Objective: To study the effects of Shensong Yangxin capsules and amiodarone on heart rate variability indexes and inflammatory factors in the treatment of patients with arrhythmia. Methods: The research subjects were selected patients with arrhythmia admitted in our hospital's department from October 2018 to October 2019, and a total of 70 patients were divided into two groups. 35 subjects in the control group were treated with amiodarone alone, and 35 subjects in the observation group were treated with Shensongyangxin capsule in combination with amiodarone for comparisons in treatment effects, heart rate variability indices, inflammatory factors, and adverse reactions. Results: After observation, the treatment efficiency in the observation group was higher than that in the control group (P<0.05). Before treatment, there was no difference in heart rate variability indices between the two groups (P>0.05), after treatment, the improvement in patients of the observation group was better than that of the control group (P<0.05). After treatment, the inflammatory factors of the two groups of patients decreased, and the indicators of the observation group were comparably lower than those of the control group (P<0.05). In the comparison of adverse reactions, the observation group showed a lower incidence of adverse reactions after treatment than the control group (P<0.05). Conclusion: In the treatment of patients with arrhythmia, the combined use of Shensong Yangxin capsules and amiodarone has more significant therapeutic effects, which can effectively improve the patient's heart rate variability indices, reduce the inflammatory factors, and relieve the symptoms in patients. The incidence of adverse reactions is low and the safety is quite high, therefore it has great value in clinical applications.

Objective: This study aims to compare the clinical application value of high-flux dialysis with low-flux dialysis in patients without significantly improved renal function after cervical cancer and obstructive renal failure catheterisation. Methods: This prospective randomised study was conducted from January 2018 to December 2019. Eighty cervical cancer patients with obstructive renal failure who showed no significant renal function improvement after catheterisation were randomised into two groups (n = 40 in each group) in the Second People’s Hospital of Yibin City. High-flux and low-flux dialysis were employed in the experimental group and the control group, respectively. Treatments in both groups were provided every other day, with the whole course lasting one week. Data were recorded before and after dialysis included inflammatory factors such as IL-6, CRP and TNF-a, large and moderate molecular toxins (e.g., β2 micro-globulin, parathyrin (PTH) and cysteine protease inhibitor). Renal function changes during the dialysis were also recorded. Afterwards, the two groups were compared regarding the overall efficacy. Results: Both the experimental group and the control group experienced a significant decrease in IL-6, CRP, TNF-a, β2 micro-globulin, PTH and cysteine protease inhibitor, with the decrease in the experimental group being more evident (p < 0.05). After dialysis was completed, the experimental group restored renal function indicators such as Cre, CysC and serum K+ levels more quickly than the control group (p < 0.05). The effective rate was 100% for the experimental group and 87.5% for the control group. The intragroup difference in the efficacy.was significant. Conclusions: High-flux dialysis appears to be more beneficial for cervical cancer patients with obstructive renal failure, showing no significant improvement in renal function after catheterisation. It restored renal function more quickly, had more radical draining of inflammatory factors and large and moderate molecular toxins, and had a higher overall effective rate. doi: https://doi.org/10.12669/pjms.37.4.3515 How to cite this:Zhang C, Xie DQ, Ao L, Zhu L. Objective. This study was designed to probe into the changes and clinical significance of GRP78 and miR-495-3p in renal failure (RF) patients during high-flux dialysis (HFD) combined with hemoperfusion (HP). Methods. Sixty-five RF patients and 74 health check-ups who were admitted in our hospital from March 2015 to February 2017 were prospectively selected, and the related characteristics were retrospectively collected for analysis. GRP78 and miR-495-3p were detected in RF patients at admission (before treatment), 12 weeks after treatment (during treatment), 24 weeks after treatment (after treatment), and the control group at admission, and the relationship between the two and the occurrence, efficacy, and recurrence of RF was analyzed. Results. Before treatment, the GRP78 mRNA level in RF patients was higher than that in health check-ups, while the miR-495-3p level was lower ( P < 0.05 ). GRP78 mRNA in RF patients was lower than that before treatment and was the lowest after treatment. On the contrary, miR-495-3p was higher than that before treatment and was the highest after treatment ( P < 0.05 ). The two had a significant effect on predicting RF before treatment, efficacy of patients, and their recurrence after treatment (all P < 0.001 ). Conclusion. GRP78 decreased during the treatment of high-flux hemodialysis (HF-HD) combined with systemic HP in RF patients, while miR-495-3p increased. Both of them have a good reference value for RF occurrence, treatment results, and recurrence.

This study is to investigate the effect of atorvastatin combined with losartan on inflammatory factors, vascular endothelial function, and cardiovascular events in patients with diabetic nephropathy. A total of 128 patients with diabetic nephropathy treated in our hospital from January 2014 to December 2015 were selected as the study subjects, and 64 cases were randomly divided into observation group and 64 cases in the control group. The control group was treated with losartan on the basis of routine treatment, and the observation group was treated with atorvastatin on the basis of the control group. The blood lipid, inflammatory factors, changes in vascular endothelial function and cardiovascular events were compared between the two groups. The levels of total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) were not significantly different between the two groups before treatment ( P > 0.05); after treatment, the levels of TC, TG, and LDL-C in the observation group were significantly lower than those of the control group, and the level of HDL-C was significantly higher than that of the control group ( P < 0.05). The levels of high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor alpha (TNF-α), and interleukin 6 (IL-6) were not statistically different between the two groups before treatment ( P > 0.05); after treatment, the levels of hs-CRP, TNF-α, and IL-6 in the observation group were significantly lower than those of the control group ( P < 0.05), the level of HDL-C was significantly higher than that of the control group ( P < 0.05). There were no significant differences in the levels of endothelin-1 (ET-1) and nitric oxide (NO) between the two groups before treatment ( P > 0.05). After treatment, the level of ET-1 in the observation group was significantly lower than that of the control group ( P < 0.05), and the level of NO was significantly higher than that of the control group ( P < 0.05). After treatment, all patients were followed up for 2 years, and the incidence of secondary cardiovascular events in the observation group was 12.50% (8/64), which was significantly lower than 29.69% (19/64) of the control group ( P < 0.05). Combination of atorvastatin and losartan can significantly improve the levels of blood lipid, inflammatory factors, and vascular endothelial function in patients with diabetic nephropathy and can effectively reduce the incidence of cardiovascular events.

Abstract Objectives Malnutrition is the most common complication of patients with esophageal cancer, and can lead to poor prognosis and death. It has been shown that good nutritional status can help improve patient outcomes and reduce complications. In the absence of specific evidence of the effect of nutrition on patients with esophageal cancer, the purpose of this study was to investigate the effect of total nutrition management on the prognosis and complications of patients with esophageal cancer through a randomized controlled trial. Methods 96 patients with esophageal cancer concurrent chemoradiation were randomized to observation group (treated by whole-nutrition management from the Nutrition Support Group (NST)) and control group (treated by the general nutritional method) for 6 weeks approximately. Dietary survey, Patient-Generated Subjective Global Assessment (PG-SGA), body measurement, blood index, quality of life survey and psychological condition survey were assessed at baseline and every week/at the end of the study. Complications, the completion rates of therapy, short-term efficacy evaluation as well as clinical outcomes were measured before discharge. Results A total of 85 subjects completed the study (observation group = 45, control group = 40). There were significantly different in the changes in serum albumin, prealbumin, and total protein between two groups throughout the trial (P < 0.05). Radioactive esophagitis, skin symptom of the complications, the roles, emotional functions, social functions, and general health status in the quality of life were statistically different before and after the intervention (P < 0.05). The difference in the change of other indicators was not statistically significant. Conclusions Whole-course nutrition management can improve the nutritional status of patients with concurrent chemoradiotherapy of esophageal cancer, reduce the severity of radiation esophagitis and radiation skin reactions, improve the quality of life and relieve their depressive symptoms.
