Sensitivity and Specificity of Maternal Anthropometric Measures Relative to Infant Nutritional Status

This cross-sectional study was done in July 1990 at health posts of two subdistricts in Cilandak, South Jakarta, Indonesia. The purpose of this study was to assess the sensitivity and specificity of maternal anthropometric measures against the functional outcome of infant nutritional status (weight-for-age). The study population consisted of 175 mother-infant pairs, who were between 14-45 years old, and their infants, who were 0-6 months old. The indicators that identified women at risk of their infant being poor nutritional status (weight-for-age less than −1 SD) were maternal weight (sensitivity 33%, specificity 89%, and positive predictive value 26%), height (sensitivity 39%, specificity 83% and positive predictive value 21%), and calf circumference (sensitivity 50%, specificity 85% and positive predictive value 28%).

Background: Even though there are many programs run by Government to tackle the problem of malnutrition in India, problem of malnutrition is still there. Malnutrition leads to infections and even can lead to death of child in severe cases. The mortality rate is very high compared to other countries. The objective of the study to study validity of age independent various nutritional status indices in comparison to gold standard of weight for ageMethods: Present study was institution based cross sectional study carried out at SDA high school, from January 2019 to July 2019 among 58 school children aged 34-92 months of age. Anthropometric measurements like weight, height, head circumference, chest circumference, mid arm circumference were recorded as per the standard guidelines. Weight for height, weight of age, Kanawati Index, Jeliffe’s ratio, Rao index, and Dugdales index was calculated. Gold standard used was weight for age. Sensitivity, specificity, positive predictive value and negative predictive value were calculated for these indices.Results: Majority of the children belonged to 48-60 months and 61-72 months i.e. 29.3% each. Males were more than females. Prevalence of malnutrition was 60.3%, 48.3%, 51.7%, 91.4%, and 56.8% based on weight for age, Kanawati index, Rao index, Jeliffe’s ratio and Dugdale’s index respectively. Dugdale’s index was found to have high sensitivity (85.7%) and specificity (86.9%) compared to all other indices.Conclusions: Among age independent indices of nutritional status available, Dugdales index can be used as it has been found that it has remarkably higher sensitivity and specificity compared to other age independent indices of nutritional status.

The aim of the present study was to explore the nature, extent and probable causes of nutritional deficiencies among children with disabilities living in Dharavi, a slum in Mumbai, India. A cross-sectional study was conducted to investigate whether the nutritional status of children with disabilities, aged 2–6 years (n141), was worse than that of non-disabled sibling controls (n122) and neighbour controls (n162). Data on food patterns, anthropometry, micronutrient status and feeding difficulties reported by parents were collected. The mean weight for age of the children with disabilities (−2·44 (sd 1·39)Zscores;n120) was significantly lower (P<0·05) compared with the sibling (−1·70 (sd 1·20)Zscores;n109) and neighbour (−1·83 (sd 1·290)Zscores;n162) control groups. The children with disabilities had significantly lower (P<0·05) mean haemoglobin levels (92 (sd 23) g/l;n134) compared with siblings (102 (sd 18) g/l;n103) and neighbours (99 (sd 18) g/l;n153). Relative risk (RR) analysis indicated that the disabled children with feeding difficulties were significantly more likely (P<0·05) to be malnourished, by the indicator of weight for age (RR 1·1; 95 % CI 1·08, 1·20) compared with the disabled children without a feeding difficulty. They were also significantly more likely to be malnourished using the indicators of height for age (RR 1·3; 95 % CI 1·19, 1·43) and weight for height (RR 2·4; 95 % CI 1·78, 3·23) compared with the disabled children without a feeding difficulty. Feeding difficulties were identified as a risk factor for vulnerability to inadequate nutritional status among children with disabilities.

Objective: To compare the accuracy of Upper lip bite test with modified Mallampati classification for predicting the difficultlaryngoscopic intubation. Study Design: Cross sectional Study. Place and duration of study: The study was carried out at Department ofAnaesthesiology, Intensive Care and Pain management, Combined Military hospital, Rawalpindi from September 2008 to August 2009.Patients and Methods: Four hundred patients undergoing elective surgery meeting the inclusion/exclusion criteria were enrolled afterwritten informed consent. The airways of the patients were evaluated by using the modified Mallampati classification (MMP) and theUpper lip bite test (ULBT). MMP class 3 or 4 and ULBT class 3 were considered as indicators of difficult intubation. The laryngeal view wasgraded by Cormack and Lehane classification (Gold standard). Grade 1 or 2 was considered to represent easy intubation and grade 3 or 4to represent difficult intubation. Sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy werecalculated for both the tests separately by using the 2×2 table. Results: ULBT had a higher accuracy of 94%, specificity of 99.2% andpositive predictive value 70% compared to MMP accuracy of 82.7%, specificity of 84.4% and positive predictive value of 22.7%.Conclusions: The diagnostic accuracy of the Upper lip bite test was more than the modified Mallampati classification. We suggest that itbe compared with the other prevailing tests as well which are often used to assess difficult intubations.

ABSTRACT Background: Pneumonia is still causing the most deaths among children in developing countries. This disesase often occurs in children under 5 years of age. Nutritional status is a factor that is closely related to infectious diseases such as pneumonia. This study aimed to examine the correlation between nutritional status and pneumonia among 6-59 months years old in Tangerang, Banten. Subject and Methods: A cross-sectional study was conducted at Pakuhaji Community Health Center, Tangerang, Banten, from January to February 2018. A total of 29 children under five were enrolled in this study. The dependent variable was pneumonia. The independent variable was nutritional status. The data were collected from direct measurement of the children under five and questtioner. The data were analyzed by Chi-square. Results: As many as 16 children under five (55.17%) had pneumonia, 7 children under five (24.13%) were malnutrition, 3 children under five (10.34%) were short, and 5 children under five (7.24%) were thin. Nutritional status based on weight for age and weight for height had differences in nutritional status with the incidence of pneumonia among children under five, and they were statistically significant (p< 0.001). Conclusion: Nutritional status is associate with the incidence of pneumonia among 6-59 months years old (children under five) in Tangerang, Banten. Objective: To compare accuracy of manual muscle testing in detection of strength differences across grade 4 and 5 at different percentage deficits of dynamometric mean in shoulder flexors of young females.  Methods: Cross sectional research design, conducting primary analysis of strength of shoulder flexors measured through manual muscle testing and hand-held dynamometer. Three hundred young healthy females without any ailment of upper extremity or generalized health problem were selected from University of South Asia and Association of Fatima Jinnah Old Graduates, Lahore. Non-probability convenient sampling technique was used. The age group selected was 18-39 years. The strength of shoulder flexors measured through manual muscle testing and hand-held dynamometer was main outcome measure. Results: The overall diagnostic accuracy of manual muscle testing showed better results at 10% dynamometric measures. It was 63.33%(95% CI: 57.74%, 68.59%) in non-dominant shoulder flexors and 61.67% (95% CI: 56.05%, 66.99%) in dominant shoulder flexors. The positive predictive value (PPV) and specificity of manual muscle testing was better at 10% strength deficit in comparison with dynamometric measures, whereas negative predictive value (NPV) and sensitivity was better at 20% strength deficit. Contrasting association was found between positive predictive value (PPV) and sensitivity, negative predictive value and specificity.  Conclusion: The subjective area of grade 4 and 5 showed better, although not ideal, diagnostic worth at 10% deficit of dynamometric measures in shoulder flexors. Further exploration through study of contrasting trends between PPV and sensitivity by subgroup analysis is required. Continuous...

Abstract Objectives Nutrient intakes of pregnant women are essential for their health and the health of their fetus. This study aims to assess the nutrient intakes of pregnant women, residing in a rural eastern Indonesia. Methods This cross-sectional study was conducted in two sub districts of Timor Tengah Selatan (TTS) district, East Nusa Tenggara, Indonesia in 2012. A total of 155 pregnant women, at all age of pregnancy, residing at least one year prior to this study in 14 villages of two sub districts KIE and Amanuban Barat involved in this study. Anthropometric data including BMI and MUAC of pregnant women were collected by trained enumerators from the department of nutrition, Health Polytechnic of Kupang, East Nusa Tenggara. A validated Semi Quantitative-Food Frequency Questionnaire (SQFFQ) was used to collect the dietary intake of the last 3 months of pregnant women. Nutrisurvey was used to compute and analyze nutrient intakes of the study subjects. Statistical Analysis was performed using STATA version 15.1 MP. Results The average of energy and protein intakes in pregnant women were 1178.4 ± 449.6 kcal/day and 38.4 ± 18.7 gr/day respectively, representing to only 54.7% and 57.4% of the Indonesian nutrient requirement of energy and protein intakes for pregnant women. The average of vitamin C and iron intakes in pregnant women were 59.9 ± 40.1 mg/day and 7.7 ± 6.2 mg/day, representing to only 70.5% and 29.5% of the Indonesian nutrient requirement of vitamin C and iron intakes for pregnant women. The average energy and protein intakes among underweight pregnant women were 211.1 kcal/day (95% CI: 17.3 kcal/day - 404.9 kcal/day) lower and 8.2 gr/day (95% CI: 0.07 gr/day -16.3 gr/day) lower than among non underweight pregnant women adjusting for age of pregnant women, gestational age, and monthly household expenditure, education level, and race. Conclusions The present study suggests that pregnant women in rural Indonesian community experienced high deficit in energy, protein, vitamin C and iron intakes lead to poor nutritional status during pregnancy. To investigate the role of IL-21 as diagnostic marker in diagnosis of rheumatoid arthritis. Study Design: Cross sectional study. Setting: Department of Physiology and Orthopedic Gujranwala Medical College, Gujranwala. Period: October 2017 to October 2018 in one year duration. Materials and Methods: A total of 150 patients were included in the study, main variables assessed in this study were positive predictive value negative predictive value, sensitivity, specificity and accuracy of IL-21 in diagnosis of rheumatoid arthritis. SPSS version 23 was used to analyze the data. P value less than or equal to 0.05 was taken as significant. Study was started after permission from hospital ethical committee and patients were informed in detail about disease and procedure to be done. Non probability consecutive sampling was used. Results: The estimated sensitivity was 93.6%. The estimated specificity was 50%. Positive predictive value was 96.3% and negative predictive value was 35.7%. The overall accuracy was 90.6% for diagnosing rheumatoid arthritis. Conclusion: IL-21 induces MMP3 in rheumatoid arthritis patients, identification of IL-21 from synovium of patients indicates the presence of rheumatoid arthritis. We observed 90.6% diagnostic accuracy of IL-21 for rheumatoid patients taking RA factor as gold standard of diagnostic tool.

Adenosine Deaminase (ADA) is used as a well established biological marker for diagnosis of tuberculous pleuritis. The aim of this study was to assess the sensitivity and specificity of ADA in patients with pleural effusion admitted in the medical ward. This was an cross sectional study. The information was gathered only from those patients whose pleural fluid sample was sent for ADA in clinical biochemistry laboratory of KIST Medical College and Teaching hospital. The data was acquired from the medical records of patients attending medical ward. The patients were considered as TB positive if they were clini­cally diagnosed so and taken as negative otherwise. Fifty six pleuritis patients were evaluated. Using the cut off value of 30 U/L, the overall sensitivity and specificity was 93.3% and 57.1% respectively. The association between ADA findings and the clinical findings were statistically significant (p <0.001). The positive predictive value (PVP) and the negative predictive value (PVN) were 82.3% and 80% respectively. ADA is an inexpensive, rapid and simple test for analysis of tuberculous pleuritis. Though, it is a sensitive test for tuberculosis, due to its low specificity, sometimes negative cases can be considered as positive. AbstractThere is a lack of uniformity in developing and validating indicators of nutritional status among People Living with Human Immunodeficiency Virus (PLHIV). Experiences from low and middle-income countries are scant, and differences in methodological and analytical approaches affect the comparability and generalizability of findings. Therefore, this study investigated the performance of individual diversity score (IDDS) as a proxy indicator of nutritional status among PLHIV. We conducted a facility-based cross-sectional study among 423 PLHIV who were under Antiretroviral Treatment (ART) at clinics in Bahir-Dar, Ethiopia. We collected data on sociodemographic, dietary, clinical, and anthropometric measures. Dietary intake was assessed using 24-Hour dietary recall. Body Mass Index (BMI) was calculated to assess the nutritional status of study subjects. The receiver operating characteristic (ROC) curve analysis was used to assess the ability of the IDDS and Minimum Dietary Diversity for Women (MDD-W) to detect poor nutritional status. Furthermore, sensitivity, specificity, Predictive Values (PPs), and Likelihood Ratios (LRs) were calculated at different cut-off points. IDDS showed good reliability with Cronbach’s Alpha of 0.76. The Area Under the Curve (AUC) of IDDS was 78.5 (95%CI 73.9–83.4). At the IDDS cut-off of 4, the sensitivity and specificity of IDDS to indicate nutritional status were 88.0% (95%CI 81.0–93.0) and 71.0% (95%CI 66.0–76.0), respectively. The AUC of MDD-W was 74.1%, and at the cut-off of 4 the sensitivity and specificity of MDD-W to indicate undernutrition were 73.0% and 72.0%, respectively. Both IDDS and MDD-W have good accuracy as a proxy indicator for measuring the nutritional status of PLHIV. In the prevention of undernutrition among PLHIV especially in a resource-limited setting, IDDS and MDD-W can be used to assess nutritional status.

Background Early life nutrition plays a critical role in the development of better health and nutrition in adulthood. However, assessing the nutritional status of Bangladeshi children and adolescents through measurement of body composition using skinfold thickness is barely studied. The current study aims to determine children’s body composition and nutritional status, and contributing factors among children aged 2 to 15 years in the northern part of Bangladesh. Methods This is a descriptive cross-sectional study done in Bangladesh. Anthropometric methods, including multiple skinfold thickness and basic anthropometric and socio-demographic characteristics, were used. Body composition was calculated from multiple skinfold thicknesses using the standard regression equation. Nutritional status was measured using Z score according to WHO 2007 reference standard. A total of 330 children from Naogaon, Bogra and Kurigram districts in Bangladesh were examined from April 2019 to September 2019. Results The Nutritional status of 2–15 years old child is exceedingly poor in the northern part of Bangladesh. Fat mass and fat-free mass were higher among children from Kurigram district than from Bogra and Naogaon district. Body fat percentages and arm fat area were greater among female children than males. The overall prevalence of stunting, underweight and wasting was around 25%, 32% and 29%, respectively, and the rate was higher among girls and children aged 2–5 years. The average SD score for weight-for-age, height-for-age, and BMI-for-age was -1.295, -0.937 and -1.009. The median weight-for-age and height-for-age Z scores of boys and girls were below the WHO reference percentile rank. Girls were twice (OR:1.951, CI:1.150–3.331) as likely to suffer from being underweight than boys. Children who don’t practice handwashing are three times (OR:3.531, CI:1.657–7.525) more likely to be underweight. Children become underweight and stunted when their family income is not sufficient to maintain their nutritional requirements. Conclusions The children of the three northern districts had a poor nutritional status, and family income was the potential contributing factor. Therefore, interventions like the promotion of income-generating activities and integrated approaches to ensuring food diversification could be an option to address the nutritional problem of children of the three northern districts of Bangladesh.
