Introduction: Advancement in aerospace technology, and resultant change in operational capabilities has posed unique challenges on the aircrew, who is required to operate for prolonged periods seated and restrained. Examination of the effects of prolonged restrained sitting on physical and physiological states was the desired objective of the study. Material and Methods: Longitudinal evaluation of subjective appreciation of physical discomfort and pain, and physiological parameters, namely, heart rate (HR), blood pressure (BP), oxygen saturation (SpO2), skin temperature, and calf girth changes across 6-h of simulated prolonged restrained sitting was undertaken among 15 healthy male non aircrew volunteers with mean age of 31.2 ± 3.63 years. Subjective discomfort was assessed by Category Partitioning Scale (CPS) and pain by Short-Form McGill Pain Questionnaire (SF-MPQ). Results: Prolonged restraint sitting of 6-h resulted in a significant increase in calf girth. Significant changes were also observed in CPS and SF-MPQ scores indicating an increase perception of discomfort and pain. Changes in HR, MAP, SpO2, and lower limb skin temperature were also observed even though these changes were lower in magnitude. Conclusion: The pain and discomfort following 6-h of prolonged restraint sitting were found to be appreciable by the subjects. This, along with a significant change in the calf circumference and other physiological parameters were possibly due to effects of venous pooling as a result of sitting for prolonged periods. The effects so observed could have potential flight safety implications and affect mission effectiveness. However, these findings need to be examined in actual cockpit conditions where the seat is more ergonomically designed; yet the aircrew is subjected to a wide spectrum of additional aeromedical stressors.

The objective of this prospective clinical study was to analyze the pain (intensity, location and type) that patients presented after the placement of different types of orthodontic appliances: conventional, low friction, lingual and aligners. The sample consisted of 120 patients divided into four groups: conventional (CON), low friction (LF), lingual (LO) and aligners (INV). The participants were given the Short-Form McGill Pain Questionnaire (Ortho-SF-MPQ), where they had to record the pain intensity (no pain, mild, moderate or intense) and the periodontal location at different time points, from the first 4 h to 7 days after the start of treatment. In all the study groups, the most frequent location was both anterior arches, with maximum values between 56.7% (CON group at 24 h) and 30% (LO group at 4 h). The “whole mouth” and “complete lower arch” locations were indicated only by the patients in the lingual group. Regarding pain intensity, the patients reported a higher percentage of mild–moderate pain during the first 3 days of treatment (96.7% in LO at 4 h, 86.7% in CON, 83.3% in LF and 90% in INV at 24 h); later, the reported pain decreased to no pain/mild pain, especially in the lingual group, until reaching values close to zero at 7 days post-treatment. The most frequent type of pain was acute in the low friction and lingual groups (with maxima of 60% and 46.7% at 24 h, respectively). On the contrary, in the conventional (36.7% at 4 h) and Invisalign (40% at 24 h) groups, the sensitive type was the most frequent. There are differences regarding periodontal pain in its intensity, location and type according to the use of different orthodontic techniques.

Purpose. The primary aim of the study was to review the existing literature about patient-reported outcome measures (PROMs) in colorectal cancer and IBD. The secondary aim was to present a road map to develop a core outcome set via opinion gathering using social media. Method. This study is the first step of a three-step project aimed at constructing simple, applicable PROMs in colorectal surgery. This article was written in a collaborative manner with authors invited both through Twitter via the #OpenSourceResearch hashtag. The 5 most used PROMs were presented and discussed as slides/images on Twitter. Inputs from a wide spectrum of participants including researchers, surgeons, physicians, nurses, patients, and patients’ organizations were collected and analyzed. The final draft was emailed to all contributors and 6 patients’ representatives for proofreading and approval. Results. Five PROM sets were identified and discussed: EORTC QLQ-CR29, IBDQ short health questionnaire, EORTC QLQ-C30, ED-Q5-5L, and Short Form-36. There were 315 tweets posted by 50 tweeters with 1458 retweets. Awareness about PROMs was generally limited. The general psycho-physical well-being score (GPP) was suggested and discussed, and then a survey was conducted in which more than 2/3 of voters agreed that GPP covers the most important aspects in PROMs. Conclusion. Despite the limitations of this exploratory study, it offered a new method to conduct clinical research with opportunity to engage patients. The general psycho-physical well-being score suggested as simple, applicable PROMs to be eventually combined procedure-specific, disease-specific, or symptom-specific PROMs if needed.

Objective. PainVision device was a developed application for the evaluation of pain intensity. The objective was to assess the efficacy and safety of pulsed radiofrequency (PRF) combined with pharmacological therapy in the treatment of postherpetic neuralgia (PHN). We also discussed the correlation of the measurements.Method. Forty patients with PHN were randomized for treatment with PRF combined with pharmacological therapy (PRF group,n=20) or pharmacological therapy (control group,n=20) at postoperative 48 hours. The efficacy measure was pain degree (PD) that was assessed by PainVision and visual analog scale (VAS), short form Mcgill pain questionnaire (SF-Mcgill), and numeric rate scale sleep interference score (NRSSIS). Correlations between PD, VAS, SF-Mcgill, and NRSSIS were determined.Results. The PD for persistent pain (PP) and breakthrough pain (BTP) at postoperative 48 hours assessed by PainVision were significantly lower in PRF group than in control group (PD-PP,P<0.01; PD-BTP,P<0.01). PD and VAS were highly correlated for both persistent pain (r=0.453,ρ=0.008) and breakthrough pain (r=0.64,ρ=0.001).Conclusion. PRF was well tolerated and superior to isolated pharmacological therapy in the treatment of PHN. PainVision device showed great value in the evaluation of pain intensity and PD had an excellent correlation with VAS and SF-Mcgill.

AbstractThe aim of study was to assess the skin temperature changes over selected muscles zones of the lower limbs following an exercise performed at similar relative external workload in 10 male and 6 female high-trained cross-country skiers. The first stage of experiment involved preliminary exercise to determine individual oxygen uptake VO2max and anaerobic threshold. The second stage experimental exercise consisted of 60 min running on treadmill with 80% of determined VO2max Thermographic imaging of lower limbs was performed, before, immediately after exercise and during recovery. Physiological parameters: VO2, MET—metabolic energy equivalent, heart rate and internal temperature were assessed at rest and at the end of exercise, respectively. Infrared thermography showed that at rest the skin temperature over lower limb muscles was significantly higher in men than in women. In response to exercise an increase in skin temperature over the studied muscles was significantly higher in women than men. Since was no significant difference in skin temperature in men and women after exercise. Before the test, no physiological parameter was significantly different in women and men. Exercise test reveled significant differences between men and women in some physiological parameters such as VO2 and MET. Our study showed that there are significant differences in lower limbs skin temperature between male and female at rest but not at the end of exercise test. Women in comparison with men had a greater increase in skin temperature in response to exercise and a persistence of elevated temperature over muscles of lower limbs after exercise.

Background: Homeless people are known to suffer disproportionately with health problems that reduce physical functioning and quality of life, and shorten life expectancy. They suffer from a wide range of diseases that are known to be painful, but little information is available about the nature and prevalence of chronic pain in this vulnerable group. This study aimed to estimate the prevalence of chronic pain among homeless people, and to examine its location, effect on activities of daily living, and relationship with alcohol and drugs.Methods: We conducted face-to-face interviews with users of homeless shelters in four major cities in the United Kingdom, in the winters of 2009-11. Participants completed the Brief Pain Inventory, Short Form McGill Pain questionnaire, Leeds Assessment of Neuropathic Symptoms and Signs, and detailed their intake of prescribed and unprescribed medications and alcohol. We also recorded each participant’s reasons for homelessness, and whether they slept rough or in shelters.Findings: Of 168 shelter users approached, 150 (89.3%) participated: 93 participants (63%) reported experiencing pain lasting longer than three months; the mean duration of pain experienced was 82.2 months. The lower limbs were most frequently affected. Opioids appeared to afford a degree of analgesia for some, but whilst many reported symptoms suggestive of neuropathic pain, very few were taking anti-neuropathic drugs.Interpretation: The prevalence of chronic pain in the homeless appears to be substantially higher than the general population, is poorly controlled, and adversely affects general activity, walking and sleeping. It is hard to discern whether chronic pain is a cause or effect of homelessness, or both. Pain is a symptom, but in this challenging group it might not always be possible to treat the underlying cause. Exploring the diagnosis and treatment of neuropathic pain may offer a means of improving the quality of these vulnerable people’s lives.

Electrotherapy is commonly used for myalgia alleviation. Low-frequency stimulation (LFS) is primarily used for controlling acute and chronic pain and is a non-invasive therapy that can be easily performed with electric stimulation applied on the skin. However, little evidence exists regarding the pain alleviation effects of personal low-frequency stimulation device for home use. Moreover, no studies have compared myalgia alleviation effects between personal low-frequency stimulation (PLS) and physical therapy (PT), which are most commonly used for patients with myalgia in hospitals and clinics. Therefore, we aimed to investigate the pain alleviation effects of PLS in patients with myalgia and compare these effects with those of conventional PT (transcutaneous electrical nerve stimulation + ultrasound). In total, 39 patients with myalgia in the neck, shoulder, back, and waist areas were randomly assigned to the personal low-frequency stimulation group (PLSG: n = 20) and physical therapy group (PTG: n = 19). Both groups were treated for 3 weeks (20 min per session and 5 sessions per week). Patients were assessed for pain intensity by surface electromyography (sEMG), visual analogue scale (VAS) and a short-form McGill pain questionnaire (SF-MPQ) before and after the intervention period. Our results showed that PLSG showed a tendency of muscle relaxation with a significant decrease in sEMG in the neck (p = 0.0425), shoulder (p = 0.0425), and back (p = 0.0046) areas compared to the control group. However, there was no significant difference in waist area. Additionally, VAS scores significantly decreased between pre- and post-treatment in both PTG (p = 0.0098), and PLSG (p = 0.0304) groups, but there was no significance difference between the groups. With respect to SF-MPQ, the PLSG showed greater pain alleviation (5.23 ± 0.25) effects than the PTG (6.23 ± 0.25). Accordingly, our results suggest that PLS treatment using a home device might offer positive assistance in pain alleviation for patients with myalgia that is as equally effective as conventional PT treatment. However, further detailed studies are required considering larger samples to fully claim the effectiveness of this device.
