Abstract Background: Tibial rotation accompanying sagittal movement contains the phenomenon of screw-home movement of the knee, which plays an important role in knee stability during extension. This study aimed to investigate the alteration of screw-home movement in patients with knee osteoarthritis (OA).Methods: Participants (n =67) in this cross-sectional study were outpatients in the department of orthopedics of a general hospital and included patients with knee OA (n=31) and asymptomatic control subjects (n=36).Knee kinematic data were measured using an inertial measurement unit. The total tibial rotation was obtainedduring knee sagittal movement. The acquired angle of tibial rotation was divided into four periodseach extension and flexion. The total tibial rotation and the variation of each period were compared between the OA and control groups.Results: Significant differences arose between the OA and control groups in the total tibial rotation during knee extension and flexion(P< 0.001).The variation of tibial rotation was also significantly different between groups for all periods (P< 0.001; knee extensionat 70° to 45°,P = 0.014).Conclusions: We found a reduction in the total tibial rotation and loss of the screw-home movement in the unloaded OA knee. To normalize the screw-home movement, it is necessary to promote proper articular movement of the knee joint and suppress the hyperexcitability of the medial muscles.

Abstract Introduction. Changes in thyroid hormone level can affect the cardiovascular system. The aim of this study was to show how the Tpeak -Tend (Tpe) interval, which is a new marker of ventricular arrythmia, is affected in patients who have become euthyroid following Levothyroxine treatment for hypothyroidism, as this has not been examined previously in literature. Materials and Methods. This, cross-sectional study included a total of 119 females aged 18-45 years, separated into 3 groups as hypothyroid, euthyroid and control groups. For evaluation of the QTc and Tpe intervals, examination on precordial V5 lead was made of all the ECGs taken routinely on presentation of the patients. Results. The Tpe and QTc intervals of the hypothyroid group were determined to be significantly prolonged compared to those of the euthyroid and control groups (p < 0.001) and the values of the euthyroid and control groups were similar. A positive correlation was determined between TSH levels and Tpe and QTc intervals. Tpe interval AUC = 0.801 (%95 CI: 0.719 – 0.884) was higher than that of QTc AUC = 0.689 (%95 CI: 0.591 – 0.786). Conclusions. The Tpe duration was evaluated in respect of the risk of arrythmia in hypothyroid patients. In patients who had become euthyroid, the Tpe interval was found to be similar to that of healthy individuals and was more predictive than QTc. In the light of these findings it can be recommended that measurement of the Tpe interval should be preferred to QTc as a marker of the arrythmogenic effect in hypothyroid patients.

Objectives: The present research aimed to evaluate the relationship of supra patella effusion with pain and disability in patients with knee osteoarthritis by Ultrasonography (US). Methods: In a cross-sectional study, 60 patients with knee OA (Mean±SD score of body mass index: 29.81±5.64 kg/m2 and age: 50.48±7.57 years) were selected by nonprobability sampling method. Supra patella effusion was evaluated using an US. All study subjects completed the Visual Analogue Scale (VAS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) for pain and disability outcomes, respectively. To evaluate the relationship between effusion, disability, and pain, the Pearson’s correlation coefficient was employed.  Results: There was a poor but significant relationship between the area of effusion (r=0.27, P=0.03), the thickness of effusion (r=0.32, P=0.01), with pain. No correlation was found between the trace of effusion (r=-0.08, P=0.5) and pain. The area of effusion (r=0.1, P=0.17), the thickness of effusion (r=0.08, P=0.51), and the trace of effusion (r=0.0, P=0.9) were not correlated with disability.  Discussion: The effusion of supra patella was slightly correlated with pain. In contrast, the effusion of supra patella demonstrated no correlation with disability.

Objective: This study was designed to analyze the BCG vaccination status and compare the immune globulins levels in thehousehold contacts of tuberculous patients with non contacts of unexposed healthy peoples. Place: Gulab Devi Hospital, Post GraduateMedical Institute and Defense Housing Authority Lahore. Study design: Cross sectional study. Materials and Methods: Investigationslike ESR were done and the sera of 200 persons included in the study were tested for anti tuberculous antibodies by ELISA. BCGvaccination present or absent and scars were positive or negative in both groups were included. Results: The combined serologicalpositivity of the household contacts was 65.8% and for non-contacts was 34.1%. BCG scars were mostly absent in the householdcontacts as well as in non-contacts; But statistically IgG and IgA were present significantly by higher number in the household contacts ascompared to non-contacts, where as no significant difference in the IgM levels. These immunoglobulins status were compared with BCGscars in both study and control groups. The results were analyzed by SPSS version 14. Conclusions: Household contacts of tuberculosispatients are more susceptible to tuberculosis as compared to the non-contacts, as shown by positive and negative status ofantituberculous antibodies in the house hold contacts. More over BCG vaccination has no significant role in humoral evaluation.

Abstract Background Sarcopenia and knee osteoarthritis (OA) are two major risk factors for falls in older adults. The coexistence of these two conditions may exacerbate the risk of falls. This cross-sectional study aimed to test the hypothesis that older adults with coexisting sarcopenia and knee OA displayed an increased risk of falls experience. Methods Participants recruited from an orthopedic clinic were divided into four groups according to the presence of sarcopenia and radiographic knee OA: isolated sarcopenia, isolated knee OA, sarcopenia + knee OA, and control (i.e., non-sarcopenia with non-OA) groups. We used questionnaires to assess falls experience in the prior 12 months. We performed logistic regression analyses to evaluate the relationship between the four groups and falls experience. Results Of 291 participants (age: 60–90 years, 78.7% women) included in this study, 25 (8.6%) had sarcopenia + knee OA. Participants with sarcopenia + knee OA had 4.17 times (95% confidence interval: 0.84, 20.6) higher odds of recurrent falls (≥2 falls) than controls after adjustment for age, sex, and body mass index. The increased recurrent falls experience was not clearly confirmed in participants with isolated sarcopenia and isolated knee OA. Conclusions People with coexisting of sarcopenia and knee OA displayed increased recurrent falls experience. This study suggests a new concept, “sarcopenic knee OA”, as a subgroup associated with higher risk of falls, which should be validated in future large cohort studies. Trial registration. Background: Parasitology is a course filled with many complicated terminologies. In this study, a guide was designed to facilitate the learning of parasitology. More precisely, an annual educational calendar of medical parasitology was designed based on the weeks of education and its effectiveness was assessed in university students of medical sciences. Materials and Methods: This cross-sectional study included 174 students who were randomly divided into two intervention and control groups. At the baseline, the students were tested with a questionnaire at the beginning of each teaching session and the end of the intervention. In addition, another questionnaire was used to compare the attitude and satisfaction of students in two groups. Finally, the scores of the students were entered in SPSS 16 for analysis. Results: The results of parametric tests showed that the mean scores of laboratory and medical students during several tests in different sessions were significantly different in the two groups. Conclusion: The educational calendar is a new method in medical education, especially in the field of medical parasitology. This calendar as an educational supplement, emphasizes on students’ active learning and contributes to improving the quality of teaching the parasitology course in addition to meeting their educational needs.

ObjectivesTo examine the associations between serum magnesium (Mg) concentration with the prevalence of metabolic syndrome (MetS), diabetes mellitus (DM), hypertension (HP) and hyperuricaemia (HU) in patients with radiographic knee osteoarthritis (OA).MethodsThe present study was conducted at the Health Management Center of Xiangya Hospital. Radiographic OA was evaluated for patients aged over 40 years with basic characteristics and blood biochemical assessment. Serum Mg concentration was measured using the chemiluminescence method. MetS, DM, HP and HU were diagnosed based on standard protocols. The associations between serum Mg concentration with MetS, DM, HP and HU were evaluated by conducting multivariable adjusted logistic regression.ResultsA total of 962 patients with radiographic knee OA were included. Compared with the lowest quintile, the multivariable adjusted ORs and related 95% CIs of DM were 0.40 (95% CI 0.23 to 0.70, p=0.001), 0.33 (95% CI 0.18 to 0.60, p<0.001), 0.27 (95% CI 0.14 to 0.52, p<0.001) and 0.22 (95% CI 0.11 to 0.44, p<0.001) in the second, third, fourth and highest quintiles of serum Mg, respectively (p for trend <0.001); the multivariable adjusted ORs of HU were 0.33 (95% CI 0.19 to 0.59, p<0.001), 0.52 (95% CI 0.30 to 0.91, p=0.022) and 0.39 (95% CI 0.22 to 0.70, p=0.001) in the third, fourth and highest quintiles of serum Mg, respectively (p for trend <0.001); and the multivariable adjusted ORs of MetS were 0.59 (95% CI 0.36 to 0.94, p=0.027) in the second and 0.56 (95% CI 0.34 to 0.93, p=0.024) in the highest quintiles of serum Mg. However, the inverse association between serum Mg and the prevalence of MetS was non-linear (p for trend=0.067). There was no significant association between serum Mg and HP in patients with OA.ConclusionsThe serum Mg concentration was inversely associated with the prevalence of MetS, DM and HU in patients with radiographic knee OA.Level of evidenceLevel III, cross-sectional study.

<p class="abstract"><strong>Background:</strong> Lack of knowledge may lead to depression, anxiety and poor coping skills, which may affect the patient’s quality of life while health education is an effective intervention and can reduce pain and disability. Hence the present study was planned with the following aim to assess the knowledge regarding self-care among knee osteoarthritis patients and to develop and administer structured teaching program from knee osteoarthritis patients.</p><p class="abstract"><strong>Methods:</strong> Data related to the study were collected from knee osteoarthritis patients attending the out patients Department at Medical Institute at Bhuj district. The period of conduction of study was approximately one year. A total of 100 patients diagnosed with knee arthritis were included in the study.<strong></strong></p><p class="abstract"><strong>Results:</strong> respondents with an immediate family member with knee OA averaged a 5.5 higher knowledge score than respondents whose family members did not have the condition (p&lt;0.001). Respondents diagnosed clinically of knee OA averaged 2.06 higher knowledge score in comparison to respondents without knee OA (p=0.011).</p><p><strong>Conclusions:</strong> A higher knowledge of knee OA was found for respondents with tertiary education over those with a primary or secondary education. The results from this cross-sectional study impact important public health decisions, given that respondents over 50 years, with a family history, self-awareness, and a knee OA diagnosis, showed better knowledge of symptomatic knee OA as shown in the multivariate analysis. </p>

Background. Inflammation might play a crucial role in the pathogenesis of osteoarthritis (OA). Interleukin-34 (IL-34) is a well-known proinflammatory cytokine. Objective. The objective of this study was to detect IL-34 levels in serum and synovial fluid (SF) of patients with OA and to investigate their correlation with radiographic and symptomatic severity. Methods. One hundred and eighty-two OA patients and 69 controls were recruited. IL-34 levels were measured by enzyme-linked immunosorbent assay (ELISA). Radiographic and symptomatic severity of OA was reflected by Kellgren-Lawrence (KL) grades and Western Ontario McMaster University Osteoarthritis Index (WOMAC) scores, respectively. Results. SF IL-34 levels were independently associated with the KL grade (B=0.273, 95% CI: 0.150–0.395; P<0.001). SF IL-34 levels were significantly correlated with WOMAC scores (r=0.265, 95% CI: 0.123–0.399; P<0.001). The correlation between SF IL-34 levels and WOMAC scores was still significant after adjusting for confounding factors (B=0.020, 95% CI: 0.001–0.038; P=0.035) in OA patients. Conclusions. We found that IL-34 levels in SF were significantly associated with the radiographic and symptomatic severity of knee OA.

Introduction: Knee osteoarthritis (KOA) is the most common degenerative joint disease resulting in bone pain and disability. The aim of current study is to determine diet quality by healthy eating index (HEI)-2015 in association with pain and functional status among a sample of participants with primary knee OA. Methods: In this cross-sectional study, 220 patients with knee OA were recruited via convenience sampling in the outpatient clinics of Tabriz University of Medical Sciences between April and September 2018. The HEI-2015 score was calculated from dietary data collected using a Food Frequency Questionnaire (FFQ). Visual analogue scale, Western Ontario and McMaster Universities Osteoarthritis Index and the SF36 quality of life (QoL) questionnaire were applied to measure the pain intensity, functional status and QoL in the participants, respectively. Participants were categorized based on the quintile cutoff points of HEI score including 42-62, 63-69, 70-75, 76-78 and 79-100. Results: The mean score of HEI was 70.62±10.18 (range: 42–89). Participants with greater HEI- 2015 scores had higher total energy intake (P=0.008) and greater dietary intake of carbohydrates (P=0.01), protein (P=0.009), monounsaturated fatty acids (P=0.01), polyunsaturated fatty acids (P=0.007) and fiber (P=0.009) and lower intake of saturated fatty acids (P=0.005). Participants in higher quintiles of HEI had significantly lower pain intensity (P=0.001) and higher scores of physical function (P=0.001), pain (P=0.001) and role limitation due to physical problems (P=0.005) subscales of SF-36 QoL questionnaire in comparison with participants in lower quintiles of HEI-2015. Conclusion: The HEI-2015 score is associated with pain intensity and two domain of QoL in patients with knee OA.
