ObjectivesSome of the nutrition problems still found in Wonogiri district. To solve that problems, completeness, and timeliness of the nutrition surveillance data reporting are needed so the data can be used as the basis for making policy appropriately. Evaluation of nutrition surveillance system was conducted to find out completeness and timeliness of the nutrition surveillance data reporting.MethodThis was an evaluative study using quantitative descriptive analysis which conducted from December 2017 to January 2018. Subjects of this study were nutritionists in District Health Office and 25 Primary Health Care (PHC) which selected randomly using Slovin samples size. Data collected by interviews and observation on surveillance data reporting form.ResultsPHC that reported complete data were 6 PHC (24%). One PHC (4%) did not report malnutrition data due to poor of time management. Ten Puskesmas (40%) did not report Hb data because they assumed that it was the duty of the laboratory officer or midwife. Three PHC (12%) did not report exclusive breastfeeding data every month, because they consider that the data should be collected only once every six months. Nine PHC (36%) did not report goiter data because they considered that it was no need to report if no cases. Most PHC (52%), never report the data timely. Most nutritionists (84%) had another duty that inhibits them to finish the surveillance data timely. Only two PHC (2%) whom the nutritionists had another duty but always report the data timely.ConclusionCompleteness and timeliness of the nutrition surveillance data reporting were still unsatisfying. PHC should be encouraged to collect data completely and timely. The data collection procedure should be fixed on the indicator collected and the time period in the data collection.

Data collection techniques used are observation, tests, and documentation. The collected data was analyzed using qualitative and quantitative descriptive analysis techniques. The results of research conducted at Purbadana 2 Elementary School showed that learning using discovery learning assisted with pop up book media went well. The results of the knowledge skills in the first cycle reached an average of 3.1 with a percentage of completeness of 46%, meaning that students' storytelling skills were still included in the sufficient category. In the second cycle, the average was 3.9 with a percentage of completeness of 84%, meaning that students' storytelling skills were included in the criteria of good. Based on the results of the study, the application of the discovery learning learning model assisted by pop up book media can improve the skills of the fourth grade students in Purbadana 2 Elementary School.

This study aims to reveal (1) how the characteristics of LKPD based on Problem Based Instruction models on environmental pollution material (2) how the feasibility of LKPD based on Problem Based Instruction models on environmental pollution material (3) how practicality of LKPD based on Problem Based Instruction models on environmental pollution material. Research refers to the Borg and Gall development model to the seventh stage, namely potential and problems, data collection, product design, design validation, design revision, product testing, product revision. The research instruments used in data collection were interviews, observation, and questionnaires. The data obtained were then analyzed by quantitative descriptive analysis. The characteristics of LKPD teaching materials developed using the Problem Based Instruction model on environmental pollution materials are arranged systematically, interestingly, and clearly. Students can learn independently; the developed LKPD does not depend on other media, and students can interact directly with the environment. Based on the feasibility trial LKPD is in the "Very feasible" category. Material experts with grades X≥ 33 categories are very feasible, media experts X≥ 45 categories are very feasible, linguists X≥ 18 categories are very feasible, expert practitioners X≥ 42 categories are very feasible, and students' responses X≥ 33 categories are efficient. The results obtained can be concluded that the developed LKPD meets the very feasible category, so it is suitable for use.
