A segmented healthcare system evolved in India by 1990s, whereby the rich population depended on private hospitals while the people at the bottom of the economic pyramid went to the poor-quality public hospitals. In a democracy of equals, unequal access to services became political when COVID-19 began to put pressure on the health system. Corruption that was normalized in a segmented healthcare system could no longer be ignored. To advance the framework of social quality, we examine the corruption that unfolded during the pandemic in India from the perspective of moral foundation theory. We study the issues raised by political parties during the pandemic and court directives responding to citizen grievances. The evidence shows there was inequality of access and that courts had to intervene to try to rectify the situation. In the absence of effective governmental intervention during the pandemic, moral norms become a useful explanatory factor for social quality.

The Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) pandemic had a significant impact on the Italian healthcare system, although geographical differences were present; regions in northern Italy have been the most severely affected while regions in the south of the country were relatively spared. Otolaryngologists were actively involved in the management of the pandemic. In this work, we analyzed and compared the otolaryngology surgical activity performed during the pandemic in two large public hospitals located in different Italian regions. In northern Italy, otolaryngologists were mainly involved in performing surgical tracheotomies in COVID-19 positive patients and contributed to the management of these patients in intensive care units. In central Italy, where the burden of the infection was significantly lower, otolaryngologists focused on diagnosis and treatment of emergency and oncology patients. This analysis confirms the important role of the otolaryngology specialists during the pandemic, but also highlights specific differences between two large hospitals in different Italian regions.

Abstract Background Nepal has made significant strides in maternal and neonatal mortality over the last three decades. However, poor quality of care can threaten the gains, as maternal and newborn services are particularly sensitive to quality of care. Our study aimed to understand current gaps in the process and the outcome dimensions of the quality of antenatal care (ANC), particularly at the sub-national level. We assessed these dimensions of the quality of ANC in 17 primary, public hospitals across Nepal. We also assessed the variation in the ANC process across the patients’ socio-economic gradient. Methods We used a convergent mixed methods approach, whereby we triangulated qualitative and quantitative data. In the quantitative component, we observed interactions between providers (17 hospitals from all 7 provinces) and 198 women seeking ANC and recorded the tasks the providers performed, using the Service Provision Assessments protocol available from the Demographic and Health Survey program. The main outcome variable was the number of tasks performed by the provider during an ANC consultation. The tasks ranged from identifying potential signs of danger to providing counseling. We analyzed the resulting data descriptively and assessed the relationship between the number of tasks performed and users’ characteristics. In the qualitative component, we synthesized users’ and providers’ narratives on perceptions of the overall quality of care obtained through focus group discussions and in-depth interviews. Results Out of the 59 tasks recommended by the World Health Organization, providers performed only 22 tasks (37.3%) on average. The number of tasks performed varied significantly across provinces, with users in province 3 receiving significantly higher quality care than those in other provinces. Educated women were treated better than those with no education. Users and providers agreed that the overall quality of care was inadequate, although providers mentioned that the current quality was the best they could provide given the constraints they faced. Conclusion The quality of ANC in Nepal’s primary hospitals is poor and inequitable across education and geographic gradients. While current efforts, such as the provision of 24/7 birthing centers, can mitigate gaps in service availability, additional equipment, infrastructure and human resources will be needed to improve quality. Providers also need additional training focused on treating patients from different backgrounds equally. Our study also points to the need for additional research, both to document the quality of care more objectively and to establish key determinants of quality to inform policy.

Pelayanan publik menjadi suatu tolok ukur kinerja pemerintah yang paling kasat mata. Seiring dengan perkembangan teknologi dan banyaknya tantangan yang harus dihadapi maka pelayanan publik harus diselaraskan dengan terobosan atau inovasi. Salah satu pelayanan publik yang perlu dioptimalkan adalah Sistem Administrasi Manunggal Satu Atap (SAMSAT). Oleh karena itu, Kantor Bersama Samsat Gresik membuat aplikasi yang diresmikan pada tanggal 14 Juli 2020 adalah SAMSAT RAME (Samsat Ramah Merakyat) yaitu sebuah aplikasi pelayanan pajak serta informasi terkait pengurusan perpanjangan masa berlaku surat tanda nomor kendaraan (STNK) bermotor. Tujuan penelitian ini yaitu untuk mengetahui bagaimana optimalisasi pelayanan Kantor Bersama Samsat Gresik melalui Inovasi Aplikasi Samsat Rame (Samsat Ramah Merakyat) sehingga mampu menjawab beberapa permasalahan pelayanan publik Teknik analisis data dengan pengumpulan data, reduksi data, penyajian data, dan penarikan kesimpulan/verifikasi. Teknik pengumpulan data melalui wawancara, observasi, dokumentasi dan studi literatur.  Hasil dari penelitian ini menunjukkan bahwa aplikasi Samsat Ramah Merakyat (Samsat Rame) belum efisien karena input pelayanan masih memberatkan para pengguna jasa. Belum efektif karena tidak berhasil mencapai tujuan yang dikehendaki oleh penyelenggara. Serta kualitas hasil yang buruk dikarenakan faktor pendukung terkesan membuat aplikasi Samsat Rame di lakukan karena mendesak tanpa memikirkan jangka panjang. Sedangkan faktor penghambat lebih berpengaruh pada keberhentian inovasi Samsat Rame. Salah satu saran yang dapat diajukan yaitu meningkatkan sosialisasi dan promosi kepada masyarakat Kabupaten Gresik. Kata Kunci : Pelayanan Publik, Inovasi Pelayanan Pajak, Samsat Rame (Samsat Ramah Merakyat)   Public service becomes the most visible measure of government performance. Along with technological developments and the many challenges that must be faced, public services must be aligned with breakthroughs or innovations. One of the public services that need to be optimized is the One-Stop One-Stop Administration System (SAMSAT). Therefore, the Samsat Gresik Joint Office made an application that was inaugurated on July 14, 2020, namely SAMSAT RAME (Samsat Ramah Merakyat), which is a tax service application as well as information related to the extension of the validity period for motorized vehicle registration certificates (STNK). The purpose of this study is to find out how to optimize the services of the Samsat Gresik Joint Office through the Samsat Rame Application Innovation (Samsat Ramah Merakyat) so that it is able to answer several public service problems. Data analysis techniques are data collection, data reduction, data presentation, and conclusion / verification. Data collection techniques through interviews, observation, documentation and literature study. The results of this study indicate that the Peoples Friendly Samsat application (Samsat Rame) is not efficient because service inputs are still burdensome for service users. Not yet effective because it failed to achieve the goals desired by the organizers. As well as the poor quality of results due to supporting factors that seem to make the Samsat Rame application done because it is urgent without thinking about the long term. While the inhibiting factor has more influence on the discontinuation of the Samsat Rame innovation. One of the suggestions that can be put forward is to increase socialization and promotion to the people of Gresik Regency. Many authors point to the low incidence of occupational diseases in our country. The reasons are poor-quality preventive examinations, the lack of interest of employers in identifying these diseases, and others. However, there are no works in the literature devoted to errors in the statistical coding of diagnoses of occupational pathology and their accounting. The study aims to improve the accounting of occupational diseases in Russia. We conducted the study using the information base of the Industry Register of Persons with Occupational Diseases, developed by Burnazyan State Medical Center of the FMBA, Russia, which operating in the industry since 2011. As of 31.12.2020, the register contains information on 2,056 patients with occupational diseases, from among the employees of enterprises and organizations served by healthcare institutions of the FMBA, Russia. Errors of incorrect coding of diagnoses for occupational diseases with violation of the classification principles in the used directive materials played a negative role. The development of plans for medical and social rehabilitation measures to reduce the incidence of occupational diseases among the people working in harmful conditions. It requires correct knowledge of morbidity indicators of this nosology. In order to improve the accounting of occupational diseases in Russia, it is necessary to switch to the system of coding diagnoses of this nosology, adopted in medical statistics, and make appropriate changes to the directive documents.

The paper is devoted to the Raiky culture in the Middle Dnieper. It reveals major issues of the phenomenon of Raiky culture and their possible solutions considering the achievements of Ukrainian archeologists in this field. The genesis, chronology and features of the development of material culture of the Raiky sites in the 8th—9th centuries of the right-bank of the Dnieper are analyzed. In general the existence of the Raiky culture in the Middle Dnieper region can be described as follows. It was formed in first half of the 8th century in the Tiasmyn basin. The first wheel-made pottery has begun to manufacture quite early, from the mid-8th century (probably at the beginning of the third quarter). At the first stage, the early vessels have imitated the hand-made Raiky forms as well as the Saltovo-Mayaki imported vessels. Significant development of the material culture occurs during the second half of the 8th century. At the same time, the movement of the people of Raiky culture and the population of the sites of Sаkhnivka type has begun in the northern direction which was marked by the appearance of the Kaniv settlement, Monastyrok, and possibly Buchak. This stage is characterized by the syncretism both in the ceramic complex and in the features of design of the heating structures. Numerous influences of the people of Volyntsevo culture (and through them – of Saltovo-Mayaki one) are recorded in the Raiky culture. It is observed not only in direct imports but also in the efforts of the Raiky population to imitate the pottery of Volyntsevo and Saltovo-Mayaki cultures, however, based on their own technological capabilities. The nature of the relationship between the bearers of these cultures is still interesting. The population of Raiky accepts the imported items of Saltovo-Mayaki and Volyntsevo cultures, tries to imitate high-quality pottery of them, and even one can see the peaceful coexistence of two cultures in one settlement — Monastyrok, Buchak, Stovpyagy. However, the reverse pulses are absent. There are no tendencies to assimilate each other. Although, given the number and size of the sites, the numerical advantage of the Volyntsevo population in the region seems obvious. There is currently no answer to this question. The first third of the 9th century became the watershed. The destruction of the Bytytsia hill-fort and the charred ruins to which most of the settlements of the Volyntsevo culture has turned, is explained in the literature by the early penetration of Scandinavians into the region or as result of the resettlement of Magyars to the Northern Pontic region. In any case, this led to a change in the ethnic and cultural situation in the Dnieper basin. According to some researchers, the surviving part of the population of Volyntsevo culture migrated to the Oka and Don interfluve. For some time, but not for long, the settlements of Raiky culture remained abandoned. Apparently, after the stabilization of situation, the residents have returned which is reflected by the reconstruction of the Kaniv settlement and Monastyrok; in addition, on the latter the fortifications have been erected. The final stage of the existence of culture is characterized by contacts with the area of the left bank of Dnieper, the influx of the items of the «Danube circle», as well as the rapid development of the forms of early wheel-made pottery. The general profiling of vessels and design of the rim became more complicated, the rich linear-wavy ornament which covers practically all surface of the item became characteristic. This suggests the use of a quick hand wheel which has improved the symmetry of the vessels, as well as permitted to create the larger specimens. The evolution of the early wheel-made ceramic complex took place only by a variety of forms, however, technological indicators (dough composition, firing, density and thickness of vessel walls) indicate the actual invariability and sustainability of the manufacture tradition. The discontinuance of the functioning of the latest Raiky sites (Monastyrok and Kaniv settlements) can be attributed as the consequences of the first stages of consolidation of the Rus people in the Middle Dnieper dating to the late 9th — the turn of the 9th—10th centuries.

Safety of people and traffic police on road and the provision of prompt and appropriate treatment of injured persons in road accident are urgent concerns. The nine recommendations accordingly made are 1) Considering anyone who informs about or brings to the hospitals the accident victims as innocent until proved otherwise, 2) Annual payment by all vehicle owners (as per the cost of vehicles) to generate treatment fund for any road accident injured patients in the free general (not paying or private or extended health service) outdoor or emergency clinics or ward of the public hospitals irrespective of anyone’ fault in the accident (insurance or other agencies may be assigned to handle the amount deposited and reimbursement of the payments to the hospitals), 3) Implementation of helmet wearing by motorcycle riders and pillion riders in motorcycles, 4) Stricter fine for hazardous traffic offenses, 5) Drivers of the larger vehicles should not automatically be held responsible for any accidents involving other smaller vehicles (to prevent smaller vehicles and motorcycles to drive recklessly), 6) Drivers should not be just held responsible to bear health expenses of injured patients (which is much more than the compensation required in the event of death of injured persons); this is to encourage drivers to take injured persons immediately to hospitals and prevent inclination to allow their deaths indirectly or directly; the drivers should be proportionately fined or punished as per the traffic regulations if they are found to be negligent, 7) Safe and visible platform for the traffic police to stay on the road, 8) Provision of cost-effective respirators for traffic police and traffic supervisors, and 9) Compensation for occupational hazards to the traffic police and field traffic supervisors by distributing to them adequate proportion (e.g. one-third to one-half) of the fund collected by stricter fine paid for the hazardous traffic offences. Provision of various allowances, including for hazards, and benefits is a common practice in the country. Compensation for the occupational hazards of the traffic police provides incentives to and motivates them to remain vigilant about hazardous traffic offenses day and night everywhere and, thus, is essential for the safety of the people.

The competitive healthcare system and healthcare environment, modern hospitals with substantial investment, healthcare reforms, availability of specialized persons in hospital management, health services management, the success of quality management programs in developed nations and high interest of international health organizations like WHO has led the developing countries like Pakistan to pay due attention to quality in national healthcare system.  Since the time of independence healthcare system in Pakistan is striving for improvements. Despite of the physical infrastructure and availability of qualified workforce for service delivery there are gaps in strategic and operational planning which resulted in poor quality health services. The leaders in healthcare has little awareness that the quality management is an organized specialty to improve the quality of healthcare. It is need of the hour that healthcare leaders in Pakistan must integrate quality improvement activities in strategic and operational planning process of healthcare system. This the only way to maximize the benefits of healthcare system and restore the rapidly deteriorating public trust.

The Pesaguan Dayak tribe is a tribe in West Kalimantan Province. Most of the people of this tribe are Catholic and Protestant. In the initial observation (pre-research), it seems that their understanding of the Bible and its position in the practice of life needs attention. However, in everyday life, the Pesaguan Dayak community is still robust with the customs, ethics, and moral norms of the tribal religion. To obtain a Christian portrait of the Pasaguan Dayak tribe, the researchers used qualitative research methods, emphasizing surveys or observations and interviews. From the results of research and interviews conducted, it was found that in the daily life of the Pesaguan Dayak people, the Bible is not the primary basis for the Pesaguan Dayak tribe. Another portrait of Christianity found in the field is the absence of awareness from the Pesaguan Dayak community to reach out to Malays to believe in Jesus, even though they live next door.   Suku Dayak Pesaguan adalah suku yang berada di Provinsi Kalimantan Barat. Sebagian besar orang-orang dari suku ini beragama Katolik dan Protestan. Pada obersevasi awal (prapenelitian), tampaknya pemahaman mereka tentang Alkitab dan posisinya dalam praktik kehidupan, perlu mendapat perhatian. Dalam kehidupan sehari-hari, masyarakat suku Dayak Pesaguan masih sangat kuat dengan adat istiadat, etika dan norma-norma moral agama suku.  Untuk memperoleh potret kekristenan suku Dayak Pasaguan, maka peneliti memanfaatkan metode penelitian kualitatif, dengan menekankan pada survei atau observasi dan wawancara. Dari hasil penelitian dan wawancara yang dilakukan, ditemukan bahwa dalam kehidupan sehari-hari masyarakat suku Dayak Pesaguan, Alkitab bukanlah landasan dasar utama yang dimiliki suku Dayak Pesaguan. Potret kekristenan lainya yang ditemukan di lapangan yaitu belum adanya kesadaran dari masyarakat suku Dayak Pesaguan untuk menjangkau orang Melayu untuk percaya pada Yesus, meskipun mereka hidup bertetangga.

The introduction to this book begins with a detailed description of what news fixers are and how their work has evolved over time. Since the book focuses primarily on news fixing in the 21st century, the introduction historicizes the figure of the fixer, illuminating the fixer’s connections to the interpreters or guides hired by explorers, missionaries, and anthropologists of past centuries. This brief but necessary historicization is firmly rooted within the critical framework of postcolonial studies, a theoretical lens that helps me explain the deeply entrenched tradition of colonial dependence on regionally specific knowledge—knowledge that unfortunately did not prevent the misrepresentation and exploitation of the people living in these other places. The introduction then moves to an examination of the news fixers’ current role within the larger ecosystem of international reporting. Building off the rich literature found in the field of journalism studies, which examines the various elements of the labor of foreign correspondence, the introduction will show that a space must be made within journalism scholarship for the study of news fixers. What is more, the field of global journalism ethics also has much to gain from a closer examination of these locally based media employees.

This chapter introduces Henry Cole, who was heavily influenced by the ideas of the philosopher Jeremy Bentham and became the conduit for utilitarianism into the Royal Society of Arts. It analyzes Bentham's proposed alternative that based everything on a fundamental guiding principle of achieving the greatest happiness for the greatest number of people. It also mentions utilitarian reformers inspired by Bentham that introduced improvements for the benefit of “the masses” or the “people.” The chapter talks about Rowland Hill, a utilitarian reformer who tried to create a national prepaid postage system. It examines Hill's scheme to replace the messy postal system that favoured only the rich and powerful by promoting a flat rate so that any letter weighing under an ounce might be sent to anywhere in the country for just a penny.
