ABSTRACT Schizencephaly is an extremely rare developmental birth defect or malformation characterized by abnormal clefts in the cerebral hemispheres of the brain, extending from the cortex to the ventricles, which may be unilateral or bilateral. This case report describes the general characteristics of a psychological home care program, reporting the main theoretical and technical elements in a 12-years-old case of type II Schizencephaly. The aims of the psychological home treatment were acceptance of the new treatment reality, a reduction in aggression and anxiety, and psychological support for the patient and family. In the psychological home care, patient’s awareness of illness was developed, along with family orientation, psychoeducation, relaxation techniques, and cognitive distraction. It can be observed that a significant improvement in the affective and emotional state was achieved within the patient’s clinical framework.

ABSTRACT:Background:Intravenous methylprednisolone (IVMP) is the treatment of choice for multiple sclerosis (MS) patients undergoing acute exacerbation of disease symptoms and yet its cost has not been accurately determined. Determination of this cost in different settings is also pertinent to consideration of cost-saving alternatives to in-patient treatment.Methods:Cost analysis from the point of view of the health care system of IVMP treatment of MS patients receiving treatment in association with a selected Toronto teaching hospital in fiscal year 1994/95 was carried out. Costs of any concurrent treatments were excluded.Results:Total cost for 92 patients, based on a 4 dose regime, was estimated to be $78,527. The the cost per patient was $1,1181.84 for in-patients (IP), $714.64 for out-patients of the MS Clinic (OP) and $774.21 for patients whose treatment was initiated in the Clinic, but completed in the home (HC). Sensitivity analyses indicated: 1) IP treatment was in all cases more expensive than that of OP or HC; 2) the cost savings of OP vs. HC was sensitive to assumptions made regarding Clinic overhead, Clinic nursing costs and Home Care Program overhead.Conclusion:Alternatives to in-patient care must be considered carefully. In this study, both out-patient and in-home treatment were cost-saving alternatives to in-patient treatment, but large differences in the cost of hospital out-patient vs. in-home care could not be demonstrated.

Background. With the release of the Romanow Commission report, Canadian governments are poised to consider the creation of a national home care program. If occupational and physical therapists are to have input in shaping such a program, they will need to learn from lost opportunities of the past. Purpose. This paper provides an overview of recent reforms to home care in Ontario with an emphasis on rehabilitation services. Method. Data were collected from documents and 28 key informant interviews with rehabilitation professionals. Results. Home care in Ontario has evolved in a piecemeal manner without rehabilitation professionals playing a prominent role in program design. Practice Implications. Rehabilitation services play a critical role in facilitating hospital discharges, minimizing readmissions, and improving the quality of peoples' lives. Canadians will benefit if occupational and physical therapists seize the unique opportunity before them to provide meaningful input into creating a national home care program.

RECENT YEARS have seen a resurgence of interest in organized Home Care programs as a variety of factors have spurred the search for alternatives to hospital care. Chief among them has been the economic burden of spiraling hospital costs. Many programs have been devised to enable chronically ill persons in the older age group—the "home-bound" geriatric patient—to be supervised in their own homes. There are, however, special reasons for attempting to control the admission of children to hospitals. Illness is a time when a child becomes more dependent than usual and seems to need the security of parents and the comfort of familiar home environment. Even though enlightened hospitals now encourage visiting, many parents cannot take advantage of this for such reasons as distance and having to care for the other children at home. There is debate as to the amount of emotional harm caused by hospitalization of small children; most workers would say it does no good, and, in some cases, can lead to serious sequelae. The Home Care Program for sick children at St. Mary's Hospital in London was started in April, 1954. One of us (A.B.B.) had the opportunity of participating in this program in 1961 while serving as an Exchange Registrar from Children's Hospital (Boston). It is felt that even though conditions in the United States and Great Britain may be different, there are enough similarities to make a descriptive account of the program of interest to American physicians. AbstractSubarachnoid haemorrhage (SAH) is a consistent presentation of haemorrhagic stroke of significance to clinicians in neurocritical care, inducing consequent effects on non-neurological systems, while at the same time, rendering the brain vulnerable to secondary physiological insult modifying neurological outcome, despite control of the original point of haemorrhage. Coordinated treatment depends on comprehensive evaluation of both cerebral and systemic physiology, identifying and treating impaired function. The presence of a dedicated neurocritical care team can benefit outcome. Protocols of care have evolved to meet evidence-based challenges, discarding potentially deleterious components of hypervolaemia and haemodilution, while maintaining pressure-guided perfusion. Treatment targets have also evolved with a shift in focus away from SAH-associated vasospasm, towards actual ischaemic outcome – illustrated by lack of effectiveness of pharmaceutical treatments of vasospasm. Clinicians must consequently review pathophysiological mechanisms of injury and devise new treatment opportunities.

Objetivo: avaliar a sobrecarga de cuidadores de pacientes atendidos por um programa de atenção domiciliar público. Método: trata-se de um estudo quantitativo, descritivo, transversal, com 127 cuidadores de pacientes atendidos pelo Programa Melhor em Casa. Coletaram-se dados sociodemográficos e clínicos e aplicaram-se também a Escala Zarit Burden Interview reduzida e a Escala de Desesperança de Beck. Realizou-se análise descritiva e bivariada de dados. Resultados: verificou-se que, dos 127 cuidadores, a maioria era do sexo feminino (114=89,8%), de cor parda (81=63,8%); composta por solteiros (56=44,1%) e a média de idade foi de 46,66 anos. Destaca-se que 38,6% (49) apresentaram sobrecarga de moderada a grave, 78,7% (110) afirmaram que houve mudanças em sua rotina diária, 59,8% (76) dizem ter sofrido alguma mudança no estado emocional após iniciar o cuidado do paciente e 56,7% (72) sentem dores no corpo. Confirmaram-se, na análise bivariada, 15 variáveis independentes que obtiveram significância estatística. Conclusão: conclui-se que a sobrecarga do cuidador de pacientes da atenção domiciliar é um achado comum e cuidados para a sua prevenção devem ser estabelecidos. Conclusion: the burden of caregivers of homebound patients is a common finding, requiring care actions for its prevention. AbstractMany studies on brain–computer interface (BCI) have sought to understand the emotional state of the user to provide a reliable link between humans and machines. Advanced neuroimaging methods like electroencephalography (EEG) have enabled us to replicate and understand a wide range of human emotions more precisely. This physiological signal, i.e., EEG-based method is in stark comparison to traditional non-physiological signal-based methods and has been shown to perform better. EEG closely measures the electrical activities of the brain (a nonlinear system) and hence entropy proves to be an efficient feature in extracting meaningful information from raw brain waves. This review aims to give a brief summary of various entropy-based methods used for emotion classification hence providing insights into EEG-based emotion recognition. This study also reviews the current and future trends and discusses how emotion identification using entropy as a measure to extract features, can accomplish enhanced identification when using EEG signal.
