The mental state of the mentally ill is one of the most important factors in the treatment of the patient by a medical professional. But aggression, high-pitched communication, indifference of a doctor or health care worker can negatively affect the outcome of treatment and recovery of a patient with mental and behavioral disorders. The problem of modern medical deontology in Ukraine is the stigmatization of health workers to patients with mental disorders, which lead to self-stigmatization of both patients and their families. The purpose of the study is to enable the relatives of mentally ill people undergoing long-term inpatient treatment, to determine the quality of medical services in a psychoneurological hospital, to identify ethical and deontological shortcomings in the doctor’s relationship with relatives of mentally ill person, to determine the level of self-stigmatization in relatives of the patient by medical staff. The analysis of domestic and foreign scientific sources, bibliosemantic, analytical and statistical research methods were used in the work. A questionnaire was conducted (using a specially designed questionnaire) with elements of interviews of 92 respondents (relatives of mentally ill patients who were treated at a psychoneurological hospital in Vinnytsia). The results were processed using the licensed standardized package "Statistica 6.1". The need to comply with state building codes for health care facilities in accordance with European requirements has been proven. This will eliminate the feeling of hopelessness during hospitalization in public psychoneurological hospitals, which are a great trauma for relatives of patients. It has been shown that in order to prevent stigmatization of relatives of patients in psychoneurological hospitals, it is necessary to conduct trainings and interviews after training, which will determine the degree of stigmatization and subsequent preventive measures. It is shown that for relatives of mentally ill on the basis of psychoneurological hospitals it is necessary to equip rooms for psychodiagnostics and further psychocorrection. Development of affordable correctional programs for relatives of patients will reduce maladaptation and improve life satisfaction.

Kata kunci: kompetensi bidan, kajian kebidanan, pendidikan bidan, kurikulum kebidanan Abstract Midwives are strategic health workers who play an important role in maternal and child health services. They are required to have well competencies to run their tasks properly. Well, competencies can be achieved if the midwife's professional education providers meet the standards. Based on the Indonesian Health Workers' Assembly (MTKI) data in 2016, the average value of the DIII midwifery-competency test was only 41.08. Participants who failed the competency test were as much as 46.5%. It is still far from the expectation as the average value of health workers’ competency tests try out between 2012 to 2015 tends to decline. This study aims to identify midwife competencies based on Minister of Health's decree No. 369/MENKES/SK/III/2007 on midwives' profession standards and the results of the 2017 Research on Health Workers’ Education (Risdiknakes). The study was conducted using observation, in-depth interviews, and literature review. Informants are midwives at primary health care and midwifery experts. The results of the study indicate that midwife competencies in health care facilities are still not up to standard. Several factors in the administration of midwifery education also shape the competence of the midwives produced. The process of recruiting prospective students, the quality of lecturers, and the process of conducting midwifery education as a whole are components that must be the focus to produce midwives that comply with the competency standards in Minister of Health's decree No. 369/MENKES/SK/III/ 2007. Keywords: midwife competencies, midwifery studies, midwife education

AbstractObjectiveTo assess the adherence to Integrated Management of Childhood Illness (IMCI) guidelines in primary health care facilities in Burkina Faso and to determine the factors associated.Materials and MethodsWe used data from a large survey on health facilities, held from October 2013 to April 2014. Primary health facilities were evaluated, health workers interviewed and consultations observed. The standard guideline for an under five year’s old child consultation was the Integrated Management of Childhood Illness (IMCI).Results1,571 consultations were observed, carried out by 522 different practitioners. The danger signs were usually not checked (13.9% only checking for at least three general danger signs). The adherence for cough (74.8%), diarrhoea (64.9%), fever (83.8%) and anaemia (70.3%) was higher. The principal factors found to be associated with poorer adherence to guidelines of consultation were female sex (Rate Ratio (RR) = 0.91; 95% CI 0.86 – 0.95), non-nurse practitioner (RR=0.93; 95% CI 0.88 – 0.97), IMCI training (RR=1.06; 95% CI 1.01 – 1.11), non-satisfaction of the salary (RR=0.95 95% CI 0.91 – 0.99).ConclusionThis study highlights a poor adherence to the IMCI guidelines and by then, revealing a poor quality of under-five care. Indeed, many characteristics of health workers including gender, type of profession, training satisfaction with salary were found to be associated with this adherence. Therefore, more initiatives aiming at improving the quality of care should be developed and implemented for improving the child health care.

Quality of care plays an important role in the status of tuberculosis (TB) control, by influencing timely diagnosis, treatment adherence, and treatment completion. In this study, we aimed at establishing the quality of TB service care in Kamuli district health care centres using Donabedian structure, process, and outcomes model of health care. A cross-sectional study was conducted in 8 health care facilities, among 20 health care workers and 392 patients. Data was obtained using face-to-face interviews, an observation guide, a check list, and record review of the TB unit and laboratory registers. Data entry and analysis were done using EPI INFO 2008 and STATA 10 versions, respectively. A high number 150 (87.21%) of TB patients were not aware of all the signs to stop TB medication, and 100 (25.51%) patients received laboratory results after a period of 3–5 working days. The major challenges faced by health workers were poor attitude of fellow health workers, patients defaulting treatment, and fear of being infected with TB. One of the worst performance indicators was low percentage of cure. Comprehensive strengthening of the health system focusing on quality of support supervisions, patient follow up, promoting infection control measures, and increasing health staffing levels at health facilities is crucial.

Purpose The purpose of this study was to examine patient satisfaction with the quality of care in Ghana’s health-care facilities using a disaggregated approach. Design/methodology/approach The study was a cross-sectional national survey. A sample of 4,079 males and females in the age group of 15-49 years were interviewed. Descriptive statistics, principal component analysis and t-tests were used in statistical analysis. Findings About 70 per cent of patients were satisfied with the quality of care provided in health-care facilities in Ghana, whereas about 30 per cent of patients were fairly satisfied. Females and insured patients were more likely to be satisfied with the quality of care, compared with males and uninsured patients. Research limitations/implications Because data were obtained from a national survey, the questionnaire did not include the type of facility patients attended to find out whether satisfaction with the quality of care varied by the type of health facility. Future studies may, therefore, include this. Practical implications The study contributes to the literature on patient satisfaction with the quality of care. It highlights that long waiting time remains an intractable problem at various service delivery units of health facilities and constitutes a major source of patient dissatisfaction with the quality of care. Innovative measures must, therefore, be adopted to address the problem. Originality/value There is a paucity of research that uses a disaggregated approach to examine patient satisfaction with the quality of care at various service delivery units of health facilities. This study is a modest contribution to this research gap.

To ascertain the frequency of mental disorders in Sudan, Philippines, India, and Colombia, 925 children attending primary health care facilities were studied. Rates of between 12% and 29% were found in the four study areas. The range of mental disorders diagnosed was similar to that encountered in industrialized countries. The research procedure involved a two-stage screening in which a ten-item "reporting questionnaire" constituted the first stage. The study has shown that mental disorders are common among children attending primary health care facilities in four developing countries and that accompanying adults (usually the mothers) readily recognize and report common psychologic and behavioral symptoms when these are solicited by means of a simple set of questions. Despite this, the primary health workers themselves recognized only between 10% and 22% of the cases of mental disorder. The results have been used to design appropriate brief training courses in childhood mental disorders for primary health workers in the countries participating in the study.

Abstract Background: Birth companions have been shown to improve quality of care provided to laboring women. Various studies have shown the benefits of companion during child birth. The objective of this study was to assess the knowledge, attitude and practice of health care providers towards the involvement of birth companions during child birth in St Paul’s Hospital Millennium Medical College (SPHMMC) and its selected catchment health centers. Methods: The study used a cross-sectional study which was supplemented by qualitative study design which employed phenomenological design was conducted using in depth interview of health care providers who were on practice during the study period in SPHMMC. Result: The study included a total 58 health care providers. The finding from the study showed that great proportion of health care providers involved in the study (90 %) have positive attitude towards involvement of birth companions but majority 82.4 % didn’t allow birth companions. The commonest reasons mentioned for not involving companions include fear of breach of privacy, interference with routine medical care and risk of litigation/complaints.Conclusion and Recommendation: The findings from this study showed that majority of the health care providers are against the practice of involving birth companions. Since involvement of birth companions is one way to ensure the quality of care provided there should be an effort to increase the knowledge of health professionals on benefits of birth companions. There is also a need for improvement of the work setup. Simple measures like providing screens, providing separate rooms for laboring and post-partum women will have a significant impact on the quality of care provided.
