The aim of the work is to determine the external, anthropogenic environmental factors that negatively affect the morbidity of the population and associated nosologies. The knowledge gained in this area will allow us to identify the adverse impacts of man-made nature and identify tools for environmental quality management to promote public health. Geoinformation data will help to identify the areas with the most intense environmental situation in the Murmansk region. It will also identify the dominant diseases in these areas. Mining and metallurgical enterprises mainly contribute to the pollution of the territory of the region: Kola MMC, Severonikel, Phosagro, Kovdor mining and processing plant, North-Western Phosphorus Company, Olkon and Rusal, as well as the nuclear fleet and fish processing enterprises, power supply facilities. The air basin is dominated by fluoride compounds, which are the emissions of ethylene into the atmosphere. Data on the demographic situation and medical statistics, including data on general morbidity and mortality, were obtained from the Territorial Body of the Federal State Statistics for Murmansk Region, Ministry of Health of the Russian Federation, Department of Monitoring, Analysis and Strategic Development of Health Care, Federal State Budgetary Institution “Central Research Institute for the Organization and Informatization of Health Care” of Ministry of Health of the Russian Federation and Ministry of Natural Resources and Ecology of Murmansk Region. The medical and environmental analysis of adverse factors in the regions of Murmansk region was carried out taking into account the environmentally dependent diseases of the adult population. The assessment of the morbidity of the population in the Murmansk region showed that several areas with a tense environmental situation are identified in the region. Diseases of the respiratory system, digestive system, genitourinary system, and musculoskeletal system are leading in the structure of overall morbidity.

Subject. Considering the existing environmental situation, it becomes especially important for the State to regulate the anthropogenic footprint on the environment in the Russian Federation. Current amendments to the legislative framework for environmental security are intended to ensure the innovative development of regions concurrently with a reduction in adverse environmental effects and more active environmental policy. Objectives. The research is to devise methodological tools to evaluate the efficiency of environmental policy in regions. Methods. The research employs qualitative and quantitative methods of economic analysis, including statistical and content analysis, rating, matrix zoning. The dataset proceeds from the Federal State Statistics Service, governmental reports on the current environmental situation and environmental protection in the Russian Federation. Results. We propose our own approach to evaluating the efficiency of environmental policy. It may help trace the correlation between the quality of strategic documents and changes in environmental indicators for a certain period, flag the challenging areas in terms of the environmental policy implementation and outline possible development paths. The approach extends the list of quantification indicators in line with those ones adopted internationally and presented in the Environmental Security Strategy of the Russian Federation up to 2025. We evaluated the efficiency of the environmental policy referring to the regions of the Northwestern Federal District for the period from 2012–2016. Conclusions. Having analyzed the evaluation results, most of the Northwestern regions tend to be controversial and ambivalent in setting environmental goals and achieving them. The findings may prove useful as the analytical and data basis for articulating the environmental and economic policy of the regions.

Significance. Diseases of the circulatory system remain one of the leading causes of death and morbidity of the adult population. Over the years, the efforts of the State aimed at controlling diseases of the circulatory system have made the contribution of this disease class to the total mortality in Russia gradually reduce. Nevertheless, diseases of the circulatory system remain the leading ones among diseases, characterized by a steady growth and rapidly increasing prevalence among the population. The purpose of the study is to analyze dynamics in incidence and prevalence of the diseases of the circulatory system among the Russian adult population in 2007-2019. Material and methods. The study used data of the statistical collections of the Ministry of Health of the Russian Federation and the Federal Research Institute for Health Organization and Informatics of the Ministry of Health of Russia: "Morbidity of the adult population of the Russian Federation" for 2007-2019. The study, which is a continuous statistical observation, used the method of direct ranking, calculation of indicators of time series. Results. Diseases of the circulatory system in 2019 ranked fourth in the incidence structure of the adult population (8%), while its prevalence ranked second to none (21%). Over 13 years, the incidence of diseases of the circulatory system in adults increased by 42% (2007-2019) while its prevalence increased by 24%. The highest levels of incidence of diseases of the circulatory system in 2019 are registered in the Krasnodar Territory (9,024.3 per 100 000 population, + 324%), the Kurgan Region (7,404.2, + 103%) and the Karachayevo-Circassian Republic (7 286.2, + 268%), and prevalence - in the Altai Territory (52 557.6, + 2%), the Chuvash Republic (51 814.6, + 38%) and the Voronezh Region (45 339.4, + 59%). The maximum rates of increase in morbidity with diseases of the circulatory system from 2007 to 2019 are registered in the South Federal district (incidence - by 119%, prevalence - by 56%), among the constituent entities of the Russian Federation: incidence - in the Krasnodar Territory (by 324%), prevalence – the Karachayevo-Circassian Republic (by 136%). According to the state statistical reporting, on average, every 7th acute myocardial infarction is complicated by recurrent heart attack in Russia. Conclusions. Throughout the study period of 13 years, the leading nosological forms include: Hypertensive diseases (35%; 45%, respectively), Ischemic heart diseases (22%; 21,6%) and Cerebrovascular diseases (23%; 20%). Over half of the diseases of the circulatory system are registered in the older working ages (55-60 and over). Scope of application. The study results can be useful to regional authorities in public health in planning and implementing regional programs aimed at controlling diseases of the circulatory system.

Influenza and ARVI - the main nosological form in structure of all infectious diseases. Influenza is especially dangerous to pregnant women. Purpose: to estimate commitment of pregnant women and health workers to vaccination against influenza. Materials and methods: form No. 5, form of the statistical account No. 1 - influenza the section 1, information from the official site of Federal State Budgetary Institution Scientific Research Institute of influenza of the Ministry of Health of the Russian Federation; materials of various conferences and the congresses on a influenza problem, information and analytical reference of the Ministry of Health of the Russian Federation. There were asked 1126 pregnant women on various durations of gestation and 447 health workers in various territories of the Russian Federation. Statistical processing was carried out with use of the Microsoft Excel 2010 and IBM SPSS Statistics 20.0 programs. Results and discussion: Among the interviewed pregnant women planned or were already vaccinated against influenza only 8% of women. The most frequent causes of failure from vaccination against influenza during pregnancy: doubt in safety (41.8%), ignorance about possibilities of vaccination (21,1%), lack of need for vaccination (18,7%). Only 25% of respondents knew about entering vaccination against influenza during pregnancy into a national calendar. At the same time only in 15% of health workers suggested pregnant women to be vaccinated from influenza. 73% of the interviewed health workers didn't recommend vaccination against influenza to pregnant women because they consider undesirable an intervention in immunity of the pregnant woman, 33% doubt in safety of vaccination, 13% don't know about a possibility of vaccination of pregnant women at all, 6% have personal negative experience of vaccination, and 4% don't see need for vaccination from influenza. Conclusion: the received materials showed need of development of knowledge at health workers of a vaccinal prevention of infectious diseases, including also vaccination against influenza during pregnancy, and also to carry out broad and active information work with the population, attracting mass media.

The planning as a key function of management in health care system is focused on target indicators of population health and development of health care sector. The focusing on social economic values of target indicators and setting them as indicators of appropriateness and effectiveness of implementing activities is actual standard ofplanning. For all that, the technique of indicative planning in health care still requires to be putted to level of its theoretical substantiation (prerequisites of becoming, conceptual foundations, terms, requirements to evaluation) and implementation in practice. The example of the Republic of Ingushetia, one of the regions of the Russian Federation, was used to demonstrate becoming of indicative planning and its development with the purpose of effective management. The emphasis is made on terms system, approaches of choosing and monitoring of target indicators for indicative planning. To present the technique of indicative planning scientifically the monograph method (analysis of reports, statistical data of Rosstat, Ministry of Health of the Russian Federation and Ministry of Health of the Republic of Ingushetia) was applied. The SWOT analysis of the situation in health care of the region was applied. The infant mortality indicator became the reference point of evaluation of effectiveness of functioning of health care system and social economic reforming in the region. The level and dynamics of infant mortality indicator reflects shortcomings of medical care organization in the region and first of all absence of the three-level system of its rendering, routing according standards of medical care support of patients on main pathology profiles, including pregnant women, women in labor, newborns and children. According to situation, the investment decisions were made on the federal level. To what extent they will be effective i.e. what will be the reaction of values of target indicators of social economic development of the region, primarily indicators of infant mortality, the «road map» should demonstrate.

The aim of the experts who provide health care for patients with cystic fibrosis (CF) has been to develop clinical guidelines on cystic fibrosis for children and adults.Methods. The 2020 Clinical Guidelines were based on the European consensus documents on cystic fibrosis and the national consensus “Cystic fibrosis: definition, diagnostic criteria, therapy” (2016, 2019). When creating clinical guidelines, the experts were guided by Article 2 of the Federal Law No.323-FL “On the basics of health care in the Russian Federation” as of November 21st, 2011, and the regulatory legal documents governing the development and approval of clinical guidelines (https://rosmedex.ru/kr/).Results. The clinical guidelines were approved by the Federal State Budgetary Institution “Centre for Expertise and Quality Control of Medical Care” of the Russian Ministry of Health in August 2020. Standards of medical care for children and adults with cystic fibrosis were prepared based on these guidelines. The diagnostic algorithms have been expanded with a three-stage DNA diagnostic strategy, functional assays of CFTR channel, laboratory and instrumental diagnostic tools for assessing disorders of respiratory and other systems, diagnosis of the microbial inflammatory process, microbiological monitoring, and therapeutic approaches. The antibiotic therapy with dosages and courses is described in detail. The updated document should describe a unified tactic for the management of Russian CF patients.Conclusion. Unified clinical guidelines for children and adults should develop unified tactics for the diagnosis, therapy, and follow-up of CF patients in the Russian Federation and ensure continuity between health care services for children and adults.
