ABSTRACTThe relationship between peripheral and central immunity and how these ultimately may cause depressed behaviour has been the focus of a number of imaging studies conducted with Positron Emission Tomography (PET). These studies aimed at testing the immune-mediated model of depression that proposes a direct effect of peripheral cytokines and immune cells on the brain to elicit a neuroinflammatory response via a leaky blood-brain barrier and ultimately depressive behaviour. However, studies conducted so far using PET radioligands targeting the neuroinflammatory marker 18 kDa translocator protein (TSPO) in patient cohorts with depression have demonstrated mild inflammatory brain status but no correlation between central and peripheral immunity.To gain a better insight into the relationship between heightened peripheral immunity and neuroinflammation, we estimated blood-to-brain and blood-to-CSF perfusion rates for two TSPO radiotracers collected in two separate studies, one large cross-sectional study of neuroinflammation in normal and depressed cohorts and a second study where peripheral inflammation in healthy controls was induced via subcutaneous injection of interferon (IFN)-α. In both studies we observed a consistent negative association between peripheral inflammation, measured with c-reactive protein P (CRP), and radiotracer perfusion into and from the brain parenchyma and CSF. Importantly, there was no association of this effect with the marker of BBB leakage S100β, that was unchanged, but there was an association between the reduction of tracer perfusion in volunteers injected with interferon (IFN)-α and VEGF, a potent vascular permeability factor.These results support a different model of peripheral-to-central immunity interaction whereas peripheral inflammation causes a “stiffening” of the healthy BBB with consequent reduction of small molecule trafficking to and from the blood into the brain and CSF. This effect, on the long term, is likely to disrupt brain homeostasis and induce depressive behavioural symptoms. Moreover, given the molecular similarity between the TSPO ligands and antidepressant, this phenomenon may underlie treatment resistance in depressive cohorts with heightened peripheral status.

BACKGROUND: Inflammation affects the brain after stroke with main functions to rapidly eliminate the source of the disturbance, remove damaged tissue and then restore tissue homeostasis. High sensitive C-reactive protein (hsCRP) is a sensitive marker of inflammation and tissue injury in the arterial wall, while fractalkine is a distinct chemokine that promotes inflammatory signaling after neuronal death on ischemic stroke. We aim to investigate the association of fractalkine with hsCRP as a marker of inflammation in ischemic stroke patients.METHODS: This study was designed as a cross-sectional study. Soon after patients with ischemic stroke admitted to hospital, plasma fractalkine and hsCRP concentrations were assesed. Subjects had to be at least 30 years old and maximum 30 days of stroke onset. High inflammation was defined as hsCRP value >3 mg/L.RESULTS: High fractalkine levels were found on 24 ischemic stroke patients (49%) and mean of fractalkine 0.719 ng/mL on patients with stroke onset <7 days was higher than patients with stroke onset 7-30 days. Low fractalkine levels (<0.527 ng/mL) were found on ischemic stroke patients with onset 7-30 days accompanied by high inflammation (hsCRP >3 mg/L), but no significant correlation between fractalkine and hsCRP (p=0.613).CONCLUSION: High inflammation and low plasma fractalkine profile was found after 7 days of onset in ischemic stroke patients. No significant correlation between fractalkine and hsCRP in ischemic stroke patients.KEYWORDS: CRP, fractalkine, inflammation, ischemic stroke

Intake of industrially produced trans fatty acids (iTFAs) has previously been associated with dyslipidemia, insulin resistance, hypertension and inflammation, as well as increased cardiovascular (CV) morbidity and mortality. iTFA intake declined in Norway after the introduction of legislative bans against iTFA consumption. However, the relationship between the current iTFA intake and CV health is unclear. The aim of the present study was to investigate the association between current iTFA intake, reflected by plasma iTFA levels, and established CV risk factors. We also examined the associations between plasma ruminant TFA levels and CV risk factors. In this cross-sectional study, we included 3706 participants from a Norwegian general population, born in 1950 and residing in Akershus County, Norway. The statistical method was multivariable linear regression. Plasma iTFA levels were inversely associated with serum triglycerides (p < 0.001), fasting plasma glucose (p < 0.001), body mass index (p < 0.001), systolic and diastolic blood pressure (p = 0.001 and p = 0.03) and C-reactive protein (p = 0.001). Furthermore, high plasma iTFA levels were associated with higher education and less smoking and alcohol consumption. We found that plasma ruminant trans fatty acids (rTFA) levels were favorably associated with CV risk factors. Furthermore, plasma iTFA levels were inversely associated with CV risk factors. However, our results might have been driven by lifestyle factors. Overall, our findings suggest that the current low intake of iTFAs in Norway does not constitute a threat to CV health.

Abstrak Latar belakang: Sarkopenia adalah sindrom yang ditandai dengan penurunan massa otot disertai penurunan kekuatan otot dan atau fungsi otot. Stres oksidatif dan proses inflamasi dikenal sebagai faktor pemicu untuk sarkopenia dengan melepaskan rangsangan katabolik interleukin-6 (IL-6) dan protein C-reaktif (CRP). Penelitian ini bertujuan untuk menentukan hubungan antara IL-6 dan kadar CRP terhadap parameter sarkopenia seperti massa otot, kekuatan pegangan, dan kecepatan berjalan. Metode: Penelitian ini menggunakan desain cross-sectional analitik yang dilakukan di Desa Pedawa, Kabupaten Buleleng, Bali pada bulan Agustus 2016. Sekitar 79 responden berusia ≥ 60 tahun menggunakan teknik sampling acak stratifikasi. Variabel yang dinilai yaitu parameter sarkopenia (massa otot, kekuatan pegangan, dan kecepatan berjalan) termasuk IMT, serta IL-6 dan pemeriksaan tingkat CRP. Uji korelasi spearman dan parsial digunakan untuk menilai korelasi antara parameter IL-6, CRP, dan sarkopenia. Hasil: Kadar IL-6 dan CRP tidak berkorelasi signifikan dengan tiga parameter sarkopenia. Kadar CRP berkorelasi dengan IL-6 (r = 0.37; p = 0.001) dan IMT (r = 0.29; p = 0.009). Pada kelompok pria, IL-6  hanya berkorelasi dengan CRP (r = 0.40; p = 0.011). Sedangkan pada kelompok wanita, IL-6 berkolerasi dengan CRP (r = 0.38; p = 0.017), kecepatan berjalan (r = 0.33; p = 0.037) serta CRP berkorelasi dengan IMT (r = 0.32; p = 0.049) dan massa otot total (r = -0.32; p = 0.043).  Setelah penyesuaian untuk variabel IMT, IL-6 berkorelasi dengan CRP (r = 0.43; p = 0,001) dan massa otot total (r = -0.25; p = 0.026) serta secara signifikan berkorelasi pada kelompok kurus (IMT<18.5 kg/m2) (r = -0.50; p = 0.026). CRP tidak berkorelasi secara signifikan dengan tiga parameter sarkopenia pada uji spearman, korelasi parsial, dan uji korelasi spesifik spearman berdasarkan pada kelompok IMT. Kesimpulan: Kadar IL-6 berhubungan dengan penurunan massa otot total pada keseluruhan lansia desa Pedawa setelah penyesuaian variabel IMT. Oxidative stress and inflammatory processes are known as triggering factors for sarcopenia by releasing catabolic stimuli of interleukin-6 (IL-6) and C-reactive protein (CRP).This study aims to determine the relationship between IL-6 and CRP levels to sarcopenia parameter such as muscle mass, grip strength, and walking speed. Methods: This study was an analytic cross-sectional design conducted at Pedawa Village, Buleleng District, Bali in August 2016. About 79 respondents aged ≥ 60 years using stratified random sampling technique. The assessed variables were sarcopenia parameter (muscle mass, grip strength, and walking speed) including BMI, as well as IL-6 and CRP levels examination. Spearman and partial correlation test were used to assess the correlation among IL-6, CRP, and sarcopenia parameters. Results: IL-6 levels and CRP were not significantly correlated with the three parameters of sarcopenia. CRP levels correlated with IL-6 (r = 0.37; p = 0.001) and BMI (r = 0.29; p = 0.009). In the male group, IL-6 was only correlated with CRP (r = 0.40; p = 0.011). While in the women group, IL-6 correlated with CRP (r = 0.38; p = 0.017), walking speed (r = 0.33; p = 0.037) and CRP correlated with BMI (r = 0.32; p = 0.049) and total muscle mass (r = -0.32; p = 0.043).  After adjustment to BMI variable, IL-6 was correlated with CRP (r = 0,43; p = 0,001) and total muscle mass (r = -0.25; p = 0.026) and significantly correlated in underweight groups (BMI<18.5 kg/m2) (r=-0.50; p=0.026). CRP was not significantly correlated with the three parameters of sarcopenia on Spearman, partial correlation, and Spearman's specific correlation test based on BMI group. Conclusion: IL-6 levels were associated with total muscle mass loss after BMI adjustmentin Pedawa village’s elderly as a whole. ABSTRACT Objective To evaluate the relationship between serum retinol concentrations and subclinical infection in children from rural settlements. Methods A cross-sectional population-based study was carried out in nine rural settlements in the northeastern region of Brazil, involving 118 children aged 6 to 59 months. The relationship between serum retinol and C-Reactive Protein levels, an important marker of infectious and inflammatory processes, was investigated by multiple linear regression, controlling for demographic, socioeconomic and nutritional variables. Serum retinol and C-Reactive Protein were measured, respectively, by High Performance Liquid Chromatography and immunoturbidimetric assay in automated equipment. Results Vitamin A deficiency (retinol <0.70μmol/L) was identified in 9.3% of the children. C-Reactive Protein was the only predictor of retinol concentrations in the final regression model, causing a 0.728μmol/L reduction in retinol concentrations in the studied children (p=0.008). Conclusion Vitamin A deficiency is a problem of mild/moderate severity and measures to control infectious diseases in this population are fundamental to prevent and/or combat this problem.

Abstract: COVID-19 patients could experience many respiratory symptoms accompanied with increased inflammatory markers, such as C-reactive protein (CRP). Vitamin D is known to have a positive effect on immune system by decreasing inflammatory process. This study was aimed to evaluate the relationship between vitamin D and CRP concentration and clinical severity, and the correlation between vitamin D concentration and CRP concentration in COVID-19 patients. This was an observational and analytical study with a cross-sectional design conducted at Prof. dr. R. D. Kandou hospital. A total of 40 subjects with COVID-19 were included. Clinical severity was classified into moderate and severe. All subjects underwent vitamin D and CRP concentration examination. The results showed that there was a significant relationship between vitamin D level and clinical severity of patients with COVID-19 (p=0.0001). There was a significant relationship between CRP level and clinical severity of patients with COVID-19 (p=0.0001). Moreover, there was a negative correlation between vitamin D concentration and CRP concentration (r=-0.49, p=0.001). In conclusion, vitamin D insufficiency or deficiency is related to CRP increament and clinical severity of patients with COVID-19.Keywords: coronavirus disease 2019, vitamin D, C-reactive protein (CRP), clinical severityAbstrak: Penderita COVID-19 mengalami berbagai gejala pernapasan disertai dengan pening-katan beberapa penanda inflamasi, salah satunya C-reactive protein (CRP). Penelitian ini bertujuan untuk mengetahui apakah terdapat hubungan antara kadar vitamin D dan kadar CRP dengan klinis COVID-19, dan korelasi antara kadar vitamin D dengan kadar CRP. Jenis penelitian ini ialah studi analitik observasional dengan desain potong lintang yang dilakukan di RSUP Prof. dr. R. D. Kandou. Total sampel sebanyak 40 pasien dengan diagnosis COVID-19. Gejala klinis pasien dibagi menjadi sedang dan berat. Pemeriksaan kadar vitamin D dan kadar CRP dilakukan pada seluruh sampel. Hasil penelitian mendapatkan hubungan bermakna antara kadar vitamin D dengan klinis pasien COVID-19 (p=0,0001), dan antara kadar CRP dengan klinis pasien COVID-19 (p=0,0001). Terdapat korelasi negatif antara kadar vitamin D dengan kadar CRP (r=-0,49, p=0,001). Simpulan penelitian ini ialah insufisiensi atau defisiensi vitamin D berhubungan dengan peningkatan CRP dan klinis pasien COVID-19.Kata kunci: coronavirus disease 2019, vitamin D, C-reactive protein (CRP), gejala klinis

Background:The most common complication in patients with SLE is infection, and its clinical presentation is often indistinguishable from SLE flares. Therefore, laboratory ratios have been evaluated to differentiate between those events. Among them, ESR/CRP1, neutrophil/lymphocyte (NLR)2, and platelet/lymphocyte (PLR)3 ratios have been previously assessed with acceptable performance; however, there is no validation of those ratios in our SLE population.Objectives:To examine the predictive capacity of infection of the lymphocyte/C4 (LC4R), lymphocyte/C3 (LC3R), and ferritin/ESR (FER) ratios in SLE patients, and to evaluate the performance of ESR/CRP, NLR, AND PLR ratios in our SLE population.Methods:We conducted a cross-sectional study of SLE patients admitted to the emergency service at Hospital San Vicente Fundación (HSVF). The HSVF ethics committee approved the execution of the project.Patients were categorized into four groups according to the main cause of hospitalization: (1) infection, (2) flare, (3) infection and flare and, (4) neither infection nor flare.We calculated the median values of the ratios and their respective interquartile ranges for each group. Then, we compared those summary measures using the Kruskal-Wallis test. Subsequently, we assessed the predictive capacity of infection of each ratio using ROC curve. Finally, we carried out a logistic regression model.Results:A total of 246 patients were included, among them 90.7% were women. The median age was 28 years (IQR: 20-35 years). Regarding the outcomes, 37.0% of the patients had flares, 30.9% had neither infection nor flare, 16.7% had an infection and, 15.5% had simultaneously infection and flare. When compared the four groups, statistical significance (p<0.05) was observed. Area under the ROC curve (AUC) for infection prediction was as follows: 0.752 (sensitivity 60.5%, specificity 80.5%) for LC4R, 0.740 (sensitivity 73.2%, specificity 68.3%) for FER, 0.731 (sensitivity 77.6%, specificity 80.5%) for LC3R.In the logistic regression modeling, we observed that an increase in the risk of infection was associated with an LC4R below 66.7 (OR: 6.3, CI: 2.7 – 14.3, p <0.0001), a FER greater than 13.6 (OR: 5.9, CI: 2.8 – 12.1, p <0.0001) and an LC3R below 11.2 (OR: 4.9, CI: 2.4 – 9.8, p <0.0001).The ESR/CRP and PLR performed poorly with an AUC of 0.580 and 0.655, respectively. In contrast, the NLR showed better performance (AUC of 0.709, with a sensitivity of 80.2% and specificity of 55.7%).Figure 1.ROC curves of the evaluated ratiosConclusion:These laboratory ratios could be easy to assay and inexpensive biomarkers to differentiate between infection and activity in SLE patients. The LC4R, FER, and LC3R have a significant diagnostic performance for detecting infection among SLE patients.
