Background. Cigarette smoke may cause harm not only to active smokers but also to those in their vicinity (passive smokers). Cigarettes contain nicotine, which triggers the release of catecholamines, affecting lipid metabolism. Exposure to cigarette smoke may increase serum LDL cholesterol levels in active and passive smokers. Objective. This study aimed to analyze the correlation between serum cotinine (a metabolite of nicotine) and LDL cholesterol levels in young adults. Methods. A cross-sectional study was performed, the analysis included 122 Andalas University students, aged 17.5 - 25.9 years. Demographic data, smoking degree, serum cotinine, and LDL cholesterol levels were collected. Bivariate analysis was carried out individually on each independent and confounding variables to the dependent variable, followed by multiple hierarchical regressions analysis. Results. Serum cotinine levels in this study was 10,5 ± 6.8 ng/ml (mean±SD), and serum LDL cholesterol levels were 65,5±18,5 mg/dl (mean±SD). There was no significant correlation between serum cotinine and LDL cholesterol levels in bivariate analysis. However, serum cotinine levels had a nonlinear correlation with serum LDL cholesterol levels in the regression model that included body mass index (BMI) as the confounding variable. The adjusted r2 value in this study is 0,066, the standardized β coefficient for the BMI is 0,197 (p = 0.028), for the serum cotinine levels is -0,830 (p = 0.007), and for the squared serum cotinine levels is 0,753 (p = 0.014).

Asthma is a chronic inflammatory disease of the airways that is characterized recurrent attacks of breathlessness and wheezing, the situation varies in severity and frequency from person to person. Type a descriptive analytic study design with a cross-sectional study, conducted date collectionwas done july 19-2, 2015. in the study population was all patients in the clinic of bronchial asthma disease in hospitals DR.achmad Mochtar Bukittinggi. The sampling technique was accidental sampling using a questionnaire with a sample of 30 people. Analysis of the data was processed and analyzed using a computerized program with univariate and bivariate analysis using chi-square test (p<0,05). The results obtained the majority of respondents exposed to the dust factor in patients with bronchial asthma by 80%, exposure to cigarette smoke as much as 73.3%, and the factor of climate change as much as 70%. Research shows there is a significant association between the risk factors of dust, cigarette smoke risk factors and climate change risk factors (p = 0.046, p = 0.001 and p = 0.004). Based on this study concluded that the degree of asthma in patients with bronchial asthma recurring multi-factor causes. It is advisable in patients with bronchial asthma can determine risk factors for asthma relapse repeatedly to prevent a recurrence. Health services are expected to be able to provide counseling and health education to both family and for patients with bronchial asthma

AbstrakInfark Miokard Akut (IMA) merupakan nekrosis otot jantung akibat terganggunya kebutuhan dan suplai oksigen ke jantung secara mendadak. Faktor risikonya adalah perubahan profil lipid yaitu Kolesterol total, Kolesterol LDL. Kolesterol HDL, dan trigliserida yang dikaitkan dengan pembentukan plak aterosklerosis. Manfaat penelitian ini untuk mengetahui gambaran profil lipid pada pasien IMA. Tujuan penelitian ini untuk mengetahui gambaran profil lipid pada pasien IMA di RSUP M. Djamil Padang periode 1 Januari 2011-31 Desember 2012. Penelitian dilakukan dengan metode deskriptif dengan desain cross sectional study di bagian Rekam Medik RSUP M. Djamil Padang. Hasil penelitian menunjukkan umur terbanyak pasien IMA 45-59 tahun, Jenis kelamin terbanyak pasien IMA adalah laki-laki, perbandingannya adalah 2,7 : 1, Pasien IMA yang memiliki kadar kolesterol total tinggi 79 orang (38,92%) dan normal 124 orang (61,08%), Pasien IMA yang memiliki kadar kolesterol LDL tinggi 76 orang (37,44%) dan normal 127 orang (62,56%), Pasien IMA yang memiliki kadar kolesterol HDL rendah 145 orang (71,43%) dan normal 58 orang (28,57%), dan Pasien IMA yang memiliki kadar trigliserida tinggi 44 orang (21,67%) dan normal 159 orang (78,33%).Kata kunci: infark miokard akut, kolesterol total, kolesterol LDL, Kolesterol HDL, trigliserida AbstractAcute Myocardial Infarction (AMI) is a muscle necrosis of the heart through disruption of demand and supply of oxygen to the heart suddenly. Risk factors of AMI is a change of lipid profile (Total Cholesterol, LDL Cholesterol, HDL Cholesterol, and Triglycerides) associated with the formation of atherosclerotic plaque. The benefit of this research is to reveal the lipid profile in patients with AMI. The objective of this study was to determine the description of lipid profile in patients with AMI at RSUP M. Djamil Padang period January 1st, 2011-December 31th, 2012. The study was conducted with descriptive methods to the design of cross sectional study at the Medical Records Department of RSUP M. Djamil Padang. The results of this study indicate that the most age of the patients of AMI 45-59 years old, the most gender of AMI are male and the ratio 2.7 : 1, AMI patients with high total cholesterol levels 79 people (38.92%) and normal 124 people (61.08%), AMI patients with high LDL cholesterol levels 76 people (37,44%) normal 127 people (62.56%), AMI patients with low HDL cholesterol levels 145 people (71.43%) and normal 58 people (28.57%), and AMI Patients with high triglyceride levels 44 people (21.67%) and normal 59 people (78.33%).Keywords: acute myocardial infarction, total cholesterol, LDL cholesterol, HDL cholesterol, triglyceride

The objective of this cross-sectional study was to assess differences in the control and treatment of modifiable cardiovascular risk factors (CVRF: HbA1c, blood pressure [BP], LDL-cholesterol, body mass index, and smoking habit) according to gender and the presence of cardiovascular disease (CVD) in patients with type 2 diabetes mellitus (T2DM) in Catalonia, Spain. The study included available data from electronic medical records for a total of 286,791 patients. After controlling for sex, age, diabetes duration, and treatment received, both men and women with prior CVD had worse cardiometabolic control than patients without previous CVD; women with prior CVD had worse overall control of CVRFs than men except for smoking; and women without prior CVD were only better than men at controlling smoking and BP, with no significant differences in glycemic control. Finally, although the proportion of women treated with lipid-lowering medications was similar to (with prior CVD) or even higher (without CVD) than men, LDL-cholesterol levels were remarkably uncontrolled in both women with and women without CVD. The results stress the need to implement measures to better prevent and treat CVRF in the subgroup of diabetic women, specifically with more intensive statin treatment in those with CVD.

Objective. Coronary heart disease (CHD) is a prevalent complication of type 2 diabetes mellitus (T2DM). The proatherogenic low-density lipoprotein (LDL) cholesterol is an established risk factor of cardiovascular disease, and evidence also suggests that postprandial plasma glucose (PPG) levels closely delineate CHD mortality in diabetes. The investigators hypothesized that the addition of telehealth consultation to standard antidiabetic therapy may help to reduce postprandial glucose variability and plasma LDL cholesterol levels in patients with T2DM. Methods. This cross-sectional study enrolled patients with newly diagnosed T2DM who received standard antidiabetic therapy with or without additional telehealth consultation. Participants received blood tests for plasma lipid profile and glucose levels at diagnosis of diabetes and after one month of therapeutic intervention. Laboratory results were compared between treatment groups to determine the efficacy of complementary telehealth consultation. Results. Three hundred and seventy-five participants were enrolled. The standard treatment group had considerably greater levels of plasma LDL cholesterol than recipients of telehealth consultation (110 mg/dL vs. 93.1 mg/dL, P<0.001). Moreover, patients receiving standard treatment had greater levels of fasting plasma glucose (104 mg/dL vs. 98.5 mg/dL, P = 0.027), 2-hour PPG (169 mg/dL vs. 111 mg/dL, P<0.001), and postprandial glucose variability (65.4 mg/dL vs. 12.8 mg/dL, P<0.001) than participants under telehealth consultation. Conclusions. Telemedicine in addition to standard antidiabetic therapy helped to reduce plasma LDL cholesterol levels and postprandial glucose variability in patients with newly diagnosed T2DM. Therefore, telehealth consultation is a suitable complement to pharmacologic therapy for diabetic patients to assist in the management of proatherogenic dyslipidemia and postprandial glucose variability.

<b><i>Background/Aims:</i></b> So far detailed insights into the nursing staff’s COVID-19 symptoms, testing, and results are missing. Therefore, this study aimed to describe the setting and regional differences in the occurrence of COVID-19 among nursing staff, analysing symptoms, testing, and their respective results. <b><i>Methods:</i></b> We used an online survey to conduct this cross-sectional study among Austrian nursing staff in different settings between May 12 and July 13, 2020. This article follows the STROBE statement. The survey includes demographic data, including age, professional qualification, e.g., nurse, nurse aid, and in which federal state and setting the participants work. In addition, we asked for COVID-19 symptoms and (result of the) testing. We used descriptive statistics as well as bivariate analysis to calculate the differences. <b><i>Results:</i></b> Nearly every sixth nurse reported experiencing COVID-19 symptoms. We found statistically significant differences between the settings and the federal provinces for the reported COVID-19 symptoms, but not the test results. In total, 1.6% of the participating nurses were tested for COVID-19, of which all were positive for COVID-19. <b><i>Conclusions:</i></b> In the future during a pandemic, health care staff should be comprehensively tested in all settings. Our results showed that shifting the perspectives from critical/acute settings to other settings, such as nursing homes, is needed.

Abstract.Aim: To assess the association of breakfast skipping with overweight and obesity among children in Albania, a post-communist country in the Western Balkans, which is undergoing a long and difficult political and socioeconomic transition towards a market-oriented economy. Methods: A nationwide cross-sectional study was carried out in Albania in 2013 including a representative sample of 5810 children aged 7.0 – 9.9 years (49.5% girls aged 8.4 ± 0.6 years and 51.5% boys aged 8.5 ± 0.6 years; overall response rate: 97%). Children were measured for height and weight, and body mass index (BMI) calculated. Cut-off BMI values of the World Health Organization (WHO) and the International Obesity Task Force (IOTF) were used to define overweight and obesity in children. Demographic data were also collected. Results: Upon adjustment for age, sex, and place of residence, breakfast skipping was positively related to obesity (WHO criteria: OR = 1.5, 95% CI = 1.3–1.9; IOTF criteria: OR = 1.9, 95% CI = 1.4–2.5), but not overweight (OR = 1.1, 95% CI = 0.9–1.3 and OR = 1.1, 95% CI = 0.9–1.4, respectively). Furthermore, breakfast skipping was associated with a higher BMI (multivariable-adjusted OR = 1.05, 95% CI = 1.02–1.07). Conclusions: Our findings point to a strong and consistent positive relationship between breakfast skipping and obesity, but not overweight, among children in this transitional southeastern European population. Future studies in Albania and other transitional settings should prospectively examine the causal role of breakfast skipping in the development of overweight and obesity.

<p><em>Based on data from the Indonesia Ministry of Health in 2015, In Indonesia, new professional nurses were just 2% of the total nurses. This figure was much lower than the Philippines which has reached 40% with bachelor and master level as their education. The purpose of this study was to determine the relationship between age, and leadership with the performance of health personnel</em><em>. </em><em>The design of this research was analytical research with Cross Sectional Study. The population in this study was all nurses and doctors who served in the internal room, children, surgery and midwifery</em><em>. </em><em>Sampling using total sampling</em><em> </em><em>by questionnaires. The data was processed by univariate and bivariate analysis using Chi-square test</em><em>. </em><em>The result showed that 57,8% nurses had poor performance, 56,3% doctors had poor performance, 64,4% nurses had average age 26-35 years, 56,2% doctors had average age  36-45 years, 64.4% nurses have poor leadership, </em><em>and </em><em>50.0% of doctors have less good leadership</em><em>.</em><em> There is a relationship between age</em><em> and </em><em>leadership with the performance of health personnel.</em><em></em></p><p><strong><em> </em></strong></p><p>Berdasarkan data kemenkes RI tahun 2015 jumlah tenaga kesehatan terbanyak yaitu perawat sebanyak 147.264 orang (45,65%). Di Indonesia, perawat profesional baru mencapai 2% dari total perawat yang ada. Angka ini jauh lebih rendah dibandingkan dengan Filipina yang sudah mencapai 40% dengan pendidikan strata satu dan dua. Tujuan penelitian ini untuk mengetahui hubungan antara umur, kepemimpinan dengan kinerja tenaga kesehatan. Jenis penelitian yang digunakan adalah desain penelitian analitik dengan Cross Sectional Study. Populasi pada penelitian ini adalah semua perawat dan dokter. Pengambilan sampel dengan menggunakan Total Sampling. Pengambilan data menggunakan kuesioner. Data diolah dengan analisis univariat menggunakan statistik deskriptif dan analisis bivariat menggunakan uji Chi-square. Hasil penelitian didapatkan 57,8% perawat memiliki kinerja kurang baik, 56,3% dokter memiliki kinerja kurang baik, 64,4% perawat memiliki umur rata-rata 26-35 tahun 64,4%, 56,2% dokter memiliki umur rata-rata 36-45 tahun, 64,4% perawat memiliki kepemimpinan kurang baik, 50,0% dokter memiliki kepemimpinan kurang baik. Terdapat hubungan antara umur dan kepemimpinan dengan kinerja tenaga kesehatan.</p>

Patients with undernutrition at admission have higher risks to worsen their nutritional status, which is linked to an increase in morbidity and mortality. This study investigated the prevalence of undernutrition at admission and its associated factors. A cross-sectional study was conducted on patients aged 18 to 59 years old in Internal Medicine ward at Dr. Cipto Mangunkusumo National General Hospital, Jakarta, Indonesia, between July and September 2019. Factors that might be associated with undernutrition at admission, such as age, sex, marital status, Charlson Comorbidity Index (CCI) and type of comorbidity, depression, and neutrophil–lymphocyte ratio (NLR), were assessed. Bivariate and multivariate analyses were used to determine the associated factors. Sixty hospitalized patients with median age of 42 years and 76.7% with married status joined the study. The most common reason for hospitalization was acute gastrointestinal disease with gallstones as the most common comorbidity. Undernutrition exists in 26.7% of subjects. High CCI score was observed among 11.7% subjects and half of subjects had NLR category ≥5. Bivariate analysis revealed that unmarried status, age ≥40 years, and malignancy were associated with undernutrition at admission. Logistic regression analysis showed malignancy as an independent predictor of undernutrition during the initial hospital admission (odds ratio [OR] = 11.8; 95% confidence interval [CI]: [1.1, 125.7]). The prevalence of undernutrition at admission was 26.7%. Factors associated with an increased prevalence of undernutrition at admission were age <40 years, unmarried status, and malignancy. Malignancy was an independent factor of the prevalence of undernutrition at admission.

Abstract Background Pregnant women experience physical, physiological, and mental changes. Health-related quality of life (HRQoL) is a relevant indicator of psychological and physical behaviours, changing over the course of pregnancy. This study aims to assess HRQoL of pregnant women during different stages of pregnancy. Methods This cross-sectional study was performed using the The EuroQoL Group’s five-dimension five-level questionnaire (EQ-5D-5L) to assess the HRQoL of pregnant women, and demographic data were collected. This study was conducted in a regional university hospital in Guangzhou, China. Results A total of 908 pregnant women were included in this study. Pregnant women in the early 2nd trimester had the highest HRQoL. The HRQoL of pregnant women rose from the 1st trimester to the early 2nd trimester, and dropped to the bottom at the late 3rd trimester due to some physical and mental changes. Reports of pain/discomfort problem were the most common (46.0%) while self-care were the least concern. More than 10% of pregnant women in the 1st trimester had health-related problems in at least one dimension of whole five dimensions. In the whole sample, the EuroQoL Group’s visual analog scale (EQ-VAS) was 87.86 ± 9.16. Across the gestational stages, the HRQoL remained stable during the pregnancy but the highest value was observed in the 1st trimester (89.65 ± 10.13) while the lowest was in the late 3rd trimester (87.28 ± 9.13). Conclusions During pregnancy, HRQoL were associated with gestational trimesters in a certain degree. HRQoL was the highest in the early 2nd trimester and then decreased to the lowest in the late 3rd trimester due to a series of physical and psychological changes. Therefore, obstetric doctors and medical institutions should give more attention and care to pregnant women in the late 3rd trimester.

Background:Foot disease is a common problem in rheumatoid arthritis (RA). Therapeutic decisions are often based on clinical examination (CE) alone, which can be adversely affected by factors such as deformity, obesity, and peripheral edema. Ultrasonography (US) has previously been shown to be more sensitive than CE for detecting synovitis and tenosynovitis in RA forefeet, but few data exist for the hindfoot and ankle.Objectives:The aim of this study was to compare CE and US for the detection of hindfoot and ankle synovitis and tenosynovitis in patients with established RA.Methods:We conducted a cross-sectional study including patients with RA (ACR/EULAR 2010). Demographic data and disease parameters were collected. CE was performed by a rheumatologist for the presence or absence of tenderness, swelling, and mobility restriction of both ankles. The following tendons were examined for tenosynovitis: tibialis anterior (TA) and posterior (TP), fibularis longus (FL), and brevis (FB) (assessed together). In a second time, US examination of the tibiotalar, talonavicular, and subtalar joints and the same tendons as CE was performed by a blinded radiologist experienced in musculoskeletal imaging using a Philips HD11 device with a high-frequency linear transducer. The presence or absence of synovitis and tenosynovitis was recorded, and the composite synovitis score (power doppler / grayscale ultrasound (PDUS)) was measured for each joint. The US score of each patient was defined by the sum of the composite scores of the joints studied (0-30). A p-value <0.05 was considered significant.Results:Sixty-two feet were examined in 31 RA patients (25 women and six men) with a mean age of 54.8±10.8 years old [32-70]. The mean disease duration was 8.5±7.2 years [1-37]. Rheumatoid Factor (RF) and Anti-Citrullinated Peptides Antibodies (ACPA) were positive in 61.3% and 83.8% of cases. The mean DAS28 ESR was 3.8±1.5 [0.6-7].Clinical examination of ankles revealed tenderness in 57.4% of cases, swelling in 38.8% of cases, and restriction in the range of motion in 11.1% of cases. TA tenosynovitis was noted in 14.8% of cases, TP tenosynovitis in 22.2% of cases, and FL and FB tenosynovitis in 31.5% of cases.US showed tibiotalar synovitis in 59.3% of cases, talonavicular synovitis in 64.8% of cases, and subtalar synovitis in 46.3% of cases. TA tenosynovitis was noted in 5.6% of cases, TP tenosynovitis in 22.2% of cases, and FB and FL tenosynovitis in 25% and 11.1% of cases respectively.An association was found between clinical tenderness and US synovitis of the tibiotalar joint (p=0.013) and the talonavicular joint (p=0.027). No association was noted between clinical swelling and US synovitis in these joints.No association was noted between clinical and US tenosynovitis of TA (p=0.279), TP (p=0.436), FB (p=0.495) and FL (p=0.315).Conclusion:Clinical examination of RA ankles may be challenging and needs to be coupled with US, which is more sensitive and accurate in the detection of synovitis and tenosynovitis.Disclosure of Interests:None declared
