Pulling our lens backwards to move forward: an integrated approach to physician distress

The medical community has recently acknowledged physician stress as a leading issue for individual wellness and healthcare system functioning. Unprecedented levels of stress contribute to physician burnout, leaves of absence and early retirement. Although recommendations have been made, we continue to struggle with addressing stress. One challenge is a lack of a shared definition for what we mean by ‘stress’, which is a complex and idiosyncratic phenomenon that may be examined from a myriad of angles. As such, research on stress has traditionally taken a reductionist approach, parsing out one aspect to investigate, such as stress physiology. In the medical domain, we have traditionally underappreciated other dimensions of stress, including emotion and the role of the environmental and sociocultural context in which providers are embedded. Taking a complementary, holistic approach to stress and focusing on the composite, subjective individual experience may provide a deeper understanding of the phenomenon and help to illuminate paths towards wellness. In this review article, we first examine contributions from unidimensional approaches to stress, and then outline a complementary, integrated approach. We describe how complex phenomena have been tackled in other domains and discuss how holistic theory and the humanities may help in studying and addressing physician stress, with the ultimate goal of improving physician well-being and consequently patient care.

The pandemic caused by COVID-19 has left few countries untouched. It is a far-reaching implication on humankind, with children and adolescents, being no exception. Although the prevalence and fatality are negligible among children, a possible impact on their psychological and mental health cannot be disregarded. The unprecedented change in the way of living is bound to be having some psychological consequences on children and adolescents. The experiences gathered in childhood and adolescence are known to contribute to shaping the physical, emotional, and social well-being in adult life. Children are highly susceptible to environmental stressors. The present situation has the potential of adversely affecting the physical and mental well-being of children. To save the children from the long term consequences of this pandemic, a holistic approach integrating biological, psychological, social and spiritual methods of enhancing mental health have become essential. A concerted effort of government, Non Government Organisations (NGOs), parents, teachers, schools, psychologists, counselors and physicians are required to deal with the mental health issues of children and adolescents. This paper discusses the possible role of these agencies in the holistic intervention of this crisis.

Yoga is a holistic approach and it is quite difficult to understand the specific effects of Yoga for its many aspects such as Asana, Pranayama, Meditation, etc. Inspite of these challenges, scientific study on Yoga has been grown optimally. Several studies have shown the positive outcome of Yoga on various psychological and physiological parameters such as stress, anxiety, depression, memory, aggressiveness, etc. ( , ). Prison population is vulnerable to different psychiatric morbidities and often represent antisocial behavior ( , ). A randomized control trial has shown a significant improvement in perceived stress, positive affect, and psychological distress in prisoners after 10 weeks of Yoga practice ( ). A few meta-analysis has also suggested the positive effect of Yoga and meditation on psychological well-being and behavioral functioning in the prison population. The study has revealed that 10 weeks Yoga was associated with increased positive affect, impulse control, psychological well-being, sleep quality, and decreased aggressiveness, stress, and negative affect.

This brief review on perceived control and aging is organized according to 3 perspectives of research involving description, explanation, and modification. An extensive body of literature has utilized cross-sectional and correlational methods to describe the sociodemographic variations and outcomes associated with perceived control. This work has focused on differences in perceived control as a function of age, sex, education, socioeconomic status, and culture and has identified positive associations with many aging-related outcomes involving health and well-being. With growing evidence regarding the health benefits of perceived control in the context of a declining sense of control with aging, there has been an increased effort to uncover the mechanisms involved, with the hopes of developing methods to maintain and/or promote adaptive control beliefs throughout adulthood. Through longitudinal and experimental work, researchers are beginning to clarify the directionality and elucidate the mechanisms to explain the associations. Recent evidence from longitudinal studies shows that control beliefs have an impact on subsequent changes in health. Yet, the findings suggest that it is not a unidirectional relationship. A conceptual model suggesting an ongoing reciprocal relationship between perceived control and health and well-being is discussed. Research examining the mechanisms that link perceived control to aging-related outcomes can help to inform and to develop effective interventions that are tailored to the individual's specific barriers and goals. We consider new directions for research, including more attention to intraindividual variability and reactivity to daily challenges, such as stress, with the goal of advancing our understanding of how perceived control contributes to aging-related outcomes. More work is needed to develop strategies to enhance control beliefs in later life. Although it will not always be possible to modify control beliefs, researchers can take these beliefs into account when developing interventions. A personalized approach is recommended as a way to tailor interventions that are compatible with individuals' beliefs about control to facilitate adaptive behavior change. Conclusions focus on selected issues and considerations for future research.

The global disconnect between the Sustainable Development Goals (SDGs) and the Convention on the Rights of the Child (CRC), has been described as ‘a missed opportunity’. Since devolution, the Welsh Government has actively pursued a ‘sustainable development’ and a ‘children’s rights’ agenda. However, until recently, these separate agendas also did not contribute to each other, although they culminated in two radical and innovative pieces of legislation; the Rights of Children and Young Persons (Wales) Measure (2013) and the Well-being and Future Generations (Wales) Act (2015). This article offers a case study that draws upon the SDGs and the CRC and considers how recent guidance to Welsh public bodies for implementation attempts to contribute to a more integrated approach. It suggests that successful integration requires recognition of the importance of including children in deliberative processes, using both formal mechanisms, such as local authority youth forums, pupil councils and a national youth parliament, and informal mechanisms, such as child-led research, that enable children to initiate and influence sustainable change.

The gut microbiome is vital for normal human body functioning. The etiological and pathogenetic significance of increased intestinal permeability in disorders of various organs and systems seems to be certain. The term “microbiota-gut-brain axis” has been defined; the crucial role of the microbiota-gut-brain axis in neurological disorders has been confirmed. Gut microbiome not only contributes to digestion, metabolism and immunity, but also mediates sleep and mental health of the host via microbiota-gut-brain axis. Such elements as zinc and selenium are essential to maintain the microbial balance in the gut. Zinc contributes to gut homeostasis and Paneth cell function. Zinc has a direct impact on gut microbiota composition (for example, on some species of Staphylococcus), modulates gut microbiota reducing the fatal entry of bacteria into the bloodstream and lymphatic vessels. Thus, zinc alters microbiome due to direct cytotoxic / cytostatic effect on certain bacteria, such as staphylococci. Zinc possesses therapeutic effect in gastrointestinal infections and diarrhea. Bacterial translocation may be also reduced with Rebamipide possessing cytoprotective and antioxidant activity. Selenium in the form of selenoproteins has a number of functions in normal health and metabolism. Selenium contributes to immune system functioning and to progression of HIV to AIDS. Selenium deficiency results in cardiovascular diseases, infertility, myodegenerative disorders, and cognitive decline.

The well-being concept refers to other similar concepts related to people’s physical, mental, emotional and intellectual well-being. In broader terms, in addition to internal dimensions, it must include external dimensions as well. In discussing the concept of wellbeing, it is worth using a holistic approach – implementing this approach in practice allows for achieving measurable organisational effects.

The specific elements in the ontological planning of the social require the holistic approach of the social process, but also of the educational ones. The whole-part dichotomy as a relationship of complementarity raises the issue of the specificity of learning integration, starting from the educational purpose, marked by the formation of key competencies, recorded by knowledge, skills, attitudes. The current status of integration, of the action to make something full, complete, very complex, generates a series of renovations, including the issue of this article, focused on the idea of the need for a model of bio- (or eco-) functional integration, designating a useful process for the student in the sense of his real life or his concrete environment.

Sleep is essential component of life. Even though the research in this field develops constantly, there are still many aspects of this rather complex process that remains to be fully clarified. One of these aspects, reason why we actually sleep, is perhaps the most crucial. In this mini review we aim to address this question and discuss potential functions of sleep. Many recent scientific papers are currently available that covers similar topic. We tried to summarize these recent findings. There are certainly many ways how to approach this rather complex issue. Our article will specifically focus on role of sleep in neuronal development, synaptic plasticity, memory consolidation or mental health in general. Its role in immune system functioning will also be mentioned. Moreover, we will also consider more general functions of sleep, such as well-being of the organisms or securing survival of the individual. In conclusion, we will highlight possible main function of sleep.

Recent research shows that civilians who work with the military in war zones are often exposed to life-threatening situations that can create psychological distress. In this study, we examine whether cohesion buffers the relationship between threat and psychological distress. Using a probability sample of civilians working with the U.S. Army in Iraq and Afghanistan, we find that cohesion buffers the relationship between threat and both internalizing and externalizing forms of emotional distress, but does so nonlinearly, with buffering observed at moderate but not high levels of cohesion. This research shows that cohesion may be an important resource for the mental health of civilians working in war zones but also supports sociological theory positing that the utility of social resources for individual well-being may be obviated in tightly integrative social contexts.
