Health Services at the PKU Muhammadiyah Gandrungmangu Clinic have not made much use of information technology. Documentation of patient medical records still uses a manual recording system, causing various obstacles; for example, officers often find the same medical record number and difficulties making reports. Thus, creating a medical record information system is necessary to help manage medical record data electronically. The system was developed using the prototyping method. This system has several features: medical record management, user management, reporting system, input checking feature for vital sign results, uploading feature for supporting examination results, and patient queuing system for each poly. The results of the user convenience test using the SEQ method obtained an average value of 6 and 7. The usability test results using the SUS method also got a good response from the questionnaire questions given to the respondents, which got a total score of 90.6, meaning that the system has an excellent usability level.

Aims: Based on the observations of researchers, some health facilities still use manual processes / have not been documented by the information system resulting in slow service, this study aims to improve health services with a medical record information system. Methodology: The method used in this study is an analysis of information systems with the waterfall method and accuracy testing with ISO 9126. Results: The results of this medical record management information system run well based on black-box testing and white box results obtained both from an average value of 82 based on the ISO 9126 scale conversion table. Conclusion: Based on the results of the average value obtained from the validation test carried out on 3 expert examiners, it can be concluded that the application for the validation system for the validation and distribution of this letter has met the ISO 9126 standard with an average good interpretation of a total value of 82, and according to be able to simplify the process of Patient Medical Record Data Management without neglecting the safety aspects of the validation and distribution process, minimizing data loss, simplifying the reporting process and facilitating the processing of patient medical record data.

Abstrak Rekam medis adalah berisikan cactatan dan dokumen tentang idetitas pasien , pemeriksaan, pengobatan, tindakan, dan pelayanan lain yang telah diberikan kepada pasien. Rekam medis adalah keteranan baik yang tertulis maupun yang terekam tentang identitas, anamases, pemeriksaan laboratorium, diagnosa, segala pelyanan dan tindakan medik yang di berikan kepada pasien. Penomoran berkas rekam medis yang baik merupakan salah satu kunci keberhsilan atau kebaikan suatu manajemen rekam medis dari suatu pelayanan kesehatan, tentunya didukung dengan sistem yang baik. Tujuan dari penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi duplikasi penomoran rekam medis di Rumah Sakit Advent Medan. Jenis penelitian ini adalah deskriptif kuantitatif dengan menggunakan metode cross sectional yang dilakukan di Rumah Sakit Advent Medan pada bulan juli 2018, Populasi dalam penelitian ini adalah 11 petugas rekam medis yang terdiri dari 7 orang petugas pendaftaran dan rawat jalan 4 orang petugas pendaftaran rawat inap, Sampel dalam penelitian ini Menggunakan  total sampel, instumen penelitian yang digunakan observasi dan kuisoner,  berdasarkan hasil penelitan yang dilakukan faktor yang mempengaruhi duplikasi nomor rekam medis mayoritas Pengetahuan diperoleh 54,5% dari 11 petugas rekam medis dan dengan hasil uji statistik diperoleh  p=0.015<0.05. sehingga Ho ditolak artinya adanya pengaruh antara pengetahuan petugas rekam medis dengan duplikasi penomoran di Rumah Sakit Advent Medan. Minoritas sikap diperoleh 63,6% dan dengan hasil uji statistik diperoleh p=0,545%<0.05 sehingga Ho diterima artinya tidak ada pengaruh antara sikap dengan duplikasi penomoran. Dari hasil uji    statistik antara pengetahuan dan sikap, terdapat pengaruh pengetahuan dengan duplikasi penomoaran di Rumah Sakit Advent, Sedangkan sikap tidak ada pengaruh dengan duplikasi penomoran di Rumah Sakit Advent. Diharapkan kepada Rumah Sakit untuk memberikan evaluasi atau pelatihan kepada petugas rekam medis dibagian pendaftaran secara berkesinambungan. Kata Kunci: Rekam medis, Faktor- faktor duplikasi No rekam medis, Sistem     penomoran Abstrack Medical record is containing records and documents about patient identity, examination, treatment, actions, and other services that have been provided to patients. Medical records are written and recorded safety regarding identities, anamases, laboratory tests, diagnoses, all services and medical actions given to patients. Numbering a good medical record file is one of the keys to the success or goodness of a medical record management of a health service, of course, supported by a good system. The purpose of this study was to determine the factors that influence the duplication of medical record numbering in Medan Adventist Hospital. This type of research is quantitative descriptive using a cross sectional method conducted at Medan Adventist Hospital in July 2018, the population in this study were 11 medical record officers consisting of 7 registration and outpatient officers 4 inpatient registration officers, samples in this study Using the total sample, the research instrument used observation and questionnaire, based on the results of research conducted by factors that influence the duplication of the majority medical record number . so Ho is rejected, meaning that there is an influence between the knowledge of the medical record officer with the numbering duplication in Medan Adventist Hospital. Minority attitudes were obtained 63.6% and with statistical test results obtained p = 0.545% <0.05 so that Ho was accepted meaning that there was no influence between attitude and numbering duplication. From the results of statistical tests between knowledge and attitude, there is an influence of knowledge with duplication of numbers in Adventist Hospitals, whereas attitude has no influence with duplication of numbers in Adventist Hospitals. It is expected that hospitals will provide evaluation or training to medical records officers in the registration section on an ongoing basis.

Nagaswidak Health Center is one of the community health centers that is quite large and has complete facilities. But the problem that is often faced by officers in the puskesmas is the medical record data processing system which is still manual, causing the accumulation of patient medical record file data, in addition to patients who have been checked before and lost their medical records, it is very difficult for officers to find back, so the officer made a new medical record data. To overcome this problem, a Medical Records Filling Application was made at the Nagaswidak Health Center which includes the processing of medical records, patient data, drug data, action data, doctor data, and admin logins. So that the data search problem is not difficult, the turbo boyer moore algorithm method is applied which is expected to later be able to facilitate the search for patient data in the medical record filling application. Based on the test results Boyer Moore's Algorithm successfully applied to search for the beginning of a word, middle word, and final word. And the level of ease and usefulness of medical records application using Boyer Moore's algorithm obtained results that the level of ease is 80% and 100% usability rate.

Total information and communication systems within hospitals have been designed, but successful complete implementation, to date, has not been achieved. Limited applications with both patient medical data, notably in the clinical laboratories, and in the hospital accounting offices have been numerous. Although total programs are not yet a reality, it is apparent that the computer will serve ultimately many communication requirements, both medical and financial, within the hospital.Sound hospital management requires that costs of all component operations be known in order that value judgments concerning worth and efficiency may be made. Accrual accounting systems which match revenue and expense over the same time period are a prerequisite. Cash and modified cash hospital accounting cannot provide current reliable data for sound decision making.Costs of hospital operations cannot be evaluated unless related to the characteristics of the patient service load. Average per diem costs mean little except when large similar populations of patients are being compared. A modern hospital accrual accounting system should be able to provide information concerning the costs of caring for specific diseases in patients with known age and sex and disease severity characteristics. Without information of this type, it will not be possible to objectively evaluate alternative systems of financing and organizing patient care.Medical record management offers the promise of prospective use of patient disease information in the planning and scheduling of facilities. The prose content of medical record summaries, such as diagnostic statements in tissue pathology, radiology, and admission and discharge diagnoses, may be susceptible to non-coded, full prose input into computer controlled diagnostic files. Thesauri in the several medical specialties will be necessary for this achievement.There is little immediate prospect for complete hospital communication systems that can be made available as a package to any hospital without substantial local alteration. Pilot projects in teaching centers should be viewed for the time being as opportunities to define objectives, evaluate feasibility, and determine degree of risk and expense.A brief survey of applications in the United States which have been successfully implemented or which appear suitable for successful implementation is recorded.Eleven general principles which have been associated with successful implementation of computer applications within the UCLA Hospital are enumerated.

Background. Cases of sexual violence increase every year, victims ranging from adolescents, children to toddlers. Based on data from the Indonesian Child Protection Commission, abuse and violence against children in Indonesia in 2013 were 23 cases, in 2014 there were 53 cases, in 2015 there were 133 cases, 2017 reached 1,337 cases, and as of July 2018 there were 424 cases. Purpose. Knowing the factors that influence the law enforcement process of sexy violence cases in Semarang City. Method This study uses descriptive analytical methods for cases of violence against children, based on medical record data in hospitals, documents in Mapolrestabes, the District Attorney's Office and the Semarang City Court for the period of January 2015 to December 2018. Results. Based on research results obtained 213 experimental cases section from medical record data in hospitals in the city of Semarang. Most cases of child abuse occurred in 2018 with 72 cases. Most victims are 12-14 years old age group, female. Most types of cases are cases of intercourse. The majority of violations are persons known as victims, perpetrators not working, and most of the places of occurrence are in the defendant's house. At the time of prosecution and trial, the number of cases was significantly reduced to only 8 cases. Factors related to this include lack of evidence, difficulty in obtaining information from victims, convoluted statements of coverage, lack of election, and obtaining diversion rates. Conclusion Cases of sexual violence have increased from year to year. The process of law enforcement on this problem still has many difficulties in each manufacturing process which is still difficult to overcome.

This research is motivated by the low desire of students in writing travel reports. The lack of students' knowledge of the report concept, the lack of students' knowledge of the 5W + 1H report points of good and correct language, the lack of students' knowledge of the spatial, time and topic pattern and not yet reached KKM 75. The method used by the teacher has not been interesting, lecture method. The purpose of this study is to describe the skills of writing travel reports by using Round Club learning model which is viewed from the aspect of understanding the report concept, the use of 5W +1H report points, the spatial, time, and topic pattern.The population of this study is the students of class V Lessons Year 2017/2018 which amounted to 2 classes with the number 80. The sample of research as much as two classes taken by the sample of propotional.Class V.1 as experimental class and class V.2 as control class. The research instrument used is performance test. Provide an assessment by specifying the subject of the 5W + 1H report, as well as the spatial, time and topic pattern. Data were analyzed by 't' test by first testing normality, homogeneity, and hypothesis testing.The results showed that the average control class 68 with more than enough qualifications with standard deviation 16.96. 83 experimental class with good qualification and standard deviation of 15.42 and there is a significant influence on the result of writing skill of class V SDN 01 Nagari Bukik SikumpaSubdistrict, Lima Puluh Kota. This is evidenced by the average value of writing skills in the experiment class higher than the average value in the control class. Normality test results indicate that the two sample classes of  Lo  values in the control class -0.2141 are smaller than the normal 0.190 Lt distributed. Homogeneity test results that the variation of this study is homogeneous at a real level of 0.05, because Ftable 2.16 > Fhitung 1.21 and the results of data analysis then obtained = 2.78 > 1.70 t table, so H0 rejected and H1 accepted. It can be concluded that there is Influence.
