Puskesmas merupakan fasilitas pelayanan Kesehatan tingkat pertama yang dapat membantu mewujudkan derajat kesehatan yang optimal serta memiliki peran penting dalam pelaksanaan rekam medis (Kepmenkes RI, 2009). Puskemas Alai merupakan salah satu sarana pelayanan kesehatan di Kota Padang. Puskesmas Alai dalam memberikan layanan asuhan Kesehatan kepada masyarakat selalu menggunakan berkas rekam medis sebagai tempat penyimpan informasi pasien. Berdasarkan Permenkes No.269/MENKES/PER/III/2008 Tentang Rekam Medis maka dinyatakan rekam medis adalah berkas yang berisi catatan dan dokumen tentang identitas pasien, pemeriksaan, pengobatan, tindakan dan pelayanan lain yang telah diberikan kepada pasien. Berkas rekam medis tersebut harus dapat disimpan dengan baik agar Ketika dibutuhkan lagi bisa didapatkan dengan efektif dan efisien dan kemanan berkas rekam medis tersebut bisa terjaga dengan baik. Untuk penyimpanan berkas rekam medis agar sesuai dengan standar yang sudah ditentukan maka dibutuhkan rak penyimpanan yang mampu menyimpan semua berkas rekam medis. Menurut Internasional Federation of Health Record Organization (IFHRO) kebutuhan rak rekam medis dapat dianalisis. Berdasarkan paparan tersebut maka Kegiatan Pengabdian Kepada Masyarakat (PKM) ini merupakan sosialisasi tentang metode analisa penghitungan kebutuhan rak rekam medis di Puskesmas Alai. Hasil dari kegiatan sosialisasi ini adalah dapat digunakan sebagai masukan bagi bagian rekam medis dalam kegiatan pengadaan kebutuhan rak rekam medis pada Puskesmas Alai Padang. Kata kunci: Rekam medis, Rak, Penyimpanan, Puskesmas, Tekhnik ABSTRACT Puskesmas is a first level health service facility that can help achieve optimal health status and has an important role in the implementation of medical records (Kepmenkes RI, 2009). Puskemas Alai is one of the health service facilities in Padang City. Puskesmas Alai in providing health care services to the community always uses medical record files as a storage place for patient information. Based on Permenkes No.269 / MENKES / PER / III / 2008 concerning Medical Records, it is stated that a medical record is a file containing notes and documents about patient identity, examination, treatment, actions and other services that have been provided to patients. The medical record files must be able to be stored properly so that when needed again they can be obtained effectively and efficiently and the safety of the medical record files can be maintained properly. For storing medical record files to conform to predetermined standards, a storage rack is needed which can store all medical record files. According to the International Federation of Health Record Organization (IFHRO) the need for medical record racks can be analyzed. Based on this explanation, this Community Service Activity (PKM) is a socialization of the analysis method for calculating the need for medical record racks at Alai Public Health Center. The results of this outreach activity can be used as input for the medical record section in the procurement of medical record racks at Alai Padang Health Center. Understanding health service utilisation by community groups can be used to enhance cost-effective service delivery planning. In an inquiry into general health needs, and experiences with health service utilisation by Chinese living in Melbourne, we conducted a series of focus group discussions to explore community health-seeking behaviour. Seven focus groups were drawn from community groups and bilingual health workers in the period September-October 1999. Discussions were carried out in dialect familiar to the participants, facilitated by trained multilingual researchers, tape recorded and transcribed in Chinese and then translated into English. Cross validation was carried out by an independent researcher. Themes that emerged from these discussions included common pathways to care seeking, barriers to the use of health care services, general health concerns, and perceived validity of health information sources. Participants opt for self care when feeling unwell if the condition is perceived as ?not severe?. Use of over-the-counter medication is usually the first course of action. There is a tendency to ?wait and see? when feeling unwell. The use of Traditional Chinese Medicine Practitioners (TCMP) is common, while continuing to see Orthodox Western Medicine practitioners. There are, however, common concerns about the quality of care provided by TCM practitioners and their qualifications. Language, transport, and cost are among other barriers that undermine the use of health care services. Participants reported diabetes, heart disease risk factors, peptic ulcer, hay fever and asthma, poor vision, dental problems, social isolation, and gambling among the most common health concerns. Participants accepted health information from a wide range of sources and placed greater trust in material disseminated by SBS Chinese Radio Programs and the Chinese Health Foundation; an established community organisation run by voluntary health professionals. In conclusion, the study confirmed a number of patterns by which the Chinese community sought and utilised health services and associated factors.

Sharing data is critical to advancing science, improving health, and creating advances in the delivery of health care services. The value of sharing data for cancer research purposes is well established, and there are multiple initiatives under way that address this need. However, there has been less focus on cancer patient perspectives regarding the sharing of their health information for research purposes. This study examined cancer patient perspectives on the sharing of de-identified health data for research purposes including both data from medical records and mobile applications. This cross-sectional study used survey methodology to collect data from cancer patients/survivors (N = 677). Overall, we found that participants were largely willing (71%) to share de-identified medical data and were most motivated (88%) by a desire to help other cancer patients. Patients were less likely to be comfortable sharing mobile application data (34%). It is vital that we understand patient perspectives on data sharing and work with them as partners, valuing their unique contributions, and attending to their preferences.

This research aims to understand the abuse of power on managing the health service policy and its implication that is very significant in reducing marginalized soldiers’ health due at the ontological level and sociological level. The problem is very interesting to be analyzed by conducting a qualitative research method based on public policy theory, abuse of power theory, and health services theory. Data were collected through in-depth interviews, observation, and documentation related to managing the health service policy cases in Indonesia. Data were analyzed by using interactive models are data reduction, data display, data verification, and supported by triangulation. The results were based on ontological level and sociological level using public policy perspective and power perspective for improving health service policy and practice for The Indonesian Army Forces. Vision and mission of public policy on managing health service policy are needed for providing information to stakeholders related, regarding the regulations and sanctions in health service policy. This result provides inputs for making better regulation on health service policy in Indonesia for state agencies as public officials and practitioners.

In returning the outpatient medical record file to thesection of the Medical Record Unit, Assembling there was a delay. This is because the return of medical record files has not been carried out according to Standard Operating Procedures, as a result, causing delays in the reporting system. The method used is qualitative using a descriptive approach. Data collection techniques by observation, interviews and literature study. This study aims to determine the return of former medical records of outpatients at Dayeuhkolot Health Center. The results of the study prove that the delay in returning outpatient medical record files at the Dayeuhkolot Health Center with presentations during the 1 week study amounted to 63 or 22% of 285 medical record files. Efforts have been made to disseminate information to all officers related to the efforts made by the person in charge of COVID-19 patients in returning medical record files on time.

Abstract - Health services include the recording of the patient's medical record . Medical records were used to aid the treatment process. The number of medical records continues to grow proportional to the number of patients. Tens of thousands of sheets of paper used to record medical record requires effort , time and place great . The amount of effort will continue to grow each day. Search one sheet of medical records among a set of storage shelves requires considerable time and risk data is not found. The risk of error in the search and storing will increase every day. The development of technology allows the implementation of technology in the process of record-keeping. Changes in the form of digital medical records will reduce the need of a previous process. Labor, time and place required by the help of information systems will be reduced significantly . Storage process data stored in the cloud will provide more value for the system as a patient's medical records from a health center can be accessed from other health centers. The development of this system will reduce the risk of inappropriate storage and retrieval of medical records. Grobogan Health Department that oversees health center in Grobogan are office that are ready to migrate business processes into the digital age. Development of medical record information system for the health center expected to improve the quality of service of health centers , especially in health care.

This study aims to determine and analyze the medical record storage system on the effectiveness of patient care at the Cikole Public Health Center. The research method used is quantitative with inferential statistical analysis techniques. Data collection techniques by means of observation, interviews and questionnaires. Cikole Lembang Public Health Center is one of the first level health care facilities. Based on observations, the storage system used is a regional storage system in which each region starts from the first digit. There are 2 officers who work in the medical records section of the Cikole Public Health Center. With the storage system used, there are several shortcomings, namely the occurrence of misfiles and duplication. To find out whether there is an analysis of the medical record storage system on the effectiveness of the service, the author gives a questionnaire to the Cikole Public Health Center officers. The results of the questionnaire also show that the highest percentage level is found in item no.7, namely the decentralized storage system is in accordance with the target of 71%. Then the second is item no. 8, namely a decentralized storage system that allows the provision of medical record files on time by 61%. From the results of interviews with medical records officers, suggestions and input were given: (1). The storage system used was changed to centralization so that the alignment system was not duplicated; (2). The use of bindex as an additional tool to make storage neater and easier.

The novel coronavirus (COVID-19) is an infectious disease caused by a newly discovered coronavirus. Despite strong efforts that have been taking place to control the pandemic globally, the virus is on the rise in many countries. Hence, this study assessed the maternal health care services utilization amidst the COVID-19 pandemic in West Shoa zone, Central Ethiopia. A community-based cross-sectional study was conducted among 844 pregnant women or those who gave birth in the last 6 months before the study. A multi-stage sampling technique was used to select the study participants. The data were collected through face-to-face interviews using a semi-structured questionnaire. Logistic regressions were performed to identify the presence of significant associations, and an adjusted odds ratio with 95%CI was employed for the strength and directions of association between the independent and outcome variables. A P-value of <0.05 was used to declare statistical significance. The prevalence of maternal health service utilization during the COVID-19 pandemic was 64.8%. The odds of maternal health service utilization was higher among mothers who had primary (AOR = 2.16, 95%CI: 1.29–3.60), secondary (AOR = 1.97, 95%CI: 1.13–3.44), and college and above education (AOR = 2.89, 95%CI: 1.34–6.22) than those who could not read and write. Besides, mothers who did travel 30–60 minutes (AOR = 0.37, 95%CI: 0.23–0.59) and 60-90minutes (AOR = 0.10, 95%CI: 0.05–0.19) to reach the health facility had a lower odds of maternal health service utilization than those who did travel <30 minutes. Moreover, mothers who earn 1000–2000 (AOR = 3.10, 95%CI: 1.73–5.55) and > 2000 birrs (AOR = 2.66 95%CI: 1.52–4.64) had higher odds of maternal health service utilization than those who earn <500 birrs. Similarly, the odds of utilizing maternal health service were higher among mothers who did not fear COVID-19 infection (AOR = 2.79, 95%CI: 1.85–4.20), who had not had to request permission from husband to visit the health facility (AOR = 7.24, 95%CI: 2.65–19.75), who had practicedCOVID-19 prevention measure (AOR = 5.82, 95%CI: 3.87–8.75), and used face mask (AOR = 2.06, 95% CI: 1.28–3.31) than their counterpart. Empowering mothers and creating awareness on COVID-19 preventionis recommended to improve maternal health service utilization during the COVID-19 pandemic.

In the activities of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi, a health check was carried out by a health center health worker, but there was no form of a toddler's medical record that could be used to document the results of a health examination. The target of this community service activity is the Posyandu Kutilang posyandu participants. Guidance and consultation was carried out by Lecturers and Students of the Department of Medical Records and Health Information at the Indonusa Polytechnic Surakarta when accompanying cadres of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency in documenting the health history of toddler participants. The assistance provided includes how to fill out medical record forms for toddlers, what items must be filled in, and how to manage forms. Health checks are only recorded in the agenda book for elderly examinations belonging to the Mobile Puskesmas, so that the participants of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi cannot monitor the results of monthly health checks. Results of Assistance in Making Form Design and Recording Medical Records for Toddlers at Posyandu Kutilang, in the hall of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency was attended by 11 people. Activities started according to the agreed schedule. The training participants participated in the activity enthusiastically because they felt that this activity had a role in administrative order through the implementation of recording in the form / resume of toddler medical records.
