Clinical relevance: Colon lipomas are not rare tumors, but they occur with blurred clinical features and patients arrive with complications- obstructive intestinal obstruction or bleeding. In this regard, the authors presented a clinical case of a 57-yearold patient whose diagnosis was established only by morphological examination of the surgical material.The purpose of the study was to demonstrate clinical observation of colon lipoma.Materials and methods: The case history of a 57-year-old patient diagnosed with colonic lipoma is presented.

Clinical relevance. Duodenal MALT-lymphoma, a rare disease of the duodenum with a blurred clinical picture, frequently is an incidental finding at esophagogastroduodenoscopy. Psoas abscess is an uncommon presentation on the acute medical take. However recognition and appropriate treatment is essential. This review is designed to highlight the clinical features, microbiology, diagnostic tests and treatment for this condition. In order to illustrate some of the pitfalls and complexities in the management of psoas abscess we have included a case history of a patient who was recently treated in our department.

Hundreds of thousands of bariatric surgeries are performed worldwide every year. Th ey have long been proven to be safe and eff ective in treating obesity and type 2 diabetes. Along with an unconditional positive eff ect, these interventions, especially shunting ones, are characterized by specifi c complications. In the absence of proper correction, they can become fatal for patients. One of these complications is malabsorption leading to a defi ciency of vitamins and microelements, which in most cases, is amenable to timely correction in the postoperative period. However, there are situations when it is not possible to carry out an eff ective correction and it becomes necessary to perform reconstructive interventions with the reverse inclusion of the small intestine in the digestion, which is associated with great diffi culties. Th e authors demonstrated this situation in the description of clinical observation of a patient with postoperative hypothyroidism and history of postoperative hypoparathyroidism, who underwent bariatric surgery. Impaired absorption of drugs (L-thyroxine, calcium, and vitamin D), and therefore, uncompensated hypothyroidism and hypocalcemia was an indication for reconstructive surgery.

Aim. A clinical observation of colitis conditioned by mycophenolate mofetil intake and concomitant Clostridium difficile-associated disease.Key points. Mycophenolate mofetil (MMF) is an active immunosuppressant with side effects affecting gastrointestinal tract (GIT). A 37-yo male patient with type 1 diabetes mellitus was admitted at a gastroenterology unit with clinical signs of diarrhoea with haematochezia. A history of diabetic nephropathy and related-donor pre-dialysis kidney transplantation in 2012, since when MMF intake was 2000 mg daily. Catarrhal ulcerative colitis in colonoscopy, C. difficile toxins in pathogen stool panel. Ulcerative, ischaemic colitises and the graft-versus-host disease were ruled out in examination. A positive clinical and endoscopic trend was observed upon MMF withdrawn and start of vancomycin.Conclusion. The case presented illustrates the clinical picture and diagnostic algorithm in MMF-associated colitis. The case-distinctive is association with C. difficile infection.

Objective. To analyze a rare clinical case of the aneurysmal bone cyst (ABC) of the paranasal sinuses in a 9-year-old patient.Materials and methods. We studied the data of the anamnesis, diagnostic and treatment methods contained in the medical history of the inpatient. We examined the patient and kept her under clinical observation during the process of the examination and treatment.Results. The fnal diagnosis of the ABC of the paranasal sinuses in the 9-year-old patient was verifed after the obtainment of the results of the performed clinical and morphological studies. The neoplasm was surgically removed. One year after the surgery, the MRI investigation showed no relapse. The child is in a fair condition.Conclusion. To diagnose an ABC, it is required to perform a histological examination of the tumor including the use of immunohistochemical methods.

Meckels diverticulum is a sac-like bulge of the wall of ileum formed in result of incomplete closure of the viteline duct that participates in nutrition of the embryo. Aim. To familiarize a wide community of colleagues with an interesting clinical case of a foreign body (a coin) in Meckels diverticulum. In the article, approaches to diagnostics and operative treatment of such patients are given. This clinical observation is of interest to abdominal surgeons from the point of view that Meckels diverticulum is most commonly identified by clinical symptoms present in diverticulitis, bleeding or perforating ulcer, in intestinal obstruction caused by formation of a node or of invagination. Conclusion. A situation in which a blunt-ended foreign body permitted to identify Meckels diverticulum, is described as casuistically rare.

A diagnostic conundrum of a firm, left submandibular node presenting in a 35-year-old female with a history of pain and persistent swelling is described. This paper aims to educate readers on the differential diagnostic dilemma that may be faced in general practice and discusses the importance of shared care in a multidisciplinary setting. CPD/Clinical Relevance: The presentation of this clinical case is discussed with radiographic imaging and the multidisciplinary approach undertaken for patient management. Readers can identify the steps undertaken in this case to exclude conditions in the surgical sieve.

This article describes a clinical case of lepromatous leprosy. Despite the sporadic incidence of leprosy in Russia, the presented clinical observation demonstrates that clinicians must maintain vigilance with regard to leprosy. Close attention and in-depth study of the history of the disease is required both in endemic and (in view of active migration and tourist flows) non-endemic regions, since timely diagnosis of this mycobacteriosis can present significant difficulties.
