The occurrence of carbapenemase-producing Proteus mirabilis , especially those strains producing NDM-1 and its variants, is a major public health concern worldwide. The integrative conjugative element (ICE) plays an important role in horizontal acquisition of resistance genes.

Global transmission of plasmid-mediated tigecycline resistance gene tet (X)-bearing Escherichia coli strains incurs a public health concern. However, the research focusing on the prevalence of tet (X)-positive isolates in clinical specimens is still rare, and to our knowledge, there is no such report from South Asia.

ABSTRACTThe acquisition of resistance to cephalosporins amongSalmonellaspp. is a major public health concern. This study identified clonal plasmids carryingblaTEM-52from 10Salmonella entericaserovar Infantis and Manhattan isolates from retail chicken meats that originated from a common supplier in Japan. Whole-genome analyses of the representative plasmids, including pYM4, revealed that they are 38 kb in size and that pYM4 is identical to pDKX1 from beef in Denmark, suggesting a global dissemination of resistance mediated by the plasmids.

PurposeThe purpose of this viewpoint is to discuss and analyze three major governance tools that have been implemented in the United States to target tobacco smoking as a major public health concern.Design/methodology/approachThe author highlights the negative consequences of smoking as a global and U.S public health concern and discusses three categories of governance tools implemented in the U.S. Additionally, emerging challenges in the U.S. and different sides of story in developing countries are underscored.FindingsAlthough some success has been reached in controlling smoking-related mortalities and morbidities in the U.S. and most of the countries, long-term and sustainable improvement require active surveillance and constant implementation of evidence-based policies and programs.Practical implicationsThis viewpoint discusses the governance tools that can be implemented to decrease smoking-related preventable mortalities and morbidities. Similar tools with some tuning can be used to target smoking in other nations. Additionally, these tools can be modified to target other public health-related wicked problems such as obesity, alcohol consumption, and traffic accidents.Originality/valueThis viewpoint highlights the magnitude of smoking as a major public health concern and underscores the necessity of using governance tools in targeting this issue. Additionally, it provides application examples from the United States implementable in other countries with some contextual justifications and tuning.

ABSTRACTYersinia pestisis the causative agent of plague, a fulminant disease that is often fatal without antimicrobial treatment. Plasmid (IncA/C)-mediated multidrug resistance inY. pestiswas reported in 1995 in Madagascar and has generated considerable public health concern, most recently because of the identification of IncA/C multidrug-resistant plasmids in other zoonotic pathogens. Here, we demonstrate no resistance in 392Y. pestisisolates from 17 countries to eight antimicrobials used for treatment or prophylaxis of plague.

ABSTRACTShigellaspp. represent the second most common etiologic pathogen causing childhood diarrhea in developing countries. There are no licensedShigellavaccines, and progress for such vaccines has been limited. In this issue ofClinical and Vaccine Immunology, Riddle and colleagues (M. S. Riddle, R. W. Kaminski, C. Di Paolo, C. K. Porter, R. L. Gutierrez, et al., Clin Vaccine Immunol 23:908–917, 2016,http://dx.doi.org/10.1128/CVI.00224-16) report results from a phase I study of a parenterally administered monovalent O-polysaccharide “bioconjugate” directed againstShigella flexneri2a. Ultimately, the goal is to develop a broad-spectrumShigellavaccine to address this public health concern. A parenteralShigellavaccine capable of eliciting protection in children of developing countries would be an important tool to reach this goal.

ABSTRACT A novel 139,487-bp SXT/R391 integrative and conjugative element, ICEPmiChnBCP11, was characterized in Proteus mirabilis of swine origin in China. ICEPmiChnBCP11 harbors 20 different antimicrobial resistance genes, including the clinically important rRNA methyltransferase gene cfr, the extended-spectrum β-lactamase gene blaCTX-M-65, fosfomycin resistance gene fosA3, and fluoroquinolone resistance gene aac(6′)-Ib-cr. An ISPpu12-mediated composite transposon containing various resistance genes and 10 copies of IS26 is inserted in hot spot 4. ICEPmiChnBCP11 was successfully transferred to Escherichia coli.
