Burnout among Hungarian physicians. Who are the most jeopardized?

Introduction: Burnout is one of the most challenging questions of the healthcare systems in the 21st century. Aim: The aim of this study was to analyze the Hungarian physicians’ burnout and its associations with workload factors (work hours, shift work, multiple workplaces). Method: Data of this representative, cross-sectional, online epidemiological study was obtained from online questionnaires completed by 4784 physicians. Results: Medium or high level personal accomplishment was present in 75.9% and emotional exhaustion in 58% among physicians, while medium or high level of depersonalization subscale was 53%. All of the 3 dimensions showed association with young ages (<35 years), work in in-patient care, shift hours and multiple workplaces. The physician partner increased the risk of depersonalization, however, children were protective factors. The work–home interface was associated with higher burnout scores. Conclusions: Physicians’ burnout is an important indicator of the functioning and effectiveness of the healthcare system. The results suggest the importance of prevention and intervention. Orv. Although burnout has been described as a serious hazard for personal and professional lives and has been surveyed among dentists in many countries, no study has been published regarding burnout among dentists in Lithuania. This study aimed to evaluate the burnout level among Lithuanian dentists and its association with demographic variables, job satisfaction, and other job-related variables. The data were collected among dentists online or during professional conferences while using an anonymous questionnaire (n = 380). The Maslach Burnout Inventory (MBI) was used to evaluate the burnout level. A Poisson regression was applied for the analysis of relationships between variables. We observed that 42.3% of the respondents had a high emotional exhaustion (EE) (95% confidence interval (CI): 37.4–42.3%), while 18.7% (95% CI: 15.0–22.9%) and 28,2% (95% CI: 23.4–32.6%) had high depersonalization (DP) and low personal accomplishment (PA), respectively. Nonetheless, 15.3% (95% CI: 11.8–18.9%) of the study population experienced a high level of overall burnout. An original job satisfaction index was elaborated. It was significantly associated with sum scores of all burnout dimensions: with the EE sum score (Ratio of Sum Score Means (RSSM) 1.54; 95% CI: 1.46–1.62), DP sum score (RSSM 1.59; 95% CI: 1.45–1.74), and PA sum score (RSSM 0.88; 95% CI: 0.84–0.92). It was concluded that Lithuanian dentists can be characterised by high burnout intensity and high prevalence of burnout, being especially evident in emotional exhaustion. The dentist with low job satisfaction appeared to be the most vulnerable to all burnout dimensions.

Introduction: Years of residency are the most challenging period of a medical carrier. Aim: The aim of this study was to analyze female residents’ (n = 380) workload, work satisfaction and burnout. Method: Data in this representative, cross-sectional epidemiological study were obtained from online questionnaires completed by 380 female residents. For a wider interpretation of the data, male residents (n = 176) were included in the analysis as a control group. Results: The average weakly work hours of female residents were 66 hours and 70% of them felt overloaded. The medium and high level personal accomplishment was 75.9%, the emotional exhaustion was 58% and the medium and high level of depersonalization subscale was 53%. Female residents were mostly dissatisfied with working conditions, financial status, and prestige of her work. Dissatisfaction with the Hungarian health system was about 80% and nearly a quarter of respondents were considering working abroad. Conclusions: Female residents represent the “critical mass” of the young doctors. Residents’ well-being is an important indicator of the functioning and effectiveness of the health care system. Orv. Objective: To gauge the prevalence of burnout among postgraduate trainees and consultants working in psychiatry department, Services Hospital Lahore, Pakistan. Methods: 31 trainee and consultant psychiatrists in Services Hospital Lahore, Pakistan participated in this cross-sectional study. Abbreviated Maslach Burnout Inventory (aMBI) measured burnout. It consisted of 9 items, relating to emotional exhaustion, depersonalization and personal accomplishment. Each item is scored on a seven-point Likert scale. For Emotional Exhaustion and Depersonalization, higher scores predicted greater burnout; Personal Accomplishment demonstrated the opposite, hence its scores were inverted. Participants with moderate scores in 2 or more dimensions were identified as suffering from burnout syndrome. Data was analyzed by SPSS 25.0. Result: Mean age of participants was 34.87 ± 8.06 years. 52% were female. 58% were consultants, with average experience of 7 years; 61% practiced in more than one place. 32.3% of participants had burnout syndrome. Emotional Exhaustion subscale showed the highest scores i.e. 7.06 ± 3.43. 71% of participants demonstrated moderate or high burnout in this scale. Average Depersonalization score was 2.94 ± 2.42, while that for Personal Accomplishment was 3.29 ± 2.25. Conclusion: A significant portion of the sample reported moderate and high level of emotional exhaustion. On the contrary, we found low levels of depersonalization in the sample. Most psychiatrists reported adequate levels of personal achievement. These results are reassuring because, despite the presence of emotional burnout, psychiatrists still have capacity to empathize and provide adequate patient care. Background: The aim of our study is to evaluate the prevalence of burnout among nurses practicing in the surgical units of a Moroccan university hospital.Methods: This is a qualitative cross-sectional observational study Conducted from July 2017 to December 2017 among nurses working in the 12 surgical units of the hospital. Nurses provided a self-administered questionnaire with socio-demographic and burnout data using the Maslach burnout inventory.Results: Among 183 nursing nurses, 130 (82 women/48 men) completed the questionnaires (71%). The prevalence of burnout across all grades was 86.2% (low level: 33.8 %, moderate level: 46.2%, high level: 6.2%). High emotional exhaustion was noted among 95 nurses (73.1%), a high level of depersonalization among 75 nurses (57.7%) and a low level of personal accomplishment among 12 nurses (9.2%). High emotional exhaustion and major depersonalization were observed together in 61 nurses (46.9%). Women appeared more likely to burnout (p=0.03). Nurses working in visceral surgical emergency unit, neurosurgery unit and traumatology orthopedics unit seemed to be more affected by emotional exhaustion (p=0.002).Conclusions: The prevalence of burnout among surgical nurses is worrying. Surgical nurses whose activity is much more related to emergencies are the most exposed. A preventive and therapeutic strategy is needed to reduce the extent of burnout.

BACKGROUND: Although previous studies have separately revealed that parameters such as anxiety, depression, and secondary traumatic stress (STS) are associated with burnout, there is still a limited understanding of the relationship between anxiety, depression, and STS and burnout in intensive care unit (ICU) nurses. AIMS: To investigate the relationship between levels of burnout, anxiety, depression, and STS in ICU nurses. METHOD: A cross-sectional study was conducted with ICU nurses ( N = 164) from a university hospital in eastern Turkey. The participants completed the anxiety, depression, STS, and burnout scales along with the descriptive characteristics form. The data were analyzed using descriptive statistics, correlation, and logistic regression analysis. RESULTS: The mean scores for STS, anxiety, depression, and burnout were 40.60 ± 13.77, 17.14 ± 12.90, 13.28 ± 9.75 and 41.39 ± 14.87, respectively. The results showed that, in the ICU nurses, anxiety, depression, and STS components explained 61% of emotional exhaustion, 38% of depersonalization, and 13% of personal accomplishment. CONCLUSIONS: While the present findings supported the paradigm that burnout in ICU nurses is associated with STS, anxiety, and depression, they also revealed some details about the psychopathological factors associated with burnout. These details were as follows: (1) individuals who resorted to avoidance as a component of STS on a high level were more likely to experience emotional exhaustion and depersonalization, (2) individuals with severe depressive symptoms were more likely to experience a decrease in their personal accomplishment, and (3) individuals with anxiety symptoms were more likely to experience both emotional exhaustion and personal accomplishment.

Abstract Background Physicians during the COVID-19 pandemic are working under relentless stress. This study aimed to identify the impact of the perceived fears of COVID-19 virus infection on the quality of life and the emergence of burnout syndrome among physicians in Egypt during the COVID-19 outbreak. This cross-sectional study was conducted between May 10th and June 9th, 2020, and included 320 Egyptian physicians who were working during the outbreak of the COVID-19 pandemic. The participants were interviewed using the Fear of COVID-19 scale (FCV-19S), Hospital Anxiety and Depression Scale (HADS), Maslach Burnout Inventory, and World Health Organization Quality of Life Scale (WHOQOL-BREF) for assessment of the perceived fears of COVID-19 virus infection, associated anxiety and depressive symptoms, burnout symptoms, and quality of life, respectively. Results Overall, most physicians were females (63%). Ideas about death, moderate-to-severe anxiety, and depressive symptoms were reported by 11, 28, and 29% of physicians, respectively. For burnout symptoms, high emotional exhaustion, high depersonalization, and low personal accomplishment were reported by 20, 71, and 39% of physicians, respectively. The perceived fear of COVID-19 virus infection was positively correlated with anxiety, depression, and burnout emotional exhaustion, and depersonalization symptoms, and negatively correlated with personal accomplishment and all quality of life domains. Conclusions Egyptian physicians experienced higher levels of COVID-19-related fears, anxiety, and depressive and burnout symptoms. There was a robust correlation between these perceived fears, and higher burnout symptoms, and poor quality of life among physicians. Specific interventions should be tailored to minimize the physical and mental burdens on the physicians during the COVID-19 pandemic.

<p><strong>Objective. </strong>To investigate the level and causes of stress and the risk of onset of burnout syndrome among physicians employed at the Primary Health Care Centre, Banja Luka.</p><p><strong>Subjects and Methods. </strong>Between March 1, 2018, and May 31, 2018 all physicians from the Primary Health Care Centre, Banja Luka were offered the following questionnaires to fill in: a socio-demographic questionnaire, a questionnaire for self-assessment of the level of stress and the Maslach Burnout Inventory for assessment of the risk of burnout syndrome. <strong>Results. </strong>Out of 211 physicians, 85.8% were female. A high level of stress was found in 77.7% of the subjects. Older doctors had higher levels of emotional exhaustion compared to younger doctors with a shorter length of service (r=0.236, P=0.01). Emotional exhaustion was significantly correlated with a high level of depersonalization, a low level of personal accomplishment and a high level of stress (r=0.380, r=-0.174 and r=0.574, P=0.01, P=0.04 and P&lt;0.01, respectively). Depersonalization correlated with a low level of personal accomplishment and stress (r=-0.347 and r=0.283, P&lt;0.01 and P=0.01, respectively), while the level of personal accomplishment was in a negative correlation with stress (r=-0.281, P=0.01). A high stress level was associated with a high degree of emotional exhaustion (OR 56.543; 95% CI 11.35-213.09; P&lt;0.001) as well as lack of personal accomplishment (OR 0.155; 95% CI 0.04-0.50; P=0.003).</p><p><strong>Conclusion. </strong>A high level of stress was associated with older age, female gender, as well as with a high degree of emotional exhaustion and a lack of personal accomplishment. Preventive measures are warranted.</p>

Introduction: Researchers’ interest in occupational burnout results primarily from the dangerous and extensive consequences of this phenomenon. The aim of the study was to analyze the level of occupational burnout among nurses and doctors in operating theaters. Materials and Methods: A cross-sectional survey study conducted on 325 nurses and doctors of seven hospitals in Poland. The Maslach Burnout Inventory (MBI) and the Areas of Worklife Survey (AWS) by Michael Leiter and Christina Maslach. Results: The mean values for the level of occupational burnout for the entire sample according to the scale from the Maslach Burnout Inventory by C. Maslach amounted to 14.35 for emotional exhaustion, 8.56 for depersonalization, and 11.90 for personal accomplishment; when compared to reference levels, they classified emotional exhaustion at a low level, depersonalization at an average level, and personal accomplishment at a high level of burnout. Areas of work life are predictors of occupational burnout. The analysis showed a relationship between three of the six variables. As the workload increased, so did the level of burnout among participants, and the categories of honesty and values. Conclusions: The conducted research has shown that occupational burnout among nurses and doctors in operating theaters occurs in all dimensions of this phenomenon (emotional exhaustion, depersonalization, job satisfaction). It was also shown that the areas of work life (workload, control, community, rewards, fairness, values) are predictors of occupational burnout among the respondents. This article shows how important the problem of burnout among operating theater medical staff is. Perhaps it will allow nurses and doctors to recognize this syndrome and encourage them make changes to their work to prevent burnout.

AbstractDuring the Covid-19 pandemic, nurses are at the forefront. Many nurses experience stress and fatigue due to increased workload. Stress and fatigue that is obtained at work, both physical and psychological fatigue, is known as Burnout. This study aimed to describe the incidence of Burnout experienced by nurses during the Covid 19 Pandemic. This research is a literature review. This study was looking for some articles from PubMed,Garuda, and Google Scholar, with keyword. There were 5 articles that matched the research inclusion criteria. Critical analysis of 5 articles used the JBI instrument. These articles used the mean calculation. In the Emotional Exhaustion category, the mean value was 22.75 and SD was 8.828. it meant that the burnout level in the Emotional Exhaustion category was at a moderate level. In the depersonalization category, the mean value was 7.54 with SD 4.248. it meant that the burnout level in the depersonalization category was at a moderate level. In the personal accomplishment category, the mean was 19.676 with SD of 6.7. it mean that the burnout level in this category was a high level. The nurses experience burnout during the Covid-19 pandemic. Terdapat 5 Artikel yang menggunakan perhitungan mean di dapatkan hasil kategori Emotional Exhaustion nilai mean sebanyak 22.75 dan SD 8.828 dimana hasil menunjukkan level burnout pada level sedang. Kategori depersonalization dengan nilai mean 7.54 dengan SD 4.248 dimana hasil burnout pada level sedang. Kategori personal accomplishment hasil mean 19.676 dengan SD 6.7 dimana hasil burnout menunjukkan level tinggi. Abstract BackgroundPostgraduate medical trainees are at an increased risk of burnout. Burnout has been defined as “a psychological condition of emotional exhaustion, depersonalization and reduced personal accomplishment occurring in people persistently exposed to emotional and interpersonal stressors at work”. It can arise either from within the individual, from the medical profession itself or from healthcare organisations. Burnout is associated with mental health issues in trainees, impaired work performance and suboptimal patient outcomes.MethodsA cross-sectional observational study of three cohorts of the Malta Foundation Programme was conducted using the Copenhagen Burnout Inventory (CBI) to evaluate incidence of personal-, work-related and client related burnout. The cohorts were at 3-months, 1-year and 2-years of training respectively. The CBI was administered via Google Forms after ethical clearance. Descriptive statistics and non-parametric comparative statistics were used to analyse the results through SPSS v25.0.ResultsA global response rate of 26.5% was achieved. CBI and its sub scales showed good internal validity. High scores for burnout in all three scales were identified except for client-related burnout in First Year doctors. Burnout peaked midway through the 2-year period. High numbers reported “feeling “tired” and “physically exhausted”. Males reported more burnout than females.ConclusionThe high level of burnout so early in the career of Maltese postgraduate medical trainees is of concern. More studies are needed on the causality of this burnout and what factors are associated with burnout in this population.
