This study aims at hazard identification and risk assessment in an effort to prevent work accident at the Subang State Polytechnic’s Combustion Engine Laboratory. This research is a qualitative-descriptive study with an observation stage using the hazard identification and risk assessment (HIRA) methods. Data were analyzed using Australian Standard/New Zealand Standard (AS/NZS) measurements. Conclusions from the results are: (1) occupational health and safety have not been implemented optimally in the Combustion Engine Laboratory; (2) 9 hazards were found in the laboratory consisted of 1 hazard in the H (high) category, 2 hazards in the M (moderate) category, and 6 hazards in the L (low) category, but none were in the extremely danger category; (3) recommended actions to be taken to reduce the danger in the laboratory are defining laboratory standard operating procedure, rearrangement of laboratory layout and equipment, and briefing students with clear instructions before and after practical work.Penelitian ini bertujuan untuk mengidentifikasi bahaya dan menilai risiko, harapannya dapat mencegah terjadinya kecelakaan kerja di Laboratorium Motor Bakar Politeknik Negeri Subang. Penelitian ini merupakan penelitian deskriptif kualitatif dengan tahap observasi menggunakan metode hazard identification and risk assessment (HIRA). Data selanjutnya dianalisis menggunakan pengukuran yang digunakan dalam Australian Standard/New Zealand Standard (AS/NZS). Hasil penelitian dapat disimpulkan bahwa: (1) penerapan kesehatan dan keselamatan kerja (K3) di Laboratorium Motor Bakar belum optimal; (2) ditemukan 9 bahaya (hazard) di Laboratorium Motor Bakar yang terdiri dari 1 bahaya dalam kategori H (high), 2 bahaya dalam kategori M (moderate), dan 6 bahaya dalam kategori L (low) namun tidak ada yang masuk dalam kategori bahaya ekstrem; (3) rekomendasi tindakan yang dapat dilakukan untuk mengurangi bahaya di Laboratorium Motor Bakar yaitu pembuatan SOP laboratorium, penataan kembali tata ruang dan peralatan, serta pemberian instruksi sebelum dan sesudah praktikum kepada mahasiswa.

<p style="margin-bottom: 0.04in; line-height: 0.17in; orphans: 0; widows: 0;" lang="en-US">The article describes a concept of health and safety to conduct research in Nanomaterial Laboratory in Two Public University in Malang, East Java, Indonesia. The utilization of nanomaterials in the world of education has been done in two universities in Malang, namely Malang State University and Brawijaya University. The nanomaterial laboratory as a means for research and development of nanomaterial science that often creates hazards and risks of work accidents for its users, but things that are often not realized. Dangers and risks of work accidents caused by the absence of standards. This research focuses on hazards and accidents in the nanomaterial laboratory using the HIRA (Hazard Identification and Risk Assessment) method, which is then followed by a variety of independent variables. The population in this study were nanomaterial researchers at two universities in Malang. Based on the results of trials between respondents 'competency expertise with the level of risk of workplace accidents in the nanomaterial laboratory obtained p- value 0.00 (&lt;0.05), meaning that there is a significant relationship between the respondents' scientific conservation and the level of work accident risk in the nanomaterials laboratory. Furthermore, the results of the study were also obtained between the research respondents with the level of risk of work accidents in the nanomaterial laboratory with a p-value of 0.00 (&lt;0.05), meaning that there was a significant correlation between the respondent's research experience and the level of work accident risk in the nanomaterial laboratory.</p>

ABSTRAK Rumah Potong Hewan merupakan suatu bangunan yang memiliki desain dan konstruksi khusus digunakan sebagai tempat pemotongan hewan. Aktivitas kerja di RPH memiliki potensi bahaya yang dapat mengancam keselamatan dan kesehatan kerja. Berdasarkan observasi awal, RPH Kota Pekanbaru ini tidak memiliki laporan bulanan maupun tahunan mengenai data kecelakaan, dan juga belum pernah melakukan identifikasi, dan penilaian terhadap bahaya yang ada. Adapun tujuan penelitian ini yaitu untuk menganalisis keselamatan kesehatan kerja (K3) terhadap risiko bahaya lingkungan fisik di tempat kerja dengan metode “HIRARC” (Hazard Identification, Risk Assessment, and Risk Control). Jenis penelitian ini adalah kualitatif analitik. Lokasi penenlitian dilakukan di rumah potong hewan Kota Pekanbaru, penelitian dilakukan pada bulan Juli-Agustus 2020. Informan dalam penelitian ini sebanyak 6 orang, sebagai informan utama yaitu Kepala UPTD, 2 orang informan pendukung yaitu koordinator lapangan dan dokter hewan, dan informan kunci yaitu pekerja, metode yang digunakan peneliti yaitu wawancara mendalam dan observasi langsung. Berdasarkan hasil penelitian yang dilakukan potensi bahaya tingkatan risiko “Ekstrim” seperti diseruduk sapi pada proses penggiringan sapi menuju killing box, tertimpa katrol dan sapi pada proses penggantungan dan pemindahan sapi menggunakan katrol. Rumah Potong Hewan belum pernah melakukan identifikasi bahaya, penilaian risiko akan tetapi untuk pengendalian risiko sudah dilakukan beberapa upaya di rumah potong hewan, seperti SOP, Shift kerja, dan juga APD. Rekomendasi yang diberikan yaitu diharapkan sebaiknya RPH memiliki fasilitas yang berhubungan dengan kesehatan pekerja. Perlu adanya upaya pengendalian risiko secara engineering control, contohnya, membuat kerangkeng sapi. administrative control, seperti, memberikan sosialisasi, maintenance, housekeeping dan inspeksi, terhadap alat, mesin dan pekerja. ABSTRACTSlaughterhouse is a building having the design and construction specifically used as animal slaughterhouse. Slaughterhouse’s work activities have potential of hazards which can cause harm to workers’ safety and health. Based on pre observation, the slaughterhouse in Pekanbaru does not have any monthly and annual report about the data of accident. This livestock production also has no doing identification and assessment of hazard. The purposes of this study are to analyze occupational health and safety toward risk of danger physical environment at work, this study uses Hazard Identification, Risk Assessment, and Risk Control “HIRARC” method which using likelihood and severity level of each hazard as references to define a risk value. There are 6 informants the maininfromants is a UPTD head, 2 supporting informants are the field coordinator and venetarian, and the key informant is a worker, The methode that use in this study is interview And observation. The study shows that the most extreme are getting hit by a cow in cattle herding to killing box, falling of a cow hoist material from conveyor railing in shackling and hoisting process. Slaughterhouse has never done hazard identification and risk assessment. However, for risk control have done several attempts in slaughterhouse, such as Standard Operating Procedures, work shifts, and Personal Protective Equipment. Recommendation is given that the slaughterhouse should has facilities related to health of workers. It needs an effort to risk control through engineering control such as making cow cages, administrative control, such as giving socialization, maintenance, housekeeping and inspection of tools, machines and workersKeywords : Slaughterhouse, Hazard, Risk, Physical Environtmen, HIRARC.

Zero accident was considered as the primary occupational health and safety value in manufacturing and service enterprises.  PT. ABC provided  electrical and instrument services, including a low-voltage installation, high-voltage installation and an instrument installation. This company is known to excellent electricity and instrument service providers across the country . Moreover, men’s power is used as a primary source to maintain  these kind  of services. Installation projects depend not only on the high working speed, but also on  high working  accuracy within  certain timeframes . As a result, work  accidents  have occurred with a classification varying from minor to fatal accidents.  The purpose of this research is to identify potential hazards to assess risk levels and  obtain recommendations for  preventing accident. The HIRARC method is employed as a research approach. This research is initiated through Hazard Identification, Risk Assessment and Risk Control.  This research identified that two hazards were classified as  extreme risk, three hazards as high- risk, eight hazards as moderate risk , and two hazards have been identified as low risk . Furthermore, a number of action lists have been suggested in order to minimize accident rates and finally the working conditions are designed to maintain zero accidents.

Introduction: In 2009, UNESCO declared Batik Indonesia as an Intangible Cultural Heritage of Humanity. This study examines the occupational health and safety hazards and risks that arise in a Batik company based on a case example of a Batik company in Surakarta, Indonesia. In Indonesia, Batik is categorized as traditional attire. Two kinds of Batik are produced - Batik tulis (traditionally handcrafted) and Batik printing, which uses specific printing tools. The UK Health Safety Executive (HSE) risk assessment framework is referred to in this study, which consists of: hazard identification, population exposure, risk evaluation, precaution development, recording findings, and regular appraisal of systems. Methods: Both observation and interviews (of workers) at the Batik company were utilized for this study as well as an independent risk assessment. Results: The findings highlighted physical, ergonomic, chemical, mechanical, and biological hazards, originating from the materials, tools, and working methods utilized in the batik production. Conclusion: Many of the issues highlighted by this study can be addressed via in-depth risk assessment, using the POPMAR model to frame the activities. This approach can create a positive pathway, leading to a continuous cycle of improvement which puts the workers’ health at the forefront of the business activities.Keywords: Batik, workers, hazard identification, HSE Model, risk management

This study focused on evaluating the concepts of risk assessment associated with unsafe acts according to hazard identification at Gaili Area, Khartoum North, Sudan. Approaches used in this study; Research tools: Interviews, definite questionnaire &amp; computer program for descriptive statistics – statistical package for social science (SSPS/version 22 – 2014). The study of risk assessment is conducted for workers in fuel terminals at Gaili area and analyzed using environmental health and safety concepts for eight jobs. Risk is associated with Job (driver, electrician, pump attendant, etc.), after hazards to be identified (natural, environmental, technological, biochemical, etc.). Fuel truck drivers at Gaili area showed the most highly risk job. The result showed that 60% of the incidents were caused by the hydrocarbon fuel transport drivers at the study area as the most highly risk job, followed by the electricians being the most affected job by electrical shocks during working hours followed by the pump attendants then welders followed by mechanics; including fatalities, restricted work activities, injuries, first aid and property damage. The study recommended to formulate of temporary committees such as autumn committee is not efficient in solving the problem, HSE steering committee should be a permanent committee to direct the emergency planning according to risk based assessment for identified hazards.

Need—National legislative health and safety (H&S) frameworks impose requirements but grant self-management to organisations. Consequently variability arises in management systems, and some organisations struggle to achieve successful implementation. The risk assessment process is key to the H&S management system, and could benefit from greater consistency and better external alignment with the legislative framework of the jurisdiction. Approach—The harm categories in the New Zealand (NZ) Act were adapted into a consequence scale. A non-linear scale was developed for the consequence axis to represent the disproportional nature of catastrophic harm outcomes compared to minor injuries. A hazard assessment process was devised based on systems engineering methods. Organisational decision-criteria were derived from the communications requirement in the Act, and these thresholds linked to expected treatments. Originality—A method is providing for aligning risk assessments with a national legislative framework, and integrating the technical aspects of risk assessment with the management processes. The approach also more explicitly includes recovery actions in contrast to existing methods where prevention dominates. Regarding the management aspects, it shows how thresholds may be defined relative to the legislation, to give clear expectations regarding treatment and internal communication, thereby assisting executives (‘officers’ in terms of the NZ Act) meet their duties.

Health and safety risk assessment of PT Unido Pasifik Fabrication department to create a safe and efficient and productive work environment. The fabrication department is the main pillar of company support. The purpose of this study was to analyze the potential hazards in the Production department especially the PT Unindo Pasifik Fabrication Department. In addition this study aims to determine the relationship of knowledge of workers about the potential danger with the behavior of the use of personal protective equipment. This research is an observational descriptive study in which the object of research is the machine, attitude or behavior of the workforce, work processes and work environment. Based on hazard identification, risk assessment and control, there are several hazards and risks that occur with tolerable risk, moderate risk and sustainable risk criteria. Hazard control has been carried out at PT. Unindo Pasifik that is administrative control and the provision of personal protective equipment (PPE). And while the Objective, Target and Program (OTP) are set namely reducing work accidents and reducing occupational diseases in the Fabrication department. Didalam penilaian risiko kesehatan dan keselamatan departemen Fabrikasi PT Unido Pasifik untuk menciptakan lingkugan kerja aman dan efisien serta produktif. Departemen fabrikasi merupakan penopang kegitaan perusahaan yang utama. Tujuan penelitian ini untuk menganalisis potensi bahaya di bagian Produksi khususnya Departemen Fabrikasi PT Unindo Pasifik. Selain itu penelitian ini bertujuan untuk mengetahui hubungan pengetahuan pekerja tentang potensi bahaya dengan perilaku penggunaan alat pelindung diri. Penelitian ini merupakan penelitian deskriptif observasional dimana Objek penelitian adalah mesin, sikap atau perilaku tenaga kerja, proses kerja dan lingkungan kerja. Berdasarkan identifikasi bahaya, penilaian dan pengendalan risiko terdapat beberapa bahaya dan risiko yang terjadi dengan kriteria tolerable risk, moderate risk dan sustainable risk. Pengendalian bahaya sudah dilakukan di PT. Unindo Pasifik yaitu pengendalian bersifat administrative & penyediaan alat pelindung diri (APD). Dan sedangkan Objective, Target dan Program (OTP) yang ditetapkan yaitu mengurangi kecelakaan kerja dan diharapkan dapat mengurangi penyakit yang ditimbulkan akibat kerja di deprtemen fabrikasi tersebut.

Hazard identification, risk assesment and risk control and the application of risk mapping at Hospital of the Animal Prof. Soeparwi Universitas Gadjah Mada Purpose: This research was held to analyze the work accident using HIRARC method and the applied of Risk Mapping at the hospital of the animal of Prof. Soeparwi Gadjah Mada University, Yogyakarta.Method: This research used descriptive qualitative research method. The qualitative method used to describe the condition, assess, and to recognize the work accident analysis. The data used was grabbed from Hazard Identification Risk Assessment and Risk Control (HIRARC) method and questionnaire.Results: Hazard identification used HIRARC method at the hospital of the animal of Prof. Soeparwi Gadjah Mada University was about 198. The work hazard commonly includes physical hazard, chemistry, biologist, ergonomics, and psychosocial. The risk assessment which held denoted that there were 61 low risks, 108 medium risks, and 15 high risks. The recommendations of risk restraint that can be given were technician control, administrative, and the use of personal protective equipment (PPE).Conclusion: HIRARC method had been applied at the hospital of animal of Prof. Soeparwi with the result to that grooming unit was high risk work place, assessed with risk mapping. The risk was from animal contact phisically.

Occupational Health and Safety plays a crucial role in the institution or organization. The purpose of Occupational Health and Safety (OHSAS) is to ensure the health and safety of the work. To fulfil the purpose, the institution or organization need to have a good work safety and health management system. OHSAS 18001: 2007 is an international standard for the implementation of work safety and health management system. One of the clauses contained in OHSAS 18001: 2007 are hazard identification, risk assessment, and determining controls and these can be documented in the HIRADC (Hazard identification, risk assessment, and determining control) document. HIRADC is divided into three parts: Hazard Identification, Risk Assessment, and Determining Control. HIRADC, which is annually reviewed and updated by the management, discovers and controls the potential hazards that exist around the working environment.
