Violation of medical statistics as one of the reasons for the low incidence of occupational diseases

Many authors point to the low incidence of occupational diseases in our country. The reasons are poor-quality preventive examinations, the lack of interest of employers in identifying these diseases, and others. However, there are no works in the literature devoted to errors in the statistical coding of diagnoses of occupational pathology and their accounting. The study aims to improve the accounting of occupational diseases in Russia. We conducted the study using the information base of the Industry Register of Persons with Occupational Diseases, developed by Burnazyan State Medical Center of the FMBA, Russia, which operating in the industry since 2011. As of 31.12.2020, the register contains information on 2,056 patients with occupational diseases, from among the employees of enterprises and organizations served by healthcare institutions of the FMBA, Russia. Errors of incorrect coding of diagnoses for occupational diseases with violation of the classification principles in the used directive materials played a negative role. The development of plans for medical and social rehabilitation measures to reduce the incidence of occupational diseases among the people working in harmful conditions. It requires correct knowledge of morbidity indicators of this nosology. In order to improve the accounting of occupational diseases in Russia, it is necessary to switch to the system of coding diagnoses of this nosology, adopted in medical statistics, and make appropriate changes to the directive documents.

Pelayanan publik menjadi suatu tolok ukur kinerja pemerintah yang paling kasat mata. Seiring dengan perkembangan teknologi dan banyaknya tantangan yang harus dihadapi maka pelayanan publik harus diselaraskan dengan terobosan atau inovasi. Salah satu pelayanan publik yang perlu dioptimalkan adalah Sistem Administrasi Manunggal Satu Atap (SAMSAT). Oleh karena itu, Kantor Bersama Samsat Gresik membuat aplikasi yang diresmikan pada tanggal 14 Juli 2020 adalah SAMSAT RAME (Samsat Ramah Merakyat) yaitu sebuah aplikasi pelayanan pajak serta informasi terkait pengurusan perpanjangan masa berlaku surat tanda nomor kendaraan (STNK) bermotor. Tujuan penelitian ini yaitu untuk mengetahui bagaimana optimalisasi pelayanan Kantor Bersama Samsat Gresik melalui Inovasi Aplikasi Samsat Rame (Samsat Ramah Merakyat) sehingga mampu menjawab beberapa permasalahan pelayanan publik Teknik analisis data dengan pengumpulan data, reduksi data, penyajian data, dan penarikan kesimpulan/verifikasi. Teknik pengumpulan data melalui wawancara, observasi, dokumentasi dan studi literatur.  Hasil dari penelitian ini menunjukkan bahwa aplikasi Samsat Ramah Merakyat (Samsat Rame) belum efisien karena input pelayanan masih memberatkan para pengguna jasa. Belum efektif karena tidak berhasil mencapai tujuan yang dikehendaki oleh penyelenggara. Serta kualitas hasil yang buruk dikarenakan faktor pendukung terkesan membuat aplikasi Samsat Rame di lakukan karena mendesak tanpa memikirkan jangka panjang. Sedangkan faktor penghambat lebih berpengaruh pada keberhentian inovasi Samsat Rame. Salah satu saran yang dapat diajukan yaitu meningkatkan sosialisasi dan promosi kepada masyarakat Kabupaten Gresik. Along with technological developments and the many challenges that must be faced, public services must be aligned with breakthroughs or innovations. One of the public services that need to be optimized is the One-Stop One-Stop Administration System (SAMSAT). Therefore, the Samsat Gresik Joint Office made an application that was inaugurated on July 14, 2020, namely SAMSAT RAME (Samsat Ramah Merakyat), which is a tax service application as well as information related to the extension of the validity period for motorized vehicle registration certificates (STNK). The purpose of this study is to find out how to optimize the services of the Samsat Gresik Joint Office through the Samsat Rame Application Innovation (Samsat Ramah Merakyat) so that it is able to answer several public service problems. Data analysis techniques are data collection, data reduction, data presentation, and conclusion / verification. Data collection techniques through interviews, observation, documentation and literature study. The results of this study indicate that the Peoples Friendly Samsat application (Samsat Rame) is not efficient because service inputs are still burdensome for service users. Not yet effective because it failed to achieve the goals desired by the organizers. As well as the poor quality of results due to supporting factors that seem to make the Samsat Rame application done because it is urgent without thinking about the long term. While the inhibiting factor has more influence on the discontinuation of the Samsat Rame innovation. One of the suggestions that can be put forward is to increase socialization and promotion to the people of Gresik Regency. Abstract. Introduction. Personal development in competitive and adverseconditions gives us plenty of examples that show negative behavior andincapability of making peace between nations. The situation in the world todayconfirms the previous statement, Ukraine suffers more than many other countriesbecause of the powerful neighbor from the East who forces our country to accepthis conditions. The war in Eastern Ukraine is the consequence of this irrationaland atavistic tendency. We are the victims of the aggravation between twodialectic processes of social activism: disconnection (destructive situations, ATO,loss of a family member, family break up, heightened level of aggression in thecommunity) connection (constructive and creative appearances, adaptation inconditions of forced immigration, posttraumatic growth, resocialization afterovercoming trauma, tendency for reconciliation, personal growth in a newprofession).This is the reality that requires psychological science and practice to find quickand accurate answers because the goal is to save people's mental health in militaryand mobilization conditions.Purpose. The main goal of the research is to generalize the information aboutpsychological features of the people who have experienced loss. Also to spot thestages of women's post-loss resocialization and factors that have a positive impacton that resocialization.Methodology. The authors of this paper made research on the psychologicalfeatures of the people who have experienced loss due to military situation in theEast of Ukraine and who need psychotherapy to help the process ofresocialization. Conclusions. The facts stated in the paper that helped to analyze the situationwere received during work in the period of 2015-2018 at the center of psycho-social rehabilitation at National University of "Kyiv Mohyla Academy". 5370people have addressed to the center to get help during this period. 1414 of themhave gotten individual consultations, 3953 have participated in group therapies.

Abstract Objective To investigate whether the morphologic parameters of euploid blastocyst influence the live birth rate (LBR) following single frozen-thawed embryo transfer (FET) cycles? Methods A retrospective cohort analysis involving autologous single FET cycles after preimplantation genetic testing for aneuploidy (PGT-A) through next generation sequencing (NGS) by a university-based reproductive medical center that was performed from June 2017 to September 2019.Women were divided into three age groups (< 30, 30–34 and ≥ 35 years). The primary outcome measure was LBR. Outcomes were compared to determine the association between different blastocyst quality (Good, Average and Poor), inner cell mass (ICM) grade (A and B), and trophectoderm (TE) grade (A, B and C) and LBR. Results We included 232 single FET cycles for analysis, the total LBR was 48.48%. In the youngest group (< 30 years, n = 86), LBR were compared between cycles with various blastocyst quality (72.22% for good quality, 54.55% for average quality and 34.78% for poor quality; P = 0.019), ICM grade (70.59% for grade A and 42.03% for grade B; P = 0.035) and TE grade (85.71% for grade A,57.58% for grade B and 34.78 for grade C; P = 0.015). Similarly, in the 30–34 years group, LBR ranged from 50.00% for good quality to 45.45% for poor quality (P = 0.870), from 35.29% for ICM grade A to 51.22% for ICM grade B (P = 0.291), from 60.00% for TE grade A to 45.45% for TE grade C (P = 0.634). Likewise, in the oldest group (≥ 35years, n = 47), LBR were also comparable between these subgroups, but no significant differences were seen in blastocyst morphologic parameters and LBR (P > 0.05). Conclusion After single FET cycles, the LBR was associated with morphologic parameters of euploid blastocysts, especially in women < 30 years old. However, these differences were not found in women older than 30 years. We suggested that for older women whose embryos undergoing PGT-A with NGS to be euploid have the same development potential regardless of their blastocyst morphology.

The analysis of occupational morbidity at the enterprises of the machine-building industry of the region is carried out. In recent years, there has been a sharp decrease in occupational diseases at machine-building enterprises. So, in 2015, 187 cases were established, which amounted to 68.2% of all established cases, in 2020 – only 2 cases (6.3%). The level of occupational morbidity does not reflect the true situation. Incomplete identification of patients with occupational pathology is associated with the negative attitude of the employer to the establishment of occupational diseases in employees, poor-quality certification of workplaces. The necessity to routinely carry out histopathologic examinations of surgically removed polyps in patients with chronic rhinosinusitis with bilateral nasal polyps (CRSwNP) is uncertain. The aim of this study was to describe the histopathology of polyps from a large series of patients who underwent endoscopic sinus surgery for CRSwNP and the rate of neoplasms. This is a retrospective study of the histopathological result of all patients with CRSwNP who underwent functional endoscopic nasal surgery and in whom histologic examination was performed, from 2006 to 2015, in a tertiary medical center. Data on demographics and whether a nasal lesion was suspected for neoplasm prior to surgery were analyzed. Two hundred thirty-five patients underwent bilateral nasal polypectomy and histopathologic examinations of the surgical specimens. The rate of neoplasms in nonsuspicious cases was 2.3% (5 cases) and all were inverted papilloma. To conclude, inverted papilloma and other neoplasms are considered rare incidental findings in patients presenting with bilateral nasal polyps. This study demonstrated a higher rate of the latter than that reported in the literature. Our study further emphasizes that despite the low incidence of these pathologies, a histopathologic examination of every nasal polypectomy is warranted so as not to miss potential serious pathologies. We stress that a routine histopathological examination should be practiced at least in the elderly population.

The article raises the problems of the formation of an inclusive culture of teachers working with intellectually disabled children in centers of social and professional rehabilitation for the people with special psychophysical development needs (hereinafter referred to as the SPDN). Two components of an inclusive culture are presented: tolerance and inclusive competence. The mechanism of professional and social rehabilitation of people with intellectual disabilities is shown on the example of Brest region of the Republic of Belarus.

The strong connection between people and place provides fodder for literary producers to create their works. In his anthology of poems Ruang Kota , Nassury Ibrahim draws upon Kuala Lumpur as the place that allows him to delve into the study of self and other people. Using psychogeography as an analytical tool, this article examines how Kuala Lumpur affects the mental states and behaviors of the people living in the heart of the urban city. Nassury’s poems deal with many aspects of life which include poverty, pollution and humanity. Other than his experiences, the poet uses the landscape of the city to express his own perspectives and feelings towards Kuala Lumpur. Based on the analysis, Kuala Lumpur is portrayed as a city that has developed materially but its dwellers benevolence and compassion have lessened greatly. His poems highlight urban poverty and poor urban planning in Kuala Lumpur which have led to poor quality of life in this cosmopolitan city. Dalam antologi Ruang Kota , Nassury Ibrahim memberikan tumpuan terhadap Kuala Lumpur sebagai lokasi yang menyerlahkan dirinya sebagai pengkaji kendiri dan masyarakat sekeliling. Dengan menggunakan psikogeografi sebagai lensa kritikan, makalah ini mengkaji kesan pembangunan Kuala Lumpur terhadap keadaan mental dan tingkah laku masyarakat yang tinggal di kota raya ini. Puisi Nassury memaparkan pelbagai aspek dalam kehidupan termasuklah kemiskinan, pencemaran dan kemanusiaan. Selain pengalaman beliau, Nassury telah menggunakan landskap kota tersebut untuk menitipkan perspektif dan perasaannya terhadap Kuala Lumpur. Kuala Lumpur digambarkan sebagai sebuah kota yang telah membangun secara material, akan tetapi sifat kebaikan dan belas kasihan penduduknya telah berkurangan dengan begitu ketara sekali. Puisi Nassury menunjukkan kesempitan hidup dan kualiti kehidupan yang rendah akibat kelemahan perancangan bandar yang memberikan kesan buruk kepada penduduk bandar kosmopolitan ini. A segmented healthcare system evolved in India by 1990s, whereby the rich population depended on private hospitals while the people at the bottom of the economic pyramid went to the poor-quality public hospitals. In a democracy of equals, unequal access to services became political when COVID-19 began to put pressure on the health system. Corruption that was normalized in a segmented healthcare system could no longer be ignored. To advance the framework of social quality, we examine the corruption that unfolded during the pandemic in India from the perspective of moral foundation theory. We study the issues raised by political parties during the pandemic and court directives responding to citizen grievances. The evidence shows there was inequality of access and that courts had to intervene to try to rectify the situation. In the absence of effective governmental intervention during the pandemic, moral norms become a useful explanatory factor for social quality.

Fair warning to the reader: Culture of Death: The Assault on Medical Ethics in America, by Wesley J. Smith, is hard for me to review. I work at Rush–Presbyterian–St. Luke's Medical Center, where in April 1989, Rudy Linares removed his son, Sammy, from a ventilator at gunpoint. I took the criticism hard and was critical of others who were in the outcry. In fact, some of the people Smith criticizes are people with whom I had differences. Yet, Smith's views are so harsh and alien to my understanding of the culture and practice of healthcare that I find them off-putting. Further, he and I participate in the same listserv, where I often disagree with his postings.

Background: A case of mother to child transmission (MTCT) of HIV at a medical center in Washington, DC, resulted in the implementation of universal opt-out rapid testing of patients admitted for delivery. This article evaluates the policy’s efficacy and implementation. Methods: We evaluated the implementation using the Reach, Efficacy, Adoption, Implementation, and Maintenance (RE-AIM) framework. Results: We could not evaluate decrease in MTCT rate secondary to low sample size ( n = 3324) and no true-positive results. Patients not tested ( n = 458) were predominately secondary to physician omission (93.7%) and were more likely to be White ( p < 0.01) and older ( p < 0.01). There was a negative relationship with physician omission over time. Conclusion: The policy was successfully implemented with decreasing proportions of patients not tested. Earlier inclusion of testing into standard admission orders and nurse-based approach may have expedited adoption. Given the low incidence of new HIV diagnosis in labor, we were unable to assess decrease in MTCT.
