Viral infections with aerosol transmission are one of the most common infectious diseases in the world. Their relevance is due to the wide distribution and socio-medical consequences. To study the dynamics of the incidence of viral infections with aerosol transmission in Sumy Oblast, to determine the level of influence of social and natural factors on the intensity of the epidemic process. Data from the sectoral statistical reporting of the Ministry of Health of Ukraine, Sumy Regional Laboratory Center, the Main Department of Statistics in Sumy Oblast, Sumy Regional Center of Hydrometeorology were used. Epidemiological and statistical research methods were used. It was established that in Sumy Oblast the epidemic process of aerosol viral infections was characterized by a pronounced tendency to reduced incidence of influenza (Rinc.aver.= -6.2 %) and rubella (Rinc.aver.= -22.7 %), moderate reduction tendency ‒ to mumps (Rinc.aver = -2.4 %); high intensity with no reduction in incidence ‒ for other acute respiratory diseases (Rinc.aver = 0.2 %); a sharp increase in the incidence of measles (Rinc.aver.= 23.1 %). Coronavirus infection caused by SARS-CoV-2 was found in 3% of the population. The average long-term rates of measles, rubella, and mumps vaccinations with MPR-1 and MPR-2 vaccines were 70.9 % and 61.2 %, respectively. An inverse correlation was established between air humidity, population, coefficients of natural and migratory movement and measles incidence (p<0.05). The system of epidemiological surveillance of infections of viral etiology with aerosol transmission requires new approaches to the development of preventive measures.

The review analyses measles incidence in Russia at the elimination stage. Epidemiological studies have revealed changes in the measles epidemic process in Russia. It was shown that whereas the specificity of the epidemic process in the pre-vaccination period was mainly determined by the influence of the infectious agent, lately the impact of the social factor has increased. There was an increase in the impact of decreed groups on maintaining the circulation of the virus in the Russian Federation. The aim of the study was to analyse the specificity of the measles epidemic process in Russia, to identify the major current problems of measles vaccination, to reveal reasons that prevent measles elimination in the country given the present environment, and to formulate possible ways of solving them. The article demonstrates the role of migration processes within the country and infection importation from neighboring territories in the rise of disease outbreaks. The article discusses Russian regulatory documents that are devoted to combating the spread of measles, their consistent replacement and refinement bearing in mind the changing epidemiological situation and the current WHO criteria and guidelines for the infection elimination. High-level epidemiological surveillance of the infection is a key factor in choosing the optimal strategy of measles vaccination. The article lists indicators of epidemiological surveillance effectiveness. It also highlights the importance of molecular genetic monitoring of the measles pathogen. The authors outline measures that are necessary to achieve measles elimination in the Russian Federation given the current epidemiological situation.

The widespread worldwide spread of acute respiratory diseases is an urgent problem in health care. Expressed polyetiology of respiratory diseases does not allow to limit the use of specific vaccine preparations and dictates the need to use to combat them a variety of non-specific means that stimulate the natural resistance of the human body. The main pharmacological action of sodium deoxyribonucleate is the stimulation of phagocytic activity of T-helpers and T-killers, increasing the functional activity of neutrophils and monocytes/ macrophages, providing regeneration and repair processes in the epithelial component of antiviral protection of the body. Based on the above, the study of the clinical efficacy of Derinat® in the form of spray in the treatment of acute respiratory viral infections is relevant.

West Nilefever (WNF) is the most important arbovirus infectionfor the Volgograd region. The observed decrease in the WNF incidence from 2001 to 2006, in 2008 and2009, the reducing the number ofsevere forms ofdiseases only give evidence ofa temporary regression of the epidemic process in the territory of the Volgograd region. In the territory of the Volgograd region there are prerequisites and harbingers of the complication of epidemiological situation on West Nile fever. The aim of the performed in the Volgograd region epidemiological surveillance for WNF is to provide the information necessary and sufficient for the analysis of the epidemiological situation, as well as to determine the direction of policy and activities in public health and in making management decisions.

Abstract Background : Diarrheal infections remain relevant for many countries of the world. The processes of globalization, fundamental changes in nutrition and water consumption contributed to the fact that the significance of individual infection sources, etiological structure of diarrheal infections changed. Purpose of the study: on the background of the analytical study of the incidence of diarrheal infections in Ukraine under the current conditions, to determine the factors influencing epidemic situation. Methods: The reports of the Ministry of Health of Ukraine, Main Administrations of Statistics in Kharkiv, Odesa, Zaporizhia oblasts for 2011-2018 are used in the paper. Epidemiological and statistical research methods are applied. Results: The epidemic situation with diarrheal infections in Ukraine is characterized by a low incidence of typhoid fever (from 0.012 to 0.14 per 100 thousand people), shigellosis (from 1.97-6.13 per 100 thousand people), stable incidence rates without the downward trend, salmonellosis (from 17.35 till 24.1 per 100 thousand people), high incidence of diahrreal infections of specified etiology (from 115.5 to141.9 per 100 thousand people) and diahrreal infections of unspecified etiology (from 69.76 to 107.02 per 100 thousand people). The most complicated epidemic situation is observed in economically most developed regions of the country. Most diarrheal infectionsoutbreaks are connected with catering establishments 36.5 % and with children educational establishments 26.1 %. In the region with the highest shigellosis and salmonellosis incidence direct strong correlation relationships are established between the incidence and population, density, natural population movement. The oblasts with the highest diahrreal infections of specified etiology, rotaviral enteritis, diahrreal infections of unspecified etiology incidence are at least provided with water resources and have problems with provision of high-quality drinking water. There is a need to improve the system of epidemiological surveillance over diarrheal infections by extension of the indicators of microbiological study of drinking water quality in the regions of Ukraine, where high diahrreal infections of specified etiology, rotaviral enteritis, diahrreal infections of unspecified etiology incidence is registered; by increasing frequency of food item inspections in the oblasts, where mediana shigellosis and salmonellosis incidence exceeds the average one in Ukraine. Despite significant progress in medicine, out-of-hospital cardiac (OHCA) remains one of the leading causes of death around the world. Epidemiological data suggest wide distribution of OHCA, low incidence of cardiopulmonary resuscitation attempts and low efficiency of resuscitation in the Russian Federation. Both implementation of measures for reducing mortality from OHCA and monitoring of their efficiency should be based on up-to-date, reliable data on national and regional OHCA epidemiology and performance of emergency medical services. The aim of this review is to provide a rationale for establishment of the national OHCA registry as a main instrument of collection, arrangement, storage, processing and presentation of data on OHCA epidemiology and efficiency of care provided. The review includes the analysis of OHCA epidemiological studies carried out in Russia, describes general concept and international experience of developing OHCA registries, and discusses Utstein guidelines for uniform reporting of OHCA data.

Acute respiratory viral infections are widespread in the human population. High contagiosity, a wide variety of pathogens, the possibility of rapid changes in the genotype of the virus and, accordingly,cause the development of drug resistance, make it possible to develop epidemics, and sometimes pandemics of SARS.The clinical picture of all acute respiratory diseases consists of local and general manifestations. Local symptoms include difficulty in nasal breathing, rhinorrhea, pain and discomfort in the throat, cough, hoarseness, stuffy ears, and lacrimation. General manifestations of SARS indicate the onset of viremia, and include an increase in body temperature, general weakness, fatigue, malaise, and headache. The mechanism of SARS development determines the  treatment tactics, including etiotropic, pathogenetic and symptomatic therapy.Domestic and international clinical recommendations indicate the  possible use of  symptomatic agents for  SARS and influenza.Symptomatic treatment is one of the areas of therapy for patients with influenza and other acute respiratory viral infections and allows you to alleviate the general condition, accelerate recovery and improve the quality of life of patients. The use of combined drugs is safer for patients thenthanmonocomponent drugs, and is safe for patients, in case the instructions for use are strictly observed. Such complex medicines, which provides control of the symptoms of colds and flu within 24 hours, as domestic drug are used for patients with SARS. Due to its components, drughas antipyretic, anti-inflammatory, analgesic, anti-allergic, angioprotective and vasoconstrictive effects. The synergism of the components of the combined drug allows to increase the effectiveness of therapy of patients with acute respiratory viral infections.

Introduction: Today, we are witnessing the process of forming a fundamentally new epidemiological situation on infectious mononucleosis. Over the past decade, a general increase in the incidence of infectious mononucleosis, its proportion in the structure of respiratory tract infections, and economic importance was noted in Russia. Information about the epidemic process of infectious mononucleosis in different areas is limited. Our objective was to study the features of the epidemic process of infectious mononucleosis in the Nizhny Novgorod Region in 2010–2019. Methods: We conducted a retrospective epidemiological analysis of the incidence of infectious mononucleosis in the Nizhny Novgorod Region for 2010–2019 based on official statistics using standard statistical approaches. Results and discussion: The long-term incidence rate of infectious mononucleosis in the Nizhny Novgorod Region was stable (+0.6 %) with the long-term average rate of 12.6 ± 0.6 ‰оо. An autumn-winter-spring seasonality with two distinct peaks (in November–December and May) was revealed. The majority of cases were children aged 0–14 years (72.8 ± 2.2 %). The highest incidence rates were regis�tered in the age groups of 1–2 and 3-6 years. We established a reverse trend in the disease incidence among children aged 0–6 (decrease) and 7 years and older (increase). Adolescents aged 15–17 demonstrated the most pronounced growth rate (+7.5 %). The typical annual dynamics in adults was distinguished by a clear spring-summer seasonality, the absence of a characteristic growth in the autumn months, and a low rate in December. The identified features of the epidemic process in different age groups require clarification and detailing. Conclusions: We established recent patterns and features of the epidemic process of infectious mononucleosis in the Nizhny Novgorod Region. This work is an important component of epidemiological surveillance of infection and a scientific basis for improving the existing system of preventive and anti-epidemic measures.

AbstractBackgroundEmerging mosquito-borne viruses like Zika, dengue, and chikungunya pose a major threat to public health, especially in low-income regions of Central and South America, southeast Asia, and the Caribbean. Outbreaks of these diseases are likely to have long-term social and economic consequences due to Zika-induced congenital microcephaly and other complications. Larval control of the container-inhabiting mosquitoes that transmit these infections is an important tool for mitigating outbreaks. However, metapopulation theory suggests that spatiotemporally uneven larvicide treatment can impede control effectiveness, as recolonization compensates for mortality within patches. Coordinating the timing of treatment among patches could therefore substantially improve epidemic control, but we must also consider economic constraints, since coordination may have costs that divert resources from treatment.Methodology/Principle FindingsTo inform practical disease management strategies, we ask how coordination among neighbors in the timing of mosquito control efforts influences the size of a mosquito-borne infectious disease outbreak under the realistic assumption that coordination has costs. Using an SIR/metapopulation model of mosquito and disease dynamics, we examine whether larvicide treatment triggered by surveillance information from neighboring patches reduces human infections when incorporating coordination costs. We examine how different types of coordination costs and different surveillance methods jointly influence the effectiveness of larval control. We find that the effect of coordination depends on both costs and the type of surveillance used to inform treatment. With epidemiological surveillance, coordination improves disease outcomes, even when costly. With demographic surveillance, coordination either improves or hampers disease control, depending on the type of costs and surveillance sensitivity.Conclusions/SignificanceOur results suggest coordination among neighbors can improve management of mosquito-borne epidemics under many, but not all, assumptions about costs. Therefore, estimating coordination costs is an important step for most effectively applying metapopulation theory to strategies for managing outbreaks of mosquito-borne viral infections.Author SummaryMosquito-borne viruses, such as Zika, are an urgent public health threat, particularly in tropical, low-income regions. Vector control, the main strategy for combatting outbreaks, can be challenging because the urban-adapted, container-breeding mosquitoes that transmit these viruses often exhibit metapopulation dynamics, where mortality in one population is compensated by migration from neighboring populations. The timing and spatial distribution of vector control efforts can therefore have a large impact on their efficacy. Using a model of virus transmission and vector population dynamics, we demonstrate that local mosquito control initiatives aimed at reducing the burden of Zika and other mosquito-borne infections are most effective when there is communication of surveillance findings among neighboring control agencies and coordination over the timing of mosquito reduction treatments. We find that local communication improves epidemic outcomes even when it imposes costs to resource-limited control agencies due to gains in the efficiency of mosquito control from spatial coordination.

Respiratory viruses are the leading cause of pediatric morbidity and mortality worldwide. Rapid identification of a pathogen, epidemiological surveillance, description of symptoms, and the development of preventive and therapeutic measures are keystones to limit the spread of respiratory infections. Novel viruses with specific properties are regularly discovered. This paper addresses essential data on the most common viruses provoking acute respiratory infections, including whose in children and the pattern of their course. Conventionally, the most vital concern is flu. Influenza viruses provoke seasonal outbreaks, epidemics, and pandemics. Many studies demonstrate the role of rhinovirus C in the development of bronchiolitis and, subsequently, asthma. Severe acute respiratory syndrome-related coronavirus (SARS-CoV), Middle East respiratory syndrome-related coronavirus (MERS-CoV), and SARS-CoV-2 provoking COVID-19 were recently described. Knowledge of known and newly emerging viruses is crucial for timely adequate medical care. The authors discuss major therapeutic strategies for acute respiratory infections that provide a more favorable course of infectious inflammation irrespective of viral etiology. Introduction. Measles infection in recent years has become particularly relevant in connection with the registration of outbreaks of this disease in various territoriesof the Russian Federation and abroad.The aim of the study is to characterize the epidemic process of measles in a large industrial city in the near and long term with the use of modern mathematical modeling technologies for making new management decisions on infection control at the elimination stage.Materials and methods. The research materials were data from statistical reports of measles incidence in Yekaterinburg from 1950 to 2019 (70 years of follow-up), medical documentation of measles cases, population vaccination data (form No. 6 for 2000—2018 and outpatient maps of children under two years of age vaccinated against measles), results of screening for measles IgG ofmedical organizations, data of planned serological monitoringof population immunity to measles in «indicator» groups in the period from 2013 to 2017 and the results of mathematical modeling of measles incidence in different scenarios of its prevention.Results. Under the influence of vaccination, the epidemic process of measles in Yekaterinburg — a city with a population of 1.5million inhabitants-has undergone significant changes. During the observation period, the incidence decreased to sporadic levels, there was no indigenous measles, drifts from endemic areas had no consequences, the epidemic process was under control. However, outbreaks of 72 cases in 2016 and 90 cases in 2019 have changed our view of measles as a eradicated infection.Of the factors that led to the spread of infection in the foci, the most significant were the presence of measles-susceptible children and adults, including those previously vaccinated, mainly in the periods remote from vaccination and revaccination, defects in the clinical diagnosis of measlesin the first and subsequent cases, and violations in the organization and conduct of anti-epidemic measures. Conclusion. Based on the data of mathematical modeling of the epidemic process of measles with different combinations of its determinants, for the elimination of infection, it is necessary to ensure vaccination against measles at the age of 1 year and 6 years in 95—97.5%. In vulnerable groups for infection to discuss the introduction of routine revaccination among people up to 50 years of age with an interval of 10 years.
