The article examines organizational and legal problems related to state registration, production, procurement and use of medical devices for first aid. The study significance is accounted for by the need to find solutions to problems related to equipping first aid activities with medical devices, which is becoming extremely important for the modern Russian society. Moreover, a comprehensive study of these issues has not been undertaken so far. The purpose of the study is to develop a set of proposals of an organizational and legal nature to improve quality of medical devices for first aid and equipping all potential participants of first aid delivery with such devices. To eliminate the problem of low equipment of potential participants of first aid delivery with the means of delivery, the composition of first-aid kits, packs, sets, and packages for all categories of participants have to be approval by the Ministry of Health of the Russian Federation. The authors also propose organizational and legal measures to improve interdepartmental and intradepartmental cooperation in this direction with the Russian Ministry of Health playing the coordinating role. The authors have also identified the need for improving the procedure for state registration of the production of first aid devices either though amending the Federal Law "On the Basics of Public Health Protection in the Russian Federation" regarding the provision that first-aid kits, packs, sets, and packages equipped with medical devices registered in the prescribed manner are not subject to state registration. As an alternative, the authors propose amendments and additions to the Rules of the state registration of medical devices, providing for a simplified state registration of first-aid kits, packs, sets, and packages. In addition, the authors name arguments in favor of improving the clinical trial procedure for first aid medical devices and propose changes to Article 38 of the Federal Law “On the Basics of Public Health Protection in the Russian Federation” in terms of eliminating the problems of legal techniques in the definition of the term “medical device” regarding first aid. The work shows the importance of rational and efficient assessment of control and supervision in health care for society as a whole and for its individual members. Health care is part of the system that shapes the quality of life of the population, influences the development of human potential, economic performance, on the adoption and diffusion of innovations in Russia. Control and supervision in public administration reform received much attention. The Ministry of Economic Development of the Russian Federation has developed a methodology for assessing and comparing the activities of Supervisory authorities. However, there is a need for not only implementation of the standardized approach in assessing but taking into account the specific features of activity of Supervisory authorities. The article presents the system of socially significant indicators for assessing the activities of Roszdravnadzor and its territorial offices in the RF entities. Currently the authority spends a lot of tests (about 10 thousand per year), however, the effectiveness of control and supervision is low. The health of the population remains low, satisfaction with public health services is reduced. Researching foreign experience of health systems development in countries with socially oriented economy allows to draw a conclusion about the necessity of building a qualitatively new assessment system of control and supervision, which should be based on the interests of society. The author proposes to depart from the principle of "check for validation" and change over to socially significant indicators for the government activities. The article highlights such indicators for healthcare and proposes their decomposition for fractional quantitative and qualitative assessment of activities. This system of indicators is aimed at improving the efficiency of public health care management and growth of the citizens’ health and their quality of life in a whole. The work is performed in as part of research of Financial University under the Government of the Russian Federation by order of the Government of the Russian Federation.

In the context of civil society development, citizens themselves have an important place in controlling the full implementation of rights. One of the principles of health is the "priority of child health." A study was conducted to examine the views of patients on the implementation of the principles of health care in the Russian Federation. The basis of the study was children's clinics. The sample total, formed by the method of random selection among those who applied to the clinic within a month, amounted to 1,250 mothers. The collection of information was carried out by the method of sociological survey on a specially compiled questionnaire. It has been established that in public health children face a formal attitude, lack of infrastructure of health care, and need legal assistance. The survey showed that the principle of "priority of children's health" declared by the state has some problems in practical implementation, both in the health sector and in the part of non-health-related entities. It has been established that in public health, mothers of children, as their legal representatives, often face a formal attitude, lack of infrastructure of health care, and experience an unrealized need for legal assistance. The general problems of reducing accessibility are the lack of necessary specialists, long waits for admission, but for single-parent families more significant than for full families is the factor of time of admission of specialists. The findings provided the basis for the following proposals to improve the implementation of the principle of "priority of child health": 1. It is necessary to ensure that the public is informed widely and multi-channel about the directions and measures taken by the state (programs, plans, activities, legislation) to ensure the implementation of the principle of "priority of child health"." Public monitoring should be established with the participation of civil society representatives of the degree of practical implementation of the principle of "priority of child health" by public bodies and officials. 3. Create the conditions for public discussion of specific measures to improve the implementation of the "priority of child health" principle. To make information more accessible to parents raising children, provide additional distance counseling and information opportunities through the Internet.

The priorities of scientific and technological development of the Russian Federation, established by Decree of the President of the Russian Federation № 642 of 01.12.2016, provide for the improvement of health-saving technologies, which in this article have been considered from the standpoint of their focusing on reducing health risks caused by the adverse impact of environmental factors, including by increasing the functional and adaptive reserves of the body. The article analyzes in order to improve regulatory legal documents in the field of improving the prevention of chronic non-communicable diseases, the formation of a healthy lifestyle and hygienic education of the population. Also, a critical analysis of the state of educational activities provided at the state level in the field of higher professional education and postgraduate training of medical workers in the direction (specialties): medical and preventive care, medical care, public health and healthcare. Concluded that to ensure scientific and technological development of the country's need to expand educational and enlightening activities on issues of ensuring sanitary and epidemiological welfare of the population, hygiene, public health and organization of health care in health care organizations, and also outside in all major sectors of economy and business, in the sphere of organized tourism and recreation, SPA, fitness centers, educational organizations, sports, social welfare and life of the population, especially in terms of prevention, that in accordance with Federal Law № 323 of 21.11.2011 "On the basics of protecting the health of citizens in the Russian Federation" provides for measures to eliminate the harmful influence of environmental factors. Proposals and additions to the relevant state regulatory documents of the Russian Federation that have already been approved and are currently under development are given.

The article suggests legislative implementation of certain “medical” amendments to the Constitution of the Russian Federation. The authors propose to add two articles to the Federal law “on the basics of public health protection in the Russian Federation” dated 21.11.2011 N323-FZ. In one article, it is proposed to reveal and specify the content of the newly introduced in the Constitution of the Russian Federation concept “unified legal bases of the health care system”. Another proposed article reveals the forms and content of work on the coordination of health issues, again referred by the Constitution of the Russian Federation to the joint responsibility of the Russian Federation and the subjects of the Russian Federation. According to the authors, the proposed changes in legislation will allow to ensure in practice the proper implementation of “medical” amendments to the Constitution of the Russian Federation, which in turn will ensure the improvement of legal regulation of the organization and activities of health care, and strengthen its material and technical base.

The planning as a key function of management in health care system is focused on target indicators of population health and development of health care sector. The focusing on social economic values of target indicators and setting them as indicators of appropriateness and effectiveness of implementing activities is actual standard ofplanning. For all that, the technique of indicative planning in health care still requires to be putted to level of its theoretical substantiation (prerequisites of becoming, conceptual foundations, terms, requirements to evaluation) and implementation in practice. The example of the Republic of Ingushetia, one of the regions of the Russian Federation, was used to demonstrate becoming of indicative planning and its development with the purpose of effective management. The emphasis is made on terms system, approaches of choosing and monitoring of target indicators for indicative planning. To present the technique of indicative planning scientifically the monograph method (analysis of reports, statistical data of Rosstat, Ministry of Health of the Russian Federation and Ministry of Health of the Republic of Ingushetia) was applied. The SWOT analysis of the situation in health care of the region was applied. The infant mortality indicator became the reference point of evaluation of effectiveness of functioning of health care system and social economic reforming in the region. The level and dynamics of infant mortality indicator reflects shortcomings of medical care organization in the region and first of all absence of the three-level system of its rendering, routing according standards of medical care support of patients on main pathology profiles, including pregnant women, women in labor, newborns and children. According to situation, the investment decisions were made on the federal level. To what extent they will be effective i.e. what will be the reaction of values of target indicators of social economic development of the region, primarily indicators of infant mortality, the «road map» should demonstrate.

In places of imprisonment of Russia the system of registration and monitoring ofpatients is the same as in civil health care. This system includes data on detection, diagnostic, treatment and dispensary observation of socially significant diseases. The obtained information is summarized with data concerning civil population and then is passed to Ministry of Health of the Russian Federation and profile research institutes within the framework of common federal and sectoral report forms. The systematic medical examinations, intensification of activities of infection control, support of effective treatment and coordination of measures between Ministry of Justice of Russia and Ministry of Health of the Russian Federation, international partners favored dramatic decreasing of indicator of morbidity of contingents ofpenitentiary system (up to 2.7 times during 1999-2015). Nowadays, the epidemic situation on tuberculosis in institutions of the penal executive system can be characterized as stable and controlled and having tendency to amelioration. Overall, as compared with Russian national average indices (2015), in places of imprisonment morbidity is higher: with tuberculosis 19,4 times, HIV-infection - in 26,9 times, syphilis - in 8.9 times. Among primarily registered patients of Federal penitentiary service of Russia in investigative isolation ward s in 2015 were detected 46% with tuberculosis, 84.6% with HIV-infection, 96.6% with syphilis. The number of patients infected with HIV-infection in contrast with tuberculosis has no tendency to decreasing in places of imprisonment. In 2015 percentage of infected individuals in institutions of the Federal penitentiary service of Russia began to increase in the structure of morbidity of HIV-infection in Russia in general. The cause of death of deceased with combined pathology (tuberculosis + HIV-infection) is HIV-infection mainly because almost in 90% HIV-infection has late stages. This indirectly testifies presence ofproblems related to detection of infection socially significant diseases among socially deadapted persons in civil health care.

The aim of the work is to determine the external, anthropogenic environmental factors that negatively affect the morbidity of the population and associated nosologies. The knowledge gained in this area will allow us to identify the adverse impacts of man-made nature and identify tools for environmental quality management to promote public health. Geoinformation data will help to identify the areas with the most intense environmental situation in the Murmansk region. It will also identify the dominant diseases in these areas. Mining and metallurgical enterprises mainly contribute to the pollution of the territory of the region: Kola MMC, Severonikel, Phosagro, Kovdor mining and processing plant, North-Western Phosphorus Company, Olkon and Rusal, as well as the nuclear fleet and fish processing enterprises, power supply facilities. The air basin is dominated by fluoride compounds, which are the emissions of ethylene into the atmosphere. Data on the demographic situation and medical statistics, including data on general morbidity and mortality, were obtained from the Territorial Body of the Federal State Statistics for Murmansk Region, Ministry of Health of the Russian Federation, Department of Monitoring, Analysis and Strategic Development of Health Care, Federal State Budgetary Institution “Central Research Institute for the Organization and Informatization of Health Care” of Ministry of Health of the Russian Federation and Ministry of Natural Resources and Ecology of Murmansk Region. The medical and environmental analysis of adverse factors in the regions of Murmansk region was carried out taking into account the environmentally dependent diseases of the adult population. The assessment of the morbidity of the population in the Murmansk region showed that several areas with a tense environmental situation are identified in the region. Diseases of the respiratory system, digestive system, genitourinary system, and musculoskeletal system are leading in the structure of overall morbidity.
