Interrater and Intrarater Reliability and Validity of 3 Measurement Methods for Shoulder-Position Sense

Context: Joint-position sense (JPS) plays a critical role in the stability of shoulder joint. Restoration of JPS is essential to improve rehabilitation outcomes in individuals with shoulder injury. However, the number of affordable and reliable shoulder-JPS measurement methods for everyday clinical practice is limited. Objective:To estimate reliability and validity of 3 simple shoulder-JPS measurement methods. Design: Cross-sectional study. Participants: 25 healthy men and women. Main Outcome Measure: Absolute-error scores of JPS in 3 ranges of shoulder flexion (low, mid, and high), measured with a laser pointer, an inclinometer, and a goniometer in 2 separate sessions (48 h apart). Results: Overall interrater and intrarater intraclass correlation coefficients were .86 and .78 for the laser pointer, .67 and .70 for the inclinometer, and .60 and .50 for the goniometer, respectively. There was excellent reliability in the low range for the laser pointer and inclinometer methods, but fair to good and poor reliability in mid- and high ranges, respectively. All methods showed strong validity. Conclusion: The laser pointer and inclinometer JPS measurement methods are reliable and can be used by clinicians during rehabilitation of shoulder injuries.

Context: The prevalence of shoulder pain among competitive swimmers is high, but no guidelines exist to reduce shoulder injuries. Elucidating differences between swimmers with and without shoulder pain can serve as a basis for the development of a program to prevent shoulder injury that might lead to pain and dysfunction. Objective: To determine whether physical characteristics, exposure, or training variables differ between swimmers with and without shoulder pain or disability. Design: Cross-sectional study. Setting: Multisite swimming centers. Patients or Other Participants: A total of 236 competitive female swimmers aged 8 to 77 years. Data Collection and Analysis: Participants completed the Penn Shoulder Score and underwent testing of core endurance, range of motion, muscle force production, and pectoralis minor muscle length and the Scapular Dyskinesis Test. Swimmers were grouped by age for analysis: ages 8 to 11 years (n = 42), 12 to 14 years (n = 43), 15 to 19 years (high school, n = 84), and 23 to 77 years (masters, n = 67). Comparisons were made between groups with and without pain and disability using independent t tests for continuous data and χ2 analyses and Fisher exact tests for categorical data. Results: Nine (21.4%) swimmers aged 8 to 11 years, 8 (18.6%) swimmers aged 12 to 14 years, 19 (22.6%) high school swimmers, and 13 (19.4%) masters swimmers had shoulder pain and disability. Differences that were found in 2 or more age groups between athletes with and without shoulder pain and disability included greater swimming exposure, a higher incidence of previous traumatic injury and patient-rated shoulder instability, and reduced participation in another sport in the symptomatic groups (P < .05). Reduced shoulder flexion motion, weakness of the middle trapezius and internal rotation, shorter pectoralis minor and latissimus, participation in water polo, and decreased core endurance were found in symptomatic females in single varying age groups (P < .05). Conclusions: Female competitive swimmers have shoulder pain and disability throughout their lives. Given that exposure and physical examination findings varied between athletes with and without substantial pain and disability, a program to prevent shoulder injury that might lead to pain and dysfunction appears warranted and might include exposure reduction, cross-training, pectoral and posterior shoulder stretching, strengthening, and core endurance training.

Context:A reduction in joint position sense (JPS) is sometimes a consequence of shoulder injury that may adversely affect the ability to maintain dynamic joint stability.Objective:To compare shoulder JPS between previously injured and noninjured judokas.Design:Cohort study.Participants:Twenty-nine noninjured subjects (10.93 ± 3.45 years) and eleven injured subjects (15.09 ± 3.39 years).Main Outcome Measures:JPS was tested at 45° and 80°of shoulder external rotation at 90° of abduction.Results:No signifcant difference in JPS was found between previously injured and noninjured judokas at either joint position.Conclusion:Despite evidence that JPS acuity decreases following shoulder injury, this study did not demonstrate a difference in average error between previously injured and noninjured judokas. Uncontrolled confounding factors, such as age and time since injury, may have affected the results. Sport-specifc shoulder joint loading patterns may also be an important factor that affects JPS.

Elevated plantar pressures are an important predictor of diabetic foot ulceration. The objective of this study was to determine which clinical examination variables predict high plantar pressures in diabetic feet. In a cross-sectional study of 152 male veterans with diabetes mellitus, data were collected on demographics, comorbid conditions, disease severity, neuropathy status, vascular disease, and orthopedic and gait examinations. Univariate predictors included height, weight, body surface area, body weight per square inch of foot surface area, bunion deformity, hammer toe, Romberg’s sign, insensitivity to monofilament, absent joint position sense, decreased ankle dorsiflexion, and fat pad atrophy. Variables that remained significantly associated with high plantar pressures (≥4 kg/cm2) in multivariate analysis included height, body weight per square inch of foot surface area, Romberg’s sign, and insensitivity to monofilament. These results may be useful in identifying patients who would benefit from interventions designed to decrease plantar foot pressures. (J Am Podiatr Med Assoc 93(5): 367-372, 2003)

Abstract Background: Assessment of shoulder proprioception outside the laboratory in the absence of specialized equipment remains a challenge with field-based tests often lacking good reliability, validity and feasibility. This study aimed to enhance the knowledge base surrounding the assessment of shoulder proprioception and investigated the effect of fatigue on shoulder joint position sense (JPS) amongst amateur male handball players.Method: 27 healthy recreationally active participants and 13 amateur male handball players undertook two sessions of active JPS tests using laser pen technology and a calibrated 2-dimensional target to assess test-retest reliability. The active JPS test was then utilized on the subgroup of handball players who were subjected to five bouts of a repeated throwing task in order to investigate the effect of local fatigue on shoulder JPS. Results: The intraclass correlation coefficient for the active JPS test was 0.78 (95% CI = [0.57; 0.89]). Standard error of measurement between trials was 0.70° (range 0.57°-0.90°). For the throwing task, repeated measures analysis of variance revealed a significant interaction for arm x bout (F5=2.74, p=0.028) and a significant effect for arm (F1=5.85, p=0.034). Post hoc analysis showed a significant difference between throwing arm and non-throwing arm after throwing bout one (p=0.036), three (p=0.026) and four (p=0.041). Conclusion: Assessment of the active JPS test showed acceptable reliability and measurement error. Repeated throwing to fatigue decreased shoulder JPS in amateur male handball players which indicate validity of the test to identify reduced proprioception.

Objective: The main goal of this study was to exemplify the development of a measure of continuity of care (COC) from inpatients’ perspective. This measure is focused on several aspects related to physiotherapy.Methods: A cross-sectional self-report based psychometric study was carried out in a public hospital in southeast Spain. One hundred and fifty two patients with neurological and orthopaedic disorders who received rehabilitation care during stay at hospital were included in the study. A self-report questionnaire was used to examine experiences of patients related to the three types of COC, relational, management and informational continuity. The questionnaire also includes questions about sociodemographic characteristics, patient/therapist affiliation and trust with therapist. To examine reliability were used test-retest and internal consistency. For validation analysis, there were used convergent and known group strategies.Results: Of the 19 indicators included, 13 were selected to demonstrate adequate reliability and validity. From these indicators were generated three composite measures (Relational, management and COC index) and one individual measure (Informational continuity). Test-retest reliability indicated excellent agreement (intraclass correlation coefficient [ICC] > 0.75) for the three indexes. The range of Cronbach’s value was from 0.60 to 0.73. Total scores of all the indexes were moderately correlated with the satisfaction scale (r > 0.30). Regarding the known groups, all indexes scores were similar for men and woman. However, significant differences were found for management index and for relational index, based on trust with therapist and patient/therapist affiliation, respectively.Conclusions: The continuity self-reported measures is a valid and reliable method to assessing the COC in hospitalized patients receiving physiotherapy.

Purpose: To analyze limbs joint positions’ sense and its relationship with pain intensity and duration in patients with chronic non-specific back pain. Methods: Study design: cross-sectional study. For assessment of patients, the following elements were used: general data collection protocol; evaluation of pain intensity using the Visual Analogue Scale; assessment of limb kinesthesia: determination of reposition precision of a 90-degree flexion angle in shoulder, elbow, hip and knee joints. Subjects. The study included 100 patients (88 women, 12 men) with chronic non-specific back pain, who met the selection criteria for the study. The average age of patients was 45.9 ± 11.6 years, and it ranged from 19 to 64 years. Results: All participants of the study were found to have diminished upper and lower limbs kinesthesia. The study results showed plausible (p < 0.05) relation between limbs kinesthesia and pain intensity and duration, and these correlations were significantly determined by pain localization. Conclusion: Limb kinesthesia is plausibly related to the manifestation of pain in patients with chronic non-specific back pain.

Context: Knee joint-position sense (JPS) is a key parameter for optimum performance in many sports but is frequently negatively affected by injuries and/or fatigue during training sessions. Although evaluation of JPS may provide key information to reduce the risk of injury, it often requires expensive and/or complex tools that make monitoring proprioceptive deterioration difficult. Objective: To analyze the validity and reliability of a digital inclinometer to measure knee JPS in a closed kinetic chain (CKC). Design: The validity and intertester and intratester reliability of a digital inclinometer for measuring knee JPS were assessed. Setting: Biomechanics laboratory. Participants: 10 athletes (5 men and 5 women; 26.2 ± 1.3 y, 71.7 ± 12.4 kg; 1.75 ± 0.09 m; 23.5 ± 3.9 kg/m2). Intervention: Knee JPS was measured in a CKC. Main Outcome Measures: Absolute angular error (AAE) of knee JPS in a CKC. Results: Intraclass correlation coefficient (ICC) and standard error of the mean (SEM) were calculated to determine the validity and reliability of the inclinometer. Data showed that the inclinometer had a high level of validity compared with an isokinetic dynamometer (ICC = 1.0, SEM = 1.39, P < .001), and there was very good intra- and intertester reliability for reading the inclinometer (ICC = 1.0, SEM = 0.85, P < .001). Compared with AutoCAD video analysis, inclinometer validity was very high (ICC = 0.980, SEM = 3.46, P < .001) for measuring AAE during knee JPS in a CKC. In addition, the intertester reliability of the inclinometer for obtaining AAE was very high (ICC = .994, SEM = 1.67, P < .001). Conclusion: The inclinometer provides a valid and reliable method for assessing knee JPS in a CKC. Health and sports professionals could take advantage of this tool to monitor proprioceptive deterioration in athletes.

The Mindful Eating Questionnaire is a reliable tool for the assessment of mindful eating behavior among the general population. This study aimed to determine the reliability and validity of The Malay Mindful Eating Questionnaire (MEQ-M) in a sample of overweight and obese adults. This is a cross-sectional survey which involved 144 overweight and obese adults in a selected public university. After linguistic validation of the Malay version of the MEQ, exploratory factor analysis (EFA) with varimax rotation was performed on the scale constructs. The psychometric properties of the MEQ were assessed through Cronbach’s alpha and intraclass correlation coefficient (ICC) analysis. The EFA of the MEQ produced a seven-dimensional model (58.8% of overall variances). The concurrent validity analysis between total MEQ scores and total Mindfulness Attention Awareness Scale (MAAS) scores indicated a weak non-significant correlation (p = 0.679). The internal consistency reliability of the MEQ was reasonable (Cronbach’s α = 0.64). The agreement stability of the MEQ over eight weeks was poor (ICC = 0.10). In conclusion, the psychometric properties of the Malay-translated MEQ are acceptable through construct validity and internal consistency reliability tests. This instrument may be used for assessing mindful eating habits in the Malaysian population, especially among overweight and obese adults.
