Background Obesity has become a rapidly growing epidemic worldwide, increasing the risk of morbidity and mortality in adolescents. Obesity is due to an expansion of adipose tissue mass, which is an important source of cytokines and contributes to an increase in pro-inflammatory cytokines, such as interleukin-6 (IL-6). Interleukin-6 is significantly increased in obesity and may lead to a state of insulin resistance.Objective To assess for a correlation between IL-6 levels and insulin resistance in obese adolescentsMethods We conducted a cross-sectional study from January to April 2012 in Manado, North Sulawesi. Subjects were either obese or normal body mass index (BMI) teens aged 13-18 years. Data collected were anthropometric status, BMI, and blood specimens for fasting plasma glucose levels, fasting insulin levels, and IL-6 levels. Insulin resistance was expressed as homeostatic model assessment of insulin resistance (HOMA-IR) level >2.77. Data was analyzed by Pearson’s correlation and linear regression tests to assess for a possible correlation between IL-6 levels and insulin resistance.Results The mean BMI in the obese group was 31.21 (SD 3.61) kg/m2 while the mean BMI in the normal group was 19.52 (SD 2.38) kg/m2. There was no significant association between IL-6 and the occurrence of insulin resistance (P=0.309). The log regression coefficient value of IL-6 was negative (b = -0.329).Conclusion There is no correlation between IL-6 levels and incidence of insulin resistance in obese adolescents.

The relationship between caffeine and insulin resistance (IR) has been assessed only in terms of caffeine intake, and the association between caffeine and beta cell function (BCF) remains unclear. This study examines the association between urinary caffeine and its metabolites, IR, and BCF in nondiabetic, noninstitutionalized US adults in order to account for the inter-individual differences in caffeine metabolism. Data on urinary caffeine and its metabolites, IR and BCF from adults aged 20 years and older who participated in the 2009–2010 and 2011–2012 National Health and Nutrition Examination Surveys were analyzed (n for caffeine = 994). IR and BCF were assessed using homeostatic model assessment (HOMA) and urinary caffeine and its metabolites were measured using high-performance liquid chromatography-electrospray ionization-tandem quadrupole mass spectrometry. After adjusting for all covariates, increases in urinary 1,3-DMU, 1,7-DMU, 1,3,7-TMU, theophylline, paraxanthine, caffeine, and AAMU were significantly associated with increased HOMA-IR and HOMA-β (HOMA of insulin resistance and beta cell function). Compared with individuals in the lowest quartile of urinary 1,3-DMU, 1,7-DMU, 1,3,7-TMU, theophylline, paraxanthine, caffeine, and AAMU, the regression coefficients for HOMA-IR and HOMA-β were significantly higher among those in the highest quartile. After stratification by prediabetes status, HOMA-IR and HOMA-β showed significant positive associations with urinary caffeine and its metabolites among subjects with normal fasting plasma glucose levels. Our cross-sectional study showed that caffeine and its metabolites were positively related to IR and BCF.

Background Childhood obesity is a major health concern.Obesity is due to an expansion of adipose tissue mass. This tissueproduces pro􀁞inflarmnatory cytokines, such as interleukin􀁞6 (IL--6).IL􀁞6 is considered to be the chief stimulator of the production ofhighly sensitive C􀁞reactive protein (hsCRP) in the liver. Bothmolecules are responsible for the chronic low􀁞grade inflammatorystate in obese individuals.Objective To assess a correlation between IL􀁞6 and hsCRP inobese adolescents.Methods This cross􀁞sectional study was conducted from Marchto June 2011 in Manado. Subjects were obese and normal bodymass index (BMI) teens aged 13􀁞18 years. Serum glutamicoxaloacetic transaminase (SOOT) and serum glutamic pyruvictransaminase (SOPT) levels were measured to rule out liverimpairment. IL􀁞6 and hsCRP levels were also measured. Data wasanalyzed by Pearson's correlation and linear regression to test forcorrelation between IL--6 and hsCRP levels.Results There was a strongly positive correlation between IL􀁞6and hsCRP levels in obese adolescents (r=0.79 with P<O.OOl).IL--6 and hsCRP levels were not significantly associated in subjectswith normal BMI.Conclusions There was a strongly positive correlation betweenIL--6 and hsCRP levels in obese adolescents, suggestive of anongoing, chronic, low􀁞grade inflammatory state. Background Obesity is a global problem. Even in poor and developing countries, obesity has reached alarming levels. In childhood, obesity may lead to insulin resistance. Retinol binding protein (RBP4), secreted primarily by liver and adipose tissues, was recently proposed as a link between obesity and insulin resistance. The role of RBP4 in pediatric obesity and its relationship with insulin resistance have not been well elucidated.Objective To compare RBP4 levels in obese and lean adolescents and to assess for a relationship between RBP4 levels and insulin resistance. Method This cross-sectional study was conducted in three senior high schools in Padang, West Sumatera, Indonesia. Subjects were adolescents aged 14-18 years, who were obese or normal weight (n=56). We measured subjects’ body mass index (BMI) and serum RBP4 concentrations. Insulin resistance was assessed using the homeostasis model assessment of insulin resistance (HOMA-IR) index.Results Similar RBP4 levels were found in the obese and normoweight groups (P>0.05). Higher RBP4 levels were found in the insulin resistant compared to the non-insulin resistant group, but the difference was not significant (P > 0.05).Conclusion There is no significant difference in mean RBP4 levels in obese adolescents compared to normoweight adolescents. Nor are mean RBP4 levels significantly different between obese adolescents with and without insulin resistance.

Background Childhood obesity is associated with increased risk of cardiovascular diseases and metabolic syndrome, such as insulin resistance. Clinically, insulin resistance may be manifested as acanthosis nigricans. Resistin has a biological activity that is important in glucose and lipid metabolisms and closely related to the incidence of insulin resistance.Objective To find out the association of resistin level with scale of acanthosis nigricans in adolescents obesity.Methods A cross-sectional study was conducted on 53 obese adolescents with acanthosis nigricans in senior high schools in Padang, West Sumatera. Degree of acanthosis nigricans was assessed using scale of Burke and then plasma resistin level was performed with ELISA. Data were analyzed using ANOVA and post-hoc test.Result The mean of resistin level in obese adolescents was 14.21 (SD 7.43) ng/dL. High resistin level was found in scale of acanthosis nigricans 2,3 and 4 (P=0.0001). Obese adolescents with severe degree of acanthosis nigricans has higher resistin level compared to milder acanthosis nigricans.Conclusion In obese adolescents, the higher degree of acanthosis nigricans, the higher level of plasma resistin.

Abstract Objectives Insulin resistance is the most common metabolic change associated with obesity. The present study aimed to investigate the relationship between insulin resistance and body composition especially adipose tissue in a randomized Tehrani population. Methods This study used data of 2,160 individuals registered in a cross-sectional study on were randomly selected from among subjects who were referred to nutrition counseling clinic in Tehran, from April 2016 to September 2017. Insulin resistance was calculated by homeostasis model assessment formula. The odds ratio (95% CI) was calculated using logistic regression models. Results The mean age of the men was 39 (±10) and women were 41 (±11) (the age ranged from 20 to 50 years). The risk of increased HOMA-IR was 1.03 (95% CI: 1.01–1.04) for an increase in one percent of Body fat, and 1.03 (95% CI: 1.00–1.05) for an increase in one percent of Trunk fat. Moreover, the odds ratio of FBS for an increase in one unit of Body fat percent and Trunk fat percent increased by 1.05 (adjusted odds ratio [95% CI: 1.03, 1.06]) and 1.05 (95% CI: 1.02, 1.08). Also, the risk of increased Fasting Insulin was 1.05 (95% CI: 1.03–1.07) for an increase in one unit of Body fat percent, and 1.05 (95% CI: 1.02–1.08) for an increase in one unit of Trunk fat percent. Conclusions The findings of the present study showed that there was a significant relationship between HOMA-IR, Fasting blood sugar, Fasting Insulin, and 2 h Insulin with percent of Body fat, percent of Trunk fat.

AbstractObjectiveThe purpose of the present study was to examine the association between soya food consumption and insulin resistance using baseline data of the Japan Multi-Institutional Collaborative Cohort (J-MICC) Study in Tokushima, Japan.DesignThis cross-sectional study included 1274 subjects, aged 34–70 years at baseline, living in Tokushima Prefecture between 2008 and 2013. Fasting blood samples were collected and information on lifestyle characteristics including soya food intake and medical history were obtained using a structured self-administered questionnaire. The homeostasis model assessment of insulin resistance (HOMA-IR) was measured and those with HOMA-IR≥2·5 were defined as having insulin resistance. Multiple logistic regression models were used to analyse the association between soya product intake and the prevalence of insulin resistance.SettingRural communities located in Tokushima Prefecture, Japan, between 2008 and 2013.SubjectsA total of 1148 adults (565 men and 583 women), aged 34–70 years.ResultsThe frequency of intake of miso soup, total non-fried soya products and total soya products showed significant inverse dose–response relationships with insulin resistance, after adjustments for potential confounders. When soya product intake was calculated as soya protein and isoflavone, the odds ratios of insulin resistance decreased significantly as the estimated intake of soya protein increased. Furthermore, significant inverse dose–response relationships were observed for total non-fried soya products and total soya products, after adjustment for total vegetable or total fibre consumption.ConclusionsThe present results indicate that the intake of soya products and non-fried soya products is associated with reduced insulin resistance in the Japanese population.

Abstract Objective: To describe the frequency of albuminuria in overweight and obese children and adolescents and to relate it to the severity of obesity, pubertal staging, associated morbidities and the glomerular filtration rate. Method: Cross-sectional study including 64 overweight and obese children and adolescents between 5 and 19 years of age. Data collected: weight, height, waist circumference and systemic arterial pressure. Laboratory tests: lipid profile; glycemia and insulin, used to calculate the Homeostasis Model Assessment (HOMA-IR); C-reactive protein; glutamic-pyruvic transaminase and albuminuria in an isolated urine sample (cutoff <30 mg/g). Creatinine was used to calculate the estimated glomerular filtration rate (eGFR, mL/min/1.73 m2). Results: The mean age was 11.6 ± 3.4 years, 32 (50%) and 29 (45.3%) were male and prepubertal. Forty-six (71.9%) had severe obesity. The frequency and median (min/max) of the observed values for albuminuria (> 30 mg/g) were 14 (21.9%) and 9.4 mg/g (0.70, -300.7 mg/g). The mean eGFR was 122.9 ± 24.7 mL/min/1.73 m2. There was no significant correlation between body mass index, pubertal staging, insulin and HOMA-IR with albuminuria values and neither with eGFR. Children with albuminuria tended to have higher values of diastolic blood pressure (75.0 ± 12.2 vs. 68.1 ± 12.4, p = 0.071). Conclusion: Albuminuria, although frequent in children and adolescents with obesity, was not associated with other morbidities and the glomerular filtration rate in these patients.

Health status and micronutrient intake among vegetarian and non-vegetarian in BaliBackground: Several studies have reported that vegetarian diets have health benefits for those adopting the diets.Objective: This study aimed to compare anemia status and micronutrient intake between vegetarians and non-vegetarians in Bali.Methods: This cross-sectional study was conducted in Bali. A total of 240 samples consisting of 160 vegetarians and 80 non-vegetarians were randomly selected. Hemoglobin (Hb) and fasting blood sugar levels were measured using the cyanmethemoglobin method and enzymatic colorimetric, respectively. Meanwhile, the data on micronutrient intakes were collected by interviews using the semi-quantitative food frequency questionnaire (SQFFQ). The data were analyzed using descriptive statistics, and the Mann-Whitney test was performed to analyze the differences.Results: The study’s results showed that the mean Hb levels in the vegetarian group were significantly lower than non-vegetarians (p=0.002; p<0.05), and the anemia status in vegetarian women were higher (22.5%) than non-vegetarian women (2.5%). The mean fasting blood glucose level in non-vegetarians was significantly higher than in the vegetarian group (p=0.000; p<0.05). There were no differences in zinc (Zn) intake between vegetarians and non-vegetarians, but there were significant differences (p<0.05) in vitamin C, vitamin D, vitamin B12, folic acid, magnesium (Mg), calcium (Ca), and iron (Fe) intakes.Conclusions: There were differences in anemia status and fasting blood glucose levels between vegetarians and non-vegetarians. The fasting blood glucose levels of non-vegetarians were higher than vegetarians, and there were differences in the intake of certain micronutrients between vegetarians and non-vegetarians.

Summary Background: Non-alcoholic fatty liver disease (NAFLD), the hepatic manifestation of insulin resistance, is the most common cause of chronic liver. The present study aimed to investigate the roles of Vaspin and omentin-1 in the NAFLD-related pathology including IR, inflammation and elevated blood pressure. Methods: This cross-sectional study was conducted among 83 NAFLD patients in Jahrom, Iran. Plasma levels of omentin-1, Vaspin, hs-CRP and IL-6 were measured. Anthropometric indices, lipid profiles, liver enzymes as well as abdominal ultrasonography were assessed. Results: Partial correlations controlling for age and sex showed significant positive correlation between Vaspin and fasting blood sugar (FBS), insulin, HOMA-IR, and hs-CRP. It has been observed that omentin negatively correlated with glucose levels. Moreover, a marginally significant association has been found between Omentin levels and systolic blood pressure (SBP). Conclusions: This study shows that Vaspin and Omentin-1 are associated with inflammation, insulin resistance and serum glucose levels in patients with NAFLD.
