Background and Objectives: Abuse and indiscriminate use of antibiotics, prolong hospital admission, travel history, organ transplants, immunocompromised conditions and age are parts of the factors that contribute to development of antibiotic resistance and intestinal carriage of Extended Spectrum Beta-Lactamases (ESBL) Enterobacteriaceae. These bacteria affect the course and outcome of an infection and continue to pose a challenge to infection management worldwide. This study was carried out to determine the antimicrobial susceptibility patterns and prevalence of ESBL-producing Enterobacteriaceae isolated from stool samples of HIV and AIDS patients in Ibadan, Nigeria. Materials and Methods: One hundred stool samples were collected from consenting HIV and AIDS patients accessing care in Antiretroviral (ARV) Clinic in a secondary and a tertiary health care facility in Ibadan, Nigeria. Gram-negative bacteria were isolated and identified using conventional methods. Antimicrobial susceptibility test was carried out using the Kirby Bauer disc diffusion technique. Phenotypic detection of ESBL-producing isolates was carried out using Double Disc Synergy Test (DDST). Results: A total of 240 Gram-negative bacteria were isolated comprising 100 (41.6%) Escherichia coli, 33(13.8%) Klebsiella pneumonia, 27(11.3%) Serratia marcescens, 20(8.3%) Salmonella enterica, 9(3.8%) Proteus vulgaris, 13(5.4%) Proteus mirabilis, 21(8.8%) Citrobacter freundii and 17(7.1%) Enterobacter aerogenes. Out these, 56 (23.3%) were ESBL-producers; comprising 23(41.0%) Escherichia coli, 9(16.0%) Klebsiella pneumonia, 6(10.7%) Serratia marcescens, 5(8.9%) Salmonella enterica, 3(5.3%) Proteus vulgaris, 5(8.9%) Proteus mirabilis, 2(3.6%) Citrobacter freundii and 3(5.4%) Enterobacter aerogenes. Among the ESBL producers, 45 (80.3%) and 38 (67.8%) showed resistance to trimethoprim/sulfamethoxazole and aztreonam while 3 (3.5%) showed resistance to ertapenem. Also, 96.4% (54/56) of the ESBL producers were multidrug resistant. Conclusion: This study showed that HIV and AIDS patients are reservoirs of ESBL-producing Enterobacteriaceae through faecal carriage, presenting them as likely source of dissemination of ESBL producer in community and hospital settings.

AbstractA new dimeric naphtho-γ-pyrone, 2-hydroxydihydronigerone (1), along with five compounds, nigerone (2), pyrophen (3), kojic acid (4), 4-(hydroxymethyl)-5-hydroxy-2H-pyran-2-one (5), and p-hydroxyphenylacetic acid (6), was isolated from an endophytic fungus Aspergillus niger AKRN associated with the roots of Entandrophragma congoënse. The structure of the new compound has been elucidated using spectroscopic data including 1D and 2D NMR as well as the high-resolution mass spectrometry. Compounds 1–5 showed weak antimicrobial activity on five selected Gram-negative bacteria, namely Enterobacter aerogenes (CM64), Enterobacter cloacae (BM67), Klebsiella pneumonia (K2), and Escherichia coli (ATCC8739 and ATCC10536).

Urinary tract infection caused by bacteria leads to inflammation. It is an important infection which can cause a highly morbidity rates in both males and females. The aim of the present study was determine the percentage prevalence of the UTI among the general population and the percentage of the bacterial isolates in the Urine samples. Among 150 samples bacterial isolates were obtained which included Escherichia coli, Klebsiella pneumonia, Proteus vulgaris, Pseudomonas aeruginosa. The highest prevalence of specific bacteria was found to be Escherichia coli recorded among all the UTI patients to be (55.55%) the least percentage among the bacteria were Klebsiella pneumonia, Proteus vulgaris, Pseudomonas aeruginosa and Enterobacter aerogenes which recorded 16.66%, 5.55%, 11.11% and 11.11% respectively. Results of the study showed that of femaleshad UTI more than the males.

<p class="StandardTextkrperSAR">Water samples were collected from three sites (Ogbakiri, Choba and Iwofe) of the New Calabar River and were evaluated for microbiological and physico-chemical characteristics. Bacterial isolates consisting mainly of gram-negative bacteria (<em>Escherichia coli, Proteus vulgaris, Acinetobacter</em> spp, <em>Pseudomonas aeruginosa</em>, <em>Enterobacter aerogenes, Klebsiella</em> spp., <em>Vibrio</em> sp. and <em>Nocardia asteroides</em>) were isolated from the samples. The highest total coliform counts log<sub>10</sub> 2.9 were encountered in water samples from Iwofe but the lowest counts log<sub>10</sub>1.9 occurred in the water samples from Ogbakiri. Ecological parameters varied among the sites with the highest pH value of 7.87 occurring at Iwofe and lowest of 6.12 at Choba. Maximum salinity level of 15<sup>0</sup>/<sub>00</sub> was observed at Iwofe and lowest at Choba. In contrast, maximum dissolved oxygen of 10.1ppm was observed at Choba and the minimum at Iwofe. The different bacterial profiles are probably attributable to the anthropogenic and industrial activities of the sites.</p>

Diabetic ulcers are a chronic complication of diabetes mellitus and have a high risk of infection. Severe ulcer infectionsare a significant cause of lower-extremity amputations in addition to trauma. Therefore, therapy for diabetic ulcer infectionsmust be performed immediately. This study aimed to determine the bacterial susceptibility pattern to the antibiotic indiabetic ulcer patients. This study was retrospective observational descriptive by taking the results of swab culture andantibiotic susceptibility patterns data in diabetic ulcer patients at Ulin General Hospital, Banjarmasin, in 2016-2018. Theresults showed 41 (62.1%) monomicrobial infections and 25 (37.9%) polymicrobial infections. The number of Gram-negativebacilli (57.4%) was higher than Gram-positive cocci (42.6%). The most common bacterial isolates on pus culture wereStaphylococcus aureus (26.6%), Klebsiella pneumonia (19.1%), and Escherichia coli (12.8%). Antibiotic susceptibility testresults showed that Gram-positive bacteria were sensitive to Tigecycline (100%), Nitrofurantoin (96.9%), and Linezolid(96.8%). Gram-negative bacteria were susceptible to Ertapenem (92.7%), Meropenem, and Amikacin (90.6%). S.aureusisolates were sensitive 100% to Meropenem and Tigecycline. K.peneumoniae and E.coli isolates were susceptible 100% toMeropenem and Amikacin. It was concluded in this study that the prevalence of Gram-negative bacteria in diabetic ulcerinfection was higher than Gram-positive bacteria. The most common isolated Gram-negative bacteria were K.pneumoniaeand E.coli, while the most common Gram-positive bacteria were S.aureus. The most sensitive antibiotics for K.pneumoniaeand E.coli were Meropenem and Amikacin, while the most sensitive antibiotics for S.aureus were Linezolid and Tigecycline.

Мета роботи — вивчити резистентність до антибіотиків бактеріальних збудників інфекцій сечових шляхів (ІСШ), виділених у пацієнтів урологічного стаціонару в м. Києві. Матеріали і методи. Досліджено 1612 штамів бактерій, виділених із сечі хворих з ІСШ (цистит, уретрит, пієлонефрит), госпіталізованих в урологічне відділення ДУ «Інститут урології НАМН України» у м. Києві протягом 2016 р. Серед пацієнтів переважали жінки — 1201 (74,5 %). Вік хворих становив від 17 до 74 років. Для збору даних використано медичну документацію лікарні. Мікробіологічні дослідження виконано у лабораторії мікробіології ДУ «Інститут урології НАМН України». Аналізували результати культурального дослідження зразків сечі, зібраних за наявності клінічних ознак ІСШ. Дослідження клінічного матеріалу та інтерпретацію отриманих результатів проводили загальноприйнятими методами. Вивчено чутливість уропатогенів до 31 антибіотика дискодифузійним методом відповідно до рекомендацій Інституту клінічних та лабораторних стандартів США (Clinical and Laboratory Standards Institute (CLSI)). Результати та обговорення. Аналіз мікробного спектра сечі виявив домінування серед уропатогенів штамів Escherichia coli (32,0 %), Enterococcus faecalis (19,5 %), Klebsiella pneumoniae (10,9 %), Staphylococcus epidermidis (8,9 %), S. haemolyticus (6,5 %) та Pseudomonas aeruginosa (6,4 %). Частка Enterococcus faecium, Enterobacter aerogenes і Streptococcus viridans становила відповідно 2,5, 2,2 і 1,6 %, Enterobacter cloacae, Klebsiella oxytoca, Acinetobacter baumannii, Proteus vulgaris та Providencia rettgeri — менше 1,0 %. У більшості випадків (69,7 %) мікроорганізми виділено у монокультурі, у решті випадків — у мікробних асоціа- ціях. Високу резистентність до тестованих антибіотиків виявили штами E. aerogenes (45,1 %), E. cloacae (45,7 %), E. faecium (40,9 %), E. faecalis (40,7 %), E. coli (39,9 %), P. aeruginosa (34,0 %), K. pneumoniae (28,6 %). Найбільш активними до уропатогенів були іміпенем (E. coli — 87,6 %, P. aeruginosa — 75,7 %, E. cloacae — 67,3 %, E. aerogenes — 72,6 %, K. pneumoniae — 93,2 %), меропенем (E. coli — 89,1 %, P. aeruginosa — 76,7 %, K. pneumoniae — 82,6 %), лефлоцин (E. coli — 74,5 %, ентерококи — 78,7 %, P. aeruginosa — 76,7 %, E. cloacae — 73,9 %, E. aerogenes — 80,4 %, K. pneumoniae — 83,5 %), амоксицилін/клавуланат (ентерококи — 84,6 %), фурагін (ентерококи — 82,6 %), цефоперазон (K. pneumoniae — 89,2 %, P. aeruginosa — 73,8 %), цефтріаксон (K. pneumoniae — 80,1 %). Висновки. Антибіотикорезистентність збудників ІСШ — важлива терапевтична проблема. Найбільшою активністю до уропатогенів характеризуються іміпенем, меропенем, лефлоцин, амоксицилін/ клавуланат, фурагін, цефоперазон, цефтріаксон, які можна розглядати як препарат вибору для призначення стартової терапії ІСШ. Необхідно здійснювати постійний моніторинг за резистентністю до дії антибіотиків. Політику використання антибіотиків у кожному стаціонарі слід визначати залежно від локальних даних щодо резистентності до протимікробних препаратів.

Background: Extended Spectrum Beta Lactamase (ESBL) production in gram negative bacteria confers multiple antibiotic resistance, adversely affecting antimicrobial therapy in infected individuals. ESBLs result from mutations in β-lactamases encoded mainly by the bla TEM,bla SHVand bla CTX-Mgenes. The prevalence of ESBL producing bacteria has been on the increase globally especially its upsurge among isolates from community-acquired infections. Aim: To determine ESBL prevalence and identify ESBL genes among clinical isolates in Osun State, Nigeria. Material and Methods: A cross-sectional study was carried out from August 2016 –July 2017 in Osun State, Nigeria. Three hundred and sixty Gram negative bacteria recovered from clinical samples obtained from both community and healthcare associated infections were tested. They included147 Escherichia coli(40.8%), 116 Klebsiella spp(32.2%), 44 Pseudomo-nas aeruginosa(12.2%) and23 Proteus vulgaris (6.4%) isolates. Others were Acinetobacter baumannii, Serratia rubidae, Citrobacter spp, Enterobacter spp and Salmonella typhi. Disk diffusion antibiotic susceptibility testing was carried out, isolates were screened for ESBL production and confirmed using standard laboratory procedures. ESBLs resistance genes were identified by Polymerase Chain Reaction (PCR). Results: All isolates demonstrated multiple antibiotic resistance. Resistance to ampicillin, amoxicillin with clavulanate and erythromycin was 100%, whereas resistance to Imipenem was very low (5.0%). : Overall prevalence of ESBL producers was 41.4% with Klebsiellaspp as the highest ESBL producing Enterobacteriacaea. ESBL producers were more prevalent among the hospital pathogens than community pathogens, 58% vs 29.5% (p=0.003). ESBL genes were detected in all ESBL producers with the blaCTX-Mgene predominating (47.0%) followed by blaTEM(30.9%) and blaSHVgene was the least, 22.1%. The blaCTX-Mgene was also the most prevalent in the healthcare pathogens (62%) but it accounted for only 25% in those of community origin. Conclusion: A high prevalence of ESBL producing gram negative organisms occurs both in healthcare and in the community in our environment with the CTX-M variant predominating. Efforts to control spread of these pathogens should be addressed.

ABSTRACT In 1999, 39 of 2,599 isolates of the family Enterobacteriaceae (1.5%) collected by eight private laboratories in the Aquitaine region in France produced an extended-spectrum β-lactamase (ESBL). Among these were 19 Enterobacter aerogenes isolates; 8 Klebsiella pneumoniae isolates; 6 Escherichia coli isolates; 3 Proteus mirabilis isolates; and 1 isolate each of Serratia marcescens, Morganella morganii, and Providencia stuartii. ESBL producers were isolated from 38 patients, including 33 residents of 11 clinics or nursing homes and 5 ambulatory patients. Seven different ESBLs were characterized. These mainly consisted of TEM-24 (25 isolates) and TEM-21 (9 isolates), but TEM-15 (2 isolates) and TEM-3, TEM-19, SHV-4, and CTX-M-1 (1 isolate each) were also characterized. Seven strains showed the coexistence of different TEM- and/or SHV-encoding genes, including a new SHV-1 variant, SHV-44, defined by the substitution R205L previously reported for SHV-3 in association with S238G. The epidemiology of the ESBL producers was investigated by random amplification of polymorphic DNA, typing by enterobacterial repetitive intergenic consensus PCR, analysis of resistance cotransferred with the ESBL, and analysis of the restriction profiles of the ESBL-encoding plasmids. Of the TEM-24-expressing strains, 18 were E. aerogenes isolates, including 9 from the same clinic, that were representatives of the epidemic clone disseminating in France. Of the TEM-21-producing strains that belonged to different species of the family Enterobacteriaceae (E. coli, K. pneumoniae, and P. mirabilis), 8 were isolated in the same nursing home. Outbreaks due to strain and/or plasmid dissemination in these clinic and nursing home were demonstrated. The presence of ESBL producers in five ambulatory patients probably resulted from nosocomial acquisition. Our data highlight the serious need to monitor patients for ESBL-producing Enterobacteriaceae in general practice.

Background: In recent years, Extended-spectrum beta-lactamase (ESBL) producing microorganisms have complicated treatment of infections due to resistance of ESBL producing strains to a wide range of antimicrobials. Objective: Target of this study was to determine the prevalence of ESBL producing gramnegative bacteria in neonatal sepsis cases and to reveal the antimicrobial susceptibility pattern of those isolated ESBL producers. Materials and Methods: This cross sectional study was carried out in Dhaka Medical College Hospital (DMCH) over a period of 12 months from January to December in 2016. Following isolation and identification of gram-negative bacteria from blood samples of suspected septicemic neonates, antimicrobial susceptibility test was performed by Kirby Bauer disk-diffusion method and ESBL producers were detected by Double Disk Synergy (DDS) test. Results: Among 52 Gram-negative bacteria isolated from 106 blood samples, 34.61% ESBL producers were detected and Enterobacter spp. (45%) was predominant followed by Klebsiella pneumoniae (33.33%). None of the ESBL producers was resistant to colistin and tigecycline. All ESBL producing Acinetobacter baumannii, 77.78% and 66.67% of ESBL producing Enterobacter spp and Klebsiella spp. respectively showed resistance to meropenem. All ESBL producers were resistant to piperacillintazobactam. Conclusion: Appropriate measures should be taken to prevent the spread of ESBL producing strains by combining strategies for infection prevention, control and rational use of antibiotics.
