This study examined the factors associated with success or failure on probation for 2,850 North Carolina felony probationers who were removed from supervision between July 1 and October 31, 1993. Probationers were profiled on various demographic, sentence, and probation characteristics. Chi-square analysis was used to determine differences within various subgroups of the sample, and logistic regression was used to predict failure on probation in two separate models. The findings suggest that risk assessment items used by probation officers to determine level of supervision for probationers perform well in a logistic regression equation.

Background: Musculoskeletal injuries are a primary source of disability. Understanding how risk factors predict injury is necessary to individualize and enhance injury reduction programs. Hypothesis: Because of the multifactorial nature of musculoskeletal injuries, multiple risk factors will provide a useful method of categorizing warrior athletes based on injury risk. Study Design: Prospective observational cohort study. Level of Evidence: Level 2. Methods: Baseline data were collected on 922 US Army soldiers/warrior athletes (mean age, 24.7 ± 5.2 years; mean body mass index, 26.8 ± 3.4 kg/m2) using surveys and physical measures. Injury occurrence and health care utilization were collected for 1 year. Variables were compared in healthy versus injured participants using independent t tests or chi-square analysis. Significantly different factors between each group were entered into a logistic regression equation. Receiver operating characteristic curve and accuracy statistics were calculated for regression variables. Results: Of the 922 warrior athletes, 38.8% suffered a time-loss injury (TLI). Overall, 35 variables had a significant relationship with TLIs. The logistic regression equation, consisting of 11 variables of interest, was significant (adjusted R2 = 0.21; odds ratio, 5.7 [95% CI, 4.1-7.9]; relative risk, 2.5 [95% CI, 2.1-2.9]; area under the curve, 0.73). Individuals with 2 variables had a sensitivity of 0.89, those with 7 or more variables had a specificity of 0.94. Conclusion: The sum of individual risk factors (prior injury, prior work restrictions, lower perceived recovery from injury, asymmetrical ankle dorsiflexion, decreased or asymmetrical performance on the Lower and Upper Quarter Y-Balance test, pain with movement, slower 2-mile run times, age, and sex) produced a highly sensitive and specific multivariate model for TLI in military servicemembers. Clinical Relevance: A better understanding of characteristics associated with future injury risk can provide a foundation for prevention programs designed to reduce medical costs and time lost.

This study aims to describe the effect of sanctions (individual sanctions, collective sanctions, and absence of sanctions) on cooperative behavior of individuals with medium trust in the context of corruption. Both collective sanctions and individual sanctions, are systemic, which means sanctioning behavior is exercised not by each individual but by the system. Cooperative behavior in this context means choosing to obey rules, to reject acts of corruption and to prioritize public interests rather than the personal interests. Conversely, corruption is an uncooperative behavior to the rules, and ignores the public interest and prioritizes personal interests. Research subjects were 62 students. The Chi-Square Analysis was used to see the association between the variables and the logistic regression model was applied to describe the structure of this association. Individual sanction is recommended as punishment to medium trust individuals to promote cooperative behavior in the context of corruption. The results showed that individuals with medium trust had more cooperative behavior.

Strangulation has long been associated with death in the context of sexual assault and intimate partner violence (IPV). Non-fatal strangulation (NFS) during sexual assault, which refers to strangulation or choking that does not result in death, is common and has been associated with IPV and with bodily injury; however, other factors associated with NFS are unknown. The current study examined demographic and sexual assault characteristics associated with NFS among women who received a sexual assault medical forensic exam (SAMFE). A second purpose of this study was to explore factors associated with receiving follow-up imaging orders after NFS was identified during a SAMFE. Participants ( N = 882) ranged in age from 18 to 81 ( M = 28.85), with the majority identifying as non-Hispanic White (70.4%) or Black/African American (23.4%). A total of 75 women (8.5%) experienced NFS during the sexual assault. Of these, only 13 (17.3%) received follow-up imaging orders for relevant scans. Results from a logistic regression analysis demonstrated that NFS was positively associated with report of anal penetration, intimate partner perpetration, non-genital injury, and weapon use during the assault. Results from chi-square analysis showed that among sexual assaults involving women who experienced NFS, those whose assaults involved weapon use were over four times more likely to receive imaging orders compared to assaults without weapon use. These findings have implications for criminal justice, and if incorporated into danger assessments, could potentially reduce fatalities linked to sexual assault and/or IPV. Additional work is needed to ensure that all assaults with NFS trigger a referral for imaging regardless of other assault characteristics.

OBJECTIVE Lumbosacral fractional curves in adult spinal deformity (ASD) patients often have sharp coronal curves resulting in significant pain and imbalance. Postoperative stretch neuropraxia after fractional curve correction can lead to discomfort and unsatisfactory outcomes. The goal of this study was to use radiographic measures to increase understanding of the relationship between postoperative stretch neuropraxia and fractional curve correction. METHODS In 62 ASD patients treated from 2015 to 2018, radiographic review was performed, including measurement of the distance between the lower lumbar neural foramen (L4 and L5) in the concavity and convexity of the lumbosacral fractional curve and the ipsilateral femoral heads (FHs; L4–FH and L5–FH) in pre- and postoperative anteroposterior spine radiographs. The largest absolute preoperative to postoperative change in distance between the lower lumbar neural foramen and the ipsilateral FH (ΔL4/L5–FH) was used for analysis. Chi-square analyses, independent and paired t-tests, and logistic regression were performed to study the relationship between L4/L5–FH and stretch neuropraxia for categorical and continuous variables, respectively. RESULTS Of the 62 patients, 13 (21.0%) had postoperative stretch neuropraxia. Patients without postoperative stretch neuropraxia had an average ΔL4–FH distance of 16.2 mm compared to patients with stretch neuropraxia, who had an average ΔL4–FH distance of 31.5 mm (p < 0.01). Patients without postoperative neuropraxia had an average ΔL5–FH distance of 11.1 mm compared to those with stretch neuropraxia, who had an average ΔL5–FH distance of 23.0 mm (p < 0.01). Chi-square analysis showed that patients had a 4.78-fold risk of developing stretch neuropraxia with ΔL4–FH > 20 mm (95% CI 1.3–17.3) and a 5.17-fold risk of developing stretch neuropraxia with ΔL5–FH > 15 mm (95% CI 1.4–18.7). Logistic regression analysis indicated that the odds of developing stretch neuropraxia were 15:1 with a ΔL4–FH > 20 mm (95% CI 3–78) and 21:1 with a ΔL5–FH > 15 mm (95% CI 4–113). CONCLUSIONS The novel ΔL4/L5–FH distances are strongly associated with postoperative stretch neuropraxia in ASD patients. A ΔL4–FH > 20 mm and ΔL5–FH > 15 mm significantly increase the odds for patients to develop postoperative stretch neuropraxia.

The rate of micrometastatic disease (MMD) to nonsentinel lymph nodes (NSLNs) has been shown to vary considerably in the literature. We identified patients with breast cancer with MMD (N1mi) and measured the incidence of NSLN involvement. We then compared these patients with those who had no metastasis to the SLN (N0) and those who had macrometastasis to the SLN (N2) in an attempt to better understand the behavior of patients with N1mi positivity. A retrospective analysis was conducted on 574 patients with invasive breast cancer between January 2000 and December 2007. Patients were stratified into three groups: no metastasis (N0), MMD (N1mi), and macrometastasis (N2). Chi square analysis and logistic regression models using SPSS software were applied to determine significance between groups. MMD rate was 7.7 per cent (44 of 574). Of this subset of patients, 33 underwent completion axillary dissection, and only two were found to have NSLN-positive disease. Statistical significance was achieved for NSLN positivity when comparing all three nodal groups against one another (χ22,572 = 337.084, P = 0.000). Logistic regression showed multifocality and lymphovascular invasion to be significant predictors of NSLN metastasis. NSLN positivity in patients with MMD acts similarly to node-positive disease and therefore cannot completely exclude axillary dissection from therapeutic algorithm.

Introduction While patients with acute uncomplicated appendicitis may be treated conservatively, those who suffer from complicated appendicitis require surgery. We describe a logistic regression equation to calculate the likelihood of acute uncomplicated appendicitis and complicated appendicitis in patients presenting to the emergency department with suspected acute appendicitis. Materials and methods A cohort of 895 patients who underwent appendicectomy were analysed retrospectively. Depending on the final histology, patients were divided into three groups; normal appendix, acute uncomplicated appendicitis and complicated appendicitis. Normal appendix was considered the reference category, while acute uncomplicated appendicitis and complicated appendicitis were the nominal categories. Multivariate and univariate regression models were undertaken to detect independent variables with significant odds ratio that can predict acute uncomplicated appendicitis and complicated appendicitis. Subsequently, a logistic regression equation was generated to produce the likelihood acute uncomplicated appendicitis and complicated appendicitis. Results Pathological diagnosis of normal appendix, acute uncomplicated appendicitis and complicated appendicitis was identified in 188 (21%), 525 (59%) and 182 patients (20%), respectively. The odds ratio from a univariate analysis to predict complicated appendicitis for age, female gender, log2 white cell count, log2 C-reactive protein and log2 bilirubin were 1.02 (95% confidence interval, CI, 1.01, 1.04), 2.37 (95% CI 1.51, 3.70), 9.74 (95% CI 5.41, 17.5), 1.57 (95% CI 1.40, 1.74), 2.08 (95% CI 1.56, 2.76), respectively. For the same variable, similar odds ratios were demonstrated in a multivariate analysis to predict complicated appendicitis and univariate and multivariate analysis to predict acute uncomplicated appendicitis. Conclusions The likelihood of acute uncomplicated appendicitis and complicated appendicitis can be calculated by using the reported predictive equations integrated into a web application at www.appendistat.com. This will enable clinicians to determine the probability of appendicitis and the need for urgent surgery in case of complicated appendicitis.

Purpose This study was designed to assess the risk factors associated with hypertension (HTN) and diabetes among artisans in Ogun State, Nigeria. Evidences suggest increasing prevalence, incidences and morbidity of diabetes and HTN in Nigeria. However, the purpose of this study is to plan and prioritize effective intervention programs, there is need to provide data on the prevalence and risk factors for HTN and diabetes among local groups. Design/methodology/approach In total 300 apparently healthy artisans who have never been diagnosed of diabetes or HTN were randomly selected from five communities. A structured questionnaire was used in obtaining information on the personal characteristics of the respondents. An adapted dietary habit and lifestyle questionnaire were used to assess the dietary habits and lifestyle of the respondents. The WHO global activity questionnaire was adapted and used to gather information on the physical activity level of the respondents. Random blood glucose, blood pressure and anthropometric measurements were assessed using standard instruments. Chi-square (χ2), correlations and multinomial logistic regression analysis were performed to identify significant determinants of diabetes and HTN. Findings Mean age was 34.8 ± 9.9 and prevalence of diabetes and pre-diabetes were 1 and 4.7 per cent, respectively, while HTN and pre-HTN were 48.0 and 30.3 per cent, respectively. About half (55.7 per cent) of the respondents skip at least a meal daily and 31 per cent snack daily. Most (61.4 per cent) consume alcohol and 65.7 per cent engage in high physical activity. Abdominal obesity was significantly higher among women (p = 0.004). Using the chi-square analysis, age, abdominal obesity and educational status were factors found to be significantly associated with diabetes (p = 0.002; p = 0.007; p = 0.004) while age, gender, abdominal obesity and alcohol consumption had significant association with HTN. Although not statistically significant, respondents were 0.8, 1.0 and 1.1 times more likely to be diabetic with increasing body mass index, waist circumference (WC) and age (odd ratio (OR) = 0.78; confidence intervals (CI): 0.51-1.18; OR = 1.04; CI: 0.89-1.21; OR = 1.06; CI: 0.96-1.18, respectively). Abdominal obesity was significantly associated with HTN (OR = 1.08; CI: 1.03-1.13; p = 0.001). Also, older respondents were 1.1 times more likely of becoming hypertensive (OR = 1.07; CI: 1.02-1.11; p = 0.003). Increased risk of diabetes and HTN was found among respondents with increasing age and WC. Research limitations/implications This study was cross-sectional in design; it cannot be used to establish a cause-effect relationship between diabetes, HTN and the observed variables (anthropometric characteristics, dietary habits and lifestyle risk factors). Because of the few numbers (1 per cent) of respondents identified to be diabetic several important risk factors could not be included in the model. Practical implications An understanding of the risk factors associated with diabetes and HTN among sub-groups in the population will help to plan effective interventions targeted at specific groups. Originality/value The findings of this study show the associated risk factors for diabetes and HTN among artisans in Ogun State.
