Economic aspects of village health

Given the poor quantity and quality of medical care in most villages in the developing countries, the economic determinants of village health are the supply of labour, the cash flow associated with that labour and the availability of land. The paper examines these in the three classical ‘time periods’, arguing that inability to meet labour peaks is of great significance in explaining seasonal shortage of food and chronic shortage of cash. It also explains community indifference to upkeep of social overhead capital. Substitution of capital goods for labour is socially differentiated, not least by labour availability, and leads inevitably to a regressive distribution of land and the creation or enlargement of a class of landless labourers. Under certain limited conditions this class may enjoy a rising real income with associated health-promotive expenditures. The more normal case, however, is extreme poverty, whether rural or urban, with all that that implies for the undermining of health. Land reform therefore becomes a necessary precondition of health promotion.

Purpose Providing health care to the poor is evolving in the new US marketplace. The Affordable Care Act has set goals enhancing access to health care, lowering costs and improving patient outcomes. A key segment in this evolution is the most vulnerable health-care population of all: Medicaid. This paper aims to provide a general review of how providing health care to Medicaid patients is changing including how socio-economic aspects of this vulnerable population affects the quality of the health care provided. Design/methodology/approach The paper is entirely secondary research; no primary research has been conducted. Findings Managed care Medicaid provides a risk-based model to treating a vulnerable health-care market segment. The jury is still out on whether managed care Medicaid (MCM) is improving health-care quality and saving cost, but the provision of health care to the Medicaid segment is definitely shifting from a fee-for-service model to value based payment. Very recent developments of new health-care delivery approaches present a positive outlook for improving quality and containing costs going forward. Research limitations/implications At this stage, whether or not MCM saves money or provides better health-care quality to this vulnerable population is a work in progress. Health-care marketing can impact socio-economic aspects of health care for the poor. There is a need to follow up on the positive results being documented in demonstration health-care delivery models. Practical implications At this point, there has been no long-term study of whether managed care Medicaid offers better quality of health care and cost savings. The research to date suggest that the quality of health-care delivery to the poor is improving at a lower cost to payers. Social implications Medicaid patients are an underserved market segment. Managed care Medicaid offers a new model that has the potential to provide quality care at acceptable cost. Critical to this vulnerable market segment is the need to integrate socio-economic aspects of the population with the delivery of health care. Originality/value There has been very little discussion of Medicaid overall in the marketing literature, much less any discussion of managed care Medicaid.

ABSTRAKProgram pengabdian ini ditujukan untuk memberikan informasi dan menumbuhkan kesadaran masyarakat di Desa Menggala, Kabupaten Lombok Utara, Provinsi Nusa Tenggara Barat akan potensi madu trigona dan pengembangan budidayanya dalam menyelesaikan masalah gizi buruk. Usaha peningkatan kualitas sumber daya manusia dari Desa Menggala terkendala rendahnya kualitas kesehatan penduduk di Desa Menggala, hal ini dapat diketahui dari banyaknya penduduk di Desa Menggala memiliki status gizi buruk. Program pengabdian ini dilakukan untuk memperbaiki status gizi buruk dari masyarakat di Desa Menggala. Usaha peningkatan tersebut dilakukan dengan mengadakan edukasi, diskusi tentang potensi madu trigona dan pengembangan budidayanya dalam menyelesaikan masalah gizi buruk. Tahapan pelaksaanaan kegiatan meliputi observasi awal (1 September 2021) dan edukasi (2 September 2021). Walaupun tidak seluruh anggota masyarakat di Desa Menggala dikumpulkan untuk edukasi dikarenakan mengikuti protokol Covid-19, namun pada pelaksanaannya para keluarga yang memiliki anggota keluarga berstatus gizi buruk dan kader-kader kesehatan desa dapat hadir dan mengikuti kegiatan tersebut, selain itu, antusias dan respon positif dari para peserta tersebut menghasilkan berjalannya dengan baik program pengabdian ini. Kata kunci: edukasi; gizi buruk; terapi merdeka madu trigona; desa menggala. ABSTRACTThis service program is intended to provide information and raise public awareness in Menggala Village, North Lombok Regency, West Nusa Tenggara Province about the potential of Trigona honey and the development of its cultivation in solving the problem of malnutrition. Efforts to improve the quality of human resources from Menggala Village are constrained by the low quality of health of the residents in Menggala Village, this can be seen from the large number of residents in Menggala Village who have poor nutritional status. This service program is carried out to improve the poor nutritional status of the community in Menggala Village. The improvement efforts were carried out by conducting education, discussing the potential of Trigona honey and developing its cultivation in solving the problem of malnutrition. The stages of implementing the activities include initial observation (1 September 2021) and education (2 September 2021). Although not all community members in Menggala Village were gathered for education because they followed the Covid-19 protocol, in practice, families with family members with poor nutritional status and village health cadres were able to attend and participate in the activity, in addition, enthusiastic and positive responses. of the participants produced their performance with this good program of service. Keywords: education; malnutrition; trigona honey free therapy; menggala village.

  Penempatan peran yang baik bagi Family Caregiver sangatlah membantu lansia dalam meningkatkah qualitas hidupnya, meningkatkan motivasi dalam menjalankan hidup Penelitian ini bertujuan Mengetahui hubungan peran Family Caregiver dalam pemenuhan qualitas hidup bagi lansia di Kelurahan Langenharjo Kabupaten Kendal. DesainPenelitianDeskriptifKorelasional menggunakan pendekatan Krosectional,tehnikSamplingStratified Simple Random Sampling dengan karakteristik heterogen, dari populasi mempunyai hak yang sama untuk diseleksi sebagai sampel teknik undianPengambilan data dengan menggunakan kuesioner yang telah diuji validitas dan reliabilitasnya. Uji statistik Chi-square, dengan taraf signifikasi 5%jumlah sampel pada penelitian ini 70 sampel pada Family Caregiver dari 213 populasi yang ada. Hasil penelitian dari 70 responden didapatkan Peran Family Caregiver tidak baik dengan qualitas hidup tidak baik 33 (47,1%), sedangkan Peran Family Caregiver kurang baik dengan qualitas hidup lansia baik 3 (4,3%). Untuk distribusi Peran Family Caregiver kurang baik dengan qualitas hidup lansia tidak baik sebanyak 6 responden (8,6%) sedangkan untuk distribusi Peran Family Caregiver kurang baik dengan qualitas hidup lansia baik sebanyak 23 responden (32,9%). Terakhir, untuk distribusi Peran Family Caregiver baik dengan qualitas hidup lansia tidak baik didapatkan hasil 2 responden (2, 9%) sedangkan untuk distribusi Peran Family Caregiver baik dengan qualitas hidup lansia baik didapatkan hasil 3 responden (4,3%)Menunjukkan nilai ρ value 0,001 (ρ < 0,05) berarti ada hubungan antara dukungan keluarga dengan kepatuhan lansia dalam keikutsertaan posyandu lansia. Disarankan kepada semua Family Cregiver lansia untuk mampu memahami pentingnya perhatian, dukungan bagi lansia dalammeningkatkan qualitas hidup yang lebih baik bagi lansia.   Kata kunci : Peran family caregiver, qualitas hidup, lansia.   ABSTRACT Placement of a good role for Family Caregiver is very helpful for the elderly to improve their quality of life, increase motivation in living life Research Objective: To know the relationship between the role of Family Caregiver in fulfilling quality of life for the elderly in Langenharjo Village, Kendal Regency. Descriptive Correlational Research Design uses a cross sectional approach, Sampling Stratified Simple Random Sampling technique with heterogeneous characteristics, from the population has the same right to be selected as a sample lottery technique Retrieving data using a questionnaire that has been tested for validity and reliability. Test Chi-square statistics, with a significance level of 5% the number of samples in this study 70 samples on the Family Caregiver from 213 populations. Results of the Study Of 70 respondents found the role of Family Caregiver was not good with poor quality of life 33 (47.1%) , while the role of the Family Caregiver is not good with the quality of life of a good elderly 3 (4.3%). For the distribution of the role of Family Caregiver is not good with the quality of life of the poor family as many as 6 respondents (8.6%) while for the distribution of the Role of Family Caregiver is not good with the quality of life of good elderly as many as 23 respondents (32.9%). Finally, the distribution of the Role of Family Caregiver with good quality of life for the poor is obtained by 2 respondents (2, 9%), while the distribution of the Role of Family Caregiver with good quality of life for the elderly is obtained by 3 respondents (4.3%). 0.001 (ρ <0.05) means that there is a relationship between family support and the compliance of the elderly in the participation of the elderly posyandu. The article discusses the experience of teaching and conducting spirometry of general practitioners as part of the RESPECT study (RESearch on the PrEvalence and the diagnosis of COPD and its Tobacco-related aetiology). A total of 33 trained in spirometry general practitioners performed a study of 3119 patients. Quality criteria met 84.1% of spirometric studies. The analysis of the most common mistakes made by doctors during the forced expiratory maneuver is included. The most frequent errors were expiration exhalation of less than 6s (54%), non-maximal effort throughout the test and lack of reproducibility (11.3%). Independent predictors of poor spirogram quality were male gender, obstruction (FEV1 /FVC<0.7), and the center where the study was performed. The number of good-quality spirograms ranged from 96.1% (95% CI 83.2–110.4) to 59.8% (95% CI 49.6–71.4) depending on the center. Subsequently, an analysis of the reasons behind the poor quality of research in individual centers was conducted and the identified shortcomings were eliminated. The poor quality of the spirograms was associated either with the errors of the doctors who undertook the study or with the technical malfunctions of the spirometer.

This study provides a retrospective analysis of work to ensure high quality of medical care in an unfavorable epidemic situation. The consequence of COVID-19 was the implementation of a program to prevent the spread of infection, the re-profiling of medical institu-tions, and the introduction of restrictive and anti-epidemic measures. The experience of our work has shown the effectiveness of changing the order of med-ical care, the organization of the functioning of de-partments and patient routing. The study reflects the measures implemented in the hospital departments, the Department of clinical and laboratory diagnostics, radiation diagnostics and pathology Department. To ensure the safety of patients, it was decided to place patients on a single bed according to the type of infec-tious boxes. The safety of employees was ensured by the use of personal protective equipment, minimiza-tion of contact time with patients, and preventive weekly examination of staff for SARS-CoV-2 infection. The organized and well-coordinated work of the en-tire staff of the institution made it possible to prevent the spread of COVID-19 among employees, to detect cases of infection in a timely manner, and to carry out appropriate isolation and monitoring measures. At the time of completion of infectious diseases departments, the mortality rate among patients was less than 9%. Our experience in reorganizing a multi-specialty facil-ity can be used in the future when working with pa-tients who have undergone COVID-19, as well as in the context of a worsening epidemic situation.

Purpose: To analyze the results of treatment and the quality of medical care for breast cancer patients with breast reconstruction using thoracodorsal flap (TDF). Material and methods: The study was conducted on the basis of the N.N. Petrov National Medical Research Center of Oncology at the Department of Breast Tumors for the period 2016-2017. When using the reconstruction of the breast with the help of TDF the feature was the complete intersection of the muscle in the anterior-axillary line before closing the defect of the breast tissue but with the preservation of the thoracodorsal vascular sheaf. Surgical treatment was provided to 67 patients with breast cancer including after neoadjuvant therapy. Results: One-time reconstruction was performed in 16 (23.8%) patients under the organ-preserving surgery, mastectomy in two (2.9%) patients and mastectomy with implant placement in 27 (40.2%) patients. Delayed reconstruction of the breast: TDF in combination with the implant - 20 (29.8%), the use of TDF without an implant - 2 (2.9%) patients. Complications were observed in 6 (8.9%) patients. Conclusion: Reconstruction of the breast with TDF is the method of choice and priority for patients who underwent radiation therapy with a lack of integumentary tissues to cover the implant. TDF is a «good» plastic material and could be used in patients with severe defect of breast shape after organ-preserving surgery and mastectomy. TDF is characterized by a low incidence of complications. The use of TDF does not worsen the rehabilitation of patients and does not shift the timing of adjuvant treatment.

The shift to an integrated IBS construction approach requires enhanced supply chain integration to improve the productivity as well as the poor quality of human behavioual aspect in IBS project. This paper is to identify the challenges at each tier between players to facilitate supply chain integration among the IBS players. Findings adopted from semi-structured interview revealed the critical attitude issues of human factors, lack of interaction and sharing knowledge between interdisciplinary people. The findings of this study is useful to improve integration of supply chain and enhance innovation and sharing interaction between players in the IBS Malaysian construction project environment.© 2016.
