The relevance of the emergence of innovative technologies, more sophisticated diagnostics methods, opportunities for intensive therapy, antibacterial prophylaxis, surgical methods of treatment involving minimally invasive surgery, does not solve the problem of high overall mortality in severe acute pancreatitis at this time. This indicator for the last ten years remains at a high level (10-30%), and in case of inflammatory forms of severe pancreatitis, reaches 85%. Traditional laboratory criteria for the diagnosis of pancreatic necrosis may involve leukocytosis with the destruction of the leukocyte formula and appearance of myelocytes and metamylocytes in it; improvement of leukocyte index of intoxication; improvement of the nuclear index of the destruction; blood glucose growth; growth of blood urea; growth of neutrophil-lymphocytic coefficient; change in endogenous creatinine clearance; improvement of blood transaminases. The indicator enzymes have the greatest value for the diagnosis of gastrointestinal pancreatitis in the diagnostic plan. The determining of α-amylase activity is a sensitive and valuable criterion for diagnostics of gastrointestinal pancreatitis and control over the effectiveness of treatment. Also, talking about laboratory diagnostics of acute pancreatitis it is worth noting about acute-phase proteins. The increase in the level of acute-phase proteins in inflammation is a compensatory reaction associated with the ability to inhibit cells released during destruction and cause secondary tissue damage by proteolytic enzymes, as well as to suppress autoimmune aggression. Due to this, the inflammatory process in the body is limited and autoaggression is restrained. The purpose of the study was to study the proteins of the acute phase in the blood of patients with acute pancreatitis of varying severity. Materials and methods. The study is based on the results of examination of 88 patients with acute pancreatitis of varying severity who were treated at the V.T. Zaytsev Institute of the General and Urgent Surgery of Academy of Medical Sciences of Ukraine, Kharkiv, as well as 45 donors (control group). Results and discussion. The determination of the activity of serum enzymes in the dynamics of acute destructive pancreatitis was carried out. The parameters were verified before the operation and after the operative treatment in dynamics, as well as before and after the second operation in the dynamics. Markers of tissue destruction can simultaneously serve as indicators of the severity of the inflammatory process in the pancreas and determine the quality of the performed surgical intervention. The diagnostic value of ferritin and lactofferin as markers of an acute destructive process was proved on the basis of a significant increase in their level in blood serum and exudates of patients. It has been shown that ferroproteins are reliable markers of the inflammatory process, among others, including classical acute phase proteins. At the same time, their identification is distinguished by its explicitness in the diagnosis of not only latent destructive states. Conclusion. The detected changes in the concentration of clinical and laboratory parameters of alpha-2-macroglobulin, lactoferrin, C-reactive protein, ferritin in the blood serum correlate with the degree of the inflammatory process and purulent-destructive changes in the pancreas

To evaluate possibilities of aspirin-induced gastroduodenopathy treatment in the patients with chronic ischemic heart disease by means of applying the internal endogenous prostaglandins stimulant.  Material and methods. 340 patients suffering from chronic coronary heart disease and receiving a long-term acetylsalicylic acid (ASA) therapy were examined on the base of the cardiovascular care unit of The Nizhny Novgorod Regional Clinical Hospital named after N.A. Semaschko. There were evaluated frequency, nature and severity of the aspirin-induced gastroduodenopathy. The patients with coronary heart disease and aspirin-induced gastroduodenopathy were divided in two groups. In the first group of patients there was applied rebamipide therapy (in a single daily dose 300 mg) in combination with the proton pump inhibitor (PPI) — pantoprazole. In the second group there was applied only pantoprazole therapy. For the purpose of specification of AIG pathogenetic mechanisms development, all the examined chronic coronary heart disease cases were tested on the prostaglandin E2 (PGE2) level in blood serum before the therapy beginning and after the treatment. The control group was formed of chronic coronary heart disease patients showing no AIG evidence. Statistical processing of the received data was fulfilled with the program «Statistika 10.0». Results. AIG was registered in 15% out of 340 chronic coronary heart disease patients. According to the endoscopic examination erosive disease of the body and antrum prevailed among the patients. The PGE2 level in the blood serum was significantly lower (р = 0,00087) in these patients in comparison with the control group. In association with PPI and rebamipide mixed therapy, esophagogastroduodenoscopy results showed no pathological findings in gastrointestinal mucosa and statistically significant (р = 0,00067) blood serum PGE2 level growing in all the treated patients. As a result of exclusive PPI therapy there was marked positive dynamics in endoscopic view in 19 out of 25 patients and a tendency to normalization of PGE2 level in the blood serum. However, PGE2 level growing was insignificant. Conclusion. The presented research demonstrates the possibility of AIG treatment with the use of internal endogenous prostaglandins stimulant — rebamipide in complex with proton pump inhibitor PPI therapy.

The initial inflammatory stages of the colic syndrome include changes known as acute phase response. The aim of this study was to contribute with the establishment of reference values concerning the electrophoretogram of peritoneal liquid from healthy horses and horses submitted to experimentally induced intestinal obstruction. Twenty-one horses were allotted in four groups: duodenal obstruction (DG), ileum obstruction (IG), left-dorsal colon obstruction (MG), and control group (CG). Peritoneal liquid was sampled before obtruction (T0), with 3 hours of obstruction (T3) and 6, 30, 102 and 174 hours after desobstructing (T6, T30, T102 and T174, respectively). Total protein levels were determined by the biuret method and protein fractions were obtained by SDS-PAGE electrophoresis. The acute phase proteins (APP) identified were Immunoglobulin-A, ceruloplasmin, transferrin, albumin, α1-antitrypsin, heavy and light chains of immunoglobulin-G, haptoglobin, α1-acid glycoprotein and a still unnamed protein, which was called P24. There was no difference (P>0.3) in protein levels among groups, although a significant difference (P>0.05) was observed between distinct experimental moments in each group evidencing a higher response of the APP in the obstructed groups. The APP fractioning of the peritoneal liquid was standardized to establish a standard curve for healthy equines and those submitted to induced intestinal obstruction. Moreover, it was verified that the SDS-PAGE electrophoresis was sensitive and effective to help diagnose abdominal inflammatory processes.

In the present study, we have investigated frequency of genotypes and functional alleles of genes encoding chemokines (CXCL12 rs1801157, CCL2 rs1024611), chemokine receptors (CCR5 del32, CX3CR1 rs3732378), acute phase proteins SAA rs1136743, and CD14 rs2569190 polymorphisms among Tatar obese or overweight women from the Republic of Bashkortostan.The group of patients comprised unrelated women with obesity (BMI ≥ 30 kg/m2, n = 225), females with overweight (BMI 25.0-29.9 kg/m2, n = 184), and control group of women (n = 327) BMI < 25.0 kg/m2. Genotyping was performed by PCR-RFLP analysis. Patients and controls differed in such parameters as body weight (p = 0.00001), BMI level (p = 0.001) and fasting glucose level (p = 0.0001).An association was revealed between obesity andAG-AAgenotypes (p = 0.007) andAallele (p = 0.003) of polymorphic locus rs3732378 ofCXCR1gene, as well asTTgenotype (p = 0.027) andTallele (p = 0.021) of polymorphic locus rs1136743 ofSAAgene. It has been shown that theAAgenotype of polymorphic locus rs3732378 of theCX3CR1gene is associated with increased body weight (p = 0.002) and elevated BMI (p = 0.018); theGGgenotype of polymorphic locus rs1024611 of theCCL2gene is associated with elevated fasting glucose level (p = 0.001).As based on clinical and genetic data and using logistic regression, some statistically significant differences were revealed, which allow to predict development of obesity in Tatar women.

Objective. To determine the blood serum 2-microglobulin and 2-macroglobulin concentration in patients undergoing renal replacement therapy (programmed hemodialysis) for the diagnosis of uremic pancreatitis and / or destructive pancreatitis. Materials and methods. The study involved 52 patients admitted to the Surgical Unit of Astrakhan "RZhD-Medicine" Hospital and City Clinical Hospital № 3. The blood serum 2-microglobulin and 2-macroglobulin concentration was analyzed in patients admitted on an emergency basis with suspicion of uremic pancreatitis and destructive pancreatitis, who receive renal replacement therapy (programmed hemodialysis). The control group included 50 outpatients undergoing renal replacement therapy (programmed hemodialysis). The study did not include patients with suspected pancreatitis who were not receiving renal replacement therapy. The period of the study is 20192021. Results. The concentration of blood serum 2-microglobulin is statistically higher than normal in all patients, who had received renal replacement therapy (programmed hemodialysis) in anamnesis. The most statistically high concentration of 2-microglobulin was revealed while studying patients with uremic pancreatitis (n = 34), and was (30.0 2.75 mg/l) compared with the blood serum concentration in patients with destructive pancreatitis (8 0.51 mg / l). The concentration of 2-macroglobulin was statistically lower in destructive pancreatitis (n = 18) and was 615 161 mg/l compared with uremic pancreatitis (980 216 mg/l). In the control group of outpatients (n = 50) receiving renal replacement therapy (programmed hemodialysis), no statistically significant blood serum concentrations of 2-microglobulin and 2-macroglobulin were found. Conclusions. A clear dependence of the concentration of 2-microglobulin and 2-macroglobulin on the severity of uremic pancreatitis and destructive pancreatitis was established. Statistically high values of 2-microglobulin concentrations were obtained in patients with uremic pancreatitis, and the 2-macroglobulin level was statistically low in destructive pancreatitis.

In this study, the concentrations of two acute-phase proteins (APPs), haptoglobin (Hp) and serum amyloid A (SAA), were quantitatively assessed in serum samples from cattle naturally infected with paratuberculosis (PTB). APP profiles were compared across 190 animals classified according to the different pathological forms associated with infection: uninfected (n = 59), with focal lesions (n = 73), multifocal lesions (n = 19), and diffuse paucibacillary (n = 11) and diffuse multibacillary lesions (n = 28). Our results showed a significant increase in both APPs in infected animals compared to the control group, with differences depending on the type of lesion. Hp and SAA levels were increased significantly in all infected animals, except in cows with diffuse multibacillary lesions that showed similar values to non-infected animals. The expression pattern of both APPs was similar and negatively correlated with the antibody levels against PTB. These results indicate that the release of Hp and SAA is related to the presence of PTB lesions associated with a high cell-mediated immune response and a lower bacterial load, suggesting that the pro-inflammatory cytokines that are associated with these forms are the main stimulus for their synthesis. These molecules could show some potential to be used as putative biomarkers of PTB infection, particularly for the identification of subclinical animals showing pathological forms related to latency or resistance to the development of advanced lesions.

The combined action of pesticides is one of the problems of toxicology, because in comparison to individual active substances, their mixtures can increase toxicity and delayed action effects, which is a risk to human health. It is known that plant growth regulators (PGR) when applied together with pesticides help to reduce the quantitative rates of pesticides applied, the accumulation of pesticides and heavy metals in plants, reduce the acute toxicity of xenobiotics. The combined effect of the prolonged admission of PGRs and pesticides is an unexplored area, and it is an important issue in preventive toxicology. The Aim of the Research. Determination of the nature of toxic effects of Chlorpyrifos and “Ivin” combined action at subchronic oral admission. Materials and Methods. Chlorpyrifos and PGR “Ivin” (2.6-dimethyl-N-pyridine oxide) combined action was studied on male Wistar Han rats. Substances were administered orally, via a feeding tube for 13 weeks; Chlorpyrifos was administered in the form of an aqueous emulsion with an auxiliary material OP-10 at a dose of 5 mg/kg (1/15 LD50), “Ivin” — in the form of an aqueous solution at a dose of 13 mg/kg and 0.013 mg/kg (1/100 and 1/100000 LD50 respectively), the control group of rats was given water with an auxiliary material OP-10. Signs of intoxication were recorded, body weight was measured in 4, 9 and 13 weeks, biochemical indicators of blood serum were determined (the activity of ALT and AST enzymes and alkaline phosphatase; the content of crude protein, urea, cholesterol, triglycerides and glucose were determined using kits of reagents by BioSystems (Spain); cholinesterase (ChE) activity in serum, red blood cells and brain tissue was determined with the use of methodology by Hesrtin S. using a kit of reagents produced by LLC NPP Filisit-Diagnostics (Ukraine). Results. No clinical signs of intoxication were detected in the experimental groups of rats. Under the isolated use of Chlorpyrifos a significant decrease in body weight, ChE inhibition activity in all bio substrates, with the greatest effect in the brain (by 48.8%); increase (after 9 and 13 weeks) in the activity of ALT and AST by 26.7–32.6%, 12.5–12.0%; a slight but a significant increase in the level of urea and glucose in blood serum to 17.3% and 17.6 %, respectively was detected. Under the combined action of Chlorpyrifos and “Ivin” in specified doses the body weight of rats was at control level, decrease of ChE activity was observed in all biological substrates and was at nearly the same level with the isolated use of Chlorpyrifos. There was a tendency towards the decline in ChE inhibition activity in brain tissue, and under the combined action of Chlorpyrifos with “Ivin” at higher doses after 4 weeks this effect was a significant (22.15%, p < 0.05). Conclusions. Chlorpyrifos under the isolated oral admission for 13 weeks has a weak hepatotoxic effect, as evidenced by the increase in cytolysis enzymes and urea levels in serum and causes a moderate anticholinesterase effect. “Ivin” under combined with Chlorpyrifos admission to the organism eliminates the hepatotoxic effects of Chlorpyrifos and reduces its anticholinergic effects in brain, which contributes to a less severe intoxication. The aim is to study the level of insulin-like growth factor-1 (IGF-1) and insulin-like growth factor-2 (IGF-2) in the blood serum of patients with papillary thyroid cancer, depending on the main clinical and morphological features of the disease. Materials and methods: The material was the information about 60 patients with papillary thyroid cancer (group 1). In group 2 there were 10 patients without oncopathology. All patients underwent clinical examination after total thyroidectomy before special treatment (radioiodine therapy): ultrasound diagnosis of the neck, confirmed diagnosis of papillary thyroid cancer by morphological examination of operative material. All patients underwent anthropometric studies (height, weight), on the basis of which the body mass index (BMI) was calculated. The study program also included determination of the level of thyroid-stimulating hormone of the pituitary gland (TSH), thyroglobulin (TG), antibodies to thyroglobulin (AB-TG). It was also determined the serum glucose level. In order to assess insulin resistance, the HOMA-IR index was calculated. All patients were tested for serum IGF-1 and IGF-2. Results: In the blood serum of patients with papillary thyroid cancer in 63% of patients the level of IGF-1 and in 85% – IGF-2 was probably higher than in the control group. There is a relationship between the level of IGF-1, IGF-2 and elevated level of proliferating factor – insulin in the serum of patients with papillary thyroid cancer. This may indicate an aggressive potential of the disease (i.e. clinical data on the prevalence of papillary thyroid cancer coincide with laboratory data). There was found a relationship between the expression of IGF-1, IGF-2 and insulin: at elevated levels of insulin> 24.9 μIU/ml, IGF-1 increases 4.2 times, and IGF-2 – 2.5 times. Evaluation of the relationship between the level of IGF-1 and IGF-2 and cervical lymph node involvement shows that in the absence of lesion (N0) there is an increase in these indicators by 2.2 and 1.8 times, respectively. Conclusions: The signaling system of insulin-like growth factors (IGF-1 and IGF-2) plays an important role in the occurrence and progression of malignant tumors. It is especially true for papillary thyroid cancer, so its components can be considered as potential diagnostic and prognostic markers of the disease and targets for anticancer therapy.

Infectious arthritis was modeled on white male Wistar rats by injecting a suspension of Staphylococcus aureus microorganisms into the knee joint of animals, and the effects of the synthetic hormone flosteron were also studied. At the same time, the content of acute-phase proteins (ceruloplasmin, haptoglobin, C-reactive protein (CRP)) and procalcitonin (PCT) as markers of the development of the inflammatory process and bacterial infection was studied in the blood serum of rats. Determination of the content of haptoglobin, ceruloplasmin and CRP was carried out on a Cobas 311 biochemical analyzer, the PCT concentration on a Cobas 411 analyzer using Roche Diagnostics test systems. Analysis of the results showed that when flosteron was administered, the concentration of CRP increased by 1.7 times already on the 3rd day, and by more than 4 times on the 14th day. The content of ceruloplasmin and haptoglobin also increased. The PCT concentration was at the level of the control values. Under the conditions of modeling infectious arthritis, changes in the content of acutephase proteins were observed. The greatest deviations from the reference values were found on the 14th day in animals that were injected with the hormone and S. aureus: the concentration of both CRP, the most objective biochemical marker of the inflammatory process, and PCT exceeded the physiological norm 12 times and more than 7 times, respectively. This indicates that the hormonal drug enhances the inflammatory process, which is confirmed by data that reflect changes in the content of acute-phase proteins. It can be assumed that the use of hormonal drugs contributes to the development of postoperative complications.

The purpose of the research is to increase the effectiveness of the supplement Reasil on the growth of milk-fed calves. For the scientific and economic experience, three groups of calves were formed (control, 1 and 2 experi-enced). Groups of animals of 10 heads were formed according to the principle of analogues. The indicators of live weight, growth and blood of calves were studied after Reasil use in a diet: Reasil HumicVet as a liquid form, and Reasil Humic Health – reduced to powder. In the experiment, animals of a black-and-white breed were used. The Reasil feeding in the diet contributed to an increase in the live weight of milk-fed calves in the 1-st and the 2-nd experimental groups at two months of age by 5.65 and 5.48 kg (or by 7.4 and 7.2%), respectively, compared to the indicator of the control group analogues. The average daily growth of calves of the experimental groups exceeded this indicator of control animals by 96 and 82 g, respectively. The blood of calves of the 1st and 2nd experimental groups, showed that hematoglobin was higher by 15.7 and 11.4%, red blood cells – by 16.5 and 15.1%, and the alkaline reserve – by 5.2%, compared with blood of calves from the control group, which indicates the activation of metabolic processes in the body. The total protein amount in the blood serum of animals from the experimental groups increased by 8.4 and 5.9% respectively, compared to the tests of calves from the control group. The albu-min content of animals of the 1st experimental group tested was higher by 5.3%; from the 2nd experimental group there was no significant difference compared to the control. The number of gamma-globulins in the blood of ani-mals of the experimental groups increased, which indicates an increase in defense reaction of animals. The con-tent of calcium in the blood serum of calves from the experimental groups was higher by 8.3 and 5.9%, phosphorus – by 4.8 and 2.4%, which indicates a more effective use of these mineral elements.
