Background: Seizures are the most common paediatric neurologic disorder, with 4% to 10% of children suffering at least one seizure in the first 16 years of life. Most seizures occur before age 3 years.Methods: The present study was undertaken with an objective to study prevalence of seizure disorders among the children aged 14 years and below in rural area of a district and to study some socio-demographic factors and some risk factors associated with it among the study subjects. This was a community based cross-sectional descriptive study.Results: The prevalence of seizure disorder in the area studied was 10.84%. Of the total 1200 subjects examined, 52.67% were males and 47.33% were females. Mean age of the study population was 6.48±3.33 years, median was 6 years. Age, sex, family history, and socio-economic status were highly statistically significant with risk of seizure disorders (p<0.01). H/o Head injury, H/o CNS infection were statistically age, sex, family history of seizure disorders and socio-economic status were independently associated with risk of seizure disorders (p<0.01).Conclusions: prevalence of seizure disorder in the area studied was 10.84%.Significant association was observed between seizure disorder and age, sex, socio economic status, family history of seizure disorder, h/o infections and h/o head injury. Further research is needed to better understand the intricate relationship among socio-demographic risk factors and seizure disorders in order to ensure the best possible outcomes for individuals with seizure disorders.

Background: Prevalence of gestational diabetes mellitus (GDM) vary widely depending on the region of the country, dietary habits, and socio-economic status. This study was undertaken to determine the prevalence of GDM and risk factors associated with it, in women attending an antenatal care (ANC) clinic at urban health training center in Rajkot city, Gujarat.Methods: This study enrolled women, with estimated gestational age between 24 and 28 week, attending UHC in Rajkot. After informing, women who consented to participate were given a standardized 75 g oral glucose tolerance test (OGTT). A proforma containing general information on demographic characteristics, socio-economic status, education level, parity, family history of diabetes and/or hypertension was filled up. Diabetes in pregnancy study group India (DIPSI) criteria for 75 g 2-h OGTT was used for diagnosing GDM.Results: Total of 366 women participated in the study and GDM was diagnosed in 36 (9.8%) women. Age, parity, BMI, family history of DM and hypertension were accessed and found to be not significant. History of hypertension in previous pregnancy was statistically significant with occurrence of GDM in present pregnancy.Conclusions: The prevalence of GDM was found to be 9.8 per cent in a UHC, Rajkot. Appropriate interventions are required for control of GDM and modifications of risk factors.

AbstractIntroductionSubstance abuse remains one of the major challenges in young people, as it is one of the top five causes of disability-adjusted life years (DALY). The present study aims to find the prevalence and determinants of substance use among young people attending an urban primary health center in Delhi.MethodologySystematic random sampling was used to enroll the calculated sample size of 190. Substance use was assessed using ASSIST (an Alcohol Smoking Substance Involvement Screening Tool) and brief intervention was given based on the standard guidelines of ASSIST. The total score among the substance users is calculated and divided into Grades 1, 2 or 3. Log binomial regression was performed to quantify the association between substance use and covariates such as age, sex, education, occupation, family history of substance use, socio-economic status and family type. The association was expressed in odds ratio (OR) with 95 percent confidence interval (CI).ResultThe mean age of study participants was 18.6 ± 4.1, ranges from 10 to 24 years. Out of 48 substance users, 43.7% were consuming only tobacco, 22.9% were consuming only alcohol and 33.3% were polysubstance users. The history of substance use among family members of participants was found to be 46.3%. Median substance involvement score of tobacco, alcohol and cannabis users was 19 (IQR: 14.5–22), 19 (IQR: 13.5–25) and 22.5 (IQR: 22–23), respectively. Among tobacco users, 2.7% were Grade 1 and 7.2% were Grade 2. Four (16%), 20 (80%) and one (4%) of alcohol users were Grades 1, 2 and 3, respectively. Among the cannabis users, four (100%) were in the Grade 2 category. The median age of initiation of substance use among users was 16 (range 13–21) years. The analysis shows substance use was almost 25 times (adjusted OR = 25.84, 95% CI 5.65–118.09) more common among males and it increase by 2.5 times with a decrease in socio-economic status (adjusted OR = 2.52, 95% CI 1.27–5.02) and the result is significant. The substance use was almost 7 times higher when there is a family history of substance usage (adjusted OR = 7.40, 95% CI 2.15–25.4). Residential and marital status were not significantly associated with substance use.ConclusionMale sex, lower socio-economic status, participants currently not going to school/college, family history of substance use were found to be significant predictors of substance use among the study participants.

Background: The prevalence of gestational diabetes mellitus (GDM) is increasing all over the world and varies widely depending on the region of the country, dietary habits and socio-economic status. The prevalence of GDM with its associated risk factors has important health complications for both mother and child.Objectives: The aim of this study was to evaluate the prevalence of GDM and risk factors associated with it in women attending Diabetic Association Medical College Hospital in Faridpur for ante-natal care.Materials and Methods: In this cross-sectional study, screening for GDM was performed in 303 pregnant women. Women who consented to participate underwent a standardized 2-hour 75 gm oral glucose tolerance test (OGTT). A proforma containing general information on demographic characteristics, socio-economic status, education level, parity, family history of diabetes and past history of GDM etc. was filled in. American Diabetes Association (ADA) criteria for 75 gm 2-hour OGTT was used for diagnosing GDM.Results: A total of 303 women participated in the study and GDM was diagnosed in 22 (7.3%) women. A single abnormal value was observed in additional 33 (10.89%) women. On bivariate analysis risk factors found to be significantly associated with GDM were age, household income, parity, educational level, socio-economic status, hypertension, BMI, weight gain, acanthosis nigricans, family history of diabetes and past history of GDM; but on multivariate analysis only upper middle class and presence of acanthosis nigricans were found to be significantly associated with GDM.Conclusion: This study demonstrates a high prevalence of GDM in Bangladesh. AbstractAttention deficit hyperactivity disorder is a disorder in which a person is unable to control behavior due to difficulty in processing neural stimuli, accompanied by an extremely high level of motor activity. The prevalence is much higher ranging from 8 to 77% among children with seizure disorders than in the general population. When attention deficit hyperactivity disorder presents in children with seizure disorder, it makes the treatment complicated and the prognosis poor. Hence, understanding the magnitude of attention deficit hyperactivity disorder and associated factors would be important to have a policy intention towards these people and to design appropriate interventions. Therefore, the current study was conducted to determine the comorbidity of attention deficit hyperactivity disorder and associated factors in children with seizure disorders. A hospital-based cross-sectional study was conducted by taking 260 children who have follow ups in the pediatric seizure clinic. The systematic random sampling technique was used to recruit participants. A structured, pretested and interviewer-administered questionnaire which included questions on associated factors and standard disruptive behavioral disorder rating scale was used to collect data. Data were coded, entered and cleaned by using the Epi-Data version 3.1 and exported to SPSS version 20 for further analysis. The multivariate binary logistic regression was used to check the association between independent and dependent variables. Variables with significant associations were identified based on adjusted odds ratio, with a 95% CI and p-value of < 0.05 will be considered as statistically significant. The prevalence of attention deficit hyperactivity disorder among epileptic children was found to be 115 (44.2%),with a confidence interval of (38.1–50.5),out of which only 3 (2.6%) were detected as having mental health problems by the clinician. The predominant subtype was inattentive type 96 (61.1%). Factors significantly associated with attention deficit hyperactivity disorder were male sex (AOR = 2.70 CI 1.46–4.97), family history of seizure disorder (AOR = 2.42 CI 1.26–4.65), family history of mental illnesses (AOR = 4.14 CI 1.76–9.68), sudden onset of the seizure (AOR = 2.37 CI 1.32–4.27), and uncontrolled seizure (AOR = 2.55 CI 1.41–4.61). Attention deficit hyperactivity disorder was common among children with seizure disorders in the study area. Male sex, sudden onsets of seizure, family history of seizure, and that of other psychiatric disorders as well as uncontrolled seizures were factors that increased the odds of attention deficit hyperactivity disorder. Therefore, interventions that would address such factors would help to overcome further complications.

Abstract Aetiology of transient global amnesia (TGA) remains uncertain, though many have been proposed, including ischaemic, migrainous or epileptic pathologies. We attempted to determine risk factors for TGA, as well as prognostic factors that may cause recurrence. We evaluated clinical history, family history and magnetic resonance diffusion-weighted imaging (DWI) studies of 93 prospective patients with TGA. Patients were followed from 2004-2016. Fifteen of 93 (16%) patients experienced a recurrence of TGA. Among precipitating events, physical activities inducing Valsalva-like manoeuvres were most common, followed by emotional stress. Eighty-four patients had possible comorbidities or risk factors for TGA, though no single risk factor was ubiquitous. Risk factors associated with recurrence were head injury (isolated vs. recurrent, 16.7% vs. 53.5%, p < 0.01), depression (isolated vs. recurrent, 15.4% vs 46.7%, p = 0.01) and family history of dementia (isolated vs. recurrent, 20.5% vs. 46.7%, p = 0.03). Of 15 patients with confirmed recurrent TGA, two developed dementia and four subjective memory impairment. DWI lesions were observed in 24 patients and were located anywhere within the hippocampus. DWI lesions were not significantly associated with outcomes (recurrence, subjective memory impairment, dementia). We have found that depression, previous head injury and family history of dementia may predict TGA recurrence.

Among those with recurrence, depression, previous head injury and family history of dementia may be predictive factors. Encouraging primary prevention of head injury, managing depression and assisting in development of adequate coping mechanisms may decrease incidence. Education of healthcare works will also increase diagnostic rates, allowing for improved education and comfort for patients and families.

Background: Hypertension is the one major global burden disease, causes 7.5 million deaths i.e.12.8%. Coronary Heart Disease Prevails 3-4% rural and 8-10% of urban population under 20 year’s age in India. Two fold rise in rural & six fold rise in urban areas since four decades. Environmental and Genetic factors i.e. Sex, BSA, obesity, family history of hypertension, dietary habits, physical activity, stress, race, ethnicity and socio economic status influence on children and adolescent. Many studies have established normal standards of BP for the children of different ages and races in their countries. Indian children cannot be adopted due to differences in ethnic, socio-economic, dietetic, environmental and emotional factors.Methods: A cross sectional study done with 2422 children of 5 to 16 years age school children, selected from 13 Schools (Government and Private) of Urban and Rural areas of Warangal.Results: A linear increase in mean BP with age, sex, weight, height, social status and locality. DBP has strong negative correlation with sex. i.e. female have high DBP and children of lower class and rural area has high DBP. 54 children <85th and 95th percentile and 5 children >95th percentile has comparatively high mean BP, 27 children with history of Hypertension in parents are >85th and 95th percentiles. Positive correlation with BMI in both sexes studied. A multivariate regression study confers positive strong correlation of Mean SBP and DBP with anthropometry.Conclusions: Observed similar results of SBP and DBP in both sexes, linear Increase in mean SBP and DBP with increasing age, weight, height and BMI. Family history of hypertension and high SES had direct correlation to SBP and DBP; class I SES has higher Mean SBP than class III SES.

Background  Anemia in pregnancy is responsible for many adverse effects on maternal and fetal outcome. It is the commonest hematological disorder accompanying pregnancy. Prevalence of anemia among pregnant women is fairly high worldwide, especially in developing countries. A research finding published by World Health organization revels that the prevalence of anemia among pregnant women in developing countries averages 56% (ranging between 35 to 100%). Though Nepal has decreasing trend of prevalence of anemia, it is still high (42% in 2006). Identifying the associated factors which are responsible for the development of anemia during pregnancy would be of great help in mitigating the burden to some extent. Ojective of the study was to find out the pattern of anemia and associated risk factors for anemia during pregnancy. Materials and Methods A cross-sectional study was conducted in VDC named Hansposa.  Door to door survey was carried out to collect data using pre-designed questionnaires. Ante-natal check-ups were provided with analysis of hemoglobin levels, in every individual of study population, using conventional Shali's method. Chi-square test was applied, where applicable. Results Altogether 249 pregnant women were included in the study. The proportion of anemia was found to be 46.6%, the cases of mild anemia, and moderate anemia were 51.0% and 49.0% respectively. Risk factors includes: history of complications during previous pregnancy (P<0.001), education (P<0.05), age at marriage (P=0.028), low socio-economic status (P<0.001), gravidity and parity (P<0.01), poor knowledge about anemia (P<0.001), abnormal body mass index (P<0.001) and smoking (P=0.05). Conclusion This study indicates that the prevalence of anemia during pregnancy bears a resemblance to national value. Major associated risk factors include: history of complications during previous pregnancy, low knowledge about anemia in pregnant women, women with low body mass index, age at marriage, socio-economic status, gravidity and parity, and smoking. Background: Seizure disorder a term used to include epilepsy, febrile seizures and other types. The international classification and Diagnostic tools including EEG monitoring and MRI are helpful to categorize seizure and treat them with various antiepileptic medications.Methods: Data from patients in the age group of 6 months to 5 years, over 2 months period, presenting with seizure disorders was collected in prescribed proforma and evaluated for type of seizure disorder, age of onset, family history of seizures, previous history of febrile convulsions, presence of neurological abnormality, EEG changes and response to drugs. They were followed up over a period of 2 years.Results: In this study 200 children’s with seizure disorders from a period of 2 months, were screened, reviewed and followed up for a period of two years. Incidence of seizure disorder is 4.37% with male predominance. Of which febrile seizures (52.5%), Generalized Seizures (25.5%), partial seizures (12%), besides unclassified (10%) and status epilepticus (10%). 12% of the children had positive family history of epilepsy and 14.56% had history of previous febrile convulsion. Birth injuries (19.4%) and CNS infection (38.6%) and space occupying lesions (9.7%) constitute the major etiological factors. 10% patient with seizure disorder had abnormal neurological signs. EEG Examination showed 56.8% normal and 33.2% Generalized abnormality. On follow up 51% where found regular, out of which 30% went into remission.Conclusions: Seizure disorder is quite significant condition in children. Though benign, the febrile seizures are the most common type of seizure in children. The major preventable etiological factors for seizures are Birth trauma and CNS infections. Clinical and EEG work up is necessary for proper management of seizures. Health education is needed to stress the importance of regular treatment and to educate for removal of stigmata.
