Dental implants have become a widely used dental treatment approach. It is important to identify factors that can be detrimental to dental implants and the peri-implant complex. There is controversy regarding whether occlusion plays a role in the implant and peri-implant condition. The present study aims to review the scientific literature regarding this topic. Animal and human studies, and previous reviews on the topic are included and presented. There is a wide heterogeneity among study designs. Several articles demonstrated that occlusion and occlusion overload could detrimentally affect the peri-implant condition, while other articles did not support these results. More studies are needed to help understand the mechanisms by which occlusion might play a role in the peri-implant condition.

Dental implants are becoming increasingly used in current dental practice. This increased demand has motivated manufacturers to develop varieties of product through design, but also looking for new materials used to improve surface characteristics in order to obtain a better osseointegration. But the increase in the use of implants goes to a consequent increase in the number of failures. These failures are caused either by treatment complications (peri-implantitis), by fatigue breakage under mechanical over-stress, by defective raw material, or due to errors during the insertion procedures. Although they are rare, these complications are serious in dentistry. Before to market a dental implant to clinical practitioners, the product is validated among other determinations in number of biocompatibility research. Raw material issues, details about its structure and properties are less published by the scientific literature, but all this are subject of a carefully analysis of the producers. Breaking of dental implants during surgical procedures, during the prosthetic procedures or during use (chewing, bruxism, accidents, etc.), is the second most common cause of loss of an implant after consecutive peri-implantitis rejection. Although the frequency of this type of failure for a dental implant is much smaller than those caused by the peri-implantitis, a detailed study of broken implants can explain possible causes. The use of scanning electron microscopy (SEM) in the study of the cleave areas explain the production mechanism of cleavages, starting from micro-fissures in the alloy used for the production of dental implants. These micro-fissures in weak areas of the implant (anti-rotational corners of the polygons, etc.) could generate a serious risk of cleavage first time when a higher force is applied.

Subject. Acquired edentulism has different causes and always comes with psychosomatic disturbances in patients. Patients with acquired edentulism of varying duration aim to improve the quality of life and it results in extensive use of dental implants for partial and total tooth loss treatment. Anatomic features arising as a result of tooth loss and restitutional requirements to attachment of an implant to bone tissue bring about necessity of accurate planning and surgical positioning of dental implants. Relevance. The article presents data on psychological preparation of patients for planning and placing implants, technologies of surgical template application for dental implant positioning, results of the dental service package. Objective. The article aims to show the importance of psychological preparation before using surgical templates for rational placing dental implants and preventing potential errors and complications when rendering a dental service package. Methodology. The study employed an observation method, questionnaire survey, and introspection of a number of mental and psychophysiological functions of medical staff and patients, psychophysiological state of patients in the process of receiving multidimensional dental care. Results. Conducting well-timed psychotherapeutic work with a surgeon dentist, a prosthodontist, and a patient, using contemporary local anesthetics, equipment, and dental materials provide successful multidimensional treatment of acquired edentulism under municipal clinic conditions. Conclusions. The material presented here shows the necessity of using surgical templates when delivering multidimensional dental treatment to patients with acquired edentulism, as well as the importance of psychological prevention of occupational stress among dental practitioners.

Introduction. In order to achieve the goals of oral implantology, it is of great importance to obtain tissue integration (bone and epithelial) of dental implants. Bone integration has the role of rigidly fixing the dental implant at the level of the supporting bone, while ensuring the transfer of masticatory forces to the neighboring bone, while the soft tissues integration has the role of creating a barrier around these medical devices, with the role of protecting the underlying bone and preventing the access of microorganisms. Purpose. In this material we wanted to perform a statistical analysis on the gingival integration and osseointegration of dental implants in the oral cavity. Material and method. The analysis was performed on a number of 21 patients with a total of 97 dental implants, approximately one year after the prosthetic loading of the implants. The analysis was based on bleeding index, gingival index but also on the depth of the peri-implant canal and bone resorption around the implant. Results and discussions. Most of the implants analyzed showed no signs of inflammation. A small number (less than 30%) showed incipient signs of inflammation, falling within the data in the literature. Conclusions. Early detection of tissue changes around the implant and proper treatment will reduce the risk of more serious injuries and implant failure.

Many patients travel abroad seeking cosmetic dental treatment, in particular dental implants. However, there are hundreds of dental implant systems being used globally by dentists of all training backgrounds. Furthermore, complications can occur that patients may expect their general dental practitioner in the UK to be able to manage. The following report describes the case of a 71-year-old, medically compromised patient who presented with complaints consistent with the immediate failure of dental implants that happened to be placed abroad. On closer examination, the dental implants themselves were highly questionable in their design and placement. An incidental finding was also made in the maxillary sinus. CPD/Clinical Relevance: To acknowledge that implant dentistry is increasing in popularity as patients have the freedom to access dental implant treatment globally. UK dentists are expected to manage these dental implants on the return of patients to the UK who may present with post-operative complications involving unknown dental implant systems.

Several factors affect dental implant osseointegration, including surgical issues, bone quality and quantity, and host-related factors, such as patients’ nutritional status. Many micronutrients might play a key role in dental implant osseointegration by influencing some alveolar bone parameters, such as healing of the alveolus after tooth extraction. This scoping review aims to summarize the role of dietary supplements in optimizing osseointegration after implant insertion surgery. A technical expert panel (TEP) of 11 medical specialists with expertise in oral surgery, bone metabolism, nutrition, and orthopedic surgery performed the review following the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) model. The TEP identified micronutrients from the “European Union (EU) Register of nutrition and health claims made on foods” that have a relationship with bone and tooth health, and planned a PubMed search, selecting micronutrients previously identified as MeSH (Medical Subject Headings) terms and adding to each of them the words “dental implants” and “osseointegration”. The TEP identified 19 studies concerning vitamin D, magnesium, resveratrol, vitamin C, a mixture of calcium, magnesium, zinc, and vitamin D, and synthetic bone mineral. However, several micronutrients are non-authorized by the “EU Register on nutrition and health claims” for improving bone and/or tooth health. Our scoping review suggests a limited role of nutraceuticals in promoting osseointegration of dental implants, although, in some cases, such as for vitamin D deficiency, there is a clear link among their deficit, reduced osseointegration, and early implant failure, thus requiring an adequate supplementation.

Sickle Cell Disease (SCD) is responsible for high mortality and morbidity in Brazil. It is known that SCD promotes several oral manifestations and impacts directly on the dental treatment approach. However, there is little information concerning the dentists’ prior knowledge about the illness. The aim of the present study is to assess the level of knowledge and the profile of different respondents, by means of a questionnaire applied during the seminar “O papel da Odontologia no contexto da atenção integral à DF” hosted in a dental course of a public higher education institution. The results of the present study have shown that, in general, the majority of the respondents stated that they have insufficient information about SCD, in spite of the great interest in learning. It was also found a greater interest among the graduated dentists, probably due to the fact that they have already experienced the treatment on SCD patient, since Bahia State presents the greater prevalence of the disease in Brazil. In this context, it seems clear that there is a lack of information and many doubts about the role of dentistry in the health care of patients with SCD. High prevalence diseases in Brazil should be addressed with depth more often in dentistry higher education programs. Better treatment approaches and protocols for dental care of patients with SCD are antecipated.

Relevance. According to WHO, the absence of teeth occurs in various regions of the globe in 75 % of the population [1]. In the Russian Federation, tooth extraction in 35–44 years is 5.50 %, in patients older than 44 years is 17.29 % of the total number of people applying for dental treatment. Purpose. To analyze the theoretical basis for the positioning of dental implants. Materials and methods. The authors of the article carried out a retrospective analysis of the literature data on the theoretical basis for the positioning of dental implants. It was revealed that modern implantology successfully solves many problems of rehabilitation of patients with dentition defects. The conclusion. Improper positioning of dental implants entails a number of surgical and orthopedic problems, leading to disruption of the osteointegration of dental implants and, consequently, the inability of the dental implant to function.

Background. Dental implants are one of the most successful treatments for restoring tooth function and beauty. Identifying the causes of dental implant failure is useful and vital. This review was aimed at studying the possible role of vitamin D in early implant failure. Method and Material. This review was designed based on the PRISMA guideline. Data was collected using keywords including implant, vitamin D, deficiency, failure, dental, OR tooth in international databases including PubMed, Scopus, Web of Science, Cochrane, and Embase, until 2020. Based on the inclusion and exclusion criteria, data were extracted and gathered in a checklist. Results. Finally, twelve studies were selected from five different countries. In 6 studies (2 animal studies and four human studies), there was no significant relationship between vitamin D deficiency and dental implant failure. In the other six studies (1 animal study and five human studies), there was a significant relationship in this regard. Conclusion. It is difficult to conclude the association between vitamin D and implant failure based on the literature’s researches. However, vitamin D appears to play an essential role in implant success through its effects on immune system modulation.

Az agyérbetegségek jelentőségét jelzi, hogy a cardiovascularis és tumoros betegségek után a stroke jelenti a harmadik leggyakoribb halálokot világszerte. Mivel stroke során az érintett agyállomány irreverzíbilisen károsodik, az infarktust övező épen maradt, de általában hipoperfundált agyterületek működésének serkentésével lehet a betegek állapotán javítani. Az agyi véráramlás növelése révén várható javulás a stroke-on még át nem esett, de krónikus agyi hipoperfúzióban szenvedő betegek tüneteiben is. Célkitűzés, módszer: Jelen tanulmány célja, hogy áttekintse a vinpocetin akut és krónikus agyérbetegségekben történő alkalmazásának eddig publikált humán vizsgálati eredményeit, és összefoglaló képet adjon a gyógyszer főbb indikációs területeiről, az ott mutatott hatékonyságról. Eredmények: A vinpocetin akut ischaemiás stroke-ban történő alkalmazását vizsgáló tanulmányok száma csekély, eredményeik pedig ellentmondásosak. Krónikus agyérbetegek esetében mind az egyszeri, mind a hosszú távú vinpocetinkezelést követően PET-, SPECT-, TCD- és NIRS-vizsgálatokkal a laesio körüli ép agyterületek perfúziónövekedése, fokozott glükóz- és O 2 -felhasználása igazolódott, és jelentős javulás volt észlelhető a vér reológiai paramétereiben is. A klinikai hatékonyságot értékelő nemzetközi tanulmányok metaanalízise alapján a gyógyszer per os alkalmazásával szignifikáns javulás jelentkezik a kognitív teljesítményben és a napi aktivitásban. Következtetések: A fenti vizsgálatok eredményei alapján kimondható, hogy a vinpocetin sokrétű farmakológiai hatása révén kedvezően befolyásolja az agy ép területeinek perfúzióját és metabolizmusát, a vér áramlási viszonyait, javítva ezzel a krónikus agyérbetegek életminőségét.

Цель исследования состояла в разработке неинвазивного метода диагностики состояния тканей периимплантационной зоны после постановки дентальных имплантатов, а также прогноза течения послеоперационного периода и ранней диагностики послеоперационных осложнений, таких, как мукозит и периимплантит, при одноэтапном методе дентальной имплантации. Методы. В исследовании представлена цитологическая характеристика отпечатков из области контакта имплантата с десной, в которых оценивали состояние эпителиальных клеток при различных проявлениях цитопатологии. Определяли индекс деструкции (ИД) и воспалительно-деструктивный индекс (ВДИ) в клетках воспалительного инфильтрата, которые сопоставляли с клинической картиной течения послеоперационного периода. Сроки забора цитологического материала 1, 5, 15, 25, 30, 60, 120 дней после постановки имплантатов. Количество исследованных цитограмм - 442 от 11 пациентов (по 1-2 имплантата). Общий массив использованных для вычисления интегральных показателей составил »2550. Результаты. Было установлено, что в сроки 1-5 дней после постановки дентальных имплантатов происходит резкое повышение показателей ИД (до 7500 ± 15) и ВДИ (до 80 ± 20), что отражает процесс развития в области имплантатов острых воспалительных реакций. В отдаленные сроки наблюдений отмечалось 2 варианта развития событий. Первый из них характеризовался сохранением во все сроки высоких показателей ИД и ВДИ. Для второго варианта динамики показателей было характерно их падение, что соответствовало развитию процесса остеоинтеграции дентальных имплантатов. The objectives of the investigation were to develop protocols evaluating the validity and effectiveness of the cytological method for diagnosis and prognosis of tissue conditions adjacent to a dental implant. Methods. This study examined the cytological characteristics of imprints of the gingiva-implant contact area, a. the correlation of quantitative ratios of epithelial cells either with or without hystopathological manifestations (an index of destruction, ID); b. ratio of cells of the inflammatory infiltrate (an inflammatory and destructive index, IDI); these data were researched in relation with dynamics of the clinical pictures during a postoperative period.The cytological material was obtained 1, 5, 15, 25, 30, 60, 120 days after dental implantation. Totally 442 cytogramme from 11 patients (1-2 implant per a patient) were assayed. The data array for computation of integrated indicators was composed of ca. 2550 data points. Results. A significant increase of ID (7500 ± 15) and IDI (80 ± 20) indicators, which reflects the development of acute inflammatory reactions in tissues neighboring an implant, was observed within 1-5 days following the insertion of dental implants. Long-term observations revealed two scenarios. The first scenario was characterized by the retention of high levels of ID and IDI over the period of observations. The second scenario featured the reduction of ID and IDI indices, which corresponded to the development of the osseointegration of dental implants.
