OBJECTIVE: To analyze the association of characteristics recorded at the time of birth, including weight, occurrence of asphyxia, gestation duration, maternal age and education level, with death from diseases or malformations of the circulatory system in children under 18 years of age. METHODS: The Brazilian Information System on Live Births and Information System on Mortality databases were linked and evaluated following a longitudinal cohort analysis strategy. The following independent variables were evaluated: characteristics recorded at the time of birth, including weight, occurrence of asphyxia, gestation duration, maternal age and education level. Dependent variables were death from diseases or malformations of the circulatory system in children under 18 years of age. Crude relative risks were estimated and relative risks were adjusted for the variables. RESULTS: 6,380 deaths were linked to 4,282,260 birth records, yielding 5,062 pairs considered as true. Low birth weight (RR = 2.26), asphyxia at 1 (RR = 1.72) and 5 minutes (RR = 1.51), prematurity (RR = 1.50), maternal age ≥ 40 years (RR = 2.06), and low maternal education level (RR = 1.45) increased the probability of death caused by circulatory system diseases. In the association with death by malformations of the circulatory system, the predictive variables showed the same association profile, but with greater intensity. CONCLUSIONS: Fetal and maternal factors are associated with increased mortality due to diseases and malformations of the circulatory system. Measures to control these factors and improve access to their diagnosis and treatment would contribute to reducing the number of deaths caused by diseases and malformations of the circulatory system. However, the identification of environmental influences during gestation and birth on the risk of death should be carefully considered due to being influenced by genetic factors.

Abstract Background Heat waves are associated with incresed mortalitiy, especially in old-age population. Methods We estimated relative risks for number of deaths, for the observed diagnoses, sex, and area, as well as 95% confidence intervals and excess deaths associated with heat waves occurring in the years from 2013 to 2017, for population 75+ years old. Results Statistically significant were: in 2014: all causes of deaths (RR = 1.27, CI 1.12-1.44); male, all causes of deaths (RR = 1.27, CI 1.04-1.56); female, all causes of deaths (RR = 1.26, CI 1.08-1.48); all, circulatory system diseases (RR = 1.19, CI 1.01-1.43); female, circulatory system diseases (RR = 1.23, CI 1.01-1.51); rural area (RR = 1.28, CI 1.09-1.51); urban area (RR = 1.35, CI 1.11-1.64); and in 2015: all causes of deaths (RR = 1.28, CI 1.15-1.41); male, all causes of deaths (RR = 1.27, CI 1.08-1.5); female, all causes of deaths (RR = 1.27, CI 1.12-1.45); all, circulatory system diseases (RR = 1.27, CI 1.1-1.48); male, circulatory system diseases (RR = 1.38, CI 1.06-1.79); female, circulatory system diseases (RR = 1.23, CI 1.03-1.47); rural area (RR = 1.31, CI 1.16-1.47). No significant association in 2013, 2016 and 2017 appeared. Conclusions Despite increasing heat load in observed years, we did not notice a statistically significant association between heat waves and the number of deaths in the last observed years 2016 and 2017. The results indirectly show that we are on good way with our public health efforts concerning awareness of Slovenian people about the dangers heat waves bring. Key messages Heat waves will become more intense, will last longer and will appear more frequent. We must take care after old age population. Chronic noncommucating diseaes increase and life expectancy also increases, so the burden of deaths during heat waves is expected to increase too.

Objectives: to identify the main causes for hospital admissions and deaths related to systemic arterial hypertension and diabetes mellitus (DM), and to analyze morbidity and mortality trends, in a municipality in São Paulo's countryside, by comparing two three-years periods, 2002 to 2004 and 2010 to 2012. Methods: cross-sectional study which used secondary data regarding deaths from the Information System on Mortality and concerning hospital admissions from the DataSus Hospital Information System. Univariate and multivariate statistical analyses were conducted. Results: from 2002 to 2012, 325,439 people were admitted to hospitals, 14.7% of them due to circulatory system diseases (CSD) and 0.7% due to DM. The deaths distributed as the following: 29,027 deaths (31.5%) were due to CSD; 8.06% due to cerebrovascular diseases (CVD); and 2.75% due to DM. There was a significant association between admittance and death causes and patients' gender and age in the three-year periods (p<0.001). The highest lethality in hospital admissions was found to be due to CVD (10%). That trend showed that mortality rates dropped, younger patients were admitted due to DM, and older patients were admitted due to CVD - they were more often females. Conclusion: the main causes for hospital admissions were the CSDs; main mortality causes were the CVDs in hypertensive and diabetic women. Those findings can back public policies which prioritize the promotion of health.

Introduction. The article considers an approach to organization of research activity using medical information system. Materials and methods. Authors carried out retrospective analytical cohort study of adults with temporary incapacity to work based on the data from the State Medical Institution Galichskaya District Hospital (Kostroma Region of the Russian Federation) for a period 2014–2019. Sampling was done using the medical information system. Results. As the medical information system has been operating for many years and is designed in the way that the key information gaps should be filled, we consider the results of our study on research activity in a polyclinic using medical information system successful. Discussion. The organization of research activity is discussed using results of an epidemiological study aimed at identifying the correlation between the incidence of acute respiratory diseases and further development of circulatory system diseases carried out in Galichskaya District Hospital.

ABSTRACTSub National Immunization Week (PIN) Diphtheria is an additional immunization activity simultaneously in 19 regencies/ cities as an attempt to cope with outbreak of diphtheria in East Java. Sub PIN Diphtheria coverage result in 2012 in the Sukolilo Village, Surabaya in children(2–36 months) did not fulfill the target because there are many children(2–36 months) that did not participate. This study was conducted to analyze the factors that affect mother to the children(2–36 months) exclusion in Sub PIN Diphtheria in the Sukolilo Village, Surabaya 2013, include: maternal age, maternal education level, maternal employment, number of children, mother’s level of knowledge, maternal attitudes, husband support, community support, and exposure information. This study conducted with observational analytic approach and case control design. Large sample of 90 mothers drawn by simple random sampling. Statistical analysis is used multiple logistic regression with α < 0.05. The results showed that the variables that affect the exclusion of mothers to immunize children(2–36 months) in Sub PIN Diphtheria sequentially are less maternal attitude (p = 0.000), maternal less knowledge (p = 0.005), the husband does not support (p = 0.047), and were not informed (p = 0.018). Whereas maternal age, maternal education level, maternal employment, number of children, and the community support has no effect because the p value < 0.05. Therefore it is necessary counseling and motivation to the mother, socialization to the husband, and optimize health promotion media in order to minimize the exclusion Sub PIN Diphtheria future.Keywords: children (2–36 months), diphteria, immunization, knowledge,                         attitude

Introduction. From 1960 to 2018, 282 people were enrolled in cosmonaut groups in the USSR and Russia. Extreme special training and working conditions could lead to health disorders, which may be reflected in the causes and rates of mortality in the future. The goal of this work is to study the causes and rates of mortality in two cohorts of cosmonauts: those with and without spaceflight experience. The study aim is to research the causes and mortality rates of astronauts who have made and did not fly into space. Methods. A cohort study of mortality of 263 male cosmonauts was conducted, 118 cosmonauts with spaceflight experience formed cohort 1, 145 cosmonauts without it - cohort 2. The follow-up period was 59 yrs. (01.01.1960-31.12.2018) with 8351,4 person-years obtained. The reference group for both cohorts was the male population of Russia, cohort 2 was also the reference group for the cohort 1. Mortality risk was assessed using standardized mortality ratio (SMR) with 95% confidence interval (95% CI). Results. Death risk from all causes (A00-Y98) for each cohort was significantly lower than that for the male population (SMR=0,39, 95% CI 0,28-0,54 for cohort 1; SMR=0,53, 95% CI 0,41-0,69 for cohort 2). Moreover, the risk of death for cosmonauts with spaceflight experience is also lower than for cosmonauts without it (SMR=0,66, 95% CI 0,46-0,91). The most common causes of death in both cohorts were circulatory system diseases, and cosmonauts without spaceflight experience died at a younger age. Conclusion. Further research is required to understand the true influence of extreme working conditions on cosmonauts’ health, especially in the long-term period.

Abstract Background Rare diseases (RDs) encompass a heterogeneous group of life-threatening or chronically debilitating conditions that individually affect a small number of subjects but overall represent a major public health issue globally. There are still limited data on RD burden due to the paucity of large population-based epidemiological studies. The aim of this research was to provide survival estimates of patients with a RD residing in Tuscany, Italy. Methods Cases collected in the Rare Diseases Registry of Tuscany with diagnosis between 1st January 2000 and 31th December 2018 were linked to the regional health databases in order to retrieve information on mortality of all subjects. Survival at 1, 5 and 10 years from diagnosis with 95% confidence intervals (CI) was estimated by sex, age class, nosological group and subgroup using the Kaplan–Meier method. The effect of sex, age and period of diagnosis (years 2000–2009 or 2010–2018) on survival was estimated using Cox proportional hazards regression. Results Survival at 1, 5 and 10 years from diagnosis was 97.3%, 88.8% and 80.8%, respectively. Respiratory diseases and peripheral and central nervous system disorders were characterized by the lowest survival at 5 and 10 years. Despite a modest higher prevalence of RDs among females (54.0% of the total), male cases had a significant increased risk of death (hazard ratio, HR 1.48, 95% CI 1.38–1.58). Cases diagnosed during 2010–2018 period had a risk of death significantly lower than those diagnosed during 2000–2009 (HR 0.81, 95% CI 0.82–0.96), especially for immune system disorders (HR 0.48, 95% CI 0.26–0.87), circulatory system diseases (HR 0.61, 95% CI 0.45–0.84) and diseases of the musculoskeletal system and connective tissue (HR 0.64, 95% CI 0.49–0.84). Conclusions An earlier diagnosis as well as the improvement in the efficacy of treatment resulted in a decreased risk of death over the years for specific RDs. The linkage between a population-based registry and other regional databases exploited in this study provides a large and accurate mass of data capable of estimating patients’ life-expectancy and increasing knowledge on the collective burden of RDs.

Our research project was aimed at studying the effects of an electromagnetic field of industrial frequency (EMF-IF) on employees of an energy company in Kazakhstan. Material and Methods — The object of our study was the health status of electricians (morbidity with temporary disability – MTD), engaged in the maintenance of power lines, relay protection systems and substations (220 and 500 kV) at an energy enterprise in Kazakhstan. The interrelation and dependence of the intensive MTD indicators on the hygienic factors at the workplace were determined, and the risks were calculated from the obtained data. Results — Unfavorable workplace conditions caused an increase in disorders of the musculoskeletal system (up to 77%), blood circulation (up to 65%), nervous system (up to 52%), skin diseases (up to 46.4%), as well as the manifold rise of the likelihood of neoplasm growth and respiratory diseases. For electricians, the relationships between the nervous system disorders (r=0.792), the circulatory system diseases (r=0.573), the musculoskeletal system ailments (r=0.672) and the EMF-IF parameters were discovered. At the same time, the dependence of the incidence rates of various diseases in workers on EMF, as well as moderate to high computed relative risks, implied the occupational genesis of worklace ailments: for nervous system – R2=0.628, cardiovascular system – R2=0.709, skin – R2=0.729, and musculoskeletal system – R2=0.413. Conclusion — As preventive measures for electricians, we recommended to wear individual exposure meters, to limit work in contact with EMF, to include an oncologist in the medical commission, and for trainees, to screen for oxidative stress proteins and chaperone proteins to exclude a predisposition to oncogenesis.

Diseases of the circulatory system occupy a leading position among the diseases prevalence of which is increasing with each passing year and is sustainable. The leading reasons for the circulatory diseases as revealed by prophylactic medical examination are arterial hypertension, chronic heart failure, arrhythmia and coronary heart disease. The growth in the number of circulatory system diseasescauses medical and social problem - disability of the working-age population. In order to determine the role of Automated Control System the study of morbidity, disability and mortality due tocirculatory system diseaseswas conducted in the municipal polyclinic №4 of Aktobe city (West Kazakhstan). The method of retrospective data analysis of annual statistical reports and data from the portal "Electronic Register of dispensary patients", "Automated Information System", "Drug Supply Information System" was used. The incidence of circulatory system diseasesin dispensary patients of working age had risen from 1.94% (2011) to 2.8% (2015). In the main classes of primary disability the circulatory system diseasesin terms of growth steadily occupy the first place ranking.

Th e article presents results of analytic epidemiologic study of mortality in coal miners cohort in Rostov region, who had occupational disease registered in various years. The cohort included 9980 males, the observation period was 26 years (01/01/1990–31/12/2015). Deep analysis of mortality with leading death causes in subgroups of coal miners differentiated by length of service in underground conditions. Standardized relative risk of death in the subgroups increased with longer length of service, with respiratory diseases, circulatory system diseases and malignancies.The authors studied mortality in subcohorts of the miners with leading and auxiliary occupations, whose work conditions differ in intensity of exposure to occupational hazards. For the workers with leading occupations, standardized relative risk of death with respiratory diseases, circulatory system diseases and malignancies was higher.
