The Impact of Environmental Design on Teamwork and Communication in Healthcare Facilities: A Systematic Literature Review

The purpose of this systematic review is to investigate the current knowledge about the impact of healthcare facility design on teamwork and communication by exploring the relevant literature. Teamwork and communication are behavioral factors that are impacted by physical design. However, the effects of environmental factors on teamwork and communication have not been investigated extensively in healthcare design literature. There are no published systematic reviews on the current topic. Searches were conducted in PubMed and Google Scholar databases in addition to targeted design journals including Health Environmental Research & Design, Environment and Behavior, Environmental Psychology, and Applied Ergonomics. Inclusion criteria were (a) full-text English language articles related to teamwork and communication and (b) involving any healthcare built environment and space design published in peer-reviewed journals between 1984 and 2017. Studies were extracted using defined inclusion and exclusion criteria. In the first phase, 26 of the 195 articles most relevant to teamwork and 19 studies of the 147 were identified and reviewed to understand the impact of communication in healthcare facilities. The literature regarding the impact of built environment on teamwork and communication were reviewed and explored in detail. Eighteen studies were selected and succinctly summarized as the final product of this review. Environmental design, which involves nurses, support staff, and physicians, is one of the critical factors that promotes the efficiency of teamwork and collaborative communication. Layout design, visibility, and accessibility levels are the most cited aspects of design which can affect the level of communication and teamwork in healthcare facilities.

Abstract Background and Objectives Caring for a person living with dementia requires resilience, the capacity to recover and grow from challenging situations. Despite the increasing interest in assessing and promoting resilience for dementia care partners, behaviors that indicate this attribute are not well known. The goal of this study is to synthesize the literature to identify resilience-related behaviors and develop a new framework for future validation and intervention research. Research Design and Methods We searched English-language peer-reviewed articles (January 1991 – June 2019) reporting qualitative or quantitative descriptions of resilience-related behaviors among dementia care partners. Thematic analysis was used to categorize behaviors into domains, identify the relationships among them, and generate a thematic map. Results Sixteen articles were identified according to predefined inclusion criteria. Four domains emerged: (1) problem-response behaviors (problem-solving, problem-distancing); (2) self-growth behaviors (self-care activities, spiritual-related activities, and developing and maintaining meaningful social relationships); (3) help-related behaviors (help-seeking and help-receiving), and (4) learning-related behaviors (learning from others and reflection). Definitions of each domain, identification of corresponding behaviors, and formulation of Care Partner Resilience (CP-R) behavioral framework were informed by this mixed research synthesis and other relevant literature. Discussion and Implications The CP-R framework emerged as a result of a new focus on identifying and cultivating strengths instead of evaluating strain and burden. It serves as a useful foundation for understanding the impact of specific behaviors on dementia care partner resilience. Once validated, this framework will inform the development of future measures, research, interventions, and policies for dementia care partners.

Assessing the fire safety of buildings is fundamental to reduce the impact of this threat on their occupants. Such an assessment can be done by combining existing models and existing knowledge on how occupants behave during fires. Although many studies have been carried out for several types of built environment, only few of those investigate healthcare facilities and hospitals. In this study, we present a new behavioural data-set for hospital evacuations. The data was collected from the North Shore Hospital in Auckland (NZ) during an unannounced drill carried out in May 2017. This drill was recorded using CCTV and those videos are analysed to generate new evacuation model inputs for hospital scenarios. We collected pre-movement times, exit choices and total evacuation times for each evacuee. Moreover, we estimated pre-movement time distributions for both staff members and patients. Finally, we qualitatively investigated the evacuee actions of patients and staff members to study their interaction during the drill. The results show that participants were often independent from staff actions with a majority able to make their own decision.

Objective: To systematically review the literature regarding the role of the physical environment in preventing or mitigating aggressive behavior toward healthcare professionals in acute care, outpatient, and psychiatric/behavioral health facilities. Background: Globally, the incidence of violence against healthcare professionals is alarming. Poor environmental design has been identified as a risk factor of violence toward employees. The design of the physical setting in which healthcare is provided may moderate the incidence and severity of violence against healthcare workers. Methods: We conducted electronic database searches of PubMed and CINAHL through November 2018. Result: Findings were organized according to four categories identified in the literature regarding crime prevention through environmental design (CPTED) including natural surveillance, access control, territoriality, and other CPTED elements. Fifteen studies (published between 1991 and 2017) met the inclusion criteria. Of the 15 studies, 4 incorporated environmental interventions. In the 11 remaining studies, physical environment attributes (i.e., layout, location, ambient conditions, equipment) were among the factors affecting violent incidents and staff security. Most study settings were hospital-based (11, with 10 of those specifically focused on emergency departments), followed by behavioral health facilities (4 studies). Design-focused recommendations, such as providing a second door in a triage room and a sub-waiting area inside the treatment zone, were summarized according to CPTED categories. Conclusion: This review suggests that the physical environment in healthcare facilities may affect the incidence of violence by patients or visitors against staff. Further research is needed to identify environmental design strategies that may protect the safety of healthcare professionals.

Purpose – The paper aims to present a critical review on the adoption and use of Twitter in libraries. Twitter, a microblogging service, is becoming more and more popular among libraries as a means of reaching users and as a marketing tool for their services. Design/methodology/approach – The method of systematic review is used to identify, collect and critically present all relevant literature. Papers’ aims were analyzed to identify the emergent themes and sub-themes in an effort to gain a better understanding of libraries’ use of Twitter. Findings – A systematic and critical analysis of the literature revealed the specific reasons why a library should adopt Twitter; the specific examples and thoughts on how Twitter could be used to promote libraries’ interests; the different ways Twitter had affected diverse aspects and functionalities of libraries; and the attempts to provide guidelines for librarians. Research limitations/implications – This literature review has implications for research in that libraries could explore the way public view their presence on social networking sites such as Twitter to better gain from it. In addition, it indicates what libraries should expect when they adopt Twitter. This study reviews only papers published between 2007 and 2013 in the English language. Originality/value – This paper contributes in identifying, collecting, analyzing and critically presenting published research referring to the adoption of Twitter in the field of Library Science. In addition, it identifies and discusses the main issues librarians and libraries need to consider, when faced with the challenge of adopting Twitter.

In contemporary “high-risk” society, unexpected disasters (epidemics and extreme weather) and chronic pressures (aging problems) put tremendous pressure on healthcare facilities. Enhancing the healthcare facilities' resilience ability to resist, absorb, and respond to disaster disruptions is urgent. This study presents a scientometric review for healthcare facility resilience research. A total of 374 relevant articles published between 2000 and 2020, collected from Web of Science (WoS) core collection database, Scopus database and MEDLINE database were reviewed and analyzed. The results indicated that research on resilience in healthcare facilities went through three development periods, and the research involved countries or institutions that are relatively scattered. The studies have been focused on the subject categories of engineering, public, environmental, and occupational health. The keywords of “resilience,” “hospital,” “disaster,” “healthcare,” and “healthcare facility” had the most frequency. Furthermore, based on the literature co-citation networks and content analysis, the detected seven co-citation clusters were grouped into four knowledge domains: climate change impact, strengthening resilience in response to war and epidemic, resilience assessment of healthcare facility, and the applications of information system. Moreover, the timeline view of literature reflected the evolution of each domain. Finally, a knowledge map for resilience of healthcare facilities was put forward, in which critical research contents, current knowledge gaps, and future research work were discussed. This contribution will promote researchers and practitioners to detect the hot topics, fill the knowledge gaps, and extend the body of research on resilience of healthcare facilities.

The idea of searching: This study tends to assess the impact of implementing evidence-based infection prevention in healthcare facilities in Egypt, with the aim of improving surveillance systems and altering the facility designs according to the data acquired on HAIs patterns. Background: Hospital acquired infections (HAIs) are becoming one of the major concerns for the patients and healthcare providers leading to significant increase in mortality rates, morbidity rates and financial losses for healthcare organizations. The incidence rate of HAI in Egypt was as recorded as 3.7% recently. Certain environmental interventions, implemented during construction of the healthcare facility could lead to enhanced prevention against the transmission and spread of the HAIs. Studies revealed that the integration of Surveillance programs could provide evidence for the designers to alter the healthcare facility design with the aim of infection prevention. Therefore, EBD approach is used to potentially measure psychological and physical effects of the environment design of a health facility on the patients and hospital staff. Methodology: Previous scientific literature is assessed to collect the relevant data which is then organized and analyzed in this study. A systematic review is generated based on the analytical outcomes of the selected data. Conclusion: EBD approach has the potential to prominently decrease HAIs burden in Egyptian healthcare facilities as it provides a diverse insight into the layout, equipment, and materials that contribute in the transmission of pathogens due to faulty design. Findings and recommendations: In order to improve the poor indoor quality by MEP (mechanical, electrical, and plumbing), previous studies have also indicated certain solutions including advancements in private room, improved surface selections, isolation, integration of touchless systems, and enhanced ventilation systems that must be applied in the healthcare facilities in Egypt for infection prevention.

This study aims to investigate the impact of using small group discussion technique on enhancing students’ speaking skill in English language department at Faculty of Arts and Science Kufrah –Benghazi University. The review of relevant literature revealed that previous researchers presented the advantages of small group discussion technique in teaching English around the world. So, the researcher believed that the use of small group discussion technique might promote students’ performance in speaking skill. The review of literature undeniably revealed that large group discussion technique mostly used by EFL lecturers to teach speaking skill were not solid enough. The advantage of the small group discussion technique over the large group discussion technique was investigated by testing the hypotheses and research questions set after conducting the literature review. The quasi-experimental research design as used in this study. English language department at Faculty of Arts and Science Kufrah was chosen for experimental study with 30 students. A teacher- made task was used as an instrument of the study. The results proved that, the mean performance of the students exposed to small group discussion technique is higher than that of those exposed to the large group discussion. Based on the results, recommendations were made on the need to adopt small group discussion to ameliorate students’ performance in speaking skill in Libyan universities.

Purpose: The objective of this research is to examine the role of hospital logistics in improving quality of care and patient satisfaction. Thus, the paper focuses on modeling and evaluating the relationship between these three constructs.Design/methodology/approach: In the present study, hospital logistics was specified as a second-order construct composed of five first-order constructs: physical accessibility, waiting time, consultation time, hospital hotel services, and administrative procedures. A questionnaire was developed and administered face-to-face to 384 hospitalized patients in three public healthcare facilities in Fez-Morocco. Collected data were processed and analyzed deploying the PLS-SEM method and using SmartPLS3 software. Data analysis was carried out by considering two types of patient circuits according to the admission modes in the healthcare facility: Urgent Patients Circuit and Scheduled Patients Circuit. Thus, two PLS-SEM models were evaluated and validated.Findings: Results highlighted the significant impact of hospital logistics on quality and satisfaction. In particular, the results of the two models showed that the most preponderant hospital logistics component is physical accessibility which consists of the availability and accessibility of ambulances, medical and nursing staff, support and guidance staff, technical facilities and equipment, etc. Thus, hospital managers and health system stakeholders should pay particular attention to hospital logistics activities in general and specifically to the physical accessibility to improve the quality of care and patient satisfaction.Research limitations/implications: This study only included patients from three public healthcare facilities in Fez-Morocco. Also, the model variables of hospital logistics construct are restricted and were applied in a specific context. Besides, the sample size was relatively reduced. Thus, results generalization might be limited. Further studies including more patients from other territories and including other logistics components are needed for large-scale validation of the proposed model. Originality/value: The results of this study contribute to the scientific literature on hospital logistics and its role as a lever for quality of care and patient satisfaction.

We aimed to examine the nature and extent of theory application in studies of built environment attributes and impact on adults in healthcare facilities. Many varied theories are described when providing the rationale for research into built environments in healthcare. Uncertainty exists around the right theory to frame a research question, alignment with measurement tools, and whether healthcare setting makes a difference. This poses challenges to researchers seeking to build the evidence base for built environment design that benefits patients and staff. Our multidisciplinary review team scoped the literature to determine how theories are used to inform research investigating the impact of the built environment of healthcare on adults. When researchers recorded theory at development of the study question, in data collection, and in data analysis/interpretation, we called this explicitly theory-based application. Synthesis occurred using a narrative approach. Overall, we found 17 diverse theories named in studies. Explicitly theory-based use occurred with eight theories, comprising 47% of all theories used. Five theories were named more frequently in studies out of all theories identified. In 20% of studies, theory was not used explicitly during the research inquiry. We argue that researchers must continue to strive toward explicit use of theories, similar to development of other health interventions that employ multifactorial components.

Abstract Background: The impact of misinformation about vapes’ relative harms compared with smoking may lead to increased tobacco-related burden of disease. To date, no systematic efforts have been made to chart interventions that mitigate vaping-related misinformation. We plan to conduct a scoping review that seeks to fill gaps in the current knowledge of interventions that mitigate vaping-related misinformation.Methods: A scoping review focusing on interventions that mitigate vaping-related misinformation will be conducted. We will search (no date restrictions) MEDLINE, Scopus, EMBASE, CINAHL, PsycINFO, Web of Science Core Collection, Global Health, ERIC and Sociological Abstracts. Grey literature will be identified using Disaster Lit, Google Scholar, Open Science Framework, governmental websites and preprint servers (e.g. EuropePMC, PsyArXiv, MedRxiv, JMIR Preprints). Study selection will conform to Joanna Briggs Institute Reviewers’ Manual 2020 Methodology for JBI Scoping Reviews. Only English language, original studies will be considered for inclusion. Two reviewers will independently screen all citations, full-text articles, and abstract data. A narrative summary of findings will be conducted. Data analysis will involve quantitative (e.g. frequencies) and qualitative (e.g. content and thematic analysis) methods.Discussion: Original research is urgently needed to design interventions to mitigate vaping-related misinformation.
