The aimed of this study is to compare the diagnostic value of pleural fluid total cholesterol and Light’s criteria to determine exudate or transudate. The samples used in this cross-sectional study were pleural fluid specimens sent to the Clinical Pathology Laboratory of the Dr.Wahidin Sudirohusodo Hospital Makassar during the period of August-September 2016. Data were grouped according to the type of effusion then statistically analyzed using nonparametric Mann Whitney U-test. The result of this study showed from 55 samples, there were 22 transudates and 33 exudates. The mean total cholesterol levels of exudate is higher than transudate (p=0.006). By using cut-off value of total cholesterol pleural fluid 56 mg/dL, it had sensitivity 72.7%; specificity 78.8%; Positive Predictive Value (PPV) 81.3% and Negative Predictive Value (NPV) 69.6% while Light’s criteria had sensitivity 97%; specificity 63.6%; PPV 80% and NPV 93.3%. Pleural fluid total cholesterol level with cut-off 56 mg/dL is proposed to be used for differentiating exudate and transudate because it is easier and more simple to perform than Light’s criteria.

Data about lipoprotein changes and their link with cardiovascular disease and atherosclerosis in systemic sclerosis (SSc) are still challenging. We aimed to evaluate serum lipid profile of patients with SSc and to identify potential relation with different disease specific characteristics (clinical, serological, inflammatory tests) in a cross-sectional study. Standard assessments comprised SSc-related parameters (disease subtype, clinical spectrum, immunological tests) and lipid metabolism (total cholesterol and fractions, triglycerides). Impaired lipid profile (low serum HDL- and high LDL-cholesterol, increased serum triglycerides, slightly modification in total cholesterol level) significantly correlated with diffuse SSc, activity (EUSTAR) and severity (MEDSGER), as well as seropositivity for specific antibodies (anti-centromere and anti-topoisomerase 1). The dyslipidemic profile might represent a pathobiological pathway for atherosclerosis in SSc.

Small dense LDL (sdLDL) is the LDL which particles are small and dense, it is pro-atherogenic. Increased levels of serum sdLDL areassociated with an increased risk of coronary stenosis. The aim of this study was to examine the diagnostic value of sd LDL in coronarystenosis. An analytical observational study with cross sectional approach was conducted at the Department of Clinical Pathology, MedicalFaculty of Diponegoro University/Dr. Kariadi Hospital and the Unit of Cardiac diseases during the period of March-October 2013. Thesubjects were 39 patients suspected of suffering a coronary stenosis. The diagnosis of coronary stenosis, degree of stenosis and numberof vascular stenosis was established at the time of cardiac catheterization. SdLDL assessment used a test kit. The statistical analysis usedwere unpaired t-test, Spearman correlation test, ROC analysis and diagnostic test. LDL levels in stenosis subjects, 35.4±9.01 mg/dL weresignificantly higher compared to levels in subjects that had no stenosis, 20.7±7.10 mg/dL (p<0.001; unpaired t-test). Correlation testresults showed a correlation between levels of serum sdLDL with severe degree of stenosis (correlation coefficient -0.64, p <0.001) and amoderate positive correlation between the number of vascular stenosis (Coefficient correlation 0.46; p=0.003; Spearman Correlation’sTest). The area under the curve of ROC was 0.9 (p <0.001). The cut off levels sdLDL were used to detect stenosis. The results showeda sensitivity of 85.2%, specificity of 75%, positive predictive value of 88.5%, negative predictive value of 69.2% and accuracy of 82%.Levels of serum sdLDL were associated with severe to extensive stenosis degree, and showed a good diagnostic value, thus, it can beused for screening to determine the presence of coronary stenosis.

BACKGROUND: Late-onset neonatal sepsis (LONS) detection is problematic as no single examinations (blood culture, c-reactive protein (CRP), procalcitonin (PCT)) are reliable. Toll-like receptors (TLRs), which detect the presence of pathogen-associated molecular patterns is a promising novel biomarker, but less studied in LONS. This study aimed to determine neutrophils and monocytes TLR2 and TLR4 expression in LONS and their diagnostic value. METHODS: A cross-sectional study conducted in May and June 2017 involving 52 neonates with clinical late-onset (>72 hours of age) sepsis. We examine complete blood count, I/T ratio, CRP, PCT, as well as TLR2 and TLR4 expression to compared with blood culture as the gold standard. We classified cases into proven or unproven sepsis. RESULT: The incidence of LONS was 32.6% in the subjects. The expression of TLR2 was low in LONS, while TLR4 was high. TLR4 neutrophil expression has 88.2% sensitivity, 20% specificity, 34.9% positive predictive value (PPV), 77.8% negative predictive value (NPV), and an AUC of 0.541. TLR4 monocyte expression has 92.1% sensitivity, 11.4% specificity, 34% PPV, 80% NPV, and an AUC of 0.528. The AUC of CRP is increased from 0.608 to 0.843 after combination with TLR4, comparable with CRP + PCT (AUC 0.829). CONCLUSION: The increase in TLR4 expression has good sensitivity but low specificity. TLR4 expression, in combination with CRP, could become a reliable biomarker for the diagnosis of LONS.

AbstractObjectiveTo assess coffee consumption in the Sami and Norwegian populations and to investigate the impact of unfiltered boiled coffee consumption on serum cholesterol concentrations.DesignA cross-sectional study. Information was collected by self-administrated questionnaires and total serum cholesterol was analysed. Participants were divided into three ethnic groups: Sami I (Sami used as home language in the last three generations), Sami II (at least one Sami identity marker) and Norwegian.SettingIn an area with Sami, Kven/Finnish and Norwegian populations, the SAMINOR study, 2003–2004.SubjectsA total of 5647 men and 6347 women aged 36–79 years.ResultsMore than 90 % of the study populations were coffee drinkers. Only 22 % were unfiltered coffee consumers. Sami I had the highest proportion of participants who consumed nine or more cups of unfiltered coffee per day, although the number of participants was limited. Total coffee consumption was associated with increased total cholesterol for men (P < 0·01) and women (P < 0·0001). For those who drank only unfiltered coffee, a significant association was found only in Norwegian men, adjusted for physical activity in leisure time, BMI and smoking habits (P < 0·001). From the lowest (less than five cups) to the highest (nine or more cups) unfiltered coffee consumption category, the mean total cholesterol levels increased by 0·29 mmol/l in Norwegian men.ConclusionsUnfiltered coffee consumption was lower in the present study compared to previous reports. In general, total coffee consumption was positively associated with total cholesterol levels. However, for unfiltered coffee consumption, an association was found only in Norwegian men.

Adenosine Deaminase (ADA) is used as a well established biological marker for diagnosis of tuberculous pleuritis. The aim of this study was to assess the sensitivity and specificity of ADA in patients with pleural effusion admitted in the medical ward. This was an cross sectional study. The information was gathered only from those patients whose pleural fluid sample was sent for ADA in clinical biochemistry laboratory of KIST Medical College and Teaching hospital. The data was acquired from the medical records of patients attending medical ward. The patients were considered as TB positive if they were clini­cally diagnosed so and taken as negative otherwise. Fifty six pleuritis patients were evaluated. Using the cut off value of 30 U/L, the overall sensitivity and specificity was 93.3% and 57.1% respectively. The association between ADA findings and the clinical findings were statistically significant (p <0.001). The positive predictive value (PVP) and the negative predictive value (PVN) were 82.3% and 80% respectively. ADA is an inexpensive, rapid and simple test for analysis of tuberculous pleuritis. Though, it is a sensitive test for tuberculosis, due to its low specificity, sometimes negative cases can be considered as positive. Introduction: Hormonal injection contraception is the most widely used method of contraception in Indonesia. In long-term use, one of the side effects of injection contraception is changes in lipid metabolism in the body caused by the accumulation of hormones in the body. Two types of injectional contraception are widely used in Indonesia, namely combined injection and DMPA injection; different hormonal content has various side effects on the lipid profile. This study aimed to compare the total cholesterol level between acceptors of DMPA injection and combined injection.Methods: This study used an observational analytic method with a cross-sectional approach. The sample size is 60 respondents, including 30 DMPA injection acceptors and 30 combination injection acceptors. The sampling method uses a purposive sampling technique The independent variable in this study was the total cholesterol level of the acceptor, and the dependent variable was the acceptor of DMPA injection and the combination injection acceptor. Analysis of research data used the Independent T-test with α=0.05.Results: The results of the bivariate analysis found significant differences in total cholesterol levels in the DMPA injection contraception acceptor and combination injection with p= 0.037. It was found that overall cholesterol levels of DMPA injection acceptors were higher.Conclusion: There is a difference in total cholesterol levels in the DMPA injection contraception acceptor with a combination injection contraception acceptor.

BACKGROUNDMeningiomas are the most common primary extra-axial non-glial intracranial tumors. The severe grade of meningioma, according to WHO, has the highest recurrence rate accompanied by high morbidity and mortality rates. Therefore, it is imperative to perform pre-operative assessments so the clinician can give prompt treatment to gain a better prognosis. It is a novel alternative way of predicting meningioma’s malignancy by calculating the tumor’s apparent diffusion coefficient (ADC) value. The objective of the study was to determine the value of ADC for differentiating benign and malignant meningiomas. METHODSThis cross-sectional study involved 32 subjects with clinically diagnosed or histologically verified meningioma (21 benign and 11 malignant). They underwent a head-magnetic resonance imaging (MRI) examination and biopsy. We calculated the ADC value by creating regions of interest (ROIs) on the solid part of the tumor, guided by contrast and fluid-attenuated inversion recovery (FLAIR) sequence. We analyzed the ADC value with independent t-test and Bland-Altman graphs, calculated the average difference, CI 95%, limit of agreement between observers, and ROC. RESULTSMean ADC of malignant meningiomas (0.877 ± 0.167 x 10-3 mm2/s) was significantly lower than that of benign meningiomas (0.990 ± 0.105 x 10-3 mm2/s) (p<0.05). The ADC threshold is 0.886 x 10-3 mm2/s with sensitivity 63.6%, specificity 85.7%, positive predictive value 70% and negative predictive value 81.8%. CONCLUSIONThe ADC value measurement provides a discriminative feature to differentiate between benign and malignant meningiomas. However, the clinical applicability still needs to be elucidated, as histopathological confirmation remains the mainstay of definitive diagnosis.

Abstract: Cholesterol is often seen as something that is very scary and associate high levels of cholesterol in the blood with the risk of heart disease. Increasing cholesterol levels are something to avoid because it can endanger health. The purpose of this study was to examine the relationship age, sex, and obesity with total cholesterol levels on teachers SMAN 1 Sei. Raya. The design in this study using Cross-Sectional. The population of this study is all teachers SMAN 1 Sei. Raya took with total sampling method and obtained as many as 46 respondents. From each respondent obtained data onto age, sex, obesity level and measurement of total cholesterol level. Data were analyzed by univariate and bivariate with correlation test for Kendall tau with value ɑ = 0.05. The test result using Kendall tau to analyze the relationship of age, sex, and obesity with total cholesterol showed that there is no relationship between age (p = 0.648), sex (p = 0.418), and obesity (p = 0.542) with total cholesterol levels in teachers SMAN 1 Sei. Raya.Abstrak: Kolesterol sering dilihat sebagai sesuatu yang sangat menakutkan dan tingginya kadar kolesterol dalam darah dikaitan dengan risiko penyakit jantung. Peningkatan kadar kolesterol merupakan hal yang harus dihindari, karena dapat membahayakan kesehatan. Tujuan penelitian ini adalah untuk mengkaji hubungan antara umur, jenis kelamin dan kegemukan dengan kadar kolesterol total pada guru SMAN 1 Sei. Raya. Desain dalam penelitian ini menggunakan Cross Sectional. Populasi dalam penelitian ini yaitu seluruh guru SMAN 1 Sei. Raya yang diambil dengan metode total sampling dan diperoleh sebanyak 46 orang responden. Dari setiap responden diperoleh data umur, jenis kelamin, tingkat kegemukan dan pengukuran kadar kolesterol total. Data dianalisis secara univariat dan bivariat dengan uji korelasi Kendall tau dengan nilai ɑ = 0.05. Hasil uji menggunakan kendall tau untuk menganalisis hubungan umur, jenis kelamin, dan kegemukan dengan kadar kolestrol total menunjukan bahwa tidak ada hubungan antara umur (p = 0.648), jenis kelamin (p = 0.418) dan kegemukan (p = 0.542) dengan kadar kolesterol total pada guru SMAN 1 Sei. Raya.

Background: Hypercholesterolemia is a condition in which levels of cholesterol in the blood above normal levels (≥ 240 mg/dl). Physical exercises done regularly useful in the regulation of cholesterol, is total cholesterol, LDL cholesterol and triglycerides in the blood decreased, while HDL cholesterol increased. Food has an important role in association with the incident hypercholesterolemia.Objective: The purpose of this study was to determine the effect of aerobic exercise on blood total cholesterol level of adult women in the city of Semarang.Method: This research was explanatory research with cross sectional study. Subject was women aged 20-49 years gymnastic participants, as a comparison group was a housewife who was not doing gymnastics. Twenty three samples for group2 by matching on nutritional status and age and with the sampling purposive sampling. Adequacy levels of carbohydrates, proteins, fats, and also vitamins A and C were obtained through 2x24-hour recall was not a sequence, and processed using NutriSurvey. Results of independent t-test used to determine the effect of cholesterol levels between groups, and chi square test was used to determine association with nutrient adequacy levels of total cholesterol.Results: The results showed no significant difference in mean total cholesterol level between groups of gymnastic participants (201.2 g/dl) with housewife group (208.3 g/dl). Housewife group was belonging to hypercholesterolemia by 52.2%, while in group of exercise participants at 43.5%. Sufficiency level of protein showed a role in the total cholesterol level of adult women.Conclusion: The aerobic gymnastics did not affect total cholesterol. Moderate level of protein adequacy contributes to the total cholesterol level of adult women.

Background The type, amount, and composition of a diet may affect the levels of cholesterol in blood. Itis believed that children adhering to a vegetarian diet have lower total cholesterol levels and lower body mass indexes compared to children with non-vegetarian (omnivorous) diets. We wish to compare cholesterol levels of vegetarian and non-vegetarian Indonesian children who are at risk of overweight or obesity.Objective To compare lipid profiles of vegetarian and no-vegetarian children who are at risk for overweight or obesity.Methods We performed a cross-sectional study in January and February 2010. Subjects for this study came from Denpasar, Bali. Subjects filled questionnaires as well as underwent history-taking, anthropometric measurements, and blood testing. We performed lipid profile analyses on their blood samples. We used the independent t test and Mann-Whitney test for statistical analysis of the data. The level of significance was set at P <0.05.Results Our study included forty-four children at risk for overweight or obesity with a vegetarian or non-vegetarian diet. We found that vegetarian children had lower mean total cholesterol (144 mg/dL) than that of non-vegetarian children (171 mg/dL), a statistically significant difference of P=0.014. In addition, vegetarian children had lower mean triglyceride levels (150 mg/dL) than those of non􀀶vegetarian children (264 mg/dL), a statistically significant difference of P =0 .025.Conclusion Among Balinese children at risk of overweight or obesity, vegetarians have significantly lower mean total cholesterol and triglyceride levels than non-vegetarians.
