ABSTRACT: CONTEXT AND OBJECTIVE: High blood pressure (hypertension) is the most frequent cause of morbidity and a major risk factor for cardiovascular complications. The aim here was to describe the prevalence of blood pressure greater than or equal to 140/90 mmHg in the adult Brazilian population and federal states, along with self-reported information about previous medical diagnoses of hypertension, use of medication and medical care for hypertension control. DESIGN AND SETTING: Cross-sectional study analyzing information from the National Health Survey of 2013, relating to Brazil and its federal states. METHODS: The sample size was estimated as 81,254 households and information was collected from 64,348 households. The survey consisted of interviews, physical and laboratory measurements. Systolic blood pressure was considered to be high when it was ≥ 140 mmHg and diastolic blood pressure, ≥ 90 mmHg. RESULTS: It was found that 22.8% of the population has blood pressure measurements ≥ 140/90 mmHg. The proportion was higher among men than among women: 25.8% versus 20.0%. The frequency increased with age, reaching 47.1% in individuals over 75 years and was highest in the southeast and south. 43.2% reported previous medical diagnoses of hypertension and, of these, 81.4% reported using medication for hypertension and 69.6%, going to the doctor within the past year for pressure monitoring, thus showing regular medical follow-up. CONCLUSION: These results are important for supporting measures for preventing and treating hypertension in Brazil, with the aim of achieving the World Health Organization's goal of reducing hypertension by 25% over the next decade.

Introduction: The association "uric acid and high blood pressure" is still matter of debate. Therefore, to distinguish serum uric acid as an independent factor for high blood pressure can start new trials to prevent hypertension. A paleoantropological rationale for this association was a knockout of uricase occurred during the Miocene among hominids that induced high levels of uric acid. As consequence, during food shortage times, high uric acid had an evolutionary benefit increasing salt-sensitivity for keep blood pressure and rising insulin resistance that maintaining high blood glucose levels provided fuel for the brain. Nowadays, the average levels of uric acid are substantially higher compared to chimpanzees, a uricase-deficient primate, and among remote populations as the yanomamo. Hypothesis: we assessed the hypothesis that the link between uric acid and blood pressure in individuals without hypertension is independent of sex, age, race, salt and alcohol intakes, glucose homeostasis, body-mass index and renal function. Methods: from the 15105 participants of the Brazilian Longitudinal Study for Adult Health (ELSA-Brasil) aged 35-74 years, we selected 7954 individuals (median age = 48 years-old; women =58.1; White = 56.6%) without hypertension, diabetes, previous cardiovascular diseases. The presence or not of prehypertension (7th JNC criteria) was the dependent variable and quartiles (Q) of serum uric acid, the independent variable. We applied an unconditional logistic regression adjusted for age and sex. After this, according to our hypothesis, we added the following variables: (1) 24-hour urinary sodium; (2) log Homeostasis Model Assessment (fasting blood glucose (mg/dL) х fasting insulin (mg/dL)/450), and (3) a full model, adding race, body-mass index, alcohol intake, and glomerular filtration rate by CKD-epi). Results: the uric acid quartiles ranges (md/dL) were Q1 ≤ 4.1; Q2:4.2-5.0; Q3:5.1-6.0; Q4: ≥ 6.1 and the number of participants were Q1= 1909; Q2=2029; Q3=2000; Q4= 2016. Considering Q1 as reference, the age-sex adjusted odds ratios (95% Confidence Interval) through the quartiles were: Q2= 1.22 (1.05-1.41); Q3= 1.40 (1.20-1.63); Q4= 2.03 (1.71-2.39) [P for trend <0.001]. Adding 24 hour urinary sodium, the ORs (95% CI) were: Q2= 1.19 (1.01-1.40); Q3= 1.37 (1.16-1.62); and 1.94 (1.61-2.33) [P for trend <0.001]. Adding HOMA-IR, the ORs (95% CI) were: Q2= 1.14 (0.97-1.34); Q3= 1.25(1.05-1.48); and Q4=1.62(1.34-1.96) [P for trend <0.001]. Finally, for the full model the ORs were Q2= 1.04 (0.88-1.23); Q3= 1.05 (0.88-1.26) and Q4= 1.32(1.08-1.62) [P for trend <0. 01]. Conclusion: Uric acid levels were correlated to prehypertension among a middle-aged urban population. It occurred independently of other variables classically associated to high blood pressure or the origin of higher uric acid in hominids.

Objective. To estimate the additive interaction of body mass index (BMI) and family history of hypertension (FHH) on hypertension and explore whether the interaction could be influenced by behavioural risk factors. Methods. The cross-sectional data on 5791 participants were from the China National Health Survey in Gansu province in 2016. We assessed the additive interaction by calculating the relative excess risk due to interaction (RERI), the attributable proportion due to interaction (AP), and the synergy index (SI). Results. ORs for hypertension were highest in Han (13.52, 95% CI: 9.45 to 19.34) and Yugur (13.85, 95% CI: 8.48 to 22.63) with the combination of obesity and FHH. The interaction of BMI and FHH was significant in Han people, with the RERI, AP, and SI and their 95% CIs being 2.48 (1.13 to 3.82), 0.33 (0.19 to 0.47), and 1.61 (1.26 to 2.07) for overweight and FHH and 6.32 (1.91 to 10.73), 0.47 (0.27 to 0.67), and 2.02 (1.33 to 3.07) for obesity and FHH, respectively. The interaction of BMI and FHH was not significant in Yugur people. Adjustment for behavioural risk factors had little influence on the interactions, and risks of hypertension remained increased. Conclusions. BMI and FHH were associated with hypertension, and the interaction of BMI and FHH on hypertension was significant in Han but not in Yugur people. Behavioural risk factors had little influence on the associations and interactions. The exacerbation of hypertension risks by overweight or obesity in hypertension families deserves attention in weight control and community care.

ABC (glucose, blood pressure and LDL-cholesterol) goals are basic standards of diabetes care. We aimed to assess ABC control and related factors in a representative sample of Brazilian adults with diabetes. We analyzed 465 adults with known diabetes in the Brazilian National Health Survey. The targets used were <7% for glycated hemoglobin (A1C); <140/90 mmHg for blood pressure; and <100 mg/dL for LDL-C, with stricter targets for the latter two for those with high cardiovascular (CVD) risk. Individual goals were attained by 46% (95% CI, 40.3–51.6%) for A1C, 51.4% (95% CI, 45.7–57.1%) for blood pressure, and 40% (95% CI, 34.5–45.6%) for LDL-C. The achievement of all three goals was attained by 12.5% (95% CI, 8.9–16.2%). Those with high CVD risk attained blood pressure and LDL-C goals less frequently. A1C control improved with increasing age and worsened with greater duration of diabetes. Achievement of at least two ABC goals decreased with increasing BMI and greater duration of diabetes. In sum, about half of those with known diabetes achieved each ABC goal and only a small fraction achieved all three goals. Better access and adherence to treatment and strategies to personalize goals according to specific priorities are of the essence.

Introduction: The World Health Organization has already warned of increasing non-communicable diseases among adolescents as a major public health problem. The importance of this age group also lies in the fact that many serious diseases in adulthood have their roots in adolescence. Method: A pre-designed and pre-tested questionnaire was used in class-room setting to collect information from students regarding presence of risk factors of non-communicable diseases. The respondents were also subjected to anthropometric measurements and blood pressure examination using standard operating procedures. Results: A total of 761 students of class VI-XII participated in the study of which 61.4% were boys and rests were girls. High blood pressure among boys and girls were 19.9% and 22.1% were respectively. In Bivariate analysis age > 15 years (median) (OR= 2.11), fast food intake (>3 times/week) (OR= 1.66), Alcohol consumption (OR= 2.22), less physical activity (OR=1.54), increased body mass index (OR=2.53), significantly associated with high blood pressure. In Multivariate analysis age (AOR= 2.25), fast food intake (AOR= 1.50), Alcohol consumption (OR= 2.23), less physical activity (AOR=1.71), increased body mass index (AOR=2.42) remains significant predictor. Conclusion: Detecting the risk factors of high blood pressure prevalent in the population is of utmost importance to achieve a healthy population. Formulation and dissemination of need--based, culturally acceptable and age appropriate scientific messages for school students should be conducted more proactively. Abstract Background Chilean Health System has fully implemented Universal Health Coverage (UHC) for acute cardiovascular events since 2005. Age-adjusted cardiovascular mortality has decreased, but there is limited information about coverage and quality of chronic health care given to cardiovascular disease (CVD) survivors at the national level. Purpose To assess the prevalence and quality of care in Chilean adult CVD survivors. Methods Chilean National Health Survey 2016–2017 (ENS 2016–2017) is a random stratified multistage sample of non-institutionalized population over 14 years (n=6240). Age, education, gender, rural/urban and geographical area weighted prevalence of CVD survivors (self-reported medical diagnosis of myocardial infarction or cerebrovascular attack) were calculated. High quality of care was defined as meeting six criteria simultaneously: under 70mg% LDL- C level, statin use, aspirin use, blood pressure under 130/80 mmHg, HgA1C<7 or 8 (>74-year-old) and non-smoking. Quality of care was explored using multivariate linear and logistic regression adjusting by age, gender, education and year of diagnosis (before or after UHC). Results Weighted national prevalence of CVD survivors in over 20-year-old population was 6.1%. The sample size for the CVD survivor analyses was n=455. 28.7% of CVS had their first event before the year 2005 (n=141). Overall 27.9% had LDL-C under 70mg%, 37.8% used statins, 41.4% used aspirin, 37.8% had controlled blood pressure, 78.3% were non-smokers and 84.3% had good glycemic control. National “high quality of care” prevalence in CVD survivors was 0.3%, 0.4% and 0.1% for men and women respectively. LDL and Blood pressure control prevalence (meet both criteria simultaneously) was 4,4%. In the adjusted multivariate model age was associated to a higher number of quality criteria achievement. Conclusion The number of CVD survivors in Chile is a huge challenge for the health care system. Universal coverage does not guarantee the quality of chronic life long care. Specific surveillance in high-risk population is needed to assess the system's effectiveness and accountability. Introduction: The prevalence of childhood high blood pressure is rising fast. The possibility of high blood pressure augments with increasing body mass index (BMI), inappropriate dietary intake and lifestyle problems. The aim of this study was to investigate the correlation between blood pressure, body mass index, life style and dietary habits in children and adolescents aged 6 - 18 years in Tabriz, Iran. Methods: The present study was a cross-sectional study. Using data of the Sib software (the software used in the health transformation system) were collected. This data includes anthropometric information, some demographic factors, dietary intake and lifestyle of 425 children and adolescents with overweight and obesity. The data were analyzed by STATA software (MP 4.2 potable 2017). Results: The association between systolic blood pressure and dairy products (P = 0.02), watching TV / PC (P = 0.041), BMI quarter (P <0.001), BMI (P <0.001), and age (P <0.001) as well as the association between diastolic blood pressure with BMI quarter (P <0.001), BMI (P <0.001) and age (P <0.001) was statistically significant and the other variables did not show a meaningful statistical relationship. Conclusion: Age, BMI, less dairy consumption, and watching TV/PC has a relationship with high blood pressure. Therefore, nutritionists and health workers should consider the above considerations when making recommendations in children and adolescents with overweight and obesity.

Objectives To evaluate the association of general and abdominal obesity with high blood pressure in young children. Methods A longitudinal study including 1796 participants from the Madrid region (Spain) with baseline at age 4 years and a follow-up 2 years later. Blood pressure, body mass index and waist circumference were measured during a physical examination. We evaluated the association between obesity at baseline and weight changes between the ages of 4 and 6 years and high blood pressure. Data were analysed using linear and logistic regressions adjusted for covariates. Results Obese 4 year olds (general or abdominal obesity) experienced an average 4–5 mmHg increase in systolic blood pressure and a 2.5–3 mmHg increase in diastolic blood pressure by the age of 6 years. Compared to children maintaining a non-excess weight (based on body mass index) during follow-up incident and persistent cases of excess weight (overweight or obesity) had an odds ratio (OR) for high blood pressure of 2.49 (95% confidence interval (CI) 1.50–4.13) and OR 2.54 (95% CI 1.27–5.07), respectively. Regarding abdominal obesity we estimated OR 2.81 (95% CI 0.98–8.02) for incident cases and OR 3.42 (95% CI 1.38–8.49) for persistent cases. Similar estimates for the waist–height ratio were observed. Individuals who experienced remission to non-excess weight did not have an increased risk of high blood pressure. Conclusions We observed an increased risk for high blood pressure among 4-year-olds who presented with persistent or incident cases of excess weight (body mass index) or abdominal obesity after 2 years of follow-up. Children with excess weight or obesity at baseline who remitted to non-excess weight did not exhibit an increased risk of high blood pressure.
