Inter-Dialytic Weight Gain (IDWG) atau peningkatan berat badan diantara dua tindakan hemodialisis (HD) menjadi salah satu indikator keberhasilan terapi pasien HD. Semakin tinggi IDWG, maka semakin banyak cairan yang menumpuk di dalam tubuh pasien dan semakin berat dampak yang ditimbulkan. Semakin lama seseorang menjalani HD, semestinya semakin banyak yang diketahuinya tentang penyakitnya dan cara mencegah komplikasi sehingga IDWG semestinya semakin turun. Penelitian ini bertujuan untuk mengetahui hubungan antara lama waktu menjalani hemodialisis dengan IDWG. Penelitian korelasional ini dilakukan secara cross sectional dengan melibatkan 34 pasien yang telah menjalani hemodialisis minimal 1 tahun di RSUD Dumai, dapat berkomunikasi secara verbal, dan dapat berdiri untuk menimbang berat badan. Data lama waktu menjalani hemodialisis dilihat dari catatan medis pasien. Adapun IDWG dihitung dalam periode siklus satu minggu menjalani hemodialisis menggunakan timbangan dan dicatat di lembar observasi. Data dianalisis menggunakan uji Spearman. Hasil penelitian menunjukkan median 22 serta nilai minimum dan maksimum yaitu 12 dan 86 bulan. Untuk data IDWG median adalah 3 serta nilai minimum dan maksimum yaitu 1 dan 4,5 kilogram. Analisis hubungan menunjukkan tidak ada hubungan antara lama waktu menjalani hemodialiasis dengan IDWG pada pasien hemodialisis di RSUD Dumai (p value = 0,952) dengan nilai r = 0,01. Diharapkan perawat menganalisis pengetahuan serta pemahaman pasien tentang perlunya mengontrol asupan cairan dan berat badan terutama pada pasien yang telah lama menjalani hemodialisis untuk mencegah kenaikan IDWG yang dapat memperberat kondisi pasien.Kata kunci: Cairan, IDWG, lama hemodialisis AbstractInter-Dialytic Weight Gain (IDWG) becomes an indicator of successful of hemodialysis patients treatment. More IDWG indicates more fluid accumulates in the patient’s body and more impact caused by the excess fluid. This study aimed to investigate relationship between length of undergoing hemodialysis and Inter-Dialytic Weight Gain (IDWG) in Hemodialysis Patients. The study was correlational study with cross sectional approach. Samples of the study were 34 patients. Samples were recruited from hemodialysis patients who have been undergoing hemodialysis at least 1 year in Dumai General Hospital, were able to communicate verbally and to stand up for measuring body weight, and willing to participate in the study. Length of undergoing hemodialysis was taken from medical records. IDWG was measured in one week period of cycle of undergoing hemodialysis using a scale and observation sheet. The result showed the average length of time undergoing hemodialysis was 26.65 months, SD was 15.55, median was 22 and minimum and maximum values were 12 and 86 months respectively. The mean of IDWG was 2.73 kilograms with SD was 1.046, median 3 and minimum and maximum values were 1 and 4.5 kilograms respectively. Result of the study showed that there was no relationship between length of time undergoing hemodialysis and IDWG (p value = 0.952) with r = 0.01. Nurses are expected to further analyze patients’ knowledge as well as understanding about the need to control intake of fluid and body weight to prevent problems caused by kidney damage which suffered by hemodialysis patients.Keywords: Fluid excess, hemodialysis patients, IDWG

Previous cross-sectional studies showed that immigrants from low-income to high-income countries have higher risk of cardiovascular disease and type 2 diabetes mellitus. We investigated the association between weight gain during the resettlement in South Korea and risk of metabolic syndrome (MetS) among North Korean refugees (NKRs) in this cross-sectional study. In total, 932 NKRs aged 20–80 years in South Korea voluntarily underwent health examination from 2008 to 2017. We compared the risk of MetS and its components between the weight gain group (gained ≥5 kg) and the non-weight gain group (gained <5 kg, maintained or lost body weight) during resettlement in South Korea after defection from North Korea. Multiple logistic regression analysis predicted odds ratio of MetS on the basis of weight change, adjusting for covariates and current body mass index (BMI). We also evaluated the difference in body composition of NKRs between two groups. The prevalence of MetS in the weight gain group was 26%, compared to 10% in the non-weight gain group (p-value < 0.001). The weight gain group had a two-fold higher risk of MetS than the non-weight gain group after adjusting for current BMI (odds ratio 1.875, p-value = 0.045). The prevalence of central obesity, impaired fasting glucose, elevated blood pressure, and hypertriglyceridemia were higher in the weight gain group than the non-weight gain group (36% vs. 12%, p-value < 0.001; 32% vs. 19%, p-value < 0.001; 34 vs. 25%, p-value = 0.008; 19% vs. 13%, p-value = 0.025, respectively). The analysis of body composition showed that the percentage of body fat in the weight gain group was higher than in the non-weight gain group, indicating increased fat mass rather than muscle mass in the weight gain group as their body weight increased during resettlement (33.4 ± 6.53% vs. 28.88 ± 7.40%, p < 0.005). Excess weight gain after defection from North Korea increased the risk of MetS among NKRs in South Korea. It is necessary to monitor weight change among NKRs and their effect on their metabolic health in the long term.

Background: Proton pump inhibitors (PPIs) are in routine widely prescribed to hemodialysis patients. Recent studies have reported the association of PPIs use with hypomagnesemia in patients with long term hemodialysis. This study aims to determine the frequency of hypomagnesemia in patients of hemodialysis taking proton pump inhibitors. Patients and methods: This cross-sectional study was conducted in the Department of Nephrology of Sandman Provincial Hospital Quetta from 01-6-2019 till 01-9-2019. A total of 120 patients (52 PPI users and 68 non-PPI users) who were on HD for more than 06 months were included. Data regarding age, gender, duration of hemodialysis and taking PPIs were collected. Determination of serum magnesium was made by taking 3 different samples at 2 weeks’ interval and the mean value of serum magnesium was calculated. Serum Mg2+ levels <2.0 mg/dL was taken as hypomagnesemia. A Chi-square test was applied to determine the association of PPI use with hypomagnesemia. Results: Demographic variables such as age and gender were not significantly different between the groups. There was female dominance in both groups (73% in PPI groups and 66.1% in the non-PPI group (p-value 0.65). The mean duration of dialysis was 45.3±13.8 months in PPI users versus 48.9±12.9 months in non-PPI users (p-value 0.14). There was a significantly higher frequency of hypomagnesemia in PPI users; 36 (69.3%) versus 27 (39.7%) in non-PPI users (p-value 0.001). Conclusion: The use of PPI is associated with a significant reduction in serum magnesium levels. So serum magnesium levels should be advised as routine monitoring in patients of hemodialysis taking PPIs.

Background: Breast milk and colostrums are the first feeding sources for infant, providing nutrients, growth factors and immunological components. So we conducted this study to compare the growth pattern of neonates on breast feed versus formula feed.Methods: This study was done in the Department of Pediatrics, King Edward Medical University, Mayo Hospital, Lahore from 2015 July to 2016 July as cross sectional study. The Non Probability purposive sampling technique was used. Information on type of feeding was obtained from mothers. Subsequently neonates were divided in two groups on the basis of type of feeding (i.e. breast feeding or formula feeding).Results: In this study the mean age of the patients was 16.56±6.26 days and the mean gestational age of the patients was 8.52±0.97 months. The male to female ratio of the patients was 1.3:1. Statistically there was significant difference found between the weight gain in study groups at 10th, 14th week and 4th month follow up i.e. p-value<0.05.Conclusion: The prevalence of breastfeeding in infants in our study was 52.3%. Our results showed that the breast fed infants had better weight gain compared to formula fed infants; however there was no statistically significant difference in gain in length between breast fed and formula fed infants.Med Phoenix Vol.2(1) July 2017, 18-23

Patients with hemodialysis are required to carry out routine care, diet arrangements, and fluid restrictions. The highest non-compliance in patients is fluid restriction. Excessive fluid intake can be seen from weight gain between dialysis time (IDWG). IDWG in hemodialysis patients should not be more than 3.5%, where an increase in IDWG can cause the risk of death and decreased quality of life. However, at the hospital itself, routine identification of fluid restriction non-compliance activities was not carried out, so that there were still many patients with hemodialysis who experienced an increase in IDWG in the moderate and severe categories. The purpose of this study is to explore the characteristics, fluid restriction compliance, IDWG, and quality of life in hemodialysis patients. This is done to plan the provision of further intervention in patients. The design in this study was cross-sectional with sampling techniques with simple random sampling. The results of the study characteristics of respondents at the age of 46-55 years (64.7%) more male sex (55.9%), and high school education (47.1%), all respondents were married (100%), most not working (58.8%), the average length of hemodialysis is 18 months. The mean mean liquid restriction compliance was 27.35, the mean value of IDWG was 4.97 and the mean value of quality of life was 51.72.

Fatigue and inadequacy dialysis are common problem in hemodialysis patients. The dialysis inadequacy can cause an increased progression of impaired renal function, as well as the increased morbidity and mortality, and declining productivity of hemodialysis patients. Fatigue prevalence ranged from 44,7- 97% from mild to severe. Fatigue is a common complaint of hemodialysis patients that can lower physical function and life quality. To determine the correlation between adequacy and the fatigue level of the patients with End Stage Renal Disease (ESRD) undergoing hemodialysis. This study used a descriptive analytic and cross sectional approach involving 75 respondents and the FACIT-G Questionnaire was used to collect the data. The inclusion criteria are male and female patients aged 18 -70, undergoing hemodialysis for more than 3 months with a frequency of 2 times at least 4 hours, composmentis patients. The adequacy hemodialysis was assessed using the Kt/V formula. All data were collected during the session of hemodialysis. Pearson Product moment test wes used to analyze the data. The mean dialysis adequacy was 1.43±0.380, 57(76%) only 13 (17.3%) patients had adequate dialysis (minimum laboratory standard Kt / v = 1.8) and inadequate were 62 (82.7%) patients. The mean of fatigue was 20.07 and 62 (82.7%) respondents experienced severe fatigue. There was no significant correlation between adequacy and the fatigue level of the patients with ESRD undergoing hemodialysis with p value 0.504 (α> 0.05). Mostly patients had inadequate dialysis, both adequate and inadequate dialysis patients had experience fatigue from mild to severe. Multiple individuale and personnel factors affect dialysis adequacy directly or conversely.

Phyllodes tumor is a breast fibroepithelial lesion that rarely occurred. This type of tumor has tendency to recurre and metastasize. This study aimed to determine the features of phyllodes tumor in Hasan Sadikin General Hospital in period of 2012 to 2016, histopathologically and clinically. Descriptive study with cross sectional design was conducted in Department of Pathological Anatomy, Hasan Sadikin General Hospital, Bandung, from September 2016 to May 2017. Using total sampling, the secondary data are taken from medical records with the diagnosis of phyllodes tumor. From 88 samples taken, the mean age of the tumor occurrence is 45,56 years old and the mean size of tumor diameter is 10,98 cm. Tumor with benign type has the highest prevalence, with 54 cases reported. Mastectomy is the most common procedure performed on this tumor. There are 7 recurred patients and 1 patient with metastasis. As conclusions, phyllodes tumor is a rare tumor. In Hasan Sadikin general hospital, it mostly occurs in women aged 45 years old in the benign type. Mastectomy is often performed as a choice of surgical treatment.

Background: Coronavirus disease-2019 (COVID-19) is a novel coronavirus type infection disease that was first reported at Wuhan city, Hubei province, China in December 2019. Cases of COVID-19 in Indonesia were increasing, reaching up to 287,008 confirmed cases on 30th September 2020. Sanjiwani general hospital Gianyar, Bali, one of the COVID-19 referral hospital at Gianyar, Bali, has treated as many as 149 confirmed COVID-19 cases from August to October 2020. The most significant laboratory parameter associated with COVID-19 severity was C-reactive protein (CRP) and neutrophil to lymphocyte ratio (NLR).Methods: This study is a descriptive cross-sectional study. Data were gathered through secondary data from the medical records using the total sampling method. Descriptive analyses were performed to describe the samples characteristics and to calculate the mean CRP level. Meanwhile, the chi-square test was done to investigate the association between CRP level and the severity level of COVID-19. The result is considered statistically significant if the p value is <0.05Results: Most of the samples are >60 years old (24.4%), female (56.6%), and have diabetes mellitus as their comorbid (46,7%). The mean CRP level is 8.9 mg/L. CRP level >8.9 mg/l significantly affects COVID-19 patients’ severity with the p value of 0.000 (p<0.005). The higher the initial CRP level of COVID-19 patients, the higher the severity level will be.Conclusions: There is an association between the increased CRP level at the beginning of hospital admission and the severity of COVID-19 patients.

BACKGROUND: Coronavirus disease 2019 (COVID-19) pandemic harmed the world community including hemodialysis patients. It has affected the physical and psychological status of hemodialysis patients. AIM: This study aimed to evaluate the dialysis adequacy, interdialytic weight gain, and quality of life in patients undergoing hemodialysis during the COVID-19 pandemic. METHODS: A quantitative study with a cross-sectional approach was conducted on 105 regular hemodialysis patients from three hemodialysis centers in Riau Province, Indonesia. The patients were purposively recruited. Data on hemodialysis adequacy and interdialytic weight gain were collected in April–May 2020 using an observation sheet, while the quality of life was measured using SF 36. To analyze the data, Pearson’s correlation test and linear regression were performed. RESULTS: Within the sample of 53 male patients was 50.5% and 52 female patients was 49.5%. The mean score of dialysis adequacy during April–May was 1.75, while the mean IDWG was 2.2 kg, and the mean quality of life was 91.51. There was a relationship between adequacy and IDWG (p = 0.002), and between IDWG and quality of life (p =0.015). There was no relationship between adequacy and quality of life (p = 0.360). CONCLUSION: IDWG influenced the quality of life of hemodialysis patients. Health-care professionals need to help patients to keep their IDWG in the normal range to survive within COVID-19 pandemic.

Background: The visual evoked potentials (VEP) is a valuable tool to document occult lesions of the central visual channels especially within the optic nerve. Objectives: The purpose of the present study was to observe the findings of first few cases of VEP done in the neurophysiology department of the National Institute of Neurosciences (NINS), Dhaka, Bangladesh. Methodology: This cross-sectional study was conducted in the Department of Neurophysiology at the National Institute of Neurosciences and Hospital, Dhaka, Bangladesh from September 2017 to March 2020. All patients referred to the Neurophysiology Department of NINS for VEP were included. Pattern reversal VEPs were done using standard protocol set by International Federation of Clinical Neurophysiology (IFCN). Results: The mean age of the study population was 30.70 (±12.11) years (6-68 years) with 31 (46.3%) male and 36 (53.7%) female patients. The mean duration of illness was 8.71 (±1.78) months (3 days- 120 months). Most common presenting symptom was blurring of vision (37.3%) and dimness of vision (32.8%). Patterned VEP revealed mixed type (both demyelinating and axonal) of abnormality in most cases [29(43.35)]. The most common clinical diagnosis was multiple sclerosis (29.85%) and optic neuropathy (26.87%). In the clinically suspected cases of multiple sclerosis, optic neuropathy and optic neuritis most of the cases of VEP were abnormal and the p value is 0.04 in optic neuropathy and optic neuritis. Conclusion: The commonest presentation of the patients in this series were blurring of vision and dimness of vision. The most common clinical diagnosis for which VEP was asked for, was optic neuritis and multiple sclerosis. Most abnormalities were of mixed pattern (demyelinating and axonal). ABSTRAK   Berdasarkan data di RSUD Kota Prabumulih bahwa terdapat peningkatan jumlah kejadian perdarahan post partum, tahun 2014 sebanyak 178 orang, tahun 2015 sebanyak 246 orang dan tahun 2016 sebanyak 151 orang. Penelitian ini bertujuan untuk mengetahui hubungan umur ibu dan jarak kehamilan terhadap kejadian perdarahan post partum di RSUD Kota Prabumulih Tahun 2016. Penelitian ini merupakan penelitian analitik dengan rancangan cross sectional. Populasi yang digunakan adalah semua ibu bersalin di RSUD Kota Prabumulih, sebanyak 1.296 ibu bersalin dan 306 sampel. Pengambilan sampel dengan mengunakan random sampling, analisa data menggunakan analisa univariat dan bivariat dengan menggunakan uji statistik chi-square dengan derajat kemaknaan 0,05. Hasil penelitian ini menunjukan bahwa dari 288 ibu bersalin terdapat 151 yang mengalami perdarahan post partum sebagian besar adalah ibu dengan umur resiko tinggi sebanyak 43 orang  (40,9%) dan ibu dengan jarak kehamilan resiko tinggi sebanyak 21 orang (17,3%). Hasil uju chi-square umur didapatkan p.value 0,000 < α 0,05 dan uji chi-square untuk jarak kehamilan didapatkan p.value 0,000 < α 0,05. Maka ada hubungan umur ibu dan jarak kehamilan terhadap kejadian perdarahan post partum di RSUD Kota Prabumulih Tahun 2016. ABSTRACK   Based on the data at District General Hospital (RSUD) Prabumulih, there was increasing of post-partum bleeding, in 2014, there were 178 people. In 2015, there were 246 people. And in 2016, there were 151 people. The purpose of the study was to know relationship between maternal mother and the distance and old post-partum bleedingat District General Hospital (RSUD) Prabumulih in 2013. The study was analytic researchwith cross sectional design. Population thas was used in the study was all maternal mother ar District General Hospital (RSUD) Prabumulih. It was about 1.296 maternal mother and from 306. Random sampilng was done in the study, data analyses used univariate and bivariate analyses by using chi-square statistic test with significance level 0,05. The study result showed that from 306 maternal mother, there were 151 mother who experienced old post-partum bleeding, the large of that was high maternal mother 43 people  (40,9%) and mother age high distance 21 people (17,3%). The result of chi-square test was p value 0,000 < α 0,05 and chi-square test for age was p value 0,000 < α 0,05. It meant that there was relationship between maternal mother and the distance and of post-partum bleeding at District General Hospital (RSUD) Prabumulih in 2016.
