID-19 patients. Conclusions These studies demonstrate that, unlike other well-studied causes of critical illness, endothelial dysfunction may not be characteristic of severe COVID-19 early after ICU admission. Pathways resulting in elaboration of acute phase proteins and inducing epithelial cell injury may be promising targets for therapeutics in COVID-19.

This study’s objective was to evaluate the effects of distance and stocking density on physicochemical properties and oxidative stability of meat and acute-phase proteins in Brahman crossbred cattle transported by road under hot and humid tropical conditions. Sixty Brahman crossbred heifers were subjected to road transport from a cattle feedlot farm located in Universiti Putra Malaysia (UPM), Serdang, to a commercial ruminant abattoir in Shah Alam, Selangor. Animals were assigned to long and short distances and high, medium, and low stocking densities. The results revealed that the intensity of response significantly increased in meat samples from animals subjected to long-distance transportation and higher stocking density. Alpha-1-acid glycoprotein and serum amyloid-A values increased considerably and were different from the baseline values recorded at preload. In conclusion, the current results revealed that the color, pH, shear force values, water holding capacity (WHC), glycogen level, and malondilaldehyde assay (MDA) concentrations in meat and acute-phase proteins (APP) were affected by both distances and stocking densities, as evidenced by the significant changes recorded from the parameters above.
