Maternal Behavior Change Program (Cerdigi) Based on ABC’s Theory (Pilot Study in Serpong Subdistrict, South Tangerang). Oral health is one of the things that need attention during pregnancy. However, the behavior of dental and oral health maintenance during pregnancy is still low. Poor oral health conditions in pregnant women can have an impact, such as premature births and low birth weight (LBW) babies. ABC behavior change theory focuses on the antecedent, behavior, and consequence of behavior. The purpose of this study is to determine pregnant women’s behavior of oral health maintenance, identify their antecedent factors, behavior change interventions (DHE), and analyze the behavior consequence. This was a descriptive research with a cross-sectional design among 27 pregnant women in Serpong Subdistrict, South Tangerang. The data were collected through interviews and questionnaires. The results show 100% (27 respondents) did tooth brushing twice a day, 44,4% (12 respondents) used mouthwash once a week and 4% (1 respondent) visited a dentist during pregnancy. The antecedent factors of not visiting a dentist during pregnancy were because: 33% did not know the benefits of visiting a dentist, 26% had fear of dentist, 19% felt that visiting a dentist was expensive, 11% did not feel the need to go to a dentist, and 11% had fear of disturbing the fetus. After 9 days of intervention, the consequences on the 19 respondents were evaluated and the results show that 89.5% had not visited a dentist, 5.25% had visited a dentist, and 5.25% did not want to visit a dentist. The reason of not visiting a dentist was because of having not much time 52.8%, having nobody to accompany 11.8%, feeling lazy 11.8%, still feeling afraid of a dentist 11.8 %, and having no money 11.8%. The conclusion from this study is that the most dominant behaviors of oral health maintenance in pregnant women is to maintain it by themselves (toothbrushing and using mouthwash), while maintaining the health by visiting a dentist is still low with various antecendents.

Background Over the past decade, the prevalence of stunting has been close to 37% in children aged <5 years in Indonesia. The Baduta program, a multicomponent package of interventions developed by the Global Alliance for Improved Nutrition, aims to improve maternal and infant nutrition in Indonesia. Objective This study aims to assess the impact of the Baduta program, a package of health system strengthening and behavior change interventions, compared with the standard village health services on maternal and child nutrition. Methods The impact evaluation uses a cluster randomized controlled trial design with 2 outcome assessments. The first uses cross-sectional surveys of mothers of children aged 0-23 months and pregnant women before and after the interventions. The second is a cohort study of pregnant women followed until their child is 18 months from a subset of clusters. We will also conduct a process evaluation guided by the program impact pathway to assess coverage, fidelity, and acceptance. The study will be conducted in the Malang and Sidoarjo districts of East Java, Indonesia. The unit of randomization is the subdistricts. As random allocation of interventions to only 6 subdistricts is feasible, we will use constrained randomization to ensure balance of baseline covariates. The first intervention will be health system strengthening, including the Baby-Friendly Hospital Initiative, and training on counseling for appropriate infant and young child feeding (IYCF). The second intervention will be nutrition behavior change that includes Emo-Demos; a national television (TV) advertising campaign; local screening TV spots; a free, text message service; and promotion of low-cost water filters and hygiene practices. The primary study outcome is child stunting (low length-for-age), and secondary outcomes include length-for-age Z scores, wasting (low weight-for-length), anemia, child morbidity, IYCF indicators, and maternal and child nutrient intakes. The sample size for each cross-sectional survey is 1400 mothers and their children aged <2 years and 200 pregnant women in each treatment group. The cohort evaluation requires a sample size of 340 mother-infant pairs in each treatment group. We will seek Gatekeeper consent and written informed consent from the participants. The intention-to-treat principle will guide our data analysis, and we will apply Consolidated Standards of Reporting Trials guidelines for clustered randomized trials in the analysis. Results In February 2015, we conducted a baseline cross-sectional survey on 2435 women with children aged <2 years and 409 pregnant women. In February 2017, we conducted an end-line survey on 2740 mothers with children aged <2 years and 642 pregnant women. The cohort evaluation began in February 2015, with 729 pregnant women, and was completed in December 2016. Conclusions The results of the program evaluation will help guide policies to support effective packages of behavior change interventions to prevent child stunting in Indonesia. Abstract Objectives Women's self-help groups (SHGs) have become one of the world's largest institutional platforms of the poor, reaching over 70 million Indian women in 2020. Limited evidence exists on effects of nutrition interventions through SHGs on maternal and child nutrition outcomes. Methods The Women Improving Nutrition through Group-based Strategies (WINGS) study was a quasi-experimental impact evaluation, comparing 16 matched blocks where communities were provided support to form SHGs and improve women's livelihoods; 8 blocks (1 in each matched pair) received a 3-year nutrition intervention (NI) with nutrition education, agriculture- and rights-based information, facilitated by a trained female volunteer; the other 8 blocks received standard activities (STD) to support savings & livelihoods. We conducted repeated cross-sectional surveys of mother-child pairs in 2017–18 (n = 1609 pairs) and 2019–20 (n = 1841 pairs). We matched treatment groups over time and applied difference-in-difference (DID) regression models to estimate NI impacts. Outcomes were knowledge domains (nutrition for pregnant women, breastfeeding, complementary feeding, child health), child feeding (e.g., early breastfeeding initiation, dietary diversity, animal source food (ASF) consumption), woman's diets, woman's BMI and child anthropometry. Matching covariates included woman, child, and community characteristics. Results About 40% of women were SHG members. Nutrition intervention exposure was low; only ∼10% of NI women had heard of intervention content at endline. There were large improvements in women's knowledge in both groups. DID estimation revealed a positive NI impact on knowledge of timely introduction of ASFs to children (P < 0.05), but knowledge of nutrition for pregnant women unexpectedly improved more in the STD group (P < 0.05). No impacts were observed for any anthropometry or diet indicators except child ASF consumption (P < 0.01). Conclusions Limited impacts on nutrition outcomes may be due to limited exposure, low motivation or skills of volunteers, and a concurrent national nutrition behavior change program targeting mothers and children in all study areas. Our findings add to a growing literature on SHG-based behavior change interventions and the conditions necessary for their success. Funding Sources Bill & Melinda Gates Foundation.

Objective Health education interventions during pregnancy can influence maternal oral health (OH), maternal OH-behaviors and children’s OH. Interventions that can be delivered at anytime and anywhere, for example mobile-health (mHealth) provides an opportunity to address challenges of health education and support activation of women in underserved and rural communities to modify their health behavior. This pilot study was undertaken as a part of a mHealth initiative to determine knowledge, attitudes, and behaviors related to pregnancy and ECC prevention among women attending obstetrics/gynecology (OB/GYN) practices at a large rurally-based clinic. Methods A cross-sectional survey study was voluntarily engaged by women (n = 191) aged 18 to 59 years attending OB/GYN visits, over a 3-week period from 12/2019 to 1/2020. Survey results were analyzed applying descriptive statistics, X2 and Fisher’s Exact tests. The significance level was set at P < .0001 for all analyses. Results Approximately half of respondents were between 18 and 29 years (53%), had a college degree (55%), and 100% reported cell phone use. Whereas 53% and 31%, respectively, indicated that they were “somewhat” or “very” sure of how to prevent ECC in their children, only 9% recognized evidence of early decay and 30% did not know the purpose of fluoride. Overall, only 27% of participants correctly answered the knowledge-based questions. Further, only 57% reported their provider explained things in a way that was easy to understand. Only 24% reported seeing a dentist during their current pregnancy. Conclusions Study results suggested potential gaps in knowledge and behaviors related to ECC prevention and provided baseline data to inform future interventions to improve ECC prevention practices. Notably, majority of participants used their cell phones for making medical/dental appointments and reported using their phones to look up health-related information. This demographic represents a potentially receptive target for mHealth approaches to improve understanding of oral health maintenance during pregnancy and ECC prevention.
