The definition of hypertension or high blood pressure is a condition when blood pressure reaches a value of ≥ 140/90 mmHg. If not treated immediately, hypertension can cause serious life-threatening diseases, such as heart failure, kidney disease, and stroke. During the Covid-19 pandemic which has been going on since the beginning of 2020, most government health facilities have become centers for handling Covid 19 patients. So it is difficult for patients other than Covid 19 to come for treatment. Depok has a fairly high number of hypertensive patients and is currently having problems with regular check-ups at the puskesmas. This can make it difficult to control the condition of hypertension patients, apart from the puskesmas being the first service center for Covid-19 patients, the risk of contracting Covid-19 is also high if the patient comes to the puskesmas for control. Various studies on family empowerment in treating hypertensive patients have become one of the basics for our community service team to educate respondents who have family members with hypertension. This community service activity aims to increase knowledge about hypertension and how to control it in respondents so that family empowerment in treating hypertension patients can be improved. To determine the level of understanding of respondents on educational materials, questionnaires were filled out before and after education. The results achieved in this activity is an increase in respondents' knowledge about hypertension and how to control it with family participation.

Background: The balanced  nutrition  paradigm is currently unknown  to  the  general public, especially school-age children. The old paradigm of four healthy five perfects is still taught in schools. Whereas in the new paradigm of balanced nutrition, in addition to the principle of nutrition that must be consumed in a balanced manner, there is another message that must be conveyed, namely physical activity at least 30 minutes every day, regular exercise, drinking 8 glasses of water a day, washing hands before and after eating and always monitor your weight. Purpose: This community service activity aims to improve balanced nutrition knowledge in elementary school age children. Method: The method of community service is through lectures and games for ice breaking so that the atmosphere becomes more fun. Results: Most of the students of SDN Kalisari 1 Surabaya were female (54%), 10 years old (62%), and parents had jobs in the “other” category namely construction workers, janitors and farm workers (38%). As many as 83% of students have breakfast habits and 88% of students often access the internet. The level of student’s knowledge of balanced nutrition is included in the sufficient category (59%). There is a difference in the level of knowledge before and after counseling about the concept of balanced nutrition (α = 0.04). Conclusion: Efforts to increase knowledge of balanced nutrition with counseling and game methods are considered effective because there is a significant increase in the level of knowledge of SDN Kalisari 1 Surabaya students.

CONTEXT AND OBJECTIVES: Assessment of central blood pressure (BP) has grown substantially over recent years because evidence has shown that central BP is more relevant to cardiovascular outcomes than peripheral BP. Thus, different classes of antihypertensive drugs have different effects on central BP despite similar reductions in brachial BP. The aim of this study was to investigate the effect of nebivolol, a β-blocker with vasodilator properties, on the biochemical and hemodynamic parameters of hypertensive patients.DESIGN AND SETTING: Experimental single cohort study conducted in the outpatient clinic of a university hospital.METHODS: Twenty-six patients were recruited. All of them underwent biochemical and hemodynamic evaluation (BP, heart rate (HR), central BP and augmentation index) before and after 3 months of using nebivolol.RESULTS: 88.5% of the patients were male; their mean age was 49.7 ± 9.3 years and most of them were overweight (29.6 ± 3.1 kg/m2) with large abdominal waist (102.1 ± 7.2 cm). There were significant decreases in peripheral systolic BP (P = 0.0020), diastolic BP (P = 0.0049), HR (P < 0.0001) and central BP (129.9 ± 12.3 versus 122.3 ± 10.3 mmHg; P = 0.0083) after treatment, in comparison with the baseline values. There was no statistical difference in the augmentation index or in the biochemical parameters, from before to after the treatment.CONCLUSIONS: Nebivolol use seems to be associated with significant reduction of central BP in stage I hypertensive patients, in addition to reductions in brachial systolic and diastolic BP.

Introduction: Anaphylaxis is a life-threatening condition that paramedics are equipped to treat effectively in the field. Current literature suggests improvements in paramedic recognition and treatment of anaphylaxis could be made. The aim of this study was to compare the proportion of cases of anaphylaxis appropriately treated with epinephrine by paramedics before and after a targeted educational intervention. Methods: This was a retrospective medical records review of patients with anaphylaxis managed by primary or advanced care paramedics in five Emergency Medical Service areas in Ontario, before and after an educational module was introduced. This module included education on anaphylaxis diagnosis, recognition, treatment priorities, and feedback on the recognition and management from the before period. All paramedic call records (PCRs) coded as local allergic reaction or anaphylaxis during 12-month periods before and after the intervention were reviewed by trained data abstractors to determine if patients met an international definition of anaphylaxis. The details of interventions performed by the paramedics were used to determine primary and secondary outcomes. Results: Of the 600 PCRs reviewed, 99/120 PCRs in the before and 300/480 in the after period were included. Of the charts included, 63/99 (63.6%) in the before and 136/300 (45.3%) in the after period met criteria for anaphylaxis (p=0.002). Of the cases meeting anaphylaxis criteria, 41/63 (65.1%) in the before and 88/136 (64.7%) in the after period were correctly identified as anaphylaxis (p=0.96). Epinephrine was administered in 37/63 (58.7%) of anaphylaxis cases in the before period and 76/136 (55.9%) in the after period (p=0.70). Anaphylactic patients with only two-system involvement received epinephrine in 20/40 (50.0%) cases in the before period and 45/93 (48.4%) in the after period (p=0.86). Conclusion: There are gaps in paramedic recognition and management of anaphylaxis, particularly in cases of two-system involvement. These gaps persisted after the implementation of an educational intervention. Other quality interventions and periodic refreshers may be necessary to improve prehospital treatment of anaphylaxis. Limitations include an increase in overall cases and decrease in rate of true anaphylaxis in the after period, which may relate to better case identification after electronic PCR implementation and changes in paramedic recognition.

1. Plasma renin activity, body weight and blood pressure were measured before and after 7 days' treatment with bendrofluazide in ten hypertensive subjects. They were then treated with bendrofluazide alone (5 mg daily) for a minimum of 3 years. The diuretic was then discontinued and the measurements were repeated before and again after 7 days with bendrofluazide. The results were compared with those obtained before chronic treatment with the diuretic. 2. Chronic diuretic treatment was associated with a persistent and progressive rise in plasma renin activity, that fell promptly to pretreatment levels when diuretics were discontinued. This was associated with significant weight gain but no immediate significant rise in blood pressure. 3. When acutely challenged with bendrofluazide the patients showed a greater increase in plasma renin activity on the second occasion than on the first. Three out of five patients with an initially subnormal response had normal responses after chronic diuretic treatment. 4. Chronic diuretic treatment increased the responsiveness of the juxtaglomerular apparatus in some hypertensive patients. 5. Classification of hypertensive patients into renin subgroups may be influenced by previous therapy, even when that therapy has been discontinued for 4 weeks. In particular ‘low renin hypertension’ may be masked by recent use of diuretics, as shown by three of the five patients in this subgroup in the present study.
