Shuteye Time Compared With Bedtime: Misclassification of Sleep in Adolescent Females

Short sleep duration is associated with poorer outcomes for adolescents; however, sleep duration is often assessed (either by questionnaire or device) using self-reported bedtime (i.e., the time a person goes to bed). With sedentary activities, such as screen time, being common presleep in-bed behaviors, the use of “bedtime” may introduce error to the estimates of sleep duration. It has been proposed that self-reported “shuteye time” (i.e., the time a person starts trying to go to sleep) is used instead of bedtime. This study aimed to compare the bedtimes and shuteye times of a sample of 15- to 18-year-old female adolescents recruited from 13 high schools across New Zealand. The influence on sleep duration estimates and associations with healthy lifestyle habits was also examined. Sleep data were collected from 136 participants using actigraphy and self-report. On average, 52 min (95% confidence interval [43, 60] min) of sedentary time was misclassified as sleep when bedtime was used instead of shuteye time with actigraph data. Mean bedtimes on weekdays and weekends were 9:56 p.m. (SD = 58 min) and 10:40 p.m. (SD = 77 min), respectively. The relationship between bedtime and shuteye time was not linear—indicating that bedtime cannot be used as a proxy for shuteye time. Earlier shuteye times were more strongly associated with meeting fruit and vegetable intake and sleep and physical activity guidelines than earlier bedtimes. Using bedtime instead of shuteye time to estimate sleep duration may introduce substantial error to estimates of both sleep and sedentary time.

Evidence has grown supporting the role for short sleep duration as an independent risk factor for weight gain and obesity. The purpose of the present study was to examine the relationship between sleep duration and dietary quality in European adolescents. The sample consisted of 1522 adolescents (aged 12·5–17·5 years) participating in the European multi-centre cross-sectional ‘Healthy Lifestyle in Europe by Nutrition in Adolescence’ study. Sleep duration was estimated by a self-reported questionnaire. Dietary intake was assessed by two 24 h recalls. The Diet Quality Index for Adolescents with Meal index (DQI-AM) was used to calculate overall dietary quality, considering the components dietary equilibrium, dietary diversity, dietary quality and a meal index. An average sleep duration of ≥ 9 h was classified as optimal, between 8 and 9 h as borderline insufficient and < 8 h as insufficient. Sleep duration and the DQI-AM score were positively associated (β = 0·027, r 0·130, P< 0·001). Adolescents with insufficient (62·05 (sd 14·18)) and borderline insufficient sleep (64·25 (sd 12·87)) scored lower on the DQI-AM than adolescents with an optimal sleep duration (64·57 (sd 12·39)) (P< 0·001; P= 0·018). The present study demonstrated in European adolescents that short sleep duration was associated with a lower dietary quality. This supports the hypothesis that the health consequences of insufficient sleep may be mediated by the relationship of insufficient sleep to poor dietary quality.

<p><em>Coronary Arterial Disease (CAD) is one of cardiovaskular disease that remain leading cause death and disability. Short sleep duration is the major symptoms in patients with CAD, during recovery period after cardiac events and during cardiac rehabilitation. Benson’s relaxation is one of relaxation as modalities therapy to increase sleep duration, </em><em>however few studies related to this</em><em> technique in planned</em><em> intervention</em><em>.</em><em> This study was to measured the effectiveness of Benson’s relaxation in short sleep duration of CAD patients during cardiac rehabilitation. It was a </em><em>quasi experimental pretest posttest control group design.</em><em> This study included 29 respondens in Dr.M.Djamil Hospital were assigned to intervention group which receiving Benson’s relaxation technique (n=15) and control group with routine care (n=14). </em><em>Benson’s relaxation </em><em>technique</em><em> was administered for 5 days 2 times a day, each 20 minutes to intervention group.</em><em> Short sleep duration was measured using </em><em>sleep diary (self report).</em><em> The result indicated significant increasing in mean of  sleep duration  before and after Benson’s relaxation in intervention group </em><em>(p value &lt; 0,001). </em><em>The study concluded that </em><em>Benson’s relaxation </em><em>technique is an effective non-pharmacological intervention to increase sleep duration in CAD patients.</em></p><p><em><br /></em></p><p>Penyakit jantung koroner menjadi masalah kardiovaskular yang mengakibatkan angka mortalitas yang tinggi. Durasi tidur pendek termasuk salah satu keluhan utama pasien penyakit jantung koroner pada masa recovery setelah serangan dan menjalani rehabilitasi fase 2. Relaksasi Benson merupakan teknik relaksasi sebagai terapi modalitas untuk mengurangi keluhan durasi tidur pendek, namum belum banyak penelitian terkait intervensi ini. Penelitian ini bertujuan untuk mengidentifikasi pengaruh relaksasi Benson terhadap durasi tidur pasien penyakit jantung koroner yang menjalani rehabilitasi fase 2. Penelitian ini menggunakan desain Quasi Eksperimen dengan pendekatan <em>control group pretest posttest design</em> pada 29 responden di RSUP. Dr.M.Djamil Padang yang dibagi dalam dua kelompok (kelompok intervensi dan kelompok kontrol). Hasil penelitian menunjukan ada perbedaan rerata durasi tidur yang signifikan antara sebelum dan setelah dilakukan intervensi relaksasi Benson pada kelompok intervensi (p value &lt; 0,001). Simpulan hasil penelitian ini dapat menjadi salah satu terapi modalitas bagi perawat untuk mengatasi masalah durasi tidur pendek pada pasien penyakit jantung koroner.</p><p><em><br /></em></p>

Objective: Hypertension as a cardiovascular disease occurs due to an uncontrolled increase in blood pressure. Night shift nurses with more overweight, short sleep duration, and excessive stress levels are at risk of increase blood pressure. This study aims to analyze how the relationship between obesity, nutritional status, sleep duration and stress level influence the blood pressure of the night shift nurses.Materials and methods: The subjects in this study were night shift nurses in four hospitals. The dependent variable was blood pressure and the independent variables were nutritional status, sleep duration, and stress levels. This study was an observational analysis with a perspective cohort design in which the subjects were 312 night shift nurses. Nutritional status were identified from Body Mass Index (BMI) through anthropometric measurement, sleep duration by looking at average hours of sleep during the night service, stress levels through the Perceived Stress Scale (PSS-10) questionnaire. Blood pressure was measured using a mercury sphygmomanometer. Data were analyzed by Chi-square test and Logistic Regression.Results and Discussion: There was a significant relationship between nutritional status, sleep duration, and stress levels with blood pressure. The results of the multivariate analysis showed that the shift nurses with overweight (obesity) nutritional status are at a risk of having disorder 1.97 times, the shift nurses with sleep duration < 6 hours are at risk of having disorder 3.78 times and shift nurses with intermediate stress level at risk of having disorder 2.08 times with enhancement blood pressure.Conclusion: There is a relationship between nutritional status, sleep duration and stress level with blood pressure. Abstract Background A variety of health problems, such as metabolic syndrome (MetS), have been linked to sleep disorders. While numerous epidemiological studies have shown a U-shaped relationship between sleep duration and poor health outcomes, the results were limited and inconsistent. This study was designed to evaluate the relationship between sleep duration and MetS. Methods This population-based study was conducted on the participants aged 35–70 of Bandare-Kong Non-Communicable Diseases (BKNCD) Cohort Study, a part of Prospective Epidemiological Research Studies in IrAN (PERSIAN). MetS was diagnosed according to the National Cholesterol Education Program (NCEP) criteria and the Iranian-specific cut-off for waist circumference (≥ 95 cm). Sleep information was extracted through a standard questionnaire based on self-reported information. Data were analyzed by R software using generalized additive models (GAMs). A statistically significant level was considered as P < 0.05. Results A total of 3695 participants were included in the analyses. The mean age was 48.05 years (SD 9.36), and 2067 (55.9%) were female. The estimated Prevalence of MetS was 35.9%, and women appeared to be more likely to have MetS than men (P < 0.001). There was a non-linear and linear association between sleep duration and the risk of MetS in women and men, respectively. The lowest risk was observed among those with 7–7.5 h of sleep duration per night. Conclusion Long sleep duration was associated with increased risk of MetS and higher MetS severity score in both genders, while the short sleep duration increased the risk of Mets as well as MetS severity score just in women. The longitudinal studies would be suggested to assess the relationship between sleep quality and quantity components and MetS.

Background Studies have found that social capital (SC) is associated with the risk of cognitive decline; however, the mechanism explaining how SC leads to cognitive decline is unclear. The current study examines the mediation effect of sleep duration on the relationship between SC and cognitive decline in Chinese older adults. Methods A cross-sectional study of 955 community-dwelling aged 60 or over was conducted. The mini-mental state examination (MMSE), self-report sleep duration questionnaire, and social capital scales were administered during the face-to-face survey. The Bootstrap methods PROCESS program is employed to test the mediation model. Results After controlling for covariates, both social cohesion and social interaction were positively correlated with the MMSE score (p<0.001), and social cohesion was negatively correlated with sleep duration (p = 0.009); On the contrary, sleep duration was negatively correlated with MMSE score (p<0.001). Linear regression analysis showed social cohesion was positively associated with the MMSE score (β = 0.16, p = 0.005), while sleep duration was associated with an increased risk of cognitive decline (β = -0.72, p<0.001). Sleep duration has mediated the relationship between social cohesion and cognitive decline (explaining 21.7% of the total variance). Conclusions Social capital negatively associated with the risk of cognitive decline in this Chinese population, and sleep duration may partly explain this relationship. It may be a suggestive clue to identify those at a higher risk of progressing to cognitive impairment. Further prospective study in need to confirm this finding due to the cross-sectional design.

BACKGROUND Obesity and short sleep duration are significant public health issues. Current evidence suggests these conditions are associated with cardiovascular disease, metabolic syndrome, inflammation, and premature mortality. Increased interest in the potential link between obesity and short sleep duration, and its health consequences, has been driven by: 1) the apparent parallel increase in prevalence of both conditions in recent decades; 2) their overlapping association with cardiometabolic outcomes; and 3) the potential causal connection between the two health issues. The Short Sleep Undermines Cardiometabolic Health (SLUMBRx) Study seeks to contribute to the development of a comprehensive adiposity-sleep model, while laying the groundwork for a future program of research that will be designed to prevent and treat adiposity and sleep-related cardiometabolic disease risk factors. OBJECTIVE SLUMBRx addresses four topics pertinent to the adiposity-sleep hypothesis: 1) the relationship between adiposity and sleep duration; 2) sex-based differences in the relationship between adiposity and sleep duration; 3) influence of adiposity indices and sleep duration on cardiometabolic outcomes; and 4) the role of socioecological factors as effect modifiers in the relationship between adiposity indices, sleep, and cardiometabolic outcomes. METHODS SLUMBRx will employ a large-scale survey (n=1,000) that recruits 159 participants (53 normal weight, 53 overweight, and 53 obese) to be assessed in two phases. RESULTS Phase 1, a lab-based study, will gather objective adiposity indices (air displacement plethysmography and anthropometrics) and cardiometabolic data (blood pressure, pulse wave velocity and pulse wave analysis, and blood-based biomarker). Phase 2, a one-week, home-based study, will gather sleep-related data (home sleep testing/sleep apnea, actigraphy, sleep diaries). During Phase 2, detailed demographic and socioecological data will be collected to contextualize hypothesized adiposity and sleep-associated cardiometabolic disease risk factors. Collection and analyses of these data will yield information necessary to customize future observational and intervention research. CONCLUSIONS Precise implementation of the SLUMBRx protocol promises to provide objective, empirical data on the interaction between body composition and sleep duration. The hypotheses that will be tested by SLUMBRx are important for understanding the pathogenesis of cardiometabolic disease and for developing future public health interventions to prevent its conception and treat its consequences. Introduction: Short sleep duration has been associated with increased risk of cardiovascular and cerebrovascular disease (CVD), cognitive impairment (CI) and mortality. However, the role of sleep duration in predicting mortality in the context of CVD and CI is still not well-understood. Hypothesis: Short sleep duration is a key effect modifier of the relationship between CI associated with CVD and all-cause mortality. Methods: We addressed this hypothesis in the Penn State Adult Cohort, a random, general population sample of 1,741 middle-aged adults who were studied in the sleep lab and followed-up for 15y. An in-lab, 8-hour polysomnography was performed to ascertain sleep duration. CI associated with CVD was defined by the presence of hypertension, diabetes, heart disease and/or stroke with impaired higher-order, executive cognitive functioning, including slow processing speed. We tested the interaction between sleep duration and CI associated with CVD on all-cause mortality with multiple logistic regression while adjusting for sex, age, race, obesity, smoking, cholesterol, depression, insomnia, dementia, and sleep apnea. Results: The odds of mortality associated with CI-alone, CVD-alone, and CI associated with CVD were 1.3 (95% CI: 0.7-2.4), 1.7 (95% CI: 1.1-2.8), and 4.6 (95% CI: 2.8-7.7), respectively. As shown in Figure 1, the interaction between CI associated with CVD and sleep duration was significant (p < .01), indicating that the probability of mortality increased significantly as a function of shorter sleep duration in individuals with CI associated with CVD. Conclusion: We found that objective sleep duration modifies the relationship between CI associated with CVD and all-cause mortality in a dose-response manner. Short sleep duration in individuals with probable vascular cognitive impairment (VCI) may serve as a biomarker of the severity of central autonomic dysfunction. Future studies should examine whether improving sleep reduces the odds of mortality in individuals with VCI.

Background. Getting a sufficient amount of sleep is an important component of living a healthy lifestyle. Consequently, it is important for researchers to identify the factors that influence sleep duration. Aims. The current study has a twofold objective. The first is to see if two types of social support are associated with sleep duration. The second objective is to see if meaning in life and depressive symptoms serve as indirect pathways in the relationship between social support and sleep. Method. The data come from an internet survey of a random probability sample of adults who reside in the United States ( N = 2,287). Questions were administered on received support, satisfaction with support, meaning in life, depressive symptoms, and sleep. Results. The findings indicate that the amount of support that is received is associated with satisfaction with support, greater satisfaction with support is associated with a stronger sense of meaning in life, a stronger sense of meaning is related to fewer depressive symptoms, and fewer depressive symptoms is significantly associated with the likelihood of getting the recommended number of hours of sleep. Discussion and Conclusions. Instead of merely showing that social support is associated with sleep, our findings take a modest step toward explaining how this relationship arises (i.e., through an increased sense of meaning in life and, in turn, reduced depressive symptoms). This theoretical specificity is helpful for devising interventions to improve sleep habits.

Introduction: Short and long sleep durations are associated with heightened risk for cardiovascular disease and vascular risk factors. Elevated homocysteine is also associated with greater risk for cardiovascular disease; however, studies have yet to investigate the relationship between sleep duration and homocysteine. Hypothesis: We hypothesized that short and long sleep duration would be associated with clinical levels of homocysteine. Methods: Adults (n=2,469; ≥20y) from the 2005-2006 National Health and Nutrition Examination Survey (NHANES) were assessed for habitual sleep duration (coded as <5, 5, 6, 7, 8, 9, and ≥10hrs) and fasting plasma homocysteine levels (<10 [normative], 10 to <15 [pre-clinical] and ≥15 [clinical] μmol/L). Participants were excluded if pregnant, lactating, missing data on the primary variables, or if they had a history of cardiovascular disease, cancer, diabetes, kidney disease, or diagnosed sleep disorder. Population weighted, multinomial logistic regression analyses assessed the relationship between sleep duration and homocysteine after adjustment for age, sex, race/ethnicity, marital and menopausal status, shift work, dietary folate, alcohol intake, cotinine levels, reported physical activity, hypertension, and reported frequency of cessation of breathing at night. Results: Pre-clinical and clinical levels of homocysteine were present in 13.7% and 2.5% of the sample, respectively. The mean sleep duration was 6.9 ± 1.4 hours. In adjusted analyses, sleep duration was significantly related to homocysteine ( p < 0.001). See Table. Very short sleepers (<5hrs) were more likely to have clinical levels of homocysteine (OR: 3.01, 95%CI: 1.38, 6.57) compared to 7-hr sleepers. Conclusions: In a U.S. representative sample of adults without cardiovascular disease or other major conditions, short sleepers were at greater odds for clinical levels of homocysteine Findings suggest that homocysteine may be one mechanism linking short sleep duration to cardiovascular disease.
