Scientists’ response to global public health emergencies: A bibliometrics perspective

The unprecedented COVID-19 outbreak at the end of 2019 has produced a worldwide health crisis. Scientific research, especially international research collaboration, is crucial to deal successfully with the epidemic. This article aims to review the response modes, and especially the international collaboration characteristic, of the academic community to similar public health events in the past. Based on relevant studies of four major public health emergencies in the past, the major public health emergencies were regarded as ‘new knowledge’ in the academic field. By using knowledge diffusion indicators, such as the breadth and speed of diffusion, and combined with the development characteristics of the event, this article explores the diffusion characteristics of the four major public health emergencies in the academic exchange system and then identifies the academic community’s response mode to the outbreaks. In addition, the characteristics of international collaboration in response to the public health events and the impact of international collaboration on the academic community’s response are analysed. Through the analysis of the international collaboration network, the cooperative groups and core countries in the research collaboration network related to the major public health emergencies are obtained. In terms of COVID-19, it is found that the response speed and intensity of scientists have been significantly improved, but more focus should be given to international collaboration. Our findings could be beneficial to both decision-makers and researchers in policy formulation and conducting research, respectively, to optimally deal with COVID-19 and possible outbreaks in the future.

AbstractIt is well-established that the process of learning and capability building is core to economic development and structural transformation. Since knowledge is ‘sticky’, a key component of this process is learning-by-doing, which can be achieved via a variety of mechanisms including international research collaboration. Uncovering significant inter-country research ties using Scopus co-authorship data, we show that within-region collaboration has increased over the past five decades relative to international collaboration. Further supporting this insight, we find that while communities present in the global collaboration network before 2000 were often based on historical geopolitical or colonial lines, in more recent years they increasingly align with a simple partition of countries by regions. These findings are unexpected in light of a presumed continual increase in globalisation, and have significant implications for the design of programmes aimed at promoting international research collaboration and knowledge diffusion.

The first-ever global outbreak of COVID-19 has been intensely impacting humanity and continues to affect all segments of the population since its origin While dealing with such public health emergencies, the past experiences have testified generalised public fear and anxieties due to uncertainty, fatality, and lack of public health preparedness. The aim of the article is to explore the impact of this unprecedented environment on mental health of individuals and communities in its current magnitude. It will discuss at length some emerging concepts of the causal pathways of mental distress/disorders and suggest strategies to prevent and promote mental health in individuals during this crisis. For the general population at large, the mental health effects of COVID-19 are as important to address as are the physical health effects.

A recent report found that the number of opioid-related deaths in Ontario in the first 15 weeks of the COVID-19 pandemic was 38.2% higher than in the 15 weeks before the pandemic. Our study sought to determine if pharmacy professionals self-reported an increase or decrease in naloxone provision due to the pandemic and to identify adjustments made by pharmacy professionals to dispense naloxone during the pandemic. A total of 231 Ontario community pharmacy professionals completed an online survey. Pharmacy professionals’ barriers, facilitators, and comfort level with dispensing naloxone before and during the pandemic were identified. The sample consisted of mostly pharmacists (99.1%). Over half (51.1%) reported no change in naloxone dispensing, while 22.9% of respondents reported an increase and 24.7% a decrease. The most common adjustments made during the pandemic were training patients how to administer naloxone over video or phone, delivering naloxone kits, and pharmacy technicians offering naloxone at prescription intake. Over half (55%) of participants said the top barrier for dispensing was that patients did not request naloxone. Naloxone distribution through pharmacies could be further optimized to address the increased incidence of overdose deaths during the pandemic. Future research should investigate the reasons for changes in naloxone dispensing.

Abstract Evidence-based decision-making is central to public health. Implementing evidence-informed actions is most challenging during a public health emergency as in an epidemic, when time is limited, scientific uncertainties and political pressures tend to be high, and reliable data is typically lacking. The process of including data for preparedness and training for evidence-based decision making in public health emergencies is not systematic and is complicated by many barriers as the absence of common digital tools and approaches for resource planning and update of response plans. Health Technology Assessment (HTA) is used with the aim to improve the quality and efficiency of public health interventions and to make healthcare systems more sustainable. Many of today's public health crises are also cross-border, and countries need to collaborate in a systematic and standardized way in order to enhance interoperability to share data and to plan coordinated response. Digital health tools have an important role to play in this setting, facilitating use of knowledge about the population that can potentially affected by the crisis within and across regional and national borders. To strengthen the impact of scientific evidence on decision-making for public health emergency preparedness and response, it is necessary to better define and align mechanisms through which interdisciplinary evidence feeds into decision-making processes during public health emergencies and the context in which these mechanisms operate. Activities and policy development in the HTA network could inform this process. The objective of this presentation is to identify barriers for evidence-based decision making during public health emergencies and discuss how standardization in digital health and HTA processes may help overcome these barriers leading to more effective coordinated and evidence-based public health emergency response.

The Coronavirus disease 2019 (COVID-19) pandemic has emerged as a significant and global public health crisis. Besides the rising number of cases and fatalities, the outbreak has also affected economies, employment and policies alike. As billions are being isolated at their homes to contain the infection, the uncertainty gives rise to mass hysteria and panic. Amidst this, there has been a hidden epidemic of “information” that makes COVID-19 stand out as a “digital infodemic” from the earlier outbreaks. Repeated and detailed content about the virus, geographical statistics, and multiple sources of information can all lead to chronic stress and confusion at times of crisis. Added to this is the plethora of misinformation, rumor and conspiracy theories circulating every day. With increased digitalization, media penetration has increased with a more significant number of people aiding in the “information pollution.” In this article, we glance at the unique evolution of COVID-19 as an “infodemic” in the hands of social media and the impact it had on its spread and public reaction. We then look at the ways forward in which the role of social media (as well as other digital platforms) can be integrated into social and public health, for a better symbiosis, “digital balance” and pandemic preparedness for the ongoing crisis and the future.

During the last decade, the Caribbean Hispanic islands experienced accelerated demographic aging, representing the fastest aging region within Latin America. Age-related non-communicable diseases, including dementia, are now reported at high prevalence. The Caribbean islands share similar genetic ancestry, culture, migration patterns, and risk profiles, providing a unique setting to understand dementia in the Caribbean-Hispanics. This perspective article aimed to describe the impact of dementia in the Caribbean, at a local and regional level and reflect on research strategies to address dementia. We report on 10/66 project findings, described research projects and regional plans for the region. According to our results, the prevalence of dementia in the Caribbean is the highest in Latin America, with 11.7% in Dominican Republic, 11.6% in Puerto Rico, and 10.8% in Cuba. Preliminary data from new waves of the 10/66 study shows increasing numbers of dementia cases. Furthermore, dementia is expected to be one of the most serious medical and social issues confronted by Caribbean health systems. However, there is a scarcity of knowledge, awareness, and health services to deal with this public health crisis. In light of the new evidence, local and regional strategies are underway to better understand dementia trends for the region and develop policies aimed to decrease the impact of dementia. Implementation of our national plans is critical to deal with an aging population with high dementia rates. Current recommendations include emphasizing public health prevention campaigns to address modifiable risk factors and expand support to caregiver and family interventions.

Purpose: To explore the conditions under which enforcement likely is, or is not, an effective strategy to increase adherence to recommended protective behaviours during COVID-19. Design/Methodology/Approach: Relevant search terms were entered into three key databases (Google Scholar, medXriv, psyArXiv) to identify both peer-reviewed and pre-publication articles that reported empirical data relating to the impact of enforcement on adherence with recommended public health behaviours. Findings: Enforcement is less effective when: it is applied inconsistently; rules are ambiguous; behaviours are unobservable. Providing food and financial support, creating social norms and increasing trust are more effective in addressing specific barriers and fostering voluntary adherence. Where enforcement forms part of the strategy for policing COVID-19 regulations, rules must be clear and local responders must be given time and opportunity to plan their response as new rules are implemented. Research limitations/implications: This narrative review was limited to papers published in English and available either via the databases searched, or via reference searching; some relevant studies may therefore not have been identified. Practical implications: Findings inform an understanding of the impact of enforcement on adherence and facilitate the development of recommendations for increasing adherence to protective behaviours during COVID-19. Given the importance of public adherence, these recommendations are not only useful in the context of COVID-19 but also for future public health emergencies. Originality/value: This narrative review is the first to explore the circumstances under which enforcement can increase or reduce adherence with COVID-19 guidelines, generating recommendations for improved public adherence.

The online public opinion is the sum of public views, attitudes and emotions spread on major public health emergencies through the Internet, which maps out the scope of influence and the disaster situation of public health events in real space. Based on the multi-source data of COVID-19 in the context of a global pandemic, this paper analyzes the propagation rules of disasters in the coupling of the spatial dimension of geographic reality and the dimension of network public opinion, and constructs a new gravity model-complex network-based geographic propagation model of the evolution chain of typical public health events. The strength of the model is that it quantifies the extent of the impact of the epidemic area on the surrounding area and the spread of the epidemic, constructing an interaction between the geographical reality dimension and online public opinion dimension. The results show that: The heterogeneity in the direction of social media discussions before and after the “closure” of Wuhan is evident, with the center of gravity clearly shifting across the Yangtze River and the cyclical changing in public sentiment; the network model based on the evolutionary chain has a significant community structure in geographic space, divided into seven regions with a modularity of 0.793; there are multiple key infection trigger nodes in the network, with a spatially polycentric infection distribution.

ObjectiveTo evaluate the role of the European Union (EU) as a research collaborator in the UK’s success as a global leader in healthcare research and innovation and quantify the impact that Brexit may have.DesignNetwork and regression analysis of scientific collaboration, followed by simulation models based on alternative scenarios.SettingInternational real-world collaboration network among all countries involved in robotic surgical research and innovation.Participants772 organisations from industry and academia nested within 56 countries and connected through 2397 collaboration links.Main outcome measuresResearch impact measured through citations and innovation value measured through the innovation index.ResultsGlobally, the UK ranks third in robotic surgical innovation, and the EU constitutes its prime collaborator. Brokerage opportunities and collaborators’ geographical diversity are associated with a country’s research impact (c=211.320 and 244.527, respectively; p<0·01) and innovation (c=18.819 and 30.850, respectively; p<0·01). Replacing EU collaborators with US ones is the only strategy that could benefit the UK, but on the condition that US collaborators are chosen among the top-performing ones, which is likely to be very difficult and costly, at least in the short term.ConclusionsThis study suggests what has long been argued, namely that the UK-EU research partnership has been mutually beneficial and that its continuation represents the best possible outcome for both negotiating parties. However, the uncertainties raised by Brexit necessitate looking beyond the EU for potential research partners. In the short term, the UK’s best strategy might be to try and maintain its academic links with the EU. In the longer term, strategic relationships with research powerhouses, including the USA, China and India, are likely to be crucial for the UK to remain a global innovation leader.
