ASSORTMENT ANALYSIS OF MEDICAL DEVICES FOR MONITORING THE ACTIVITY OF THE CARDIOVASCULAR SYSTEM IN AN OUTPATIENT SETTING

atients with cardiovascular diseases need systematic monitoring of the main parameters of the cardiovascular system on an outpatient basis using special medical devices. The aim of the study is to conduct an assortment analysis of medical devices intended for use at home by patients with cardiovascular diseases. Materials and methods: The objects of the study were clinical recommendations for the treatment of diseases of the cardiovascular system (12); State Register of Medical Devices of the Russian Federation; Internet sites of pharmacy chains of the Republic of Tatarstan (5), sites of Internet pharmacies (1); data of the reference service “003” State Unitary Enterprise “Medical equipment and pharmacy of Tatarstan”. The work uses the methods of content analysis, logical, structural, graphic analysis. Price segmentation was carried out using the Sturgess grouping method. Results. An analysis of clinical guidelines for the prevention and treatment of diseases of the cardiovascular system was carried out to identify the main control parameters required on an outpatient basis. The groups of medical devices used by patients on their own have been determined: tonometers, pulse oximeters and diagnostic tests. The analysis of the State Register of Medical Products and Pharmacy Assortment of the Republic of Tatarstan was carried out in order to identify the availability of medical products to the population. Price segmentation of the studied groups of medical devices was carried out in order to determine consumer opportunities; the indicators of the completeness of the assortment were calculated. A wide range of tonometer models is presented in pharmacies of the Republic of Tatarstan. More than 60% of mechanical and automatic blood pressure monitors are in the low segment; the indicator of the completeness of the assortment Kp = 0.83, which allows satisfying the needs of patients. 50% of the assortment group “pulse oximeters” sold from pharmacies belongs to the low price segment; assortment completeness indicator Kp = 0.29. Diagnostic tests implemented in the Republic of Tatarstan are represented by express tests for the determination of ketones in blood and urine; assortment completeness indicator Kp = 0.31. The price segmentation of test systems is largely determined by the need to use additional instruments (analyzers). 75% of the total number of express tests presented on the pharmaceutical market of the Republic of Tatarstan belong to the middle price segment. Conclusions. As of March 2021, 950 medical devices used in the diagnosis and treatment of cardiovascular diseases were registered on the domestic market. At the same time, 22.6% can be used at home and are sold to the population by pharmacy organizations. For self-monitoring of indicators of the cardiovascular system on an outpatient basis, tonometers, test systems and heart rate monitors are used. Their share of the total number of registered medical devices used in the diagnosis and treatment of cardiovascular diseases is 17%. The price segmentation of the studied groups of medical devices indicates the availability of their purchase by customers with high and medium paying capacity. The prospects for expanding the range of medical products for independent use by patients with cardiovascular diseases in the regional pharmaceutical market have been identified.

Aim. Marketing research in the segment of herbal medicines for the treatment of the diseases of the cardiovascular system in the Republic of Belarus. Methods. The study of the healthcare market was carried out using analytical and statistical methods by analyzing data from the State Register of Medicines of the Republic of Belarus (as of 01.01.2020) and the database of sales of medicines and biologically active food additives «IQVIA» (data on the sale of medicines for 2019). Data processing was carried out using the Microsoft Office Excel 2016 software package. Results. As of 01.01.2020, 51 herbal medicines from group C (according to the anatomical-therapeutic-chemical classification «Cardiovascular system») were registered in the Republic of Belarus by 29 manufacturers from 10 countries. Most herbal medicines for treatment of cardiovascular diseases are imported (54.90%). According to originality criterion generic herbal medicines from this segment prevail, both in terms of the number of registered names (78.43%) and sales volumes in cash (53.86%) and in kind (96.49%). In terms of composition, monocomponent herbal medicines prevail (56.86%). Among monocomponent medicines the majority of herbal medicines for the treatment of cardiovascular diseases are domestic (31.37% of the total number of registered herbal medicines for the treatment of cardiovascular diseases), while among the combined ones the greater part is imported (33.33%). In terms of sales in kind, monocomponent herbal medicines prevail (74.98%), whereas in monetary terms combined ones predominate (40.65%). Conclusion. It is a promising perspective for domestic enterprises to develop combined herbal medicines for the treatment of cardiovascular diseases in order to provide the population with more affordable medicines from this segment, as well as to implement the principles of import substitution in the healthcare market of the Republic of Belarus.

Introduction: Cardiovascular diseases are the leading cause of death both in Serbia and in the rest of the world. It has been shown that as many as 80% of them are preventable. Control of serum lipid levels is one of the most important tasks of cardiovascular diseases prevention. Aim: The aim of the study was to analyze the use of serum lipid-modifying drugs in Serbia, Norway and Finland in the period 2015-2017. Methods: Data on drugs use during 2015, 2016 and 2017 were taken from the official websites of national drug regulatory authorities: the Serbian Medicines and Medical Devices Agency, the Norwegian Institute of Public Health and the Finnish Medicines Agency. Use was expressed as DDD/1000 inhabitants/day according to the Anatomical Therapeutic Chemical classification. Results: The share of drugs used for treatment of cardiovascular diseases in total drugs use was the largest in all three countries during the observed period. The use of lipidmodifying agents was 3-4 times lower in Serbia than in Norway or Finland. Of all lipidmodifying drugs, statins are most commonly prescribed in all three countries. Atorvastatin and rosuvastatin are the most widely used in Serbia, and simvastatin and atorvastatin in Norway and Finland. Conclusions: Use of lipid-modifying drugs in Serbia is lower than in Norway and Finland, but it is constantly increasing. This use in Serbia still represents the smallest share of all drugs for the treatment of cardiovascular diseases.

The purpose of this article was to analyze the pharmaceutical market of the tissue sealing agents group of the Republic of Belarus from 2010 to 2018 based on the data presented in the Medmarket database and the State Register of Medicines of the Republic of Belarus of UE «Center of Expertise and Testing in Healthcare». The group of tissue sealing agents (B02F) was found to be characterized by high growth rates and a relatively large market share. During the studied period, it was represented on the Belarusian pharmaceutical market by only one drug - Tachocomb (manufactured by Takeda Pharmaceutical Company Limited, Japan). When analyzing its dynamics in the period from 2010 to 2018 it was found that, in general, the change in the volume of the market for tissue sealing agents correlates with the change in the volume of the total market for hemostatic drugs over the years. During 2010-2018 there is an increase in the volume of sales for this local sealant, both in monetary (by 1.51) and in natural (by 2.01) expressions with a gradual fall of the price. It has been concluded that the development of the tissue sealing agents by domestic manufacturers is urgent and expedient.

Результаты исследования представлены в виде диаграммы и списка. Также приведены цены на глюкометры и тест-полоски, зарегистрированные в Казахстане и популярные по запросу. В связи с тем, что в настоящее время в нашей стране растет число людей, страдающих сахарным диабетом, проверка уровня сахара в крови является обязательной, но не всем больным доступны зарубежные глюкометры и тест-полоски для них, поэтому стране необходимо уделять больше внимания отечественному производству This article presents the results of a marketing study of the pharmaceutical market of the Republic of Kazakhstan on medical products, including glucose meters and test strips. The results of the study are presented in the form of a diagram and a list. Prices for blood glucose meters and test strips registered in Kazakhstan and popular on request are also shown. Due to the fact that currently the number of people suffering from diabetes is growing in our country, checking the blood sugar level is mandatory, but not all patients have access to foreign blood glucose meters and test strips for them, so the country needs to pay more attention to domestic production.

Abstract In the elderly population, pathological inflammation has been associated with ageing-associated diseases. The term ‘inflammageing’, which was used for the first time by Franceschi and co-workers in 2000, is associated with the chronic, low-grade, subclinical inflammatory processes coupled to biological ageing. The source of these inflammatory processes is debated. The senescence-associated secretory phenotype (SASP) has been proposed as the main origin of inflammageing. The SASP is characterised by the release of inflammatory cytokines, elevated activation of the NLRP3 inflammasome, altered regulation of acetylcholine (ACh) nicotinic receptors, and abnormal NAD+ metabolism. Therefore, SASP may be ‘druggable’ by small molecule therapeutics targeting those emerging molecular targets. It has been shown that inflammageing is a hallmark of various cardiovascular diseases, including atherosclerosis, hypertension, and adverse cardiac remodelling. Therefore, the pathomechanism involving SASP activation via the NLRP3 inflammasome; modulation of NLRP3 via α7 nicotinic ACh receptors; and modulation by senolytics targeting other proteins have gained a lot of interest within cardiovascular research and drug development communities. In this review, which offers a unique view from both clinical and preclinical target-based drug discovery perspectives, we have focused on cardiovascular inflammageing and its molecular mechanisms. We have outlined the mechanistic links between inflammageing, SASP, interleukin (IL)-1β, NLRP3 inflammasome, nicotinic ACh receptors, and molecular targets of senolytic drugs in the context of cardiovascular diseases. We have addressed the ‘druggability’ of NLRP3 and nicotinic α7 receptors by small molecules, as these proteins represent novel and exciting targets for therapeutic interventions targeting inflammageing in the cardiovascular system and beyond.

Cardiovascular diseases (CVD) may be mediated through increases in the cardiovascular risk factors. Hemoglobin A1c (HbA1c) also called glycated hemoglobin is presently used for the diagnosis and management of diabetes. It has adverse effects on cardiovascular system. This review deals with its synthesis and effects on the cardiovascular system. The serum levels of HbA1c have been reported to be affected by various factors including, the lifespan of erythrocytes, factors affecting erythropoiesis, agents interfering glycation of Hb, destruction of erythrocytes, drugs that shift the formation of Hb, statins, and drugs interfering the HbA1c assay. Levels of HbA1c are positively correlated with serum glucose and advanced glycation end products ( AGE), but no correlation between AGE and serum glucose. AGE cannot replace HbA1c for the diagnosis and management of diabetes because there is no correlation of AGE with serum glucose, and because the half-life of protein with which glucose combines is only 14-20 days as compared to erythrocytes which have a half-life of 90-120 days. HbA1c is positively associated with CVD such as the carotid and coronary artery atherosclerosis, ischemic heart disease, ischemic stroke and hypertension.HbA1c induces dyslipidemia, hyperhomocysteinemia, and hypertension, and increases C-reactive protein, oxidative stress and blood viscosity that would contribute to the development of cardiovascular diseases. In conclusion, HbA1c serves as a useful marker for the diagnosis and management of diabetes. AGE cannot replace HbA1c in the diagnosis and management of diabetes. There is an association of HbA1c with CVD which be mediated through modulation of CVD risk factors.

AbstractResearch funded projects are often concerned with the development of proof-of-concept products. Consequently, activities related to verification and validation testing (VVT) are often not considered in depth, even though various design iterations are carried out to refine an idea. Furthermore, the introduction of additive manufacturing (AM) has facilitated, in particular, the development of bespoke medical products. End bespoke products, which will be used by relevant stakeholders (e.g. patients and clinicians) are fabricated with the same manufacturing technologies used during prototyping. As a result, the detailed design stage of products fabricated by AM is much shorter. Therefore, to improve the market-readiness of bespoke medical devices, testing must be integrated within the development from an early stage, allowing better planning of resources. To address these issues, in this paper, a comprehensive VVT framework is proposed for research projects, which lack a VVT infrastructure. The framework builds up on previous studies and methods utilised in industry to enable project key experts to capture risks as early as the concept design stage.
